1Robotic surgery for rectal cancer: A systematic review of current practice显示文摘AIM: To give a comprehensive review of current litera-ture on robotic rectal cancer surgery.METHODS: A systematic review of current literature via PubMed and Embase search engines was per-formed to identify relevant articles from january 2007 to november 2013. The keywords used were: 'robotic surgery', 'surgical robotics', 'laparoscopic computer-assisted surgery', 'colectomy' and 'rectal resection'. RESULTS: After the initial screen of 380 articles, 20 pa-pers were selected for review. A total of 1062 patients(male 64.0%) with a mean age of 61.1 years and body mass index of 24.9 kg/m2 were included in the review.Out of 1062 robotic-assisted operations, 831(78.2%) anterior and low anterior resections, 132(12.4%) in-tersphincteric resection with coloanal anastomosis, 98(9.3%) abdominoperineal resections and 1(0.1%) Hart-mann's operation were included in the review. Robotic rectal surgery was associated with longer operative time but with comparable oncological results and anastomotic leak rate when compared with laparoscopic rectal surgery. CONCLUSION: Robotic colorectal surgery has con-tinued to evolve to its current state with promising re-sults; feasible surgical option with low conversion rate and comparable short-term oncological results. 2Application of constructed wetlands for wastewater treatment in tropical and subtropical regions(2000–2013)显示文摘Constructed wetlands(CWs) have been successfully used for treating various wastewaters for decades and have been identified as a sustainable wastewater management option for developing countries. With the goal of promoting sustainable engineered systems that support human well-being but are also compatible with sustaining natural(environmental)systems, the application of CWs has become more relevant. Such application is especially significant for developing countries with tropical climates, which are very conducive to higher biological activity and productivity, resulting in higher treatment efficiencies compared to those in temperate climates. This paper therefore highlights the practice,applications, and research of treatment wetlands under tropical and subtropical conditions since 2000. In the present review, removal of biochemical oxygen demand(BOD) and total suspended solid(TSS) was shown to be very efficient and consistent across all types of treatment wetlands. Hybrid systems appeared more efficient in the removal of total suspended solid(TSS)(91.3%), chemical oxygen demand(COD)(84.3%), and nitrogen(i.e.,80.7% for ammonium(NH)4-N, 80.8% for nitrate(NO)3-N, and 75.4% for total nitrogen(TN))as compared to other wetland systems. Vertical subsurface flow(VSSF) CWs removed TSS(84.9%), BOD(87.6%), and nitrogen(i.e., 66.2% for NH4-N, 73.3% for NO3-N, and 53.3% for TN)more efficiently than horizontal subsurface flow(HSSF) CWs, while HSSF CWs(69.8%)showed better total phosphorus(TP) removal compared to VSSF CWs(60.1%). 6Would integrated Western and traditional Chinese medicine have more benefits for stroke rehabilitation? A systematic review and meta-analysis显示文摘Background Stroke is a major cause of death or long-term disability worldwide.Many patients with stroke receive integrative therapy consisting of Western medicine(WM)and routine rehabilitation in conjunction with Chinese medicine(CM),such as acupuncture and Chinese herbal medicine.However,there is no available evidence on the effectiveness of the combined use of WM and CM interventions in stroke rehabilitation.Aims The purpose of this meta-analysis is to evaluate the results of all individual studies to assess the combined use of CM and WM in stroke rehabilitation compared with WM only.Methods The Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were followed.MEDLINE,EMBASE,Cochrane and China National Knowledge Infrastructure(CNKI)were searched.The included outcomes were dependency,motor function,depression and swallowing function.Subgroup analysis was performed,and publication bias was assessed using funnel plots.Summary of review 58 studies and 6339 patients were included in the meta-analysis.Subgroup analysis revealed that combined therapy comprising both acupuncture and WM had a superior effect on improving dependency and swallowing function compared with standard WM therapy alone.Potential superiority of combined therapy comprising CM and WM in improving depression compared with standard WM therapy was also found.Conclusions Our results indicate that the combined use of CM and WM could be more efficacious in stroke rehabilitation compared with the use of WM therapy alone.However,most studies were short in duration(2 to 4 weeks)and prone to different types of biases,which prevents making any conclusion regarding the long-term effects and raises concerns regarding true efficacy in context of high likelihood of Hawthorn bias.So,more randomised controlled trials with more rigorous design and longer duration of treatment and follow-up need to be conducted to compare WM alone versus WM and CM combined.Linda LD Zhong Ya Zheng Alexander Y Lau Norman Wong Liang Yao Xingyao Wu Tengteng Shao Zhenxing Lu Huijuan Li Chun Sum Yuen Jianwen Guo Suzanne Lo Janita Chau Kam Wa Chan Bacon Fung Leung Ng Zhaoxiang Bian Edwin Chau-leung Yu 2022Stroke & Vascular Neurology2022,7,1:7
7A GLP-1 analog lowers ER stress and enhances protein folding to ameliorate homocysteine-induced endothelial dysfunction显示文摘Hyperhomocysteinemia(HHcy)is an independent risk factor for cardiovascular diseases and increases mortality in type 2 diabetic patients.HHcy induces endoplasmic reticulum(ER)stress and oxidative stress to impair endothelial function.The glucagon-like peptide 1(GLP-1)analog exendin-4 attenuates endothelial ER stress,but the detailed vasoprotective mechanism remains elusive.The present study investigated the beneficial effects of exendin-4 against HHcy-induced endothelial dysfunction.Exendin-4 pretreatment reversed homocysteine-induced impairment of endothelium-dependent relaxations in C57BL/6 mouse aortae ex vivo.Four weeks subcutaneous injection of exendin-4 restored the impaired endothelial function in both aortae and mesenteric arteries isolated from mice with diet-induced HHcy.Exendin-4 treatment lowered superoxide anion accumulation in the mouse aortae both ex vivo and in vivo.Exendin-4 decreased the expression of ER stress markers(e.g.,ATF4,spliced XBP1,and phosphorylated eIF2α)in human umbilical vein endothelial cells(HUVECs),and this change was reversed by cotreatment with compound C(CC)(AMPK inhibitor).Exendin-4 induced phosphorylation of AMPK and endothelial nitric oxide synthase in HUVECs and arteries.Exendin-4 increased the expression of endoplasmic reticulum oxidoreductase(ERO1α),an important ER chaperone in endothelial cells,and this effect was mediated by AMPK activation.Experiments using siRNA-mediated knockdown or adenoviral overexpression revealed that ERO1αmediated the inhibitory effects of exendin-4 on ER stress and superoxide anion production,thus ameliorating HHcy-induced endothelial dysfunction.The present results demonstrate that exendin-4 reduces HHcy-induced ER stress and improves endothelial function through AMPK-dependent ERO1αupregulation in endothelial cells and arteries.AMPK activation promotes the protein folding machinery in endothelial cells to suppress ER stress.Chak Kwong Cheng Jiang-Yun Luo Chi Wai Lau William Chi-shing Cho Chi Fai Ng Ronald Ching Wan Ma Xiao Yu Tian Yu Huang 2021Acta Pharmacologica Sinica2021,42,10:6
8Advances in lithium niobate photonics:development status and perspectives显示文摘Lithium niobate(LN)has experienced significant developments during past decades due to its versatile properties,especially its large electro-optic(EO)coefficient.For example,bulk LN-based modulators with high speeds and a superior linearity are widely used in typical fiber-optic communication systems.However,with everincreasing demands for signal transmission capacity,the high power and large size of bulk LN-based devices pose great challenges,especially when one of its counterparts,integrated silicon photonics,has experienced dramatic developments in recent decades.Not long ago,high-quality thin-film LN on insulator(LNOI)became commercially available,which has paved the way for integrated LN photonics and opened a hot research area of LN photonics devices.LNOI allows a large refractive index contrast,thus light can be confined within a more compact structure.Together with other properties of LN,such as nonlinear/acousto-optic/pyroelectric effects,various kinds of high-performance integrated LN devices can be demonstrated.A comprehensive summary of advances in LN photonics is provided.As LN photonics has experienced several decades of development,our review includes some of the typical bulk LN devices as well as recently developed thin film LN devices.In this way,readers may be inspired by a complete picture of the evolution of this technology.We first introduce the basic material properties of LN and several key processing technologies for fabricating photonics devices.After that,various kinds of functional devices based on different effects are summarized.Finally,we give a short summary and perspective of LN photonics.We hope this review can give readers more insight into recent advances in LN photonics and contribute to the further development of LN related research.Guanyu Chen Nanxi Li Jun Da Ng Hong-Lin Lin Yanyan Zhou Yuan Hsing Fu Lennon Yao Ting Lee Yu Yu Ai-Qun Liu Aaron J.Danner 2022Advanced Photonics2022,4,3:6
9Hydrogen generation from ammonia electrolysis on bifunctional platinum nanocubes electrocatalysts显示文摘The ammonia electrolysis is a highly efficient and energy-saving method for ultra-pure hydrogen generation, which highly relies on electrocatalytic performance of electrocatalysts. In this work, high-quality platinum(Pt) nanocubes(Pt-NCs) with 4.5 nm size are achieved by facile hydrothermal synthesis. The physical morphology and structure of Pt-NCs are exhaustively characterized, revealing that Pt-NCs with special {100} facets have excellent uniformity, good dispersity and high crystallinity. Meanwhile, the electrocatalytic performance of Pt-NCs for ammonia electrolysis are carefully investigated in alkaline solutions, which display outstanding electroactivity and stability for both ammonia electrooxidation reaction(AEOR) and hydrogen evolution reaction(HER) in KOH solution. Furthermore, a symmetric Pt-NCs||Pt-NCs ammonia electrolyzer based on bifunctional Pt-NCs electrocatalyst is constructed, which only requires 0.68 V electrolysis voltage for hydrogen generation. 11Is there an association between Helicobacter pylori infection and irritable bowel syndrome? A meta-analysis显示文摘BACKGROUND Irritable bowel syndrome(IBS)is a prevalent and debilitating gastrointestinal condition.Research has reported persistent,low-grade mucosal inflammation and significant overlaps between patients with IBS and those with dyspepsia,suggesting a possible pathogenic role of Helicobacter pylori(H.pylori)in IBS.This study therefore aimed to provide the first systematic review and meta-analysis on the association between H.pylori infection and IBS.AIM To investigate the association between H.pylori infection and IBS.METHODS Using the keywords“H.pylori OR Helicobacter OR Helicobacter pylori OR infection”AND“irritable bowel syndrome OR IBS”,a preliminary search of PubMed,Medline,Embase,Cochrane Database of Systematic Reviews,Web of Science,Google Scholar and WanFang databases yielded 2924 papers published in English between 1 January 1960 and 1 June 2018.Attempts were also made to search grey literature.RESULTS A total of 13 clinical studies were systematically reviewed and nine studies were included in the final meta-analysis.Random-effects meta-analysis found a slight increased likelihood of H.pylori infection in patients with IBS,albeit this was not statistically significant(pooled odds ratio 1.47,95%confidence interval:0.90-2.40,P=0.123).It must also be acknowledged that all of the available studies reported only crude odd ratios.H.pylori eradication therapy also does not appear to improve IBS symptoms.Although publication bias was not observed in the funnel plot,there was a high degree of heterogeneity amongst the studies included in the meta-analysis(I2=87.38%).CONCLUSION Overall,current evidence does not support an association between IBS and H.pylori infection.Further rigorous and detailed studies with larger sample sizes and after H.pylori eradication therapy are warranted.Qin Xiang Ng Nadine Xinhui Foo Wayren Loke Yun Qing Koh Vanessa Jing Min Seah Alex Yu Sen Soh Wee Song Yeo 2019World Journal of Gastroenterology2019,25,37:5

15Escitalopram-induced liver injury: A case report and review of literature显示文摘BACKGROUND Depression is a growing public health problem that affects over 350 million people globally and accounts for approximately 7.5%of healthy years lost due to disability.Escitalopram,one of the first-line medications for the treatment of depression,is a selective serotonin reuptake inhibitor and one of the most commonly prescribed antidepressant medications worldwide.Although thought to be generally safe and with minimal drug-drug interactions,we herein present an unusual case of cholestatic liver injury,likely secondary to escitalopram initiation.CASE SUMMARY A 56-year-old Chinese lady presented with fever and cholestatic liver injury two weeks after initiation of escitalopram for the treatment of psychotic depression.Physical examination was unremarkable.Further investigations,including a computed tomography scan of the abdomen and pelvis and tests for hepatitis A,B and C and for autoimmune liver disease were unyielding.Hence,a diagnosis of escitalopram-induced liver injury was made.Upon stopping escitalopram,repeat liver function tests showed downtrending liver enzymes with eventual normalization of serum aspartate aminotransferase and alanine aminotransferase one-week post-discharge.CONCLUSION Clinicians should be aware of the possibility of escitalopram-induced liver injury when initiating depressed patients on antidepressant treatment.This requires extra vigilance as most patients may remain asymptomatic.Measurement of liver function tests could be considered after initiation of antidepressant treatment,especially in patients with pre-existing liver disease.Qin Xiang Ng Christl Suet Kwan Yong Wayren Loke Wee Song Yeo Alex Yu Sen Soh 2019World Journal of Hepatology2019,11,10:3

17Detection of Stage I nasopharyngeal carcinoma by serologicscreening and clinical examination显示文摘In a prospective study, 42 048 adults residing in Zhongshan City, Guangdong, China, were followed for 16 years, and 171 of them developed nasopharyngeal carcinoma (NPC). Although Epstein-Barr virus (EBV) antibody levels of the cohort fluctuated, the antibody levels of 93% of the patients with NPC were raised and maintained at high levels for up to 10 years prior to diagnosis. This suggests that the serologic window affords an opportunity to monitor tumor progression during the preclinical stage of NPC development, facilitating early NPC detection. We reviewed the clinical records of the 171 patients with NPC in the prospective study to assess the efficacy of early NPC detection by serologic screening and clinical examination. Of the 171 patients, 51 had Stage I tumor (44 were among the 73 patients detected by clinical examination and 7 were among the 98 patients presented to outpatient department). Initial serologic screening predicted 58 (95.1%) of the 61 patients detected within 2 years. The risk of the screened population (58/3093) raised 13 times relative to cohort (61/42 048) during this period. Clinical examination detected all the 58 predicted cases, and 35 (60.3%) of which were diagnosed with Stage I tumor. The serologic prediction rate fell to 33.6% (37/110) 2 to 16 years after screening. The proportion of cases detected by clinical examination fell to 40.5% (15/37). The proportion of Stage I tumors among the cases detected by clinical examination during both periods remained at about 60%. 19Room temperature Ⅲ–Ⅴ nanolasers with distributed Bragg reflectors epitaxially grown on(001) silicon-on-insulators显示文摘Efficient, scalable, bufferless, and compact Ⅲ–V lasers directly grown on(001)-oriented silicon-on-insulators(SOIs) are preferred light sources in Si-photonics. In this article, we present the design and operation of Ⅲ–V telecom nanolaser arrays with integrated distributed Bragg reflectors(DBRs) epitaxially grown on industry-standard(001) SOI wafers. We simulated the mirror reflectance of different guided modes under various mirror architectures, and accordingly devised nanoscale DBR gratings to support high reflectivity around1500 nm for the doughnut-shaped TE01 mode. Building from InP/InGaAs nanoridges grown on SOI, we fabricated subwavelength DBR mirrors at both ends of the nanoridge laser cavities and thus demonstrated room-temperature low-threshold InP/InGaAs nanolasers with a 0.28 μm^2 cross-section and a 20 μm effective cavity length. 20Hepatic venous outflow obstruction after piggyback liver transplantation by an unusual mechanism:Report of a case显示文摘Hepatic venous outflow obstruction after piggyback liver transplantation is a very rare complication. An unusual mechanism aggravating it is reported. A 33-year-old man with end-stage hepatitis B liver cirrhosis underwent a piggyback orthotopic liver transplantation using a full-size cadaveric graft. Two months after transplantation, he developed gross ascites refractory to maximal diuretic therapy. Doppler ultrasound showed patent portal and hepatic veins. Serial computed tomography scans revealed a hypoperfused right posterior segment of the liver which subsequently underwent atrophy. Hepatic venography demonstrated a high-grade stenosis with an element of torsion of venous drainage at the anastomosis. The stenosis was successfully treated with repeated percutaneous balloon angioplasty. The patient remained asymptomatic six months afterwards with complete resolution of ascites and peripheral edema. We postulate that liver allograft segmental hypoperfusion and atrophy may aggravate or result in a hepatic venous outflow problem by the mechanism of torsion effect.
