| 2 | Micro RNAs potential utility in colon cancer: Early detection, prognosis, and chemosensitivity显示文摘Over the past decade, research has shown that aberrant expression of micro RNA(mi RNA) is involved in colorectal cancer development and progression. Micro RNAs are small sequences of non-coding RNA that regulate expression of genes involved in important cellular functions, such as cell differentiation, multiplication, and apoptosis. A specific mi RNA may display the effects of a tumor suppressor or oncogene. Altered mi RNA expression is found in colorectal cancer(CRC) and patterns of mi RNA expression correlate with CRC detection and outcome. Studies also have examined the use of circulating serum mi RNA and fecal mi RNA expression as non-invasive markers for early detection. | 3 | Association between serum vitamin D levels and gastric cancer:A retrospective chart analysis显示文摘AIM To determine whether there is an increased risk of gastric adenocarcinoma associated with vitamin D deficiency(VDd).METHODS A retrospective case control study was performed of all patients diagnosed with gastric adenocarcinoma between 2005 and 2015.After we excluded the patients without a documented vitamin D level,49 patients were included in our study.RESULTS The average age of patients with gastric adenocarcinoma and documented vitamin D level was 64 years old(95%CI: 27-86) and average vitamin D level was 20.8 mg/d L(95%CI: 4-44).Compared to a matched control group,the prevalence of VDd/insufficiency in patients with gastric adenocarcinoma was significantly higher than normal vitamin D levels(83.7% vs 16.3%).Forty-one patients(83.7%) with adenocarcinoma showed VDd/insufficiency compared to 18(37%) patients with normal vitamin D level without gastric cancer(OR: 8.8,95%CI: 5-22,P value < 0.0001).The average age of males with gastric adenocarcinoma diagnosis was 60 years old vs 68 years old for females(P = 0.01).StageⅡ gastric adenocarcinoma was the most prevalent in our study(37%).CONCLUSION We reported a positive relationship between VDd and gastric adenocarcinoma,that is to say,patients with decreased VDd levels have an increased propensity for gastric adenocarcinoma. | Neil Vyas Rafael Ching Companioni Melik Tiba Hassan Alkhawam Carmine Catalano Robert Sogomonian Joel Baum Aaron Walfish | 2016 | World Journal of Gastrointestinal Oncology2016,8,9: | 10 |

The need to develop more chemotherapeutic agents to combat this disease is critical. Cyclin dependent kinases(CDKs), along with its binding partner cyclins, serve to control the growth of cells through the cell cycle. A new class of drugs, termed CDK inhibitors, has been studied in preclinical and now clinical trials. These inhibitors are believed to act as an anti-cancer drug by blocking CDKs to block the uncontrolled cellular proliferation that is hallmark of cancers like colorectal cancer. | 8 | Fecal transplant policy and legislation显示文摘Fecal microbiota transplantation(FMT) has garnered significant attention in recent years in the face of a reemerging Clostridium difficile(C.difficile) epidemic.Positive results from the first randomized control trial evaluating FMT have encouraged the medical community to explore the process further and expand its application beyond C.difficile infections and even the gastrointestinal domain.However promising and numerous the prospects of FMT appear,the method remains limited in scope today due to several important barriers,most notably a poorly defined federal regulatory policy.The Food and Drug Administrationhas found it difficult to standardize and regulate the administration of inherently variable,metabolically active,and ubiquitously available fecal material.The current cumbersome policy,which classifies human feces as a drug,has prevented physicians from providing FMT and deserving patients from accessing FMT in a timely fashion,and subsequent modifications seem only to be temporary.The argument for reclassifying fecal material as human tissue is well supported.Essentially,this would allow for a regulatory framework that is sufficiently flexible to expand access to care and facilitate research,but also appropriately restrictive and centralized to ensure patient safety.Such an approach can facilitate the advancement of FMT to a more refined,controlled,and aesthetic process,perhaps in the form of a customized and wellcharacterized stool substitute therapy. | Dinesh Vyas Apoorva Aekka Arpita Vyas | 2015 | World Journal of Gastroenterology2015,21,1: | 4 |
| 9 | Stool therapy may become a preferred treatment of recurrent Clostridium difficile?显示文摘Fecal enemas were first reported to successfully treat life threatening enterocolitis in 1958, but fecal therapy to treat Clostridium difficile (C. difficile ) infection has remained esoteric and not well investigated until recently. In the past few years, systematic reviews of case series and case reports of fecal microbiota transplant for recurrent C. difficile infection have become available and validate use of fecal transplant for C. difficile enterocolitis. Methods of fecal transplant reported in the literature include: nasogastric tube, gastroscope, duodenal tube, colonoscopy, rectal tube, and fecal enemas administered at home; no method has been shown to be superior. A recent randomized study published in New England Journal of Medicine found fecal transplant to be superior to oral vancomycin alone in treatment of recurrent C. difficile enterocolitis. | 17 | Screening colonoscopy:The present and the future显示文摘In the United States,colorectal cancer(CRC)is the second leading cause of mortality in men and women.We are now seeing an increasing number of patients with advanced-stage diagnosis and mortality from colorectal cancer before 50 years of age,which requires earlier screening.With the increasing need for CRC screening through colonoscopy,and thus endoscopists,easier and simpler techniques are needed to train proficient endoscopists.The most widely used approach by endoscopists is air insufflation colonoscopy,where air distends the colon to allow visualization of the colonic mucosa.This technique is uncomfortable for patients and requires an anesthetist to administer sedation.In addition,patients commonly complain about discomfort post-op as air escapes into the small bowel and cannot be adequately removed.Current research into the use of water insufflation colonoscopies has proved promising in reducing the need for sedation,decreasing discomfort,and increasing the visibility of the colonic mucosa.Future direction into water insufflation colonoscopies which have shown to be simpler and easier to teach may increase the number of proficient endoscopists in training to serve our aging population. | Chelsea V Hayman Dinesh Vyas | 2021 | World Journal of Gastroenterology2021,27,3: | 2 |
