| 1 | Modeling sustainability:population,inequality,consumption,and bidirectional coupling of the Earth and Human Systems显示文摘Over the last two centuries,the impact of the Human System has grown dramatically,becoming strongly dominant within the Earth System in many diferent ways.Consumption,inequality,and population have increased extremely fast,especially since about 1950,threatening to overwhelm the many critical functions and ecosystems of the Earth System.Changes in the Earth System,in turn,have important feedback efects on the Human System,with costly and potentially serious consequences.However,current models do not incorporate these critical feedbacks.We argue that in order to understand the dynamics of either system,Earth System Models must be coupled with Human System Models through bidirectional couplings representing the positive,negative,and delayed feedbacks that exist in the real systems.In particular,key Human System variables,such as demographics,inequality,economic growth,and migration,are not coupled with the Earth System but are instead driven by exogenous estimates,such as United Nations population projections.his makes current models likely to miss important feedbacks in the real Earth–Human system,especially those that may result in unexpected or counterintuitive outcomes,and thus requiring diferent policy interventions from current models.he importance and imminence of sustainability challenges,the dominant role of the Human System in the Earth System,and the essential roles the Earth System plays for the Human System,all call for collaboration of natural scientists,social scientists,and engineers in multidisciplinary research and modeling to develop coupled Earth–Human system models for devising efective science-based policies and measures to beneit current and future generations. | Safa Motesharrei Jorge Rivas Eugenia Kalnay Ghassem R.Asrar Antonio J.Busalacchi Robert F.Cahalan Mark A.Cane Rita R.Colwell Kuishuang Feng Rachel S.Franklin Klaus Hubacek Fernando Miralles-Wilhelm Takemasa Miyoshi Matthias Ruth Roald Sagdeev Adel Shirmohammadi Jagadish Shukla Jelena Srebric Victor M.Yakovenko Ning Zeng | 2016 | National Science Review2016,3,4: | 34 |
| 2 | Clinical impact of endoscopy position detecting unit(UPD-3) for a non-sedated colonoscopy显示文摘AIM:To evaluate whether an endoscopy position detecting unit(UPD-3) can improve cecal intubation rates, cecal intubation times and visual analog scale(VAS) pain scores, regardless of the colonoscopist's level of experience.METHODS:A total of 260 patients(170 men and 90women)who underwent a colonoscopy were divided into the UPD-3-guided group or the conventional group(no UPD-3 guidance).Colonoscopies were performed by experts(experience of more than 1000colonoscopies)or trainees(experience of less than 100colonoscopies).Cecal intubation rates,cecal intubation times,insertion methods(straight insertion:shortening the colonic fold through the bending technique;roping insertion:right turn shortening technique)and patient discomfort were assessed.Patient discomfort during the endoscope insertion was scored by the VAS that was divided into 6 degrees of pain.RESULTS:The cecum intubation rates,cecal intubation times,number of cecal intubations that were performed in<15 min and insertion methods were not significantly different between the conventional group and the UPD-3-guided group.The number of patients who experienced pain during the insertion was markedly less in the UPD-3-guided group than in the conventional group.Univariate and multivariate analysis showed that the following factors were associated with lower VAS pain scores during endoscope insertion:insertion method(straight insertion)and UPD-3guidance in the trainee group.For the experts group,univariate analysis showed that only the insertion method(straight insertion)was associated with lower VAS pain scores.CONCLUSION:Although UPD-3 guidance did not shorten intubation times,it resulted in less patient painduring endoscope insertion compared with conventional endoscopy for the procedures performed by trainees. | Masakatsu Fukuzawa Junichi Uematsu Shin Kono Sho Suzuki Takemasa Sato Naoko Yagi Yuichiro Tsuji Kenji Yagi Chika Kusano Takuji Gotoda Takashi Kawai Fuminori Moriyasu | 2015 | World Journal of Gastroenterology2015,21,16: | 5 |
| 3 | Endocytoscopic narrow-band imaging efficiency for evaluation of inflammatory activity in ulcerative colitis显示文摘AIM:To assess the efficacy of endocytoscopic narrowband imaging(EC-NBI)for evaluating the severity of inflammation in ulcerative colitis(UC).METHODS:This retrospective study was conducted at a single tertiary care referral center.We included UC patients who underwent colonoscopy with endocytoscopy from July 2010 to December 2013.ECNBI was performed,and the images were evaluated by assessing visibility,increased vascularization,and the increased calibers of capillaries and were classified as Obscure,Visible or Dilated.Obscure was indicative of inactive disease,while Visible and Dilated were indicative of acute inflammation.This study received Institutional Review Board approval.The primary outcome measures included the diagnostic ability of EC-NBI to distinguish between active and inactive UC on the basis of histological activity.The conventional endoscopic images were classified according to the Mayo endoscopic score.A score of 0 or 1 indicated inactive disease,whereas a score of 2 indicated active disease.RESULTS:Fifty-two patients were enrolled.There was a strong correlation between the EC-NBI findings and the histological assessment(r=0.871,P<0.01).The sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of EC-NBI for diagnosing acute inflammation were 84.0%,100%,87.1%,100%,and 92.3%,respectively,while those for the Mayo endoscopic score were 100%,40.7%,100%,61.0%,and 69.2%,respectively.Compared with conventional endoscopy,EC-NBI was superior in diagnostic specificity,negative predictive value,and accuracy(P<0.001,P=0.001 and P=0.047,respectively).CONCLUSION:The EC-NBI finding of capillaries in the rectal mucosa was strongly correlated with histological inflammation and aided in the differential diagnosis between active and inactive UC. | Yasuharu Maeda Kazuo Ohtsuka Shin-ei Kudo Kunihiko Wakamura Yuichi Mori Noriyuki Ogata Yoshiki Wada Masashi Misawa Akihiro Yamauchi Seiko Hayashi Toyoki Kudo Takemasa Hayashi Hideyuki Miyachi Fuyuhiko Yamamura Fumio Ishida Haruhiro Inoue Shigeharu Hamatani | 2015 | World Journal of Gastroenterology2015,21,7: | 4 |
