| 1 | 进展期胃癌行腹腔镜与开腹D2远端胃癌根治术的并发症发生率和死亡率的随机对照研究显示文摘目的腹腔镜根治性远端胃切除(radical laparoscopic distal gastrectomy,LG)+D2淋巴结清扫术用于治疗进展期胃癌其安全性和有效性仍存在争议。本研究设计了一个前瞻性随机对照研究来对比腹腔镜手术和传统开腹手术行根治性远端胃切除术+D2淋巴结清扫术在治疗进展期胃癌的安全性和疗效。研究对象和方法自2012年9月至2014年12月,1056例临床分期为T(2-4a)N0-3M0的胃癌患者被纳入本研究。患者被随机分为腹腔镜根治性远端胃切除术+D2淋巴清扫组(LG组)(n=528)和开腹远端胃切除术(open gastrectomy,OG)+D2淋巴清扫组(OG组)(n=528)。共有15位来自中国14个研究中心的经验丰富的外科医生参与此次研究。根据意向性治疗分析原则比较LG组(n=519)和OG组(n=520)手术后30天内的并发症发生率和死亡率。术后并发症严重程度分级根据Clavien-Dindo分级方法。结果 LG组和OG组患者的D2淋巴结清扫术的合格率相似(99.4%vs.99.6%;P=0.845)。术后并发症发生率在LG组为15.2%,OG组为12.9%,统计学无明显差异(相差2.3%,95%CI:-1.9-6.6,P=0.285)。死亡率在LG组为0.4%,而OG组无一例死亡(相差0.4%,95%CI:-0.4-1.4,P=0.249)。两组并发症的严重程度分布相似(P=0.314)。结论有经验的外科医生可以在进展期胃癌患者上安全开展腹腔镜下根治性远端胃切除术+D2淋巴结清扫。 | Hu Y Huang C Sun Y 戴伟钢 | 2016 | 消化肿瘤杂志(电子版)2016,8,3: | 202 |
| 2 | Estrogen enhances the functions of CD4^(+)CD25^(+)Foxp3^(+) regulatory T cells that suppress osteoclast differentiation and bone resorption in vitro显示文摘Cross-talk has been shown to occur between the immune system and bone metabolism pathways.In the present study,we investigated the impact of CD4^(+)CD25^(+)Foxp3^(+) regulatory T(Treg)cells on osteoclastogenesis and bone resorption.Treg cells that were isolated and purified from peripheral blood mononuclear cells(PBMCs)of healthy adults inhibited both the differentiation of osteoclasts(OCs)from human embryo bone marrow cells(BMCs)and the pit formation in a dose-dependent manner.In cell cocultures,the production levels of both interleukin-10(IL-10)and transforming growth factor-beta 1(TGF-β1)were proportionally upregulated as the ratio of Treg cells to BMCs was increased,and the inhibition of OC differentiation and bone resorption by Treg cells was completely reversed by anti-IL-10 and anti-TGF-β1 antibodies.Treatment of BMC and Treg cell cocultures with 17β-estradiol(E2)at concentrations between 10^(-7) and 10^(-9) mol/l suppressed OC differentiation and bone resorption more efficiently than it did in cultures of BMCs alone;this enhanced suppression occurred via the stimulation of Treg cell IL-10 and TGF-b1 expression.These data suggest that Treg cells suppress OC differentiation and bone resorption by secreting IL-10 and TGF-β1.E2 enhances the suppressive effects of Treg cells on OC differentiation and bone resorption by stimulating IL-10 and TGF-β1 secretion from these cells.Therefore,Treg cell-derived IL-10 and TGF-b1 are likely involved in the regulation of E2 on bone metabolism and represent potential therapeutic targets for the treatment of postmenopausal osteoporosis(PMO). | CY Luo L Wang C Sun DJ Li | 2011 | Cellular & Molecular Immunology2011,8,1: | 33 |

| 10 | Rational use of mesenchymal stem cells in the treatment of autism spectrum disorders显示文摘Autism and autism spectrum disorders(ASD) refer to a range of conditions characterized by impaired social and communication skills and repetitive behaviors caused by different combinations of genetic and environmental influences. Although the pathophysiology underlying ASD is still unclear, recent evidence suggests that immune dysregulation and neuroinflammation play a role in the etiology of ASD. In particular, there is direct evidence supporting a role for maternal immune activation during prenatal life in neurodevelopmental conditions. Currently, the available options of behavioral therapies and pharmacological and supportive nutritional treatments in ASD are only symptomatic. Given the disturbing rise in the incidence of ASD, and the fact that there is no effective pharmacological therapy for ASD, there is an urgent need for new therapeutic options. Mesenchymal stem cells(MSCs) possess immunomodulatory properties that make them relevant to several diseases associated with inflammation and tissue damage. The paracrine regenerative mechanisms of MSCs are also suggested to be therapeutically beneficial for ASD.Thus the underlying pathology in ASD, including immune system dysregulation and inflammation, represent potential targets for MSC therapy. | 11 | 笋芽顶端分生组织在竹秆横切面形态形成中的作用与机制研究显示文摘竹秆的横切面主要由表皮、皮层、维管组织、基本组织及髓腔(早期为髓组织)组成。这些组织均由笋芽茎顶端分生组织分化发育而来,且其在生长发育过程中促使了竹秆横切面形态(本文特指:竹秆横切面外轮廓形状、围度及壁-腔结构)的形成。然而,人们对于这一生长发育过程的生物学机制知之甚少。本研究利用解剖学、数学模拟等方法,对 20 种具不同秆径竹种的笋芽顶端分生组织进行了分析。研究结果发现,笋芽顶端分生组织指状形态的维持是竹子产生圆柱形竹秆的基础;在顶端分生组织细胞数目增加的同时,线性增加原体细胞数目应是竹子在演化过程中产生大径竹秆且保持规则壁-腔结构的方式。进一步通过研究一个竹壁增厚且壁-腔架构紊乱的稳定变异体———实肚竹( Phyllostachys nidularia f. farcta)发现,其笋芽顶端分生组织细胞数目减少且原体/原套细胞数比值降低使其产生了高比例的壁组分组织(指基本分生组织与肋状分生组织)与低比例的髓组织。上述 2 种组织的比例失衡,使得实肚竹笋芽在后续发育中壁组分组织随机挤入到了髓组织中,最终形成了实肚竹异常的竹秆横切面形态(竹壁多样化增厚且壁-腔结构紊乱)。此外,相对较小的顶端分生组织,使得实肚竹笋芽产生了较少的内源激素,如其生长素含量仅约为其原种篌竹的 67. 5%,各细胞分裂素含量也显著低于篌竹笋芽。转录组分析进一步发现,在实肚竹笋芽中与激素及代谢相关的功能基因呈显著下调表达。这些生理与分子变化最终导致了实肚竹竹秆矮化、秆径减小且生物量降低。本论文的研究结果为研究竹秆横切面发育提供了一个重要的视角,同时也为实肚竹厚壁茎秆的形成提出了一个合理的机制。 | Wang Y Sun X Ding Y Fei Z Jiao C Fan M Yao B Xin P Chu J Wei Q 王雨珺 魏强 | 2019 | 世界竹藤通讯2019,17,3: | 5 |

| 16 | Gynecologic oncologists involvement on ovarian cancer standard of care receipt and survival显示文摘AIM:To examine the influence of gynecologic oncolo-gists(GO) in the United States on surgical/chemotherapeutic standard of care(SOC), and how this translates into improved survival among women with ovarian cancer(OC).METHODS: Surveillance, Epidemiology, and End Result(SEER)-Medicare data were used to identify 11688 OC patients(1992-2006). Only Medicare recipients with an initial surgical procedure code(n = 6714) were included. Physician specialty was identified by linking SEER-Medicare to the American Medical Association Masterfile. SOC was defined by a panel of GOs. Multivariate logistic regression was used to determine predictors of receiving surgical/chemotherapeutic SOCand proportional hazards modeling to estimate the effect of SOC treatment and physician specialty on survival. RESULTS: About 34% received surgery from a GO and 25% received the overall SOC. One-third of women had a GO involved sometime during their care. Women receiving surgery from a GO vs non-GO had 2.35 times the odds of receiving the surgical SOC and 1.25 times the odds of receiving chemotherapeutic SOC(P < 0.01). Risk of mortality was greater among women not receiving surgical SOC compared to those who did [hazard ratio = 1.22(95%CI: 1.12-1.33), P < 0.01], and also was higher among women seen by non-GOs vs GOs(for surgical treatment) after adjusting for covariates. Median survival time was 14 mo longer for women receiving combined SOC. CONCLUSION: A survival advantage associated with receiving surgical SOC and overall treatment by a GO is supported.
