| 1 | Thrombocytopenia after liver transplantation:should we care?显示文摘Transient thrombocytopenia is a common phenomenon after liver transplantation. After liver transplantation(LT), platelet count decreases and reaches a nadir on postoperative days 3-5, with an average reduction in platelet counts of 60%; platelet count recovers to preoperative levels approximately two weeks after LT. The putative mechanisms include haemodilution, decreased platelet production, increased sequestration, medications, infections, thrombosis, or combination of these processes. However, the precise mechanisms remain unclear. The role of platelets in liver transplantation has been highlighted in recent years, and particular attention has been given to their effects beyond hemostasis and thrombosis. Previous studies have demonstrated that perioperative thrombocytopenia causes poor graft regeneration, increases the incidence of postoperative morbidity, and deteriorates the graft and decreases patient survival in both the short and long term after liver transplantation. Platelet therapies to increase perioperative platelet counts, such as thrombopoietin, thrombopoietin receptor agonist, platelet transfusion, splenectomy, and intravenous immunoglobulin treatment might have a potential for improving graft survival, however clinical trials are lacking. | 2 | Extrahepatic metastasis of hepatocellular carcinoma to the paravertebral muscle:A case report显示文摘Identification of extrahepatic metastases(EHM) of hepatocellular carcinoma(HCC) has been paradoxically increasing due to an increase in the survival of HCC patients. However, metastasis of HCC to the skeletal muscle tissue is extremely rare. We describe a unique case of HCC metastasizing to the paravertebral muscle. A 55-year-old man with a history of hepatitis B cirrhosis underwent partial liver resection with complete removal of HCC. Three months later, a computed tomography(CT) scan showed intrahepatic recurrence. The tumors were treated with yttrium-90 microspheres, transcatheter arterial chemoembolization, and sorafenib. Six months later, a CT scan showed an enhancing lesion of the left paravertebral muscle that on biopsy were consistent with metastatic HCC. The tumor was treated with stereotactic hypo-fractionated imageguided radiation therapy(SHFRT). A follow-up scan 3 mo post-radiotherapy revealed a stable appearance of the paravertebral muscle metastasis. Because of the progression in the intrahepatic tumors, the patient was treated with capecitabine, which was changed to dasatinib 6 mo later. The patient passed away three years after the primary surgical resection. Management of EHM poses an extreme challenge. This is the first case of HCC with EHM to the paravertebral muscle in which stability of disease was achieved using SHFRT. | 3 | Preliminary Study on Tensile and Impact Properties of Kenaf/Bamboo Fiber Reinforced Epoxy Composites显示文摘The application of natural fibers as reinforcement in composite material has increased due to environmental concerns,low cost,degradability and health concerns.The purpose of this study is to identify the best type of bamboo fibers to be used as reinforcement for kenaf(K)/bamboo hybrid composite.There were three types of bamboo fibers evaluated in this study which include bamboo mat(B),bamboo fabric(BF)and bamboo powder(BP).Chemical composition of B,BF,BP and K fibers were analyzed in this study.The effect of different types of bamboo fibers on tensile,impact,and morphological properties were investigated.The B/epoxy composites displayed the highest tensile strength(53.03 MPa)while K/epoxy composite had the highest tensile modulus(4.71 GPa).Scanning electron micrographs of B/epoxy composites displayed better fiber/matrix interfacial bonding in comparison to other studied composites.Results showed that impact strength of BF-based composite was highest(45.70 J/m).In conclusion,the tensile strength of B/epoxy composite is superior to the other bamboo reinforced composites and will be further evaluated in the next study. | Ahmad Safwan Mohammad Jawaid Mohamed T.H.Sultan Azman Hassan | 2018 | Journal of Renewable Materials2018,6,5: | 2 |

| 17 | Wound dehiscence with continuous versus interrupted mass closure of transverse incisions in children with absorbable suture:a randomized controlled trial显示文摘Introduction Numerous meta-analyses done on adults suggest superiority of continuous mass closure technique,but any such study does not exist for the pediatric age group.The results in adults cannot be applied to pediatrics because of numerous physiologic and anatomic differences.Methods This is a single-blinded,randomized controlled trial,1:1 parallel groups,that compares the frequency of dehiscence between the interrupted and continuous mass closure techniques for transverse incisions in pediatric patients.The age range was from birth to 12 years.We sampled 350 patients undergoing emergency or elective exploratory laparotomies in our pediatric surgery unit.Blocked randomization was used and only the patients remained blinded during the intervention.One group was closed with interrupted mass closure(group A)and the other group with continuous mass closure technique(group B).We had to drop 50 patients for not meeting the inclusion criteria.results The wound dehiscence rate for group A was 1.34%(4 patients)and for group B was 3.0%(9 patients).Significance was calculated usingχ^(2)(p<0.156).The global wound dehiscence rate was 4.34%(13 patients).The maximum number of patients dehisced on the fifth postoperative day,while the range was 4-11 days.The only statistically significant confounding factor was wound classification(p<0.002).Discussion Statistically there is no significant difference between interrupted and continuous mass closures techniques in terms of wound dehiscence.The dirty wounds are at a maximum risk of developing wound dehiscence irrespective of the technique used.We need to strictly adhere to the basic principles of closure especially when dealing with dirty wounds.trial registration number TCTR20150318001. | Safwan Khan Muhammad Saleem Nabila Talat | 2019 | World Journal of Pediatric Surgery2019,2,2: | 0 |

| 19 | Dynamic ^(18)F-fluorodeoxyglucose positron emission tomography/CT in hibernoma: enhanced tracer uptake mimicking liposarcoma显示文摘We report on two cases of patients with fat-equivalent masses in computed tomography(CT),referred to our department for dynamic positron emission tomography/CT(dPET/CT)with18F-fluorodeoxyglucose(18FFDG)in order to investigate their dignity.Both qualitative and quantitative information,as derived from dPET/CTs,couldn’t exclude a high-grade liposarcoma:Visual evaluation,revealed a large hypermetabolic focus of intense18F-FDG uptake in each patient(average SUVs 8.3 and 11.3).Regression-based parametric imaging demonstrated an enhanced distribution volume,which correlates to perfusion,and a high phosphorylation rate that correlates to cell viability.Kinetic analysis,based on a two-tissue compartment model demonstrated an enhanced FDG transport k1and an enhanced phosphorylation rate k3.A non-compartmental approach based on fractal dimension revealed also enhanced values.However,final diagnosis was based on biopsy,which revealed hibernoma,a benign brown fat tumor.Brown adipose contains increased numbers of mitochondria and a high-rate of glucose metabolism.Therefore,they have increased FDG uptake.The evaluation of lipomatous lesions on CT,with high FDG uptake,should include the possibility of hibernoma as a differential diagnosis. | Christos Sachpekidis Safwan Roumia Matthias Schwarzbach Antonia Dimitrakopoulou-Strauss | 2013 | World Journal of Radiology2013,5,12: | 0 |
| 20 | Rabi oscillation generation in the microring resonator system with double-series ring resonators显示文摘In this paper, a microring resonator(MRR) system using double-series ring resonators is proposed to generate and investigate the Rabi oscillations. The system is made up of silicon-on-insulator and attached to bus waveguide which is used as propagation and oscillation medium. The scattering matrix method is employed to determine the output signal intensity which acts as the input source between two-level Rabi oscillation states, where the increase of Rabi oscillation frequency with time is obtained at the resonant state. The population probability of the excited state is higher and unstable at the optical resonant state due to the nonlinear spontaneous emission process.
