1Clinical features,histology,and histogenesis of combined hepatocellular-cholangiocarcinoma显示文摘Combined hepatocellular-cholangiocarcinoma(CHC) is a rare tumor with poor prognosis,with incidence ranging from 1.0%-4.7% of all primary hepatic tumors.This entity will be soon renamed as hepato-cholangiocarcinoma.The known risk factors for hepatocellular carcinoma(HCC) have been implicated for CHC including viral hepatitis and cirrhosis.It is difficult to diagnose this tumor pre-operatively.The predominant histologic component within the tumor largely determines the predominant radiographic features making it a difficult distinction.Heterogeneous and overlapping imaging features of HCC and cholangiocarcinoma should raise the suspicion for CHC and multiple core biopsies(from different areas of tumor) are recommended before administering treatment.Serum tumor markers CA19-9 and alpha-fetoprotein can aid in the diagnosis,but it remains a challenging diagnosis prior to resection.There is sufficient data to support bipotent hepatic progenitor cells as the cell of origin for CHC.The current World Health Organization classification categorizes two main types of CHC based on histo-morphological features:Classical type and CHC with stem cell features.Liver transplant is one of the available treatment modalities with other management options including transarterial chemoembolization,radiofrequency ablation,and percutaneous ethanol injection.We present a review paper on CHC highlighting the risk factors,origin,histological classification and therapeutic modalities.Shweta Gera Mark Ettel Gabriel Acosta-Gonzalez Ruliang Xu 2017World Journal of Hepatology2017,9,6:13
2Utility of flexible fiberoptic bronchoscopy for critically ill pediatric patients:A systematic review显示文摘AIM: To investigate the diagnostic yield, therapeutic efficacy, and rate of adverse events related to flexible fiberoptic bronchoscopy(FFB) in critically ill children. METHODS: We searched Pub Med, SCOPUS, OVID, and EMBASE databases through July 2014 for English language publications studying FFB performed in the intensive care unit in children < 18 years old. We identified 666 studies, of which 89 full-text studies were screened for further review. Two reviewers independently determined that 27 of these studies met inclusion criteria and extracted data. We examined the diagnostic yield of FFB among upper and lower airway evaluations, as well as the utility of bronchoalveolar lavage(BAL). RESULTS: We found that FFB led to a change in medical management in 28.9%(range 21.9%-69.2%) of critically ill children. The diagnostic yield of FFB was 82%(range 45.2%-100%). Infectious organisms were identified in 25.7%(17.6%-75%) of BALs performed, resulting in a change of antimicrobial management in 19.1%(range: 12.2%-75%). FFB successfully reexpanded atelectasis or removed mucus plugs in 60.3%(range: 23.8%-100%) of patients with atelectasis. Adverse events were reported in 12.9%(range: 0.5%-71.4%) of patients. The most common adverse effects of FFB were transient hypotension, hypoxia and/or bradycardia that resolved with minimal intervention, such as oxygen supplementation or removal of the bronchoscope. Serious adverse events were uncommon; 2.1% of adverse events required intervention such as bag-mask ventilation or intubation and atropine for hypoxia and bradycardia, normal saline boluses for hypotension, or lavage and suctioning for hemorrhage. 3Could the number of CT angiograms be reduced in emergency department patients suspected of pulmonary embolism?显示文摘BACKGROUND:This study was undertaken to identify the prevalence of pulmonary embolism(PE) in the emergency department(ED) of an urban teaching hospital and also to test a Bayesian model in estimating the number of CT pulmonary angiography(CTA) expected to be performed in an emergency department.METHODS:The data for this study was obtained through a retrospective review of electronic medical records for all ED patients suspected of PE who underwent chest CTA or ventilation perfusion scanning(V/Q) between 2009 and 2010.The data is presented as means and standard deviation for continuous variables and percentages with 95%confidence intervals(95%CI) for proportions.The prevalence of PE was used as pre-test probability in the Bayesian model.Post-test probability was obtained using a Fagan nomogram and likelihood ratios for CTA.RESULTS:A total of 778 patients(560 females) with mean age of 50 years(range 18-98 years)were enrolled(98.3%underwent chest CTA and 1.7%underwent V/Q scan).A total of 69 patients had PE,rendering an overall prevalence of 8.9%(95%CI,7.1%to 11.1%) for PE.We calculated that132 CTA's per year could be avoided in our institution,without compromising safe exclusions of PE(keeping post-test probability of PE below 2%).CONCLUSIONS:Despite differences in our patient populations and /or study designs,the prevalence of PE in our institution is about average compared to other institutions.Our proposed model for calculating redundant chest CTA is simple and can be used by institutions to identify overuse of CTA.Zehtabchi Shahriar Rinnert Stephan Malhotra Shweta Subramanian Arun Timberger Mathew Patel Brijal Toro David Hassan Khaled Sinert Richard 2012World Journal of Emergency Medicine2012,3,3:7
4Genetic polymorphism in CD14 gene, a co-receptor of TLR4 associated with non-alcoholic fatty liver disease显示文摘AIM To evaluate the pathogenic role of toll-like receptor(TLR) gene polymorphisms in patients with nonalcoholic fatty liver disease(NAFLD).METHODS Two hundred and fifty subjects(NAFLD = 200, healthy volunteers = 50) underwent polymerase chain reaction and restriction fragment length polymorphism to assess one polymorphism in the toll-like receptor 2(TLR2) gene(A753G), two polymorphisms in the TLR4 gene(TLR4 Asp299 Gly and Thr399 Ile allele), and two polymorphisms in the cluster of differentiation 14(CD14)(C-159 T and C-550T) gene, a co-receptor of TLR4. Association of TLR gene polymorphisms with NAFLD and its severity was evaluated by genetic models of association.RESULTS On both multiplicative and recessive models of gene polymorphism association, there was significant association of CD14 C(-159) T polymorphism with NAFLD; patients with TT genotype had a 2.6 fold increased risk of developing NAFLD in comparison to CC genotype. There was no association of TLR2 Arg753 Gln, TLR4 Asp299 Gly, Thr399 Ile, and CD14 C(-550) T polymorphisms with NAFLD. 6Repetitive Sequences in Plant Nuclear DNA:Types, Distribution, Evolution and Function显示文摘Repetitive DNA sequences are a major component of eukaryotic genomes and may account for up to 90% of the genome size. They can be divided into minisatellite, microsatellite and satellite sequences. Satellite DNA sequences are considered to be a fast-evolving component of eukaryotic genomes, comprising tandemly-arrayed, highly-repetitive and highly-conserved monomer sequences. The monomer unit of satellite DNA is 150–400 base pairs(bp) in length.Repetitive sequences may be species- or genus-specific, and may be centromeric or subtelomeric in nature. They exhibit cohesive and concerted evolution caused by molecular drive, leading to high sequence homogeneity. Repetitive sequences accumulate variations in sequence and copy number during evolution, hence they are important tools for taxonomic and phylogenetic studies, and are known as ‘‘tuning knobs' ' in the evolution. Therefore, knowledge of repetitive sequences assists our understanding of the organization, evolution and behavior of eukaryotic genomes. Repetitive sequences have cytoplasmic, cellular and developmental effects and play a role in chromosomal recombination. 9Self-lubricating bidirectional carbon fiber reinforced smart aluminum composites by squeeze infiltration process显示文摘Self-lubrication is one of the smart material properties required for producing components with enhanced wear resistance and low coefficient of friction.Bidirectional(BD)satin weave polyacrylonitrile(PAN)based carbon fiber(Cf)fabric preform was successfully infiltrated with Al 6061 alloy by squeeze infiltration process.The infiltrated composite shows uniform distribution of carbon fibers in the matrix with the elimination of porosities,fiber damage and close control on the formation of deleterious aluminum carbide(Al4C3)phase.Cf/Al composite exhibits remarkable wear resistance compared to unreinforced alloy due to the formation of self-lubricating tribolayer on the pin surface,which intercepts the contact of matrix metal to counter surface.The BD carbon fiber enhanced the hardness and compressive strength of the composite by restraining the plastic flow behavior of matrix.High resolution transmission electron microscopy shows the presence of Al2O3 and MgAl2O4 spinel,confirmed by EDS and SAD pattern,at the composite interface.The composite shows a lower density of 2.16 g/cm^3 which is a major ad vantage for weight reduction compared to the monolithic alloy(2.7 g/cm^3).K.M.Sree Manu L.Ajay Raag T.P.D.Rajan B.C.Pai Vijay Petley Shweta Namdeo Verma 2019Journal of Materials Science & Technology2019,35,11:4

12Luminescence properties of near-UV excitable yellow-orange light emitting warm CaSrAl_2SiO_7:Sm^(3+) phosphors显示文摘Single-phase CaSrAl_2 SiO_7:Sm^(3+) phosphors were synthesized by traditional high temperature solid state reaction method. Formation of samples and phase analysis were confirmed by X-ray diffraction technique. Morphology was done by field emission scanning electron microscopy and elemental compositions were confirmed by energy dispersive X-ray analysis. Present phosphors have tetragonal crystallography with space group P42_1 m. Average crystallite size was calculated by using Scherrer and Williamson-Hall method. Photoluminescence study of CaSrAl_2 SiO_7:Sm^(3+) phosphor was investigated.Under different excitation wavelengths, PL spectra consist of four emission bands at 564, 570, 601 and650 nm. The emission bands located at 564 and 570 nm are associated with the transition ~4 G_(5/2)→~6 H_(5/2)while emission bands at 601 and 650 nm are due to ~4 G_(5/2)→~6 H_(7/2) and ~4 G_(5/2)→~6 H_(9/2),respectively.Intense emission was obtained when phosphor was excited under 404 nm wavelength. Non-radiative energy transfer process involved in concentration quenching, was also discussed. CIE coordinate is found in yellow-orange region, hence CaSrAl_2SiO_7:Sm^(3+) phosphors emit yellow-orange light when efficiently excited by near UV(~400 nm) LED chip. Color purity and CCT of the phosphor were determined; CCT suggests that present phosphor is a good candidate as a warm yellow-orange color emitting phosphor. Effect of different heating rates and different UV exposure time on the TL glow curve of the phosphor was investigated. Activation energies and kinetic parameters for different traps were calculated by using peak shape method. TL emission spectrum was also recorded. 13Strategic planning and designing of a hospital disaster manual in a tertiary care, teaching, research and referral institute in India显示文摘BACKGROUND: As per the 'Disaster Management Act, 2005' of India, it is mandatory for government hospitals in India to prepare a disaster plan. This study aimed to prepare a disaster manual of a 1 900 bed tertiary care hospital, in consultation and involvement of all concerned stakeholders.METHODS: A committee of members from hospital administration, clinical, diagnostic and supportive departments worked on an initial document prepared according to the Act and gave their inputs to frame a final disaster manual.RESULTS: The prepared departmental standard operating procedures involved 116 people(doctors and paramedical staff), and were then synchronized, in 12 committee meetings, to produce the final hospital disaster manual.CONCLUSIONS: The present disaster manual is one of the few comprehensive plans prepared by the stakeholders of a government hospital in India, who themselves form a part of the disaster response team. 14Analysis of structural,optical and electrical properties of metal/p-ZnO-based Schottky diode显示文摘A systematic study of the behaviour of Pd/p-ZnO thin film Schottky diode has been reported.The p-type ZnO thin film with improved stability has been grown on n-type Si by doping ZnO with copper.Seebeck measurement confirmed the p-type nature of Cu-doped ZnO thin film.The X-ray diffraction spectra of the deposited film revealed polycrystalline nature with preferred growth orientation of (101) of ZnO film.The surface morphological study demonstrated the conformal deposition of a thin film over n-Si wafer.The estimated bandgap of Cu-doped ptype ZnO thin film from ellipsometric measurement turns out to be 3.14 eV at 300 K.The measured electrical parameters of the proposed Pd/p-ZnO Schottky diode have also been validated by the results of numerical simulation obtained by using ATLAS^(TM) device simulator.Lucky Agarwal Shweta Tripathi P.Chakrabarti 2017Journal of Semiconductors2017,38,10:3
15Antioxidant and hepatoprotective effects of Boswellia ovalifoliolata bark extracts显示文摘Paracetamol(PCM) hepatotoxicity is related to reactive oxygen species(ROS) formation and excessive oxidative stress; natural antioxidant compounds have been tested as an alternative therapy. This study evaluated the hepatoprotective activity of an alcoholic extract of Boswellia ovalifoliolata(BO) bark against PCM-induced hepatotoxicity. BO extract also demonstrated antioxidant activity in vitro, as well as scavenger activity against 2, 2-diphenyl-1-picrylhydrazyl. Administration of PCM caused a significant increase in the release of transaminases, alkaline phosphatase, and lactate dehydrogenase in serum. Significant enhancement in hepatic lipid peroxidation and marked depletion in reduced glutathione were observed after parac intoxication with severe alterations in liver histology. BO treatment was able to mitigate hepatic damage induced by acute intoxication of PCM and showed a pronounced protective effect against lipid peroxidation, deviated serum enzymatic variables, and maintained glutathione status toward control. 17Evaluation of functional outcome and stability of sutureless scleral tunnel fixated IOLs in children with ectopia lentis显示文摘AIM:To evaluate functional outcome of sutureless scleral tunnel intraocular lens(SSTIOL)in children with crystalline lens subluxation of more than 7 clock hours.METHODS:A prospective interventional study was conducted consisting of 45 eyes of 44 children in age group 6-18 y having>7 clock hours of lens subluxation who underwent lensectomy-vitrectomy followed by SSTIOL implantation.Primary outcome was improvement in best corrected visual acuity(BCVA)and secondary outcomes were assessment of intraocular lens(IOL)tilt using ultrasound biomicroscopy(UBM),mean change in astigmatism at last follow-up of 1 y and associated complications.RESULTS:The mean preoperative and postoperative BCVA was 1.05±0.28 and 0.64±0.45(log MAR)respectively(P=0.001)at last follow-up.The mean astigmatism preoperatively and postoperatively was-4.17±2.69 D and-1.86±1.25 D respectively(P=0.011).Significant IOL tilt(>5 degrees)was present in 5 cases.The mean percentage endothelial loss was 3.65%±1.92%.The most serious complication encountered was retinal detachment seen in 2 cases.CONCLUSION:SSTIOL implantation provides efficient visual rehabilitation in children provided there is stringent case selection.We recommend caution in children having white-to-white distance>12 mm and presence of peripheral retinal degenerations.Anju Rastogi Prateek Kumar Shweta Dhiman Manisha Mishra Kamlesh AnAND Ankita Bhardwaj 2020International Journal of Ophthalmology(English edition)2020,13,1:2

19Organic acids production from lactic acid bacteria:A preservation approach显示文摘The lactic acid bacteria(LAB)are a group of gram-positive bacteria capable of converting carbohydrate substrates into organic acids and producing a wide range of metabolites.Organic acids,including propionic,formic,acetic acid,and lactic acid,create an unfavorable environment for the growth of spoilage and pathogenic microorganisms.Also,they are employed as monomers or starting ingredients for food-based supplements,biomaterials,and biodegradable polymers.Organic acids from LAB have the potential to prevent spoilage and improve food taste that enhances consumer acceptance and appeal.Organic acids can be useful as starting materials in multiple applications,including food,chemical,cosmetic,pharmaceutical,and beverage industries owing to their various functional properties.Additionally,the use of LAB for organic acids production can replace petroleum-based processes due to their ability to utilize inexpensive and renewable feedstocks.This article aims to provide the potential of LAB as a biological catalyst for producing different organic acids.Also,the current review will give a critical snapshot of the strategies to improve microbial organic acid production and potential application of organic acids and discuss new challenges and opportunities for developing novel economically effective and sustainable nutritional sources and techniques.Sneh Punia Bangar Shweta Suri Monica Trif Fatih Ozogul 2022Food Bioscience2022,46,2:2
20新型N-(2-芳基氨基苯基)-2,3-二苯基喹喔啉-6-磺胺类药物作为潜在抗疟疾、抗真菌和抗细菌药物的合理药物设计、合成和生物学评价显示文摘目的磺胺、磺胺嘧啶和氨苯砜是第一批通过破坏叶酸生物合成过程来治疗疟疾的磺胺类药物,叶酸生物合成过程是寄生虫存活的关键。因此,我们旨在通过合理的药物设计方法合成新型N-(2-芳基氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺衍生物。方法按常规方法合成所有化合物,并用光谱分析(1H核磁共振和质谱)对产物进行表征。使用薄层色谱法(TLC)监测反应进程。分析了所有衍生物在疟原虫半胱氨酸蛋白酶falcipain-2变构位点的有效结合模式。采用肉汤稀释法测定抗菌和抗真菌活性。结果S6(N-(2-噻唑-4-基)-乙酰基-氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺与S9(N-(1H-苯并[d]咪唑-2-基)氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺形成5个氢键;S8(N-(2-1H-咪唑-2-基)氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺和S10(N-(1-苯并[d]咪唑-5-基)氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺与酶的变构位点形成四个氢键。考虑到与目标酶的对接分数和氢键的形成,新的衍生物被加工用于湿实验室合成。所有新合成的衍生物都具有体外抗疟疾、抗真菌和抗菌活性。与标准品相比,所有衍生物对恶性疟原虫株表现出足够的敏感性。此外,化合物S9和S10显示出最有效的双重抗微生物和抗疟疾活性。它们在分子对接研究中也显示出强大的分子相互作用。结论基于以上结果,我们认为N-(2-芳基氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺衍生物具有作为抗疟药、抗真菌药和抗菌剂的优异生物潜力。Ahmed Hassen Shntaif Sharuk Khan Ganesh Tapadiya Anand Chettupalli Shweta Saboo Mohd Sayeed Shaikh Falak Siddiqui Ramkoteswra Rao Amara 2021Digital Chinese Medicine2021,4,4:2
