1单向复合材料高应变率下材料特性的研究(英文)显示文摘在船舶领域,复合材料及夹芯复合材料板梁结构已大量应用在舰船结构中,尤其游艇及复合材料高速艇上。在运行过程中,这些结构有时很可能会受到如砰击、碰撞等动态载荷或高速冲击载荷。然而到目前为止,大多数的结构设计所用到的材料特性仍基于低应变率情况下的准静态测试结果。该文研究了高速艇等结构中的纤维增强复合材料高应变率情况下的材料特性,通过引入一种粘弹性模型来模拟并分析研究对象。并在世界先进的INSTRON高应变率材料试验机上设计并完成了相关试验,验证了该粘弹性模型、确定了模型中的材料参数。该粘弹性模型然后被用来研究分析应变率对单向复合材料机械特性的影响。王玮波 R. 2Genetic risks and familial associations of small bowel carcinoma显示文摘Adenocarcinoma of small intestines(SBA) is a relatively rare malignancy with poor outcomes due to delayed diagnosis.Fifty percent of patients have metastases on presentation and therefore early detection and treatment offers the best long term outcomes.Certain genetic polyposis syndromes and familial diseases are associatedwith increased risks for SBA.These include familial adenomatous polyposis(FAP),Lynch syndromes(LS),Juvenile polyposis syndrome,Peutz-Jeghers syndrome,Crohn's disease(CD) and celiac disease.Mutations in APC gene,Mismatch repair genes,STK11 gene,and SMAD4 gene have been implicated for the genetic diseases respectively.While there are no specific inherited genetic mutations for CD,genome-wide association studies have established over 140 loci associated with CD.CpG island mutations with defects in mismatch repair genes have been identified in celiac disease.Significant diagnostic advances have occurred in the past decade and intuitively,it would seem beneficial to use these advanced modalities for surveillance of these patients.At present it is debatable and no clear data exists to support this approach except for established guidelines to diagnose duodenal polyps in FAP,and LS.Here we discuss the genetic alterations,cancer risks,signaling mechanisms and briefly touch the surveillance modalities available for these genetic and clinical syndromes.English language articles from Pub Med/Medline and Embase was searched were collected using the phrases 'small-bowel adenocarcinoma,genetics,surveillance,familial adenomatous polyposis,lynch syndromes,Peutz-Jeghers syndrome,juvenile polyposis syndrome,CD and celiac disease'.Figures,tables and schematic diagram to illustrate pathways are included in the review.Santosh Shenoy 2016World Journal of Gastrointestinal Oncology2016,8,6:9

4Goblet cell carcinoids of the appendix:Tumor biology,mutations and management strategies显示文摘Malignant neoplasms of the appendix are rare and represent less than 1% of gastrointestinal cancers.Goblet cell carcinoids(GCC) tumors are a distinctive group of heterogeneous appendiceal neoplasm that exhibit unique clinical and pathologic features.This review focuses on the current diagnostic procedures,pathogenesis,possible signaling mechanisms and treatment options for GCC.Perspectives for future research are discussed.The tumor likely arises from pluripotent intestinal epithelial crypt base stem cells.Previous findings of Notch signaling as a tumor suppressor in Neuroendocrine tumors may have a similar role in this tumor too.Loss of Notch signaling may be the driver mutation with other successive downstream mutations likely favors them into progressing and behavior similar to poorly differentiated adenocarcinoma with minimal neuroendocrine differentiation.A multidisciplinary approach is suggested for optimal outcomes.Surgery remains the main treatment modality.Simple appendectomy may be sufficient in early stages while right hemicolectomy is recommended for advanced tumors.Cytoreductive surgery with heated intraperitoneal chemotherapy may improve survival in a select few with metastatic peritoneal disease.These tumors have an unpredictable behavior even in early stages and local recurrence and delayed metastases may be seen.Lifelong surveillance is warranted.Santosh Shenoy 2016World Journal of Gastrointestinal Surgery2016,8,10:7
5Adult intussusception:A case series and review显示文摘AIM To identify factors differentiating pathologic adult intussusception(AI) from benign causes and the need for an operative intervention. Current evidence available from the literature is discussed.METHODS This is a case series of eleven patients over the age of 18 and a surgical consultation for 'Intussusception' at a single veteran's hospital over a five-year period(2011-2016). AI was diagnosed on computed tomography(CT) scan and or flexible endoscopy(colonoscopy). Surgical referrals were from the emergency room, endoscopy suites and the radiologists. RESULTS A total of 11 cases, 9 males and 2 females were diagnosed with AI. Median age was 58 years. Abdominal pain and change in bowel habits were most common symptoms. CT scan and or colonoscopy diagnosed AI, in ten/eleven(90%) patients. There were 6 small bowel-small bowel, 4 ileocecal, and 1 sigmoid-rectal AI. 8 patients(72%) needed an operation. Bowel resection was required and definitive pathology was diagnosed in 7 patients(63%). Five patients had malignant and 2 patients had benign etiology. Small bowel enteroscopy excluded pathology in 4 cases(37%) with AI. Younger patients tend to have a benign diagnosis.CONCLUSION Majority of AI have malignant etiology however idiopathic intussusception is being seen more frequently. Operative intervention remains the mainstay however, certain small bowel intussusception especially in younger patients may be a benign, physiological, transient phenomenon and laparoscopy with reduction or watchful waiting may be an acceptable strategy. 6Evaluation of antiinflammatory activity of Tephrosia purpurea in rats显示文摘Objective:To evaluate the antiinflammatory activity of orally administered ethanolic extract of Tephrosia purpurea in acute and subacute inflammation in rats.Methods:An ethanolic extract of Tephrosia purpurea was prepared.Carrageenan induced paw edema and cotton pellet granuloma were the models for acute and subacute inflammation respectively.Four groups of rats in each model were treated orally with 2%gum acacia,100 mg /kg of aspirin,500 mg/kg and 1 000 mg/kg of ethanolic extract of Tephrosia purpurea respectively.In carrageenan induced paw edema model, subplantar injection of 1%carrageenan was made into the hind paw of the rats sixty minutes after the administration of the respective drugs.The paw volume was measured immediately after injection of carrageenan,at 3 hours and at 6 hours.Then percentage inhibition of edema was calculated.In the cotton pellet granuloma model,animals were administered drugs for six days after placing cotton pellets in the axilla on each side.On the 7th day,dry weight of granuloma was calculated.Results:The rats treated with Tephrosia purpurea did not exhibit any significant decrease in paw volume and serum ceruloplasmin levels as compared to the control and aspirin treated groups in the acute inflammation model;while,there was a significant(P < 0.01) decrease in the weight of granuloma in Tephrosia purpurea and aspirin treated groups as compared to control in subacute inflammation.Conclusions:The ethanolic extract of orally administered Tephrosia purpurea shows significant antiinflammatory effect in subacute inflammation but not in acute inflammation in rats.Shenoy Smita Shwetha K Prabhu K Maradi R Bairy KL Shanbhag T 2010Asian Pacific Journal of Tropical Medicine2010,3,3:5
7Routine diagnosis of intestinal tuberculosis and Crohn's disease in Southern India显示文摘AIM:To investigate whether routinely measured clinical variables could aid in differentiating intestinal tuberculosis(ITB)from Crohn’s disease(CD).METHODS:ITB and CD patients were prospectively included at four South Indian medical centres from October 2009 to July 2012.Routine investigations included case history,physical examination,blood biochemistry,ileocolonoscopy and histopathological examination of biopsies.Patients were followed-up after 2 and 6 mo of treatment.The diagnosis of ITB or CD was re-evaluated after 2 mo of antituberculous chemotherapy or immune suppressive therapy respectively,based on improvement in signs,symptoms and laboratory variables.This study was considered to be an exploratory analysis.Clinical,endoscopic and histopathological features recorded at the time of inclusion were subject to univariate analyses.Disease variables with sufficient number of recordings and P<0.05 were entered into logistic regression models,adjusted for known confounders.Finally,we calculated the odds ratios with respective confidence intervals for variables associated with either ITB or CD.RESULTS:This study included 38 ITB and 37 CD patients.Overall,ITB patients had the lowest body mass index(19.6 vs 22.7,P=0.01)and more commonly reported weight loss(73%vs 38%,P<0.01),watery diarrhoea(64%vs 33%,P=0.01)and rural domicile(58%vs 35%,P<0.05).Endoscopy typically showed mucosal nodularity(17/31 vs 2/37,P<0.01)and histopathology more frequently showed granulomas(10/30vs 2/35,P<0.01).The CD patients more frequently reported malaise(87%vs 64%,P=0.03),nausea(84%vs 56%,P=0.01),pain in the right lower abdominal quadrant on examination(90%vs 54%,P<0.01)and urban domicile(65%vs 42%,P<0.05).In CD,endoscopy typically showed involvement of multiple intestinal segments(27/37 vs 9/31,P<0.01).Using logistic regression analysis we found weight loss and nodularity of the mucosa were independently associated with ITB,with adjusted odds ratios of 8.6(95%CI:2.1-35.6)and 18.9(95%CI:3.5-102.8)respectively.Right lower abdominal quadrant pain on examination and involvement of≥3 intestinal segments were independently associated with CD with adjusted odds ratios of 10.1(95%CI:2.0-51.3)and 5.9(95%CI:1.7-20.6),respectively.CONCLUSION:Weight loss and mucosal nodularity were associated with ITB.Abdominal pain and excessive intestinal involvement were associated with CD.ITB and CD were equally common.Geir Larsson Thrivikrama Shenoy Ramalingom Ramasubramanian Leena Kondarappassery Balakumaran Milada Cvancarova Smstuen Gunnar Aksel Bjune Bjφrn Allan Moum 2014World Journal of Gastroenterology2014,20,17:5
8Filling Simulation of Three-dimension Braided Composite in Resin Transfer Molding显示文摘This paper measured permeability of three-dimension braided preform by radial technology. The results show that principal permeability tensor coincided with their braiding axial direction. The software of one dimensional flow filling mold was designed using Visual C++ language. Filling time is predicted and validated. The result showed that the filling time of the mold centerline agrees with the prediction value. The filling time of the mould edge is shorter than that of the prediction. 10Diagnostic efficacy of Ziehl-Neelsen method against fluorescent microscopy in detection of acid fast bacilli显示文摘Objective:To investigate the application of Ziehl-Neelsen(Z-N) and fluorescent microscopy in detection of acid fast bacilli(AFB).Methods:Duplicate smears were prepared from 260 sputum samples and stained with Z-N and fluorescent staining(FS) methods.The efficiency of both methods in primary diagnosis of tuberculosis were evaluated.Results:The smears were positive for AFB in 15(5.77%) samples by Z-N staining method and in 16(6.15%) samples by FS method.The sensitivity and specificity of Z-N staining method against FS method were 93.75% and 100%respectively.Conclusions:Though lesser cost-effective than Z-N,FS method is a more sensitive and better case finding tool in detection of AFB.Soham Gupta Vishnu Prasad Shenoy Indira Bairy Muralidharan S 2010Asian Pacific Journal of Tropical Medicine2010,3,4:3

14The ApMat marker can resolve Colletotrichum species: a case study with Mangifera indica显示文摘Anthracnose disease caused by the Colletotrichum gloeosporioides species complex is a major problem worldwide.In this study,we investigated the phylogenetic diversity of 207 Indian Colletotrichum isolates,associated with symptomatic and asymptomatic tissues of mango,belonging to this species complex.Phylogenetic analyses were performed based on a 6-gene dataset(act,cal,chs1,gapdh,ITS and tub2),followed by ApMat sequence-analysis.The ApMat-based phylogeny was found to be superior as it provided finer resolution in most of the species-level clades.Importantly,the ApMat marker identified seven lineages within C.siamense sensu lato,including C.jasmini-sambac,C.hymenocallidis,C.melanocaulon,C.siamense sensu stricto and three undesignated,potentially novel lineages.In this study,C.fragariae sensu stricto,C.fructicola,C.jasmini-sambac,C.melanocaulon and five undesignated,potentially novel lineages were found to be associated with mango tissues.There is a need to develop a consensus among mycologists as to which genes should be used to define and delimit a Colletotrichum species and in the mean time mycologists should voluntarily restrain from describing new species based on inadequate datasets.Gunjan Sharma Navinder Kumar Bevan S.Weir Kevin D.Hyde Belle Damodara Shenoy 2013Fungal Diversity2013,,4:3
15Anal carcinoma in giant anal condyloma, multidisciplinary approach necessary for optimal outcome:Two case reports and review of literature显示文摘BACKGROUND Anal cancers are caused by human papilloma virus(HPV). Buschke-Lowenstein tumor also known as giant anal condyloma(GCA) is a variant of giant neglected anal tumors arising from warts caused by HPV infection. HPV are a family of double-stranded DNA viruses and primarily cause sexually transmitted disease of the genitalia and oropharyngeal mucosa. These tumors are slow growing;locally destructive large verrucous masses.CASE SUMMARY We present a series of two cases with large anal tumors harboring invasive cancers and highlight their presentation and management. Tumors with high risk HPV subtypes(HPV 16, 18, 31, 33) may progress into invasive squamous cell carcinoma(SCC). Untreated GCA can attain enormous size and extend into the pelvic organs and bony structures. Some tumors show malignant degeneration into SCC and are often difficult to diagnose given the large size of the tumors.Complete surgical excision with negative margins is the treatment of choice and necessary to prevent recurrence. This is often not feasible and leaves large surgical wounds with tissue defects with delay in healing and increases postoperative morbidity. Pelvic reconstructive techniques including muscle flaps and grafts are often necessary to close the defects. Human immunodeficiency virus and immunocompromised patients generally do poorly with standard treatments.CONCLUSION A multidisciplinary team of colorectal and plastic surgeons, medical and radiation oncologists along with combination treatment modalities are necessary when malignant transformation occurs in GCA, for optimal outcomes.Santosh Shenoy Murali Nittala Yazen Assaf 2019World Journal of Gastrointestinal Oncology2019,11,2:2

17高分辨率CT在耳硬化症诊断中的应用价值显示文摘目的评估高分辨率CT(HRCT)检查在耳硬化症诊断中的价值。方法回顾性分析经手术确诊且资料完整的21例(31耳)耳硬化症患者的临床资料,年龄21~62岁,平均38.2±11.4岁。所有患者术前接受0.625 mm薄层HRCT检查,依据HRCT检查结果分为三组(因拟单侧手术,故以单侧21耳分组):HRCT阴性组(4耳)、窗前型组(11耳)和混合型组(6耳),与术中确诊比较HRCT诊断耳硬化症的检出率,比较各组术前和术后3个月的纯音气导(air conduction,AC)、骨导(bone conduction,BC)阈值和气骨导差(air bone gap,ABG)值。结果手术确诊为耳硬化症的21例患者中有17例患者术前HRCT检查发现耳硬化病变,检出率80.95%(17/21),其中10例(10/17,58.82%)为双侧耳硬化症。CT阴性组、窗前型组和混合型组术前平均AC阈值分别为52.8±13.9、57.7±10.0和62.7±16.7 dB HL,差异无统计学意义(P>0.05);三组术后3个月平均AC阈值分别为26.6±11.6、40.3±27.0和58.8±8.8 dB HL,差异有统计学意义(P<0.05);三组术前平均BC阈值分别为25.6±10.1、30.5±6.5和37.1±14.4 dB HL,术后分别为25.3±10.7、29.9±17.6和45.0±13.0 dB HL,均无统计学差异(P>0.05)。术前三组间ABG差异无统计学意义(P>0.05),术后HRCT阴性组ABG明显小于混合型组(P<0.05),与CT阴性组和窗前型组相比,混合型组患者术前和术后气、骨导阈值均较高;CT阴性组(P=0.014)和窗前型组(P=0.044)术后的AC听力较混合型组明显提高。术后混合型组和窗前型组中各有1耳发生感音神经性听力损失。结论 HRCT对于耳硬化症的诊断有较高的阳性率,尤其是窗前型;HCRT诊断为混合型耳硬化症者听力下降严重,术后听力改善不如CT阴性及窗前型者。谢立 刘爱国 Tejvansh Shenoy Imrit 周良强 刘磊 2020听力学及言语疾病杂志2020,28,6:2
