17前列腺癌根治术与经直肠超声引导下前列腺穿刺的Gleason评分及肿瘤偏侧性:一项比较研究显示文摘本研究的目的在于比较经直肠超声引导下前列腺穿刺(TRUSBX)与前列腺癌根治术(RP)后Gleason 评分以及肿瘤偏侧性的诊断。一些导致两种操作结果的不一致的因素也被纳入分析。在回顾的318例病例中,有191例被纳入比较研究。我们将患者按Gleason评分分为两组:中/高Gleason评分组(≥7)及低Gleason评分组(<6)。两组间的比较采用了探索性分析。我们亦比较了TRUSBX与RP术后肿瘤偏侧性的诊断。与RP相比,TRUSBX组高估了6%的病例而低估了24%的病例的Gleason 评分; 同时,TRUSBX组高估了2.6%的病例而低估了46%的病例的肿瘤偏侧性。在低估了肿瘤偏侧性的TRUSBX病例中,穿刺标本的体积较小(P<0.05),而穿刺标本的体积与低估的Gleason评分无显著相关性。同时,前列腺体积与低估或高估的可能性无显著相关性(P>0.05)。因此,我们的研究提示TRUSBX导致低估前列腺癌患者的Gleason评分和肿瘤偏侧性的可能性较高。穿刺碎块的体积有可能是影响TRUSBX低估偏侧性和高估Gleason评分的重要因素。由于12点穿刺导致低估Gleason评分和肿瘤偏侧性的可能性较高,我们认为不应单独用这种类型的穿刺来指导治疗前列腺癌。Renan A Pereira Roberto S Costa Valdair F Muglia Fabio Franca Silva Joyce S Lajes Rodolfo B Dos Reis Gyl EB Silva 2015Asian Journal of Andrology2015,17,5:1
