Tenofovir vs lamivudine plus adefovir in chronic hepatitis B:TENOSIMP-B study显示文摘AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADV) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with LAM.METHODS This study was a Phase IV prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADV for at least 6 mo, without prior resistance to ADV and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were analysed.RESULTS Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. During study development, hepatitis B virus DNA(HBV-DNA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Alanine aminotransferase(ALT) values at the end of the study were similar in the 2 groups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). No significant changes were observed in creatinine or serum phosphorus values in either group. No significant differences between the 2 groups were noted in the identification of adverse effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occurred were serious. Treatment adherence was 95.5% and 83.3% in the TDF and the LAM+ADV groups, respectively(P = 0.488). The costs associated with hospital resource consumption were significantly lower with the TDF treatment than the LAM+ADV treatment(€4943 ± 1059 vs €5811 ± 1538, respectively, P < 0.001).CONCLUSION TDF monotherapy proved to be safe and not inferior to the LAM+ADV combination therapy in maintaining virologic response in patients with CHB and previous LAM failure. In addition, the use of TDF generated a significant savings in hospital costs.Manuel Rodríguez Juan Manuel Pascasio Enrique Fraga Javier Fuentes Martín Prieto Gloria Sánchez-Antolín Jose Luis Calleja Esther Molina María Luisa García-Buey María Angeles Blanco Javier Salmerón María Lucía Bonet Jose Antonio Pons Jose Manuel González Miguel Angel Casado Francisco Jorquera 2017World Journal of Gastroenterology2017,23,41:16
Pathogenesis of occult chronic hepatitis B virus infection显示文摘Occult hepatitis B infection(OBI) is characterized by hepatitis B virus(HBV) DNA in serum in the absence of hepatitis B surface antigen(HBsAg) presenting HBsAg-negative and anti-HBc positive serological patterns.Occult HBV status is associated in some cases with mutant viruses undetectable by HBsAg assays;but more frequently it is due to a strong suppression of viral replication and gene expression.OBI is an entity with world-wide diffusion. The failure to detect HBsAg,despite the persistence of the viral DNA,is due in most cases to the strong suppression of viral replication and gene expression that characterizes this'occult'HBV infection;although the mechanisms responsible for suppression of HBV are not well understood. The majority of OBI cases are secondary to overt HBV infection and represent a residual low viremia level suppressed by a strong immune response together with histological derangements which occurred during acute or chronic HBV infection.Much evidence suggests that it can favour the progression of liver fibrosis and the development of hepatocellular carcinoma.Rocio Aller de la Fuente María L Gutiérrez Javier Garcia-Samaniego Conrado Fernández-Rodriguez Jose Luis Lledó Gregorio Castellano 2011World Journal of Gastroenterology2011,17,12:12
Serum biomarkers and risk of hepatocellular carcinoma recurrence after liver transplantation显示文摘Liver transplantation(LT) is the only potentially curative treatment for selected patients with cirrhosis and hepatocellular carcinoma(HCC) who are not candidates for resection. When the Milan criteria are strictly applied, 75% to85%of 3-to 4-year actuarial survival rates are achieved, but up to 20% of the patients experience HCC recurrence after transplantation. The Milan criteria are based on the preoperative tumor macromorphology, tumor size and number on computed tomography or magnetic resonance imaging that neither correlate well with posttransplant histological study of the liver explant nor accurately predict HCC recurrence after LT, since they do not include objective measures of tumor biology. Preoperative biological markers, including alpha-fetoprotein, desgamma-carboxiprothrombin or neutrophil-to-lymphocyte ratio and platelet-tolymphocyte ratio, can predict the risk for HCC recurrence after transplantation. These biomarkers have been proposed as surrogate markers of tumor differentiation and vascular invasion, with varied risk magnitudes depending on the defined cutoffs. Different studies have shown that the combination of one or several biomarkers integrated into prognostic models predict the risk of HCC recurrence after LT more accurately than Milan criteria alone. In this review, we focus on the potential utility of these serum biological markers to improve the performance of Milan criteria to identify patients at high risk of tumoral Published online: January 27, 2019 recurrence after LT.Liver transplantation(LT) is the only potentially curative treatment for selected patients with cirrhosis and hepatocellular carcinoma(HCC) who are not candidates for resection.
Umbilical cord as a mesenchymal stem cell source for treating joint pathologies显示文摘Articular cartilage disorders and injuries often result in life-long chronic pain and compromised quality of life.Regrettably, the regeneration of articular cartilage is a continuing challenge for biomedical research. One of the most promising therapeutic approaches is cellbased tissue engineering, which provides a healthy population of cells to the injured site but requires differentiated chondrocytes from an uninjured site. The use of healthy chondrocytes has been found to have limitations.A promising alternative cell population is mesenchymal stem cells(MSCs),known to possess excellent proliferation potential and proven capability for differentiation into chondrocytes. The“immunosuppressive”property of human MSCs makes them an important candidate for allogeneic cell therapy. The use of allogeneic MSCs to repair large defects may prove to be an alternative to current autologous and allogeneic tissue-grafting procedures. An allogeneic cell-based approach would enable MSCs to be isolated from any donor, expanded and cryopreserved in allogeneic MSC banks, providing a readily available source of progenitors for cell replacement therapy. These possibilities have spawned the current exponential growth in stem cell research in pharmaceutical and biotechnology communities. Our objective in this review is to summarize the knowledge about MSCs from umbilical cord stroma and focus mainly on their applications for joint pathologies.María Carmen Arufe Alexandre De la Fuente Isaac Fuentes Francisco Javier De Toro Francisco Javier Blanco 2011World Journal of Orthopedics2011,2,6:7
Gene expression changes in spleens of the wildlife reservoir species,Eurasian wild boar(Sus scrofa),naturally infected with Brucella suis biovar 2显示文摘Brucella suis is responsible for swine brucellosis worldwide.Of the five different B.suis biovars(bv.),bv.2 appears restricted to Europe where it is frequently isolated from wild boar and hares, can infect pigs and can cause human brucellosis. In this study, the differential gene expression profile was characterized in spleens of Eurasian wild boar naturally infected with B.suis bv.2.Of the 20,201 genes analyzed in the microarray,633 and 1,373 were significantly(fold change > 1.8;P < 0.01) upregulated and downregulated, respectively,in infected wild boar. The analysis was focused on genes that were over represented after conditional test for biological process gene ontology.Upregulated genes suggested that B.suis bv.2 infection induced cell maturation, migration and/or proliferation in infected animals. The genes downregulated in infected wild boar impaired the activity of several important cellular metabolic pathways such as metabolism, cytoskeleton organization and biogenesis, immune response and lysosomal function and vesicle-mediated transport. In addition, the response to stress, sperm fertility, muscle development and apoptosis seemed to be also impaired in infected animals. These results suggested that B.suis bv.2 may use strategies similar to other smooth brucellae to facilitate intracellular multiplication and the development of chronic infections. To our knowledge, this is the first report of the analysis of gene expression profile in hosts infected with B.suis bv.2,which is important to understand the molecular mechanisms at the host-pathogen interface in the main reservoir species with possible implications in the zoonotic cycle of the pathogen.Ruth C.Galindo Pilar M.Muoz María J.de Miguel Clara M.Marin Javier Labairu Miguel Revilla José M.Blasco Christian Gortazar José de la Fuente 2010Journal of Genetics and Genomics2010,37,11:6
Hypothalamic-pituitary-adrenal axis function during perinatal depression显示文摘Abnormal function of the hypothalamic–pituitary–adrenal(HPA) axis is an important pathological finding in pregnant women exhibiting major depressive disorder. They show high levels of cortisol proinflammatory cytokines, hypothalamic-pituitary peptide hormones and catecholamines, along with low dehydroepiandrosterone levels in plasma. During pregnancy, the TH2 balance together with the immune system and placental factors play crucial roles in the development of the fetal allograft to full term. These factors, when altered, may generate a persistent dysfunction of the HPA axis that may lead to an overt transfer of cortisol and toxicity to the fetus at the expense of reduced activity of placental 11β-hydroxysteroid dehydrogenase type 2. Epigenetic modifications also may contribute to the dysregulation of the HPA axis. Affective disorders in pregnant women should be taken seriously, and therapies focused on preventing the deleterious effects of stressors should be implemented to promote the welfare of both mother and baby.Phillipe Leff Gelman Mónica Flores-Ramos Margarita López-Martínez Carlos Cruz Fuentes Juan Pablo Reyes Grajeda 2015Neuroscience Bulletin2015,31,3:6
Glucocorticosteroid therapy in inflammatory bowel diseases: From clinical practice to molecular biology显示文摘Inflammatory bowel diseases(IBDs),such as ulcerative colitis and Crohn's disease, are chronic pathologies associated with a deregulated immune response in the intestinal mucosa, and they are triggered by environmental factors in genetically susceptible individuals.Exogenous glucocorticoids(GCs)are widely used as anti-inflammatory therapy in IBDs.In the past, patients with moderate or severe states of inflammation received GCs as a first line therapy with an important effectiveness in terms of reduction of the disease activity and the induction of remission. However, this treatment often results in detrimental side effects. This downside drove the development of second generation GCs and more precise(non-systemic)drugdelivery methods.Recent clinical trials show that most of these new treatments have similar effectiveness to first generation GCs with fewer adverse effects. The remaining challenge in successful treatment of IBDs concerns the refractoriness and dependency that some patients encounter during GCs treatment.A deeper understanding of the molecular mechanisms underlying GC response is key to personalizing drug choice for IBDs patients to optimize their response to treatment. In this review,we examine the clinical characteristics of treatment with GCs,followed by an in depth analysis of the proposed molecular mechanisms involved in its resistance and dependence associated with IBDs.This thorough analysis of current clinical and biomedical literature may help guide physicians in determining a course of treatment for IBDs patients and identifies important areas needing further study.Karen Dubois-Camacho Payton A Ottum Daniel Franco-Munoz Marjorie De la Fuente Alejandro Torres-Riquelme David Díaz-Jiménez Mauricio Olivares-Morales Gonzalo Astudillo Rodrigo Quera Marcela A Hermoso 2017World Journal of Gastroenterology2017,23,36:6

Inflammation, lipid metabolism and cardiovascular risk in rheumatoid arthritis: A qualitative relationship?显示文摘Life expectancy in patients with rheumatoid arthritis(RA)is reduced compared to the general population owing to an increase in cardiovascular diseases(CVD)not fully explained by traditional cardiovascular risk factors. In recent years, interest has been focused on the alterations in lipid metabolism in relation to chronic inflammation as one of the possible mechanisms involvedin the pathogenesis of atherosclerosis of RA patients.Research regarding this issue has revealed quantitative alterations in lipoproteins during the acute-phase reaction, and has also demonstrated structural alterations in these lipoproteins which affect their functional abilities.Although many alterations in lipid metabolism have been described in this regard, these structural changes associated with inflammation are particularly important in high-density lipoproteins as they affect their cardioprotective functions. In this respect, excessive oxidation in low-density lipoprotein(LDL)and increased lipoprotein(a)with a predominance of smaller apolipoprotein(a)isoforms has also been reported. This article will discuss proinflammatory high-density lipoproteins(pi HDL),oxidized LDL and lipoprotein(a).Elevated concentrations of these lipoproteins with marked pro-atherogenic properties have been observed in RA patients, which could help to explain the increased cardiovascular risk of these patients.Carmen García-Gómez Maria Bianchi Diana de la Fuente Lina Badimon Teresa Padró Emili Corbella Xavier Pintó 2014World Journal of Orthopedics2014,5,3:4
Hepatitis B and immunosuppressive therapies for chronic inflammatory diseases: When and how to apply prophylaxis, with a special focus on corticosteroid therapy显示文摘Currently immunosuppressive and biological agentsare used in a more extensive and earlier way in patients with inflammatory bowel disease, rheumatic or dermatologic diseases. Although these drugs have shown a significant clinical benefit, the safety of these treatments is a challenge. Hepatitis B virus(HBV) reactivations have been reported widely, even including liver failure and death, and it represents a deep concern in these patients. Current guidelines recommend to preemptive therapy in patients with immunosuppressants in general, but preventive measures focused in patients with corticosteroids and inflammatory diseases are scarce. Screening for HBV infection should be done at diagnosis. The patients who test positive for hepatitis B surface antigen, but do not meet criteria for antiviral treatment must receive prophylaxis before undergoing immunosuppression, including corticosteroids at higher doses than prednisone 20 mg/d during more than two weeks. Tenofovir and entecavir are preferred than lamivudine because of their better resistance profile in long-term immunosuppressant treatments. There is not a strong evidence, to make a general recommendation on the necessity of prophylaxis therapy in patients with inflammatory diseases that are taking low doses of corticosteroids in short term basis or low systemic bioavailability corticosteroids such as budesonide or beclomethasone dipropionate. In these cases regularly HBV DNA monitoring is recommended, starting early antiviral therapy if DNA levels begin to rise. In patients with occult or resolved hepatitis the risk of reactivation is much lower, and excepting for Rituximab treatment, the prophylaxis is not necessary.Pilar López-Serrano Elsa de la Fuente Briongos Elisa Carrera Alonso Jose Lázaro Pérez-Calle Conrado Fernández 2015World Journal of Hepatology2015,7,3:4
Subjective experience and visual attention to a historic building:A real-world eyetracking study显示文摘The objective of this exploratory study is to discover the relations between visual attention to an architectural work and the subjective experiences produced during its observation.A subjective experience with a building is the specific manner in which an architectural element or the building as a whole are presented to a person. The qualities of the element as how they are perceived and the inner world of the observer are both considered in this subjective experience. The aims of this study are to describe the experience that a building generates in people, pinpoint what has attracted the participant's attention during each view of the building in an itinerary;and understand which aspects of this selected case study have made it an object of attend on.An eye-tracking study was carried out using a portable eye tracker. This tool allowed the participants to walk freely around the exterior of a historic train station in the city of Monterrey,Mexico. Two groups of participants, with routes starting from opposite sides of the building, contemplated it while using the eye tracker.Eye tracking allowed for the identification of the architectural elements that were objects of attention, the lengths of observation, and the points of view from which the elements were observed.Other data collection techniques, such as the think-aloud protocol and a special type of survey, were used to deeply understand the experiences that accompanied the visual exploration of the historic building. Results suggest that the participants observed the different architectural elements for a time that was neither influenced by the route used to explore the building nor the point where that route was initiated. The architectural elements identified as examples of high-quality architecture and perceived as aesthetically pleasing by the participants during the itinerary were observed for longer times.Luis Alfonso de la Fuente Suarez 2020Frontiers of Architectural Research2020,9,4:3
New horizons for uncommon mutations in non-small cell lung cancer: BRAF, KRAS, RET, MET, NTRK, HER2显示文摘The 2004 discovery of EGFR mutations, followed by ALK rearrangements, ushered in a targeted therapy era for advanced non-small cell lung cancer(NSCLC).Tyrosine kinase inhibitors targeting gene alterations have substantially improved survival and quality of life for patients with NSCLC.In the last decade, rearrangements of the ROS1 oncogene have been incorporated into healthcare practice that are applicable to another small subgroup of patients who benefit from similar targeted strategies.Recent genome studies of lung adenocarcinoma have identified other possible therapeutic targets, including RET,NTRK fusions,c-MET alterations, and activating mutations in KRAS,BRAF,and HER2,all with frequencies greater than 1%.Lung cancers harbouring these genome changes can potentially be treated with agents approved for other indications or under clinical development. This review updates the therapeutic arsenal that especially targets those genes.Maria Eugenia Olmedo Raquel Cervera Luis Cabezon-Gutierrez Yolanda Lage Elena Corral de la Fuente Ana Gomez Rueda1,Xabier Mielgo-Rubio Juan Carlos Trujillo Felipe Counago 2022World Journal of Clinical Oncology2022,13,4:3
Automated Chinese medicinal plants classification based on machine learning using leaf morpho-colorimetry,fractal dimension and visible/near infrared spectroscopy显示文摘The identification of Chinese medicinal plants was conducted to rely on ampelographic manual assessment by experts.More recently, machine learning algorithms for pattern recognition have been successfully applied to leaf recognition in other plant species. These new tools make the classification of Chinese medicinal plants easier, more efficient and cost effective. This study showed comparative results between machine learning models obtained from two methods:i)a morpho-colorimetric method and ii)a visible(VIS)/Near Infrared(NIR)spectral analysis from sampled leaves of 20 different Chinese medicinal plants.Specifically, the automated image analysis and VIS/NIR spectral based parameters obtained from leaves were used separately as inputs to construct customized artificial neural network(ANN)models. Results showed that the ANN model developed using the morpho-colorimetric parameters as inputs(Model A)had an accuracy of 98.3%in the classification of leaves for the 20 medicinal plants studied. In the case of the model based on spectral data from leaves(Model B),the ANN model obtained using the averaged VIS/NIR spectra per leaf as inputs showed 92.5%accuracy for the classification of all medicinal plants used.Model A has the advantage of being cost effective, requiring only a normal document scanner as measuring instrument. This method can be adapted for non-destructive assessment of leaves in-situ by using portable wireless scanners.Model B combines the fast,non-destructive advantages of VIS/NIR spectroscopy, which can be used for rapid and non-invasive identification of Chinese medicinal plants and other applications by analyzing specific light spectra overtones from leaves to assess concentration of pigments such as chlorophyll, anthocyanins and others that are related active compounds from the medicinal plants.Jinru Xue Sigfredo Fuentes Carlos Poblete-Echeverria Claudia Gonzalez Viejo Eden Tongson Hejuan Du Baofeng Su 2019International Journal of Agricultural and Biological Engineering2019,12,2:3
Digital surface model applied to unmanned aerial vehicle based photogrammetry to assess potential biotic or abiotic effects on grapevine canopies显示文摘Accurate data acquisition and analysis to obtain crop canopy information are critical steps to understand plant growth dynamics and to assess the potential impacts of biotic or abiotic stresses on plant development.A versatile and easy to use monitoring system will allow researchers and growers to improve the follow-up management strategies within farms once potential problems have been detected. This study reviewed existing remote sensing platforms and relevant information applied to crops and specifically grapevines to equip a simple Unmanned Aerial Vehicle(UAV)using a visible high definition RGB camera. The objective of the proposed Unmanned Aerial System(UAS)was to implement a Digital Surface Model(DSM)in order to obtain accurate information about the affected or missing grapevines that can be attributed to potential biotic or abiotic stress effects. The analysis process started with a three-dimensional(3D)reconstruction from the RGB images collected from grapevines using the UAS and the Structure from Motion(SfM)technique to obtain the DSM applied on a per-plant basis. Then, the DSM was expressed as greyscale images according to the halftone technique to finally extract the information of affected and missing grapevines using computer vision algorithms based on canopy cover measurement and classification. To validate the automated method proposed, each grapevine row was visually inspected within the study area. The inspection was then compared to the digital assessment using the proposed UAS in order to validate calculations of affected and missing grapevines for the whole studied vineyard. Results showed that the percentage of affected and missing grapevines was 9.5%and 7.3%,respectively from the area studied. Therefore, for this specific study, the abiotic stress that affected the experimental vineyard(frost)impacted a total of 16.8%of plants. This study provided a new method for automatically surveying affected or missing grapevines in the field and an evaluation tool for plant growth conditions, which can be implemented for other uses such as canopy management, irrigation scheduling and other precision agricultural applications.Su Baofeng Xue Jinru Xie Chunyu Fang Yulin Song Yuyang Sigfredo Fuentes 2016International Journal of Agricultural and Biological Engineering2016,9,6:3

The westward lithospheric drift, its role on the subduction and transform zones surrounding Americas:Andean to cordilleran orogenic types cyclicity显示文摘We investigate the effect of the westerly rotation of the lithosphere on the active margins that surround the Americas and find good correlations between the inferred easterly-directed mantle counterflow and the main structural grain and kinematics of the Andes and Sandwich arc slabs. In the Andes, the subduction zone is shallow and with low dip, because the mantle flow sustains the slab;the subduction hinge converges relative to the upper plate and generates an uplifting doubly verging orogen. The Sandwich Arc is generated by a westerly-directed SAM(South American) plate subduction where the eastward mantle flow is steepening and retreating the subduction zone.In this context, the slab hinge is retreating relative to the upper plate, generating the backarc basin and a low bathymetry single-verging accretionary prism. In Central America, the Caribbean plate presents a more complex scenario:(a) To the East, the Antilles Arc is generated by westerly directed subduction of the SAM plate, where the eastward mantle flow is steepening and retreating the subduction zone.(b) To the West, the Middle America Trench and Arc are generated by the easterly-directed subduction of the Cocos plate, where the shallow subduction caused by eastward mantle flow in its northern segment gradually steepens to the southern segment as it is infered by the preexisting westerly-directed subduction of the Caribbean Plateau. In the frame of the westerly lithospheric flow, the subduction of a divergent active ridge plays the role of introducing a change in the oceanic/continental plate’s convergence angle, such as in NAM(North American)plate with the collision with the Pacific/Farallon active ridge in the Neogene(Cordilleran orogenic type scenario). The easterly mantle drift sustains strong plate coupling along NAM,showing at Juan de Fuca easterly subducting microplate that the subduction hinge advances relative to the upper plate. This lower/upper plate convergence coupling also applies along strike to the neighbor continental strike slip fault systems where subduction was terminated(San Andreas and Queen Charlotte). The lower/upper plate convergence coupling enables the capture of the continental plate ribbons of Baja California and Yakutat terrane by the Pacific oceanic plate, transporting them along the strike slip fault systems as para-autochthonous terranes. This Cordilleran orogenic type scenario,is also recorded in SAM following the collision with the Aluk/Farallon active ridge in the Paleogene, segmenting SAM margin into the eastwardly subducting Tupac Amaru microplate intercalated between the proto-LiquineOfqui and Atacama strike slip fault systems, where subduction was terminated and para-autochthonous terranes transported. In the Neogene, the convergence of Nazca plate with respect to SAM reinstalls subduction and the present Andean orogenic type scenario.Eugenio Aragon Fernando D'Eramo Marco Cuffaro Carlo Doglioni Eleonora Ficini Lucio Pinotti Silvina Nacif Manuel Demartis Irene Hernando Tomas Fuentes 2020Geoscience Frontiers2020,11,4:2
