| 1 | New aspects in the pathomechanism and diagnosis of the laryngopharyngeal reflux-clinical impact of laryngeal proton pumps and pharyngeal p H metry in extraesophageal gastroesophageal reflux disease显示文摘AIM:To determine the laryngeal H+K+-ATPase and pharyngeal p H in patients with laryngopharyngeal reflux(LPR)-symptoms as well as to assess the symptom scores during PPI therapy.METHODS:Endoscopy was performed to exclude neoplasia and to collect biopsies from the posterior cricoid area(immunohistochemistry and PCR analysis).Immunohistochemical staining was performed with monoclonal mouse antibodies against human H+K+-ATPase.Quantitative real-time RT-PCR for each of the H+K+-ATPase subunits was performed.The p H values were assessed in the aerosolized environment of the oropharynx(Dxp H Catheter) and compared to a subsequently applied combined p H/MII measurement.RESULTS:Twenty patients with LPR symptoms were included.In only one patient,the laryngeal H+K+-ATPase was verified by immunohistochemical staining.In another patient,real-time RT-PCR for each H+K+-ATPase subunit was positive.Fourteen out of twenty patients had pathological results in Dxp H,and 6/20 patients had pathological results in p H/MII.Four patients had pathological results in both functional tests.Nine out of twenty patients responded to PPIs.CONCLUSION:The laryngeal H+K+-ATPase can only be sporadically detected in patients with LPR symptoms and is unlikely to cause the LPR symptoms.Alternative hypotheses for the pathomechanism are needed.The role of pharyngeal p H-metry remains unclearand its use can only be recommended for patients in a research study setting. | Valentin Becker Romina Drabner Simone Graf Christoph Schlag Simon Nennstiel Anna Maria Buchberger Roland M Schmid Dieter Saur Monther Bajbouj | 2015 | World Journal of Gastroenterology2015,21,3: | 14 |
| 2 | Clinical significance of NOD2/CARD15 and Toll-like receptor 4 gene single nucleotide polymorphisms in inflammatory bowel disease显示文摘AIM: To evaluate the role of genetic factors in the pathogenesis of Crohn's disease (CD) and ulcerative colitis (UC), we investigated the single nucleotide poly- morphisms (SNPs) of NOD2/CARD15 (R702W, G908R and L1007fi nsC), and Toll-like receptor 4 (TLR4) genes (D299G and T399I) in a selected inflammatory bowel disease (IBD) population coming from Southern Italy. METHODS: Allele and genotype frequencies of NOD2/ CARD15 (R702W, G908R and L1007finsC) and TLR4 (D299G and T399I) SNPs were examined in 133 CD pa-tients, in 45 UC patients, and in 103 healthy controls. A genotype-phenotype correlation was performed. RESULTS: NOD2/CARD15 R702W mutation was sig-nificantly more frequent in CD (9.8%) than in controls (2.4%, P = 0.001) and in UC (2.3%, P = 0.03). No sig-nificant difference was found between UC patients and control group (P > 0.05). In CD and UC patients, no signifi cant association with G908R variant was found. L1007f insC SNP showed an association with CD (9.8%) compared with controls (2.9%, P = 0.002) and UC patients (2.3%, P = 0.01). Moreover, in CD patients, G908R and L1007finsC mutations were significantly associated with different phenotypes compared to CD wild-type patients. No association of IBD with the TLR4 SNPs was found in either cohort (allele frequencies: D299G-controls 3.9%, CD 3.7%, UC 3.4%, P > 0.05; T399I-controls 2.9%, CD 3.0%, UC 3.4%, P > 0.05). | 3 | Speckle tracking echocardiography to assess regional ventricular function in patients with apical hypertrophic cardiomyopathy显示文摘AIM To explore regional systolic strain of midwall and endocardial segments using speckle tracking echocardiography in patients with apical hypertrophic cardiomyopathy(HCM).METHODS We prospectively assessed 20 patients(mean age 53 ± 16 years,range:18-81 years,10 were male),with apical HCM. We measured global longitudinal peak systolic strain(GLPSS) in the midwall and endocardium of the left ventricle. RESULTS The diastolic thickness of the 4 apical segments was 16.25 ± 2.75 mm. All patients had a normal global systolicfunction with a fractional shortening of 50% ± 8%. In spite of supernormal left ventricular(LV) systolic function,midwall GLPSS was decreased in all patients,more in the apical(-7.3% ±-8.8%) than in basal segments(-15.5% ±-6.93%),while endocardial GLPPS was significantly greater and reached normal values(apical:-22.8% ±-7.8%,basal:-17.9% ±-7.5%). CONCLUSION This study shows that two-dimensional strain was decreased mainly confined to the mesocardium,while endocardium myocardial deformation was preserved in HCM and allowed to identify subclinical LV dysfunction. This transmural heterogeneity in systolic strain had not been previously described in HCM and could be explained by the distribution of myofibrillar disarray in deep myocardial areas. | 8 | T_3-induced liver AMP-activated protein kinase signaling:Redox dependency and upregulation of downstream targets显示文摘AIM:To investigate the redox dependency and promotion of downstream targets in thyroid hormone(T3)-induced AMP-activated protein kinase(AMPK)signaling as cellular energy sensor to limit metabolic stresses in the liver.METHODS:Fed male Sprague-Dawley rats were given a single ip dose of 0.1 mg T3/kg or T3 vehicle(Na OH0.1 N;controls)and studied at 8 or 24 h after treatment.Separate groups of animals received 500 mg N-acetylcysteine(NAC)/kg or saline ip 30 min prior T3.Measurements included plasma and liver 8-isoprostane and serumβ-hydroxybutyrate levels(ELISA),hepaticlevels of m RNAs(q PCR),proteins(Western blot),and phosphorylated AMPK(ELISA).RESULTS:T3 upregulates AMPK signaling,including the upstream kinases Ca2+-calmodulin-dependent protein kinase kinase-βand transforming growth factor-β-activated kinase-1,with T3-induced reactive oxygen species having a causal role due to its suppression by pretreatment with the antioxidant NAC.Accordingly,AMPK targets acetyl-Co A carboxylase and cyclic AMP response element binding protein are phosphorylated,with the concomitant carnitine palmitoyltransferase-1α(CPT-1α)activation and higher expression of peroxisome proliferator-activated receptor-γco-activator-1αand that of the fatty acid oxidation(FAO)-related enzymes CPT-1α,acyl-Co A oxidase 1,and acylCo A thioesterase 2.Under these conditions,T3 induced a significant increase in the serum levels ofβ-hydroxybutyrate,a surrogate marker for hepatic FAO.CONCLUSION:T3 administration activates liver AMPK signaling in a redox-dependent manner,leading to FAO enhancement as evidenced by the consequent ketogenic response,which may constitute a key molecular mechanism regulating energy dynamics to support T3preconditioning against ischemia-reperfusion injury. | Luis A Videla Virginia Fernández Pamela Cornejo Romina Vargas Paula Morales Juan Ceballo Alvaro Fischer Nicolás Escudero Oscar Escobar | 2014 | World Journal of Gastroenterology2014,20,46: | 3 |

| 12 | Potential Use of a Poultry Manure Digestate as a Biofertiliser:Evaluation of Soil Properties and Lactuca sativa Growth显示文摘Anaerobic digestion is a process that is widely used for the treatment of organic wastes. The digestate can be used as a soil amendment or crop fertiliser. The aims of our work were to evaluate 1) the physicochemical composition and pathogen content in a digestate from poultry manure, according to international regulations, and 2) the effect of its soil application on the major chemical and biological soil properties and on the growth of Lactuca sativa. The experiment consisted of two groups of pots(with and without crop). Treatments applied to each group were as follows: low and high doses of digestate and inorganic fertiliser, and no application(control)(low dose: 70 kg nitrogen(N) ha^(-1) and 21 kg phosphorus(P) ha^(-1); high dose: 210 kg N ha^(-1) and 63 kg P ha^(-1)). Soil samples were taken 7 and 34 d(harvest) after treatment applications. Heavy metal and pathogen contents in the digestate were below the upper limit values. Despite the high pH and electrical conductivity values of the digestate, both soil parameters presented acceptable values for crop growth. Although there were no initial increases in total inorganic N and available P in soil with digestate application, an increase in the fresh weight of crop was observed with the high dose application. This is probably associated with the slow nutrient release from the digestate during the development of the crop. | 14 | COVID-19 and liver injury:Pathophysiology,risk factors,outcome and management in special populations显示文摘The coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus-2 is an ongoing health concern.In addition to affecting the respiratory system,COVID-19 can potentially damage other systems in the body,leading to extra-pulmonary manifestations.Hepatic manifestations are among the common consequences of COVID-19.Although the precise mechanism of liver injury is still questionable,several mechanisms have been hypothesized,including direct viral effect,cytokine storm,hypoxic-ischemic injury,hypoxiareperfusion injury,ferroptosis,and hepatotoxic medications.Risk factors of COVID-19-induced liver injury include severe COVID-19 infection,male gender,advanced age,obesity,and underlying diseases.The presentations of liver involvement comprise abnormalities in liver enzymes and radiologic findings,which can be utilized to predict the prognosis.Increased gamma-glutamyltransferase,aspartate aminotransferase,and alanine aminotransferase levels with hypoalbuminemia can indicate severe liver injury and anticipate the need for intensive care units’hospitalization.In imaging,a lower liver-to-spleen ratio and liver computed tomography attenuation may indicate a more severe illness.Furthermore,chronic liver disease patients are at a higher risk for severe disease and death from COVID-19.Nonalcoholic fatty liver disease had the highest risk of advanced COVID-19 disease and death,followed by metabolic-associated fatty liver disease and cirrhosis.In addition to COVID-19-induced liver injury,the pandemic has also altered the epidemiology and pattern of some hepatic diseases,such as alcoholic liver disease and hepatitis B.Therefore,it warrants special vigilance and awareness by healthcare professionals to screen and treat COVID-19-associated liver injury accordingly. | Romina Roshanshad Amirhossein Roshanshad Reza Fereidooni Mahnaz Hosseini-Bensenjan | 2023 | World Journal of Hepatology2023,15,4: | 2 |
