| 2 | Antibiotic prophylaxis in variceal hemorrhage:Timing,effectiveness and Clostridium difficile rates显示文摘AIM:To investigate if antibiotics administered within 8 h of endoscopy reduce mortality or increase the incidence of Clostridium difficile infection(CDI).METHODS:A 2-year retrospective analysis of all patients who presented with first variceal hemorrhage was undertaken.The primary outcome measure was 28-d mortality.Secondary outcome measures were 28-d rebleeding rates and 28-d incidence of CDI.All patients were admitted to a tertiary liver unit with a consultantled,24-h endoscopy service.Patients received standard care including terlipressin therapy.Data collection included:primary and secondary outcome measures,timing of first administration of intravenous antibiotics,eti-ology of liver disease,demographics,endoscopy details and complications.A prospective study was undertaken to determine the incidence of CDI in the study population and general medical inpatients admitted for antibiotic therapy of at least 5 d duration.Statistical analysis was undertaken using univariate,non-parametric tests and multivariate logistic regression analysis.RESULTS:There were 70 first presentations of variceal hemorrhage during the study period.Seventy percent of cases were male and 65.7% were due to chronic alcoholic liver disease.In total,64/70(91.4%) patients received antibiotics as prophylaxis during their admission.Specifically,53/70(75.7%) received antibiotics either before endoscopy or within 8 h of endoscopy [peri-endoscopy(8 h) group],whereas 17/70(24.3%) received antibiotics at > 8 h after endoscopy or not at all(non peri-endoscopy group).Overall mortality and rebleeding rates were 13/70(18.6%) and 14/70(20%),respectively.The periendoscopy(8 h) group was significantly less likely to die compared with the non peri-endoscopy group [13.2% vs 35.3%,P = 0.04,odds ratio(OR) = 0.28(0.078-0.997)] and showed a trend towards reduced rebleeding [17.0% vs 29.4%,P = 0.27,OR = 0.49(0.14-1.74)].On univariate analysis,the non peri-endoscopy group [P = 0.02,OR = 3.58(1.00-12.81)],higher model for end-stage liver disease(MELD) score(P = 0.02),presence of hepatorenal syndrome [P < 0.01,OR = 11.25(2.24-56.42)] and suffering a clinical episode of sepsis [P = 0.03,OR = 4.03(1.11-14.58)] were significant predictors of death at 28 d.On multivariate logistic regression analysis,lower MELD score [P = 0.01,OR = 1.16(1.04-1.28)] and periendoscopy(8 h) group [P = 0.01,OR = 0.15(0.03-0.68)] were independent predictors of survival at 28 d.The CDI incidence(5.7%) was comparable to that in the general medical population(5%).CONCLUSION:Antibiotics administered up to 8 h following endoscopy were associated with improved survival at 28 d.CDI incidence was comparable to that in other patient groups. | Matthew RL Brown Graeme Jones Kathryn L Nash Mark Wright Indra Neil Guha | 2010 | World Journal of Gastroenterology2010,16,42: | 13 |
| 3 | Effect of Sc and Zr additions on microstructures and corrosion behavior of Al-Cu-Mg-Sc-Zr alloys显示文摘The effects of adding the alloy element Sc to Al alloys on strengthening, recrystallization and modification of the grain microstructure have been investigated. The combination of Sc and Zr alloying not only produces a remarkable synergistic effect of inhibition of recrystallization and refinement of grain size but also substantially reduce the amount of high-cost additional Sc. In this work, the microstructures and corrosion behavior of a new type of Al-Cu-Mg-Sc-Zr alloy with Sc/Zr ratio of 1/2 were investigated.The experimental results showed that the Sc and Zr additions to Al-Cu-Mg alloy could strongly inhibit recrystallization, refine grain size, impede the segregation of Cu element along the grain boundary and increase the spacing of grain boundary precipitates. In addition, adding Sc and Zr to Al-Cu-Mg alloy effectively restricts the corrosion mechanism conversion associated with Al2 Cu Mg particles, which resulted in the change of the cross-section morphology of inter-granular corrosion from an undercutting to an elliptical shape. The susceptibility to inter-granular corrosion was significantly decreased with increasing Sc and Zr additions to the Al-Cu-Mg alloy. | 6 | High-resolution flood modeling of urban areas using MSN_Flood显示文摘Although existing hydraulic models have been used to simulate and predict urban flooding, most of these models are inadequate due to the high spatial resolution required to simulate flows in urban floodplains. Nesting high-resolution subdomains within coarser-resolution models is an efficient solution for enabling simultaneous calculation of flooding due to tides, surges, and high river flows. MSN_Flood has been developed to incorporate moving boundaries around nested domains, permitting alternate flooding and drying along the boundary and in the interior of the domain. Ghost cells adjacent to open boundary cells convert open boundaries, in effect, into internal boundaries. The moving boundary may be multi-segmented and non-continuous, with recirculating flow across the boundary. When combined with a bespoke adaptive interpolation scheme, this approach facilitates a dynamic internal boundary. Based on an alternating-direction semi-implicit finite difference scheme,MSN_Flood was used to hindcast a major flood event in Cork City resulting from the combined pressures of fluvial, tidal, and storm surge processes. The results show that the model is computationally efficient, as the 2-m high-resolution nest is used only in the urban flooded region.Elsewhere, lower-resolution nests are used. The results also show that the model is highly accurate when compared with measured data. The model is capable of incorporating nested sub-domains when the nested boundary is multi-segmented and highly complex with lateral gradients of elevation and velocities. | 7 | Effects of dynamic recrystallization and strain-induced dynamic precipitation on the corrosion behavior of partially recrystallized Mg-9Al-lZn alloys显示文摘The corrosion susceptibility of recrystallized and un-recrystallized grains in equal channel angular pressed(ECAPed)Mg-9Al-lZn(AZ91)alloys immersed in chloride containing media was investigated through immersion testing and an electrochemical microcell technique coupledwith high resolution techniques such as scanning Kelvin probe force microscopy(SKPFM),transmission electron microscopy(TEM),andelectron backscatter diffraction(EBSD).During ECAP,dynamic recrystallization(DRX)and strain-induced dynamic precipitation(SIDP)simultaneously occurred,resulting in a bimodal grain structure of original elongated coarse grains and newly formed equiaxed fine grainswith a large volume fraction ofβ-Mg17Al12 precipitates.Corrosion preferentially initiates and propagates in the DRXed grains,owing tothe greater microchemistry difference between theβ-Mg17Al12 precipitates formed at the DRXed grain boundaries and the adjacentα-Mgmatrix,which induces a strong microgalvanic coupling between these phases.Additionally,the weaker basal texture of the DRXed grainsalso makes these grains more susceptible to electrochemical reactions than the highly textured un-DRXed grains.The influence of dynamicrecrystallization and dynamic precipitation was also studied in ECAPed alloys with differenl levels of deformation strain through corrosion andelectrochemical techniques.Increasing the strain level led to a more uniform corrosion with a shallow penetration depth,lower corrosion ratevalues,and higher protective ability of the oxide film.Furthermore,higher levels of strain resulted in greater hardness values of the ECAPedalloys.The superior corrosion resistance and strength of the ECAPed alloys with increasing strain level was attributed to the combination ofsmaller DRXed grain size,higher DRX ratio,and higher volume fraction of uniformly distributed fineβ-Mg17Al12 precipitates.c 2020 Published by Elsevier B.V.on behalf of Chongqing University. | Yenny Cubides Dexin Zhao Lucas Nash Digvijay Yadav Kelvin Xie Ibrahim Karaman Homero Castaneda | 2020 | Journal of Magnesium and Alloys2020,8,4: | 3 |
| 8 | Dynamic phase transition behavior of a LiMn_(0.5)Fe_(0.5)PO_(4) olivine cathode material for lithium-ion batteries revealed through in-situ X-ray techniques显示文摘LiMn_(0.5)Fe_(0.5)PO_(4) has attracted great interest due to its good electrochemical performance and higher operating voltages.This has led to a greater than 30 percent higher energy density than for commercial Li Fe PO4 olivine cathodes.Understanding the phase transition behaviors and kinetics of this material will help researchers to design and develop next generation cathodes for Li-ion batteries.In this study,we investigated non-equilibrium phase transition behaviors in a LiMn_(0.5)Fe_(0.5)PO_(4) cathode material during charge–discharge processes by varying current rates(C-rates)using synchrotron in-situ X-ray techniques.These methods included wide angle X-ray scattering(in-situ WAXS)and X-ray absorption spectroscopy(in-situ XAS).The WAXS spectra indicate that the phase transition of LiMn_(0.5)Fe_(0.5)PO_(4) material at slow C-rates is induced by a two-phase reaction.In contrast,at a high C-rate(5 C),the formation of an intermediate phase upon discharge is clearly observed.Concurrently,the oxidation numbers of the redox reactions of Fe^(2+)/Fe^(3+)and Mn^(2+)/Mn^(3+)were evaluated using in-situ XAS.This combination of synchrotron in-situ X-ray techniques gives clear insights into the non-equilibrium phase transition behavior of a LiMn_(0.5)Fe_(0.5)PO_(4) cathode material.This new understanding will be useful for further developments of this highly promising cathode material for practical commercialization. | Sujeera Pleuksachat Phongsit Krabao Sarawut Pongha Viyada Harnchana Pawinee Klangtakai Wanwisa Limphirat Siriwat Soontaranon Jeffrey Nash Nonglak Meethong | 2022 | Journal of Energy Chemistry2022,31,8: | 2 |
