1Endoscopic retrograde cholangiopancreatography in periampullary diverticulum: The challenge of cannulation显示文摘Periampullary diverticulum(PAD) is duodenal outpunching defined as herniation of the mucosa or submucosa that occurs via a defect in the muscle layer within an area of 2 to 3 cm around the papilla. Although PAD isusually asymptomatic and discovered incidentally during endoscopic retrograde cholangiopancreatography(ERCP), it is associated with different pathological conditions such as common bile duct obstruction, pancreatitis, perforation, bleeding, and rarely carcinoma. ERCP has a low rate of success in patients with PAD,suggesting that this condition may complicate the technical application of the ERCP procedure. Moreover, cannulation of PAD can be challenging, time consuming, and require the higher level of skill of more experienced endoscopists. A large portion of the failures of cannulation in patients with PAD can be attributed to inability of the endoscopist to detect the papilla. In cases where the papilla is identified but does not point in a suitable direction for cannulation, different techniques have been described. Endoscopists must be aware of papilla identification in the presence of PAD and of different cannulation techniques, including their technical feasibility and safety, to allow for an informed decision and ensure the best outcome. 3Load-settlement behaviour of a strip footing resting on iron ore tailings as a structural fill显示文摘This study presents a laboratory investigation of load-settlement behaviour of a strip footing resting on iron ore tailings used as a structural fill.The footing was placed at various depths in the tailings bed.The relative density of the tailings was varied as D_r = 50%,70%and 90%.An incremental load was applied on the footing while observing the settlement until the failure took place.The results obtained for tailings were compared with those for the sandy soil.It is observed that the load-bearing capacity and stiffness increase with an increase in footing embedment depth and relative density.Compared to load-settlement behaviour of Perth sandy soil,the tailings fill could have as high as 22 times and 13.5 times the load-bearing capacity and stiffness,respectively.Therefore,the replacement of sandy soil with iron ore tailings for structural fills is cost-effective,and moreover,this application contributes to environmental sustainability in construction.Kuranchie Francis Atta Shukla Sanjay Kumar Habibi Daryoush Kazi Monir 2016International Journal of Mining Science and Technology2016,26,2:8
4强迫症患者、强迫型人格障碍患者和健康对照者之间的认知灵活性和计划能力比较(英文)显示文摘背景:强迫症患者(Obsessive Compulsive Disorder,OCD)与强迫型人格障碍患者(Obsessive Compulsive Personality Disorder,OCPD)的认知功能还没有得到充分的研究。目标:验证OCD和OCPD患者的认知灵活性和计划能力。方法:本研究在德黑兰心理咨询门诊选定了25例OCD患者和20例OCPD患者,他们在既往两周都没有服药,并且从大学工作人员和当地社区居民中选定了25名健康对照者。对所有参与者均进行28项一般健康问卷(28-item version of the General Health Questionnaire,GHQ-28)、威斯康星卡片分类测验(Wisconsin Card Sorting Test,WCST)、和伦敦塔试验(Tower of London test,TOL))。本研究运用WCTS的两个测量指标(持续错误数和完成分类数)评估认知灵活性以及TOL的三个测量指标(12个试验总共的移动次数、总应答时和计划时间)评估计划能力。结果:2组患者的当前心理困扰程度显著高于对照组。在控制人口学因素和心理困扰水平后,OCD患者和OCPD患者在WCST测试中比对照组更容易出现持续性错误,而OCD患者(不包括OCPD患者)的完成分类数比对照组显著减少。OCD患者和OCDP患者都比对照组需要更多的移动次数来完成12项TOL任务,并且OCD患者比OCPD患者和对照组需要花更长的时间来完成任务。结论:与健康对照组相比,OCD和OCPD患者的认知灵活性和计划能力都有所受损,并且OCD和OCPD患者之间的认知功能测试也存在一些差异。我们需要对OCD和OCPD患者进行长期随访研究评估在强迫症状严重性加重与减轻时认知功能的改变,从而决定此类认知评估指标对于强迫症是否有诊断或临床关联性。Negin PAAST Zohreh KHOSRAVI Amir Hossein MEMARI Monir SHAYESTEHFAR Mohammad ARBABI 2016上海精神医学2016,28,1:7
5In vitro α-amylase inhibitory activity and in vivo hypoglycemic effect of methanol extract of Citrus macroptera Montr.fruit显示文摘Objective:To investigate the therapeutic effects of methanol extract of Citrus macroptera Montr,fruit inα-amylase inhibitory activity(in vitro)and hypoglycemic activity in normal and glucose induced hyperglycemic rats(in vivo).Methods:Fruits of Citrus macroptera without rind was extracted with pure methanol following cold extraction and tested for presence of phytochemical constituents,α-amylase inhibitory activity,and hypoglycemic effect in normal rats and glucose induced hyperglycemic rats.Results:Presence of saponin,steroid and terpenoid were identified in the extract.The results showed that fruit extract had moderateα-amylase inhibitory activity[IC_(50)value=(3.638±0.190)mg/mL]as compared to acarbose.Moreover at 500 mg/kg and 1000 mg/kg doses fruit extract significantly(P<0.05 and P<0.01 respectively)reduced fasting blood glucose level in normal rats as compared to glibenclamide(5 mg/kg).In oral glucose tolerance test,500 mg/kg dose significantly reduced blood glucose level(P<0.05)at 2 h but 1000 mg/kg dose significantly reduced blood glucose level at 2 h and 3 h(P<0.05 and P<0.01 respectively)whereas glibenclamide(5 mg/kg)significantly reduced glucose level at every hour after administration.Overall time effect is also considered extremely significant with F value=23.83 and P value=0.0001 in oral glucose tolerance test.Conclusion:These findings suggest that the plant may be a potential source for the development of new oral hypoglycemic agent.Nizam Uddin Rakib Hasan Monir Hossain Arjyabrata Sarker A.H.M.Nazmul Hasan A.F.M.Mahmudul Islam Mohd.Motaher H.Chowdhury Sohel Rana 2014Asian Pacific Journal of Tropical Biomedicine2014,4,6:4
