1在 laparoscopic 肝外科的快轨道的节目: 理论或事实?显示文摘 AIM:To analyze our results after the introduction of a fast-track(FT) program after laparoscopic liver surgery in our Hepatobiliarypancreatic Unit.METHODS:All patients(43) undergoing laparoscopic liver surgery between March 2004 and March 2010 were included and divided into two consecutive groups:Control group(CG) from March 2004 until December 2006 with traditional perioperative cares(17 patients) and fast-track group(FTG) from January 2007 until March 2010 with FT program cares(26 patients).Primary endpoint was the influence of the program on the postoperative stay,the amount of re-admissions,morbidity and mortality.Secondarily we considered duration of surgery,use of drains,conversion to open surgery,intensive cares needs and transfusion.RESULTS:Both groups were homogeneous in age and sex.No differences in technique,time of surgery or conversion to open surgery were found,but more malignant diseases were operated in the FTG,and then transfusions were higher in FTG.Readmissions and morbidity were similar in both groups,without mortality.Postoperative stay was similar,with a median of 3 for CG vs 2.5 for FTG.However,the 80.8% of patients from FTG left the hospital within the first 3 d after surgery(58.8% for CG).CONCLUSION:The introduction of a FT program after laparoscopic liver surgery improves the recovery of patients without increasing complications or re-admissions,which leads to a reduction of the stay and costs.Belinda Sánchez-Pérez José Manuel Aranda-Narváez Miguel Angel Suárez-Muoz Moises elAdel-delFresno José Luis Fernández-Aguilar Jose Antonio Pérez-Daga Ysabel Pulido-Roa Julio Santoyo-Santoyo 2012World Journal of Gastrointestinal Surgery2012,4,11:18
2Clinical presentation and early predictors for poor outcomes in pediatric myocarditis:A retrospective study显示文摘BACKGROUND Myocarditis is an important cause of morbidity and mortality in children, leading to long-term sequelae including chronic congestive heart failure, dilated cardiomyopathy, heart transplantation, and death. The initial diagnosis of myocarditis is usually based on clinical presentation, but this widely ranges from the severe sudden onset of a cardiogenic shock to asymptomatic patients. Early recognition is essential in order to monitor and start supportive treatment prior to the development of severe adverse events. Of note, many cases of fulminant myocarditis are usually misdiagnosed as otherwise minor conditions during the weeks before the unexpected deterioration.AIM To provide diagnostic clues to make an early recognition of pediatric myocarditis.To investigate early predictors for poor outcomes.METHODS We conducted a retrospective cross-sectional single-center study from January 2008 to November 2017 at the Pediatric Department of our institution, including children < 18-years-old diagnosed with myocarditis. Poor outcome was defined as the occurrence of any of the following facts: death, heart transplant, persistent left ventricular systolic dysfunction or dilation at hospital discharge(early poor outcome), or after 1 year of follow-up(late poor outcome). We analyzed different clinical features and diagnostic test findings in order to provide diagnostic clues for myocarditis in children. Multivariable stepwise logistic regression analysis was performed using all variables that had been selected by univariate analysis to determine independent factors that predicted a poor early or late outcome in our study population.RESULTS A total of 42 patients [69% male; median age of 8(1.5-12) years] met study inclusion criteria. Chest pain(40%) was the most common specific cardiac symptom. Respiratory tract symptoms(cough, apnea, rhinorrhea)(38%),shortness of breath(35%), gastrointestinal tract symptoms(vomiting, abdominal pain, diarrhea)(33%), and fever(31%) were the most common non-cardiac initial complaints. Tachycardia(57%) and tachypnea(52%) were the most common signs on the initial physical exam followed by nonspecific signs of respiratory tract infection(44%) and respiratory distress(35%). Specific abnormal signs of heart failure such as heart murmur(26%), systolic hypotension(24%), gallop rhythm(20%), or hepatomegaly(20%) were less prevalent. Up to 43% of patients presented an early poor outcome, and 16% presented a late poor outcome. In multivariate analysis, an initial left ventricular ejection fraction(LVEF) < 30%remained the only significant predictor for early [odds ratio(OR)(95%CI) = 21(2-456), P = 0.027) and late [OR(95%CI) = 8(0.56-135), P = 0.047) poor outcome in children with myocarditis. LVEF correlated well with age(r = 0.51, P = 0.005),days from the initiation of symptoms(r =-0.31, P = 0.045), and N-terminal probrain natriuretic peptide levels(r = 0.66, P < 0.001), but not with troponin T(r =-0.05, P = 0.730) or C-reactive protein levels(r =-0.13, P = 0.391). N-terminal probrain natriuretic peptide presented a high diagnostic accuracy for LVEF < 30% on echocardiography with an area under curve of 0.931(95%CI: 0.858-0.995, P <0.001). The best cut-off point was 2000 pg/mL with a sensitivity of 90%,specificity of 81%, positive predictive value of 60%, and negative predictive value of 96%.CONCLUSION The diagnosis of myocarditis in children is challenging due to the heterogeneous and unspecific clinical presentation. 4Vanishing bile duct syndrome in human immunodeficiency virus: Nevirapine hepatotoxicity revisited显示文摘Vanishing bile duct syndrome (VBDS) refers to a group of disorders characterized by prolonged cholestasis as a result of destruction and disappearance ofintrahepatic bile ducts. Multiple etiologies have been indentifi ed including infections, neoplastic disorders, autoimmune conditions and drugs. The natural history of this condition is variable and may involve resolution of cholestasis or progression with irreversible damage. VBDS is extremely rare in human immunodeficiency virus (HIV)-infected patients and anti-retroviral therapy has never been implicated as a cause. We encountered a young pregnant female with HIV and VBDS secondary to anti-retroviral therapy. 5Mayombian ethnic,vegetables low intake,insulin treatment,diabetic nephropathy and severe diabetic retinopathy are determinants of blindness in diabetic Africans显示文摘AIM:to determine the frequency and causes of blindness in diabetic Africans.METHODS:The study was a cross-sectional survey carried out among known black diabetics consecutively admitted at the Teaching Hospital,University of Kinshasa,between 2005 and 2007.Examination methods included interviewer-administered structured question naire,eye examinations(visual acuity,tonometry,funduscopy),and fasting plasma glycaemia test.RESULTS:Of the 227 patients examined,15.9% had blindness.Univariate analyses showed significant association between female,severity of diabetic retinopathy,Mayombian ethnic group,use of insulin treatment,low intake of vegetables,diabetic nephropathy,open angle glaucoma and blindness in all diabetics.After logistic regression,only diabetic nephropathy,use of insulin treatment,macular oedema,Mayombian ethnic group and vegetables low intake were the independent risk factors of blindness in all diabetics.However,after logistic regression in the sub-group with diabetic retinopathy,only open angle glaucoma and proliferative diabetic retinopathy were the independent determinants of blindness.CONCLUSION:The majority of the causes of blindness in these diabetic Africans are avoidable.It is recommended that appropriate diabetes care,nutrition education,periodic eye examination and laser photocoagulation facilities should be provided for treating diabetics in sub-Saharan Africa.Mvitu Muaka Moise Longo-Mbenza Benjamin Cibanda Yokobo Enoch Longo Phemba Igor 2013International Journal of Ophthalmology(English edition)2013,6,5:6
6Retinopathy in non diabetics, diabetic retinopathy and oxidative stress: a new phenotype in Central Africa?显示文摘AIM:To evaluate the rates of retinopathy without diabetes and diabetic retinopathy(DR),associated with some markers of oxidative stress,antioxidants and cardiometabolic risk factors.METHODS:We determined the prevalence of DR in 150type 2 diabetes mellitus(T2DM)patients,that of retinopathy in 50 non diabetics,the levels of body mass index(BMI),waist circumference(WC),blood pressure,lipids,8-isoprostane,8-hydroxydeoxyguanosine(8-oHdG),gamma-glutamyl transferase(GGT),oxidized low density lipoprotein(LDL)(OxLDL),thiobarbituric acid reacting substances(TBARS),reduced glutathione(GSH),superoxide dismutase(SOD),uric acid,creatinine,albumin,total antioxidant status(TAOS),zinc,selenium,magnesium,vitamin C,vitamin D,vitamin E,glucose,apolipoprotein B(ApoB).RESULTS:The prevalences of DR at 53y and Rtp at62y were 44%(n=66)and 10%(n=5),respectively.Thehighest levels of 8-isoprostane,8-OHdG,TBARS,SOD,and OxLDL were in DR.The lowest levels of vitamin D,vitamin C,TAOS,and vitamin E were in DR.In the casecontrol study discriminant analysis,the levels of vitamin C,vitamin D,ApoB,8-OHdG,creatinine,Zn,vitamin E,and WC distinguished significantly non-diabetics without DR(controls),T2DM patients without DR and T2DM patients with DR.CONCLUSION:Anticipation of DR onset is significantly associated with the exageration of oxidative stress biomarkers or decrease of antioxidants in African type 2diabetics.Prevention of oxidative stress and abdominal obesity is needed.Supplementation in vitamin C,D,and E should be recommended as complement therapies of T2DM.Benjamin Longo-Mbenza Moise Mvitu Muaka Wayiza Masamba Lucien Muizila Kini Igor Longo Phemba Dalida Kibokela Ndembe Doris Tulomba Mona 2014International Journal of Ophthalmology(English edition)2014,7,2:4
7Clinical relevance of aberrant polypoid nodule scar after endoscopic submucosal dissection显示文摘AIM To describe a series of patients with aberrant polypoid nodule scar developed after gastric endoscopic submucosal dissection(ESD), and to discuss its pathogenesis and clinical management. METHODS We reviewed retrospectively the endoscopic database of two academic institutions located in Brazil and Japan and searched for all patients that underwent ESD to manage gastric neoplasms from 2003 to 2015. The criteria for admission in the study were:(1) successful en bloc ESD procedure with R0 and curative resection confirmed histologically;(2) postoperative endoscopic examination with identification of a polypoid nodule scar(PNS) at ESD scar;(3) biopsies of the PNS with hyperplastic or regenerative tissue, reviewed by two independent experienced gastrointestinal pathologists, one from each Institution. Data were examined for patient demographics, Helicobacter pylori status, precise neoplastic lesion location in the stomach, tumor size, histopathological assessment of the ESD specimen, and postoperative information including medical management, endoscopic and histological findings, and clinical outcome.RESULTS A total of 14 patients(10 men/4 women) fulfilled the inclusion criteria and were enrolled in this study. One center contributed with 8 cases out of 60 patients(13.3%) from 2008 to 2015. The second center contributed with 6 cases(1.7%) out of 343 patients from 2003 to 2015. Postoperative endoscopic follow-up revealed similar findings in all patients: A protruded polypoid appearing nodule situated in the center of the ESD scar surrounded by convergence of folds. Biopsies samples were taken from PNS, and histological assessment revealed in all cases regenerative and hyperplastic tissue, without recurrent tumor or dysplasia. Primary neoplastic lesions were located in the antrum in 13 patients and in the angle in one patient. PNS did not develop in any patient after ESD undertaken for tumors located in the corpus, fundus or cardia. All patients have been followed systematically on an annual basis and no malignant recurrence in the ESD scar has been identified(mean follow-up period: 45 mo).CONCLUSION PNS may occur after ESD for antral lesions and endoscopically look concerning, especially for the patient or the family doctor. 8In vitro Ruminal Gas Production Kinetics of Four Fodder Trees Ensiled With or Without Molasses and Urea显示文摘This study investigated if the addition of urea (U), molasses (M) or their 1:1 (v/v) mixture during ensiling increases the nutritional value of forage from four fodder trees (Prunus persica, Leucaena esculenta, Acacia farnesiana, and Prunus domestica). Forage samples of fodder trees were collected in triplicate (three individual samples of each species) and subjected to an in vitro gas production (GP) procedure. Fermentation at 24 h (GP 24), short-chain volatile fatty acids (SCFA), and microbial crude protein production (MCP), in vitro organic matter digestibility (OMD), metabolizable energy (ME) and dry matter degradability (DMD) were estimated. Forage samples were incubated for 72 h in an incubator at 39oC and the volume of GP was recorded at 2, 4, 6, 8, 10, 12, 24, 48, and 72 h of incubation using the reading pressure technique. The rumen fermentation profiles were highest for P. persica, which showed the highest (P<0.0001) DMD, ME, OMD, SCFA, GP 24 and MCP. On the other hand L. esculenta had the lowest (P<0.0001) DMD, SCFA, MCP; P. domestica had the lowest (P<0.0001) OMD. The addition of M to silage increased (P<0.0001) ME and OMD, as well as GP. However, the addition of U and the mixture of U and M reduced (P<0.0001) DMD, ME, OMD, SCFA, GY 24 and MCP. These results show that P. persica has the highest nutritive value and L. esculenta the lowest for ruminants. 10Cardiovascular impact of COVID-19 with a focus on children: A systematic review显示文摘BACKGROUND Since the beginning of the pandemic,coronavirus disease-2019(COVID-19)in children has shown milder cases and a better prognosis than adults.Although the respiratory tract is the primary target for severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),cardiovascular involvement is emerging as one of the most significant and life-threatening complications of SARS-CoV-2 infection in adults.AIM To summarize the current knowledge about the potential cardiovascular involvement in pediatric COVID-19 in order to give a perspective on how to take care of them during the current pandemic emergency.METHODS Multiple searches in MEDLINE,PubMed were performed using the search terms“COVID-19”or“SARS-CoV-2'were used in combination with“myocardial injury”or'arrhythmia'or“cardiovascular involvement”or'heart disease'or'congenital heart disease'or“pulmonary hypertension”or'long QT'or“cardiomyopathies”or“channelopathies”or'Multisystem inflammatory system'or'PMIS'or“MIS-C”or”Pediatric multisystem inflammatory syndrome'or'myocarditis'or'thromboembolism to identify articles published in English language from January 1st,2020 until July 31st,2020.The websites of World Health Organization,Centers for Disease control and Prevention,and the Johns Hopkins Coronavirus Resource Center were reviewed to provide up to date numbers and infection control recommendations.Reference lists from the articles were reviewed to identify additional pertinent articles.Retrieved manuscripts concerning the subject were reviewed by the authors,and the data were extracted using a standardized collection tool.Data were subsequently analyzed with descriptive statistics.For Pediatric multisystemic inflammatory syndrome temporally associated with COVID-19(PMIS),multiple meta-analyses were conducted to summarize the pooled mean proportion of different cardiovascular variables in this population in pseudo-cohorts of observed patients.RESULTS A total of 193 articles were included.Most publications used in this review were single case reports,small case series,and observational small-sized studies or literature reviews.The meta-analysis of 16 studies with size>10 patients and with complete data about cardiovascular involvement in children with PMIS showed that PMIS affects mostly previously healthy school-aged children and adolescents presenting with Kawasaki disease-like features and multiple organ failure with a focus on the heart,accounting for most cases of pediatric COVID-19 mortality.They frequently presented cardiogenic shock(53%),ECG alterations(27%),myocardial dysfunction(52%),and coronary artery dilation(15%).Most cases required PICU admission(75%)and inotropic support(57%),with the rare need for extracorporeal membrane oxygenation(4%).Almost all of these children wholly recovered in a few days,although rare deaths have been reported(2%).Out of PMIS cases we identified 10 articles reporting sporadic cases of myocarditis,pulmonary hypertension and cardiac arrythmias in previously healthy children.We also found another 10 studies reporting patients with preexisting heart diseases.Most cases consisted in children with severe COVID-19 infection with full recovery after intensive care support,but cases of death were also identified.The management of different cardiac conditions are provided based on current guidelines and expert panel recommendations.CONCLUSION There is still scarce data about the role of cardiovascular involvement in COVID-19 in children.Based on our review,children(previously healthy or with preexisting heart disease)with acute COVID-19 requiring hospital admission should undergo a cardiac workup and close cardiovascular monitoring to identify and treat timely life-threatening cardiac complications.Moises Rodriguez-Gonzalez Ana Castellano-Martinez Helena Maria Cascales-Poyatos Alvaro Antonio Perez-Reviriego 2020World Journal of Clinical Cases2020,8,21:3
11Liver dysfunction and SARS-CoV-2 infection显示文摘Severe acute respiratory syndrome coronavirus 2 infection is the cause of coronavirus disease 2019(COVID-19),which predominantly affects the respiratory system;it also causes systemic and multi-organic disease.Liver damage is among the main extrapulmonary manifestations.COVID-19-associated liver injury is defined as any liver damage occurring during the disease course and treatment of COVID-19 in patients with or without pre-existing liver disease,and occurs in approximately one in five patients.Abnormal liver test results have been associated with a more severe course of COVID-19 and other complications,including death.Mechanisms linking COVID-19 to liver injury are diverse.Particular consideration should be made for patients with pre-existing liver disease,such as metabolic dysfunction-associated fatty liver disease,chronic liver disease due to viral or autoimmune disease,liver transplant carriers,or cirrhosis,given the risk for more severe outcomes.This manuscript summarizes the current lines of evidence on COVID-19-associated liver injury regarding pathophysiology,clinical significance,and management in both patients with or without preexisting liver disease,to facilitate clinicians’access to updated information and patient care.Finally,we mention the ideas and recommendations to be considered for future research.Abraham Edgar Gracia-Ramos Joel Omar Jaquez-Quintana Raúl Contreras-Omaña Moises Auron 2021World Journal of Gastroenterology2021,27,26:2
12Production and activity of matrix metalloproteinases during liver fibrosis progression of chronic hepatitis C patients显示文摘BACKGROUND Matrix metalloproteinases(MMPs)participate in the degradation of extracellular matrix compounds,maintaining the homeostasis between fibrogenesis and fibrolytic processes in the liver.However,there are few studies on the regulation of liver MMPs in fibrosis progression in humans.AIM To assess the production activity and regulation of matrix metalloproteinases in liver fibrosis stages in chronic hepatitis C(CHC).METHODS A prospective,cross-sectional,multicenter study was conducted.CHC patients were categorized in fibrosis grades through FibroTest®and/or FibroScan®.Serum MMP-2,-7,and-9 were determined by western blot and multiplex suspension array assays.Differences were validated by the Kruskal-Wallis and Mann-Whitney U tests.The Spearman correlation coefficient and area under the receiver operating characteristic curve were calculated.Collagenolytic and gelatinase activity was determined through the Azocoll substrate and zymogram test,whereas tissue inhibitor of metalloproteinase-1 production was determined by dot blot assays.RESULTS Serum concentrations of the MMPs evaluated were higher in CHC patients than in healthy subjects.MMP-7 distinguished early and advanced stages,with a correlation of 0.32(P<0.001),and the area under the receiver operating characteristic displayed moderate sensitivity and specificity for MMP-7 in F4(area under the receiver operating characteristic,0.705;95%confidence interval:0.605-0.805;P<0.001).Collagenolytic activity was detected at F0 and F1,whereas gelatinase activity was not detected at any fibrosis stage.Tissue inhibitor of metalloproteinase-1 determination showed upregulation in F0 and F1 but downregulation in F2(P<0.001).CONCLUSION High concentrations of inactive MMPs were present in the serum of CHC patients,reflecting the impossibility to restrain liver fibrosis progression.MMPs could be good diagnostic candidates and therapeutic targets for improving novel strategies to reverse liver fibrosis in CHC.Moises Martinez-Castillo Abigail Hernandez-Barragan Ivonne Flores-Vasconcelos Marina Galicia-Moreno Dorothy Rosique-Oramas Jose Luis Perez-Hernandez Fatima Higuera-De la Tijera Eduardo E Montalvo-Jave Aldo Torre-Delgadillo Paula Cordero-Perez Linda Muñoz-Espinosa David Kershenobich Gabriela Gutierrez-Reyes 2021World Journal of Hepatology2021,13,2:2

14Alteration of chaperonin60 and pancreatic enzyme in pancreatic acinar cell under pathological condition显示文摘AIM: To investigate the changes of chaperonin60 (Cpn60)and pancreatic enzymes in pancreatic acinar cells, and to explore their roles in the development of experimental diabetes and acute pancreatitis (AP).METHODS: Two different pathological models were replicated in Sprague-Dawley rats: streptozotocininduced diabetes and sodium deoxycholate-induced AP. The contents of Cpn60 and pancreatic enzymes in different compartments of the acinar cells were measured by quantitative immunocytochemistry.RESULTS: The levels of Cpn60 significantly increased in diabetes, but decreased in AP, especially in the zymogen granules of the pancreatic acinar cells. The elevation of Cpn60 was accompanied with the increased levels of pancreatic lipase and chymotrypsinogen in diabetes.However, a decreased Cpn60 level was accompanied by high levels of lipase and chymotrypsinogen in AP.The amylase level was markedly reduced in both the pathological conditions.CONCLUSION: The equilibrium between Cpn60 and pancreatic enzymes in the acinar cells breaks in AP, and Cpn60 content decreases, suggesting an insufficient chaperone capacity.
