| 1 | APACHE II、BISAP、Ranson评分和改良CTSI对急性胰腺炎严重程度(基于Atlanta 2012分类)预测价值的比较显示文摘目的:本研究前瞻性比较了急性生理和慢性健康状况评分(APACHEⅡ)、急性胰腺炎严重程度床边指数(BISAP)、Ranson评分与改良CT严重度指数(CTSI)这四种方法,评估印度北部一家三甲医院急性胰腺炎患者疾病严重程度(基于Atlanta 2012分类)的准确性。方法:前瞻性纳入2015年3月至2016年9月间我院收治的50例急性胰腺炎患者,对所有入组病例进行APACHEⅡ、BISAP和Ranson评分,并基于胰腺对比增加CT进行改良CTSI评估。基于受试者工作特征(ROC)曲线评估这些评分系统预测急性胰腺炎严重程度的最佳阈值和曲线下面积(AUC),并对这些评分系统的预测价值进行前瞻性比较。结果:50例急性胰腺炎患者中14例被评估为重症急性胰腺炎。15例患者出现胰腺坏死,14例出现持续器官功能衰竭,14例需要收治重症监护室(ICU)。改良CTSI对重症急性胰腺炎、胰腺坏死、器官衰竭、入住ICU的预测价值均为最高,其AUC分别为0.919、0.993、0.893和0.993。APACHEⅡ对重症急性胰腺炎和器官衰竭的预测价值均仅次于改良CTSI,其AUC分别为0.934和0.931。APACHEⅡ对胰腺坏死、器官衰竭、入住ICU的预测具有较高的敏感性(分别为93.33%、92.86%和92.31%)和阴性预测值(分别为96.15%、96.15%和95.83%)。结论:APACHEⅡ是预测急性胰腺炎严重程度的一项有效的评估系统,有助于筛选出病情危重、需要早期复苏和尽快转诊上级医院的急性胰腺炎患者,尤其在一些医疗资源匮乏的发展中国家。 | Anubhav Harshit Kumar Mahavir Singh Griwan | 2018 | Gastroenterology Report2018,6,2: | 27 |
| 2 | Homocysteine mediates transcriptional changes of the inflammatory pathway signature genes in human retinal pigment epithelial cells显示文摘AIM:To test whether homocysteine(Hcy) can influence the transcriptional profile, we hypothesized that Hcy can lead to the induction of proinflammatory molecules in the retinal cells of aging people.METHODS:An unbiased in vitro inflammatory pathway focused study was designed employing retinal pigment epithelial(RPE) cell line, ARPE-19. Cells were cultured in the presence or absence of Hcy to capture target genes' expression profile. Three different concentrations of Hcy were added in the culture medium of confluent monolayers. c RNAs were made from the isolated total RNAs and the labeled c RNA probes were hybridized to microarrays specific for human disease pathway inflammatory cytokines, chemokines and their receptor gene micro-array panels as per manufacture's recommendations. Two Hcy up-regulated molecules:IL6 and CEBPB were further validated via Western blot analysis. Hcy's effect on ARPE-19 cellular morphology and genomic DNA integrity were also evaluated.RESULTS:Gene microarray analyses of RPE cells in response to Hcy treatment revealed alterations in the expressions of several inflammatory gene transcripts such as CCL5, CEBPB, IL13RA2, IL15 RA, IL6, IL8 and CXCL3 that were up-regulated. The transcripts for C3, CCL2, IL11 RA and IL18 genes exhibited down-regulation. The IL6 and CEBPB expressions were subsequently validated at the protein levels. Treatment of the retinal cells with increasing Hcy concentration influenced their density in culture however their morphology and DNA integrity remained unaffected. | 3 | A hypothesis for treating inflammation and oxidative stress with hydrogen sulfide during age-related macular degeneration显示文摘Age-related macular degeneration(AMD) is a leading cause of blindness and is becoming a global crisis since affected people will increase to 288 million by 2040.Genetics,age,diabetes,gender,obesity,hypertension,race,hyperopia,iris-color,smoking,sun-light and pyroptosis have varying roles in AMD,but oxidative stress-induced inflammation remains a significant driver of pathobiology.Eye is a unique organ as it contains a remarkable oxygengradient that generates reactive oxygen species(ROS) which upregulates inflammatory pathways.ROS becomes a source of functional and morphological impairments in retinal pigment epithelium(RPE),endothelial cells and retinal ganglion cells.Reports demonstrated that hydrogen sulfide(H_2S) acts as a signaling molecule and that it may treat ailments.Therefore,we propose a novel hypothesis that H_2S may restore homeostasis in the eyes thereby reducing damage caused by oxidative injury and inflammation.Since H_2S has been shown to be a powerful antioxidant because of its free-radicals' inhibition properties in addition to its beneficial effects in age-relatedconditions,therefore,patients may benefit from H_2S salubrious effects not only by minimizing their oxidant and inflammatory injuries to retina but also by lowering retinal glutamate excitotoxicity. | Akash K George Mahavir Singh Rubens Petit Homme Avisek Majumder Harpal S SANDhu Suresh C Tyagi | 2018 | International Journal of Ophthalmology(English edition)2018,11,5: | 3 |
| 4 | Genes and genetics in eye diseases: a genomic medicine approach for investigating hereditary and inflammatory ocular disorders显示文摘Past 25 y have witnessed an exponential increase in knowledge and understanding of ocular diseases and their respective genetic underpinnings. As a result, scientists have mapped many genes and their variants that can influence vision and health of our eyes. Based on these findings, it is becoming clear that an early diagnosis employing genetic testing can help evaluate patients' conditions for instituting treatment plan(s) and follow-up care to avoid vision complications later. For example, knowing family history becomes crucial for inherited eye diseases as it can benefit members in family who may have similar eye diseases or predispositions. Therefore, gathering information from an elaborate examination along with complete assessment of past medical illness by ophthalmologists followed by consultation with geneticists can help create a roadmap for making diagnosis and treatment precise and beneficial. In this review, we present an update on ocular genomic medicine that we believe has tremendous potential towards unraveling genetic implications in ocular diseases and patients' susceptibilities. | 5 | The role of tunica vaginalis flap in staged repair of hypospadias显示文摘Objective:The surgical repair of hypospadias is done in two stages in a select group of patients with severe anomaly.The first stage(Ⅰ)procedure consists of correction of penile shaft curvature and second stage(Ⅱ)repair involves the creation of a neourethra.This neourethra needs a cover of an intermediate layer in order to have good functional and cosmetic results.Among the various local flaps,tunica vaginalis flap is a good option for the use as an intermediate layer.Methods:We have managed 22 patients of chordee with hypospadias by staged repair.In Stage I,chordee correction was done by dividing the urethral plate and covering the penile shaft with dorsal prepucial flaps.In Stage Ⅱ,a neourethra was created and covered with tunica vaginalis flap either through the same incision(14/22)or via a subcutaneous tunnel(8/22).An indwelling catheter was kept for 10 to 12 days.Results:Eighteen(81.8%)patients had successful functional and cosmetic repair.Two patients(9.1%)had urethrocutaneous fistula of which one healed on subsequent dilatation while the other one(4.5%)needed repair.Overall fistula formation rate was 4.5%.In two patients,the external urinary meatus could be made upto subglanular or coronal level.Conclusion:Staged repair of chordee with hypospadias is valuable in selected group of patients and tunica vaginalis flap is an excellent intermediate layer to cover the neourethra.However preoperative counseling is particularly essential in patients where the external urinary meatus can be created at coronal or subglanular level. | Yogender S.Kadian Mahavir Singh Kamal N.Rattan | 2017 | Asian Journal of Urology2017,4,2: | 2 |
| 6 | Hydrogen sulfide intervention in cystathionine-β-synthase mutant mouse helps restore ocular homeostasis显示文摘AIM: To investigate the applications of hydrogen sulfide(H2 S) in eye-specific ailments in mice.METHODS: Heterozygous cystathionine-β-synthase(CBS+/-) and wild-type C57 BL/6 J(WT) mice fed with or without high methionine diet(HMD) were administered either phosphate buffered saline(PBS) or the slow-release H2 S donor: GYY4137. Several analyses were performed to study GYY4137 effects by examining retinal lysates for key protein expressions along with plasma glutamate and glutathione estimations. Intraocular pressure(IOP) was monitored during GYY4137 treatment; barium sulfate and bovine serum albumin conjugated fluorescein isothiocyanate(BSA-FITC) angiographies were performed for examining vasculature and its permeability post-treatment. Visionguided behavior was also tested employing novel object recognition test(NORT) and light-dark box test(LDBT) recordings.RESULTS: CBS deficiency(CBS+/-) coupled with HMD led disruption of methionine/homocysteine(Hcy) metabolism leading to hyperhomocysteinemia(HHcy) in CBS+/-mice as reflected by increased Hcy, and s-adenosylhomocysteine hydrolase(SAHH) levels. Unlike CBS, cystathionine-γ lyase(CSE), methylenetetrahydrofolate reductase(MTHFR) levels which were reduced but compensated by GYY4137intervention. Heightened oxidative and endoplasmic reticulum(ER) stress responses were mitigated by GYY4137 effects along with enhanced glutathione(GSH) levels. Increased glutamate levels in CBS+/-strain were prominent than WT mice and these mice also exhibited higher IOP that was lowered by GYY4137 treatment. CBS deficiency also resulted in vision-guided behavioral impairment as revealed by NORT and LDBT findings. Interestingly, GYY4137 was able to improve CBS+/-mice behavior together with lowering their glutamate levels. Blood-retinal barrier(BRB) appeared compromised in CBS+/-with vessels' leakage that was mitigated in GYY4137 treated group.
