| 1 | Measurements of dihadron correlations relative to the event plane in Au+Au collisions at√^(S)NN=200 GeV显示文摘Dihadron azimuthal correlations containing a high transverse momentum(pr)trigger particle are sensit-ive to the properties of the nuclear medium created at RHIC through the strong interactions occurring between the traversing parton and the medium,ie.jet-quenching.Previous measurements revealed a strong modification to di-hadron azimuthal correlations in Au+Au collisions with respect to ptp and d+Au collisions.The modification in-creases with the collision centrality,suggesting a path-length or energy density dependence to the je-quenching ef-fect.This paper reports STAR measurements of dihadron azimuthal correlations in mid-central(20%-60%)Au+Au collisions at√^(S)NN=200 GeV as a function of the trigger particle's azimuthal angle relative to the event plane,Ф_(s)=|Ф_(t)-ψ_(Ep)|.The azimuthal correlation is studied as a function of both the trigger and associated particle pr.The subtractions of the combinatorial background and anisotropic flow,assuming Zero Yield At Minimum(ZYAM),are described.The correlation results are first discussed with subtraction of the even harmonic(elliptic and quadrangu-lar)flow backgrounds.The away-side correlation is strongly modifed,and the modification varies withФ_(s),with a double-peak structure for out-of-plane trigger particles.The near-side ridge(long range pseudo-rapidity△_(η)correla-tion)appears to drop with increasingФ_(s)while the jet-like component remains approximately constant.The correla-tion functions are further studied with the subtraction of odd harmonic triangular flow background arising from fluc-tuations.It is found that the triangular flow,while responsible for the majority of the amplitudes,is not sufficient to explain theφs-dependence of the ridge or the away-side double-peak structure.The dropping ridge withФ_(s)could be attributed to aФ_(s)-dependent lliptie anisotropy;however,the physics mechanism of the ridge remains an open ques-tion.Even with aФ_(s)-dependent elliptic flow,the away-side correlation structure is robust.These results,with extens-ive systematic studies of the dihadron correlations as a function ofФ_(s),trigger and associated particle pT,and the pseudo-rapidity range△_(η),should provide stringent inputs to help understand the | 3 | An automated dispensing system for improving medication timing in the emergency department显示文摘BACKGROUND:Numerous medical conditions require timely medication administration in the emergency department(ED).Automated dispensing systems(ADSs)store premixed common doses at the point-of-care to minimize time to administration,but the use of such automation to improved time to medication administration has not been studied.Since vancomycin is a commonly used empiric antimicrobial,we sought to quantify the effect of using an ADS on time to drug delivery in patients presenting to the ED.The study aimed to determine the efficacy of utilizing an ADS to improve time to administration of vancomycin and determine any negative effects on dosing appropriateness.METHODS:The institional review board approved the retrospective quality improvement study took place in a single,urban academic tertiary care ED with an annual census of 80 000.Study subjects were all patients receiving vancomycin for the management of sepsis between March 1 to September 30,2008 and the same time period in 2009.The primary outcome was the proportion of patients who received vancomycin within one hour of bed placement and the secondary outcome was dosing appropriateness.RESULTS:Sixty-three patients had weight and dosing information available(29 before and 34after intervention)and were included in the study.Before intervention,no patient received vancomycin in less than 60 minutes,while after intervention 14.7%of the patients received it in less than 60minutes(difference in proportions 14.7%,95%CI 0.39%-30.0%,P=0.04).A similar proportion of the patients received correct dosing before and after intervention(44.8%vs.41.2%,difference in proportions 3.7%,95%CI-20.0%-26.7%,P=0.770).CONCLUSION:The use of an ADS may improve the timing of medication administration in patients presenting to the ED without affecting dosing appropriateness. | Michael J.Ward Jeremy S.Boyd Nicole J.Harger John M.Deledda Carol L.Smith Susan M.Walker Jeffrey D.Hice Kimberly W.Hart Christopher J.Lindsell Stewart W.Wright | 2012 | World Journal of Emergency Medicine2012,3,2: | 6 |

| 12 | The effects of trans-resveratrol on insulin resistance,inflammation,and microbiota in men with the metabolic syndrome:A pilot randomized,placebo-controlled clinical trial显示文摘Background and Aim:The metabolic syndrome(MetS)is a pathological condition comprised of abdominal obesity,insulin resistance,hypertension,and hyperlipidemia.It has become a major threat globally,resulting in rapidly increasing rates of diabetes,coronary heart disease,and stroke.The polyphenol resveratrol(RES)is believed to improve glucose homeostasis and insulin resistance by activating sirtuin,which acetylates and coactivates downstream targets and affects glucose and lipid homeostasis in the liver,insulin secretion in the pancreas,and glucose uptake in skeletal muscle.We studied the effects of RES on insulin resistance,glucose homeostasis,and concomitant effects on adipose tissue metabolism and fecal microbiota in insulin-resistant subjects with the MetS.Methods:A total of 28 obese men with the MetS were studied during a 35-day stay in the Rockefeller University Hospital metabolic unit.Subjects were randomized to receive RES 1 g orally twice daily or placebo while kept weight stable and consuming a western-style diet.At baseline,and after 30 days of RES or placebo administration,subjects underwent testing that included a euglycemic,hyperinsulinemic clamp,2-h oral glucose tolerance test(GTT),resting energy expenditure,daily blood pressure monitoring,abdominal adipose tissue biopsy,and fecal and blood collections.Results:RES induced no changes in insulin resistance but reduced the 120-min time point and the area under the curve for glucose concentration in the 2-h GTT.In post-hoc analysis,Caucasian subjects showed a significant improvement in insulin sensitivity and glucose homeostasis after GTT,whereas non-Caucasians showed no similar effects.Levels of fasting plasma RES and its primary metabolite dihydroresveratrol were variable and did not explain the racial differences in glucose homeostasis.RES administration to Caucasian subjects leads to an increase in several taxa including Akkermansia muciniphila.Conclusions:RES 2 g administered orally to obese men with MetS and insulin resistance marginally altered glucose homeostasis.However,in a small group of Caucasians,insulin resistance and glucose homeostasis improved.No concomitant changes in adipose tissue metabolism occurred,but fecal microbiota showed RES-induced changes.Relevance for Patients:The MetS increases the risk of diabetes,heart disease,and stroke.A major component of the syndrome is insulin resistance,resulting in systemic inflammation and hyperinsulinemia.The primary treatment consists of lifestyle changes,improved diet,and increased physical activity.This is often unsuccessful.In this study,RES was well tolerated.In Caucasian men,it significantly improved insulin sensitivity and glucose homeostasis.Similar results were found in studies that consisted exclusively of Caucasian men.However,RES presents a novel addition to the current treatment of the MetS and its sequelae. | Jeanne M.Walker Patricia Eckardt Jose OAleman Joel Correa da Rosa Yupu Liang Tadasu Iizumi Stephane Etheve Martin JBlaser Jan LBreslow Peter RHolt | 2018 | Journal of Clinical & Translational Research2018,4,2: | 1 |
