2帕博利珠单抗单用或与放疗联用治疗转移性非小细胞肺癌:两个随机试验的汇总分析显示文摘背景放疗可以提高整个机体对免疫治疗的应答。在Ⅱ期PEMBRO-RT研究和Ⅰ/Ⅱ期MDACC研究中,患有转移性非小细胞肺癌(NSCLC)的患者被随机分配入组,接受免疫治疗(帕博利珠单抗)+放疗联合疗法,或免疫治疗单一疗法。当上述2个研究单独分析时,联合疗法组显示出潜在获益。由于每个研究的样本量较小,缓解率和结局并未显示出统计学意义,然而却有显著的临床获益。因此,本研究进行汇总分析,来判断放疗是否会改善转移性NSCLC患者的免疫治疗应答。方法PEMBRO-RT和MDACC研究纳入标准:患者年龄≥18岁,患有转移性NSCLC,且有≥1处未经放疗照射的病灶,以便进行射野外应答监测。PEMBRO-RT研究纳入曾接受过化疗患者,MDACC研究纳入曾接受过治疗或新诊断患者。2个研究中的患者均未接受过免疫治疗。在PEMBRO-RT研究中患者被等比例随机分配入组,并根据吸烟状态进行分层(分为<10年组和≥10年组)。MDACC研究的患者根据放疗计划可行性被等比例随机分配入2个受试组。由于联合治疗组的干预本质,每个研究中的放疗均不适用盲法。在2个研究中,不论是否进行放疗,均静脉滴入帕博利珠单抗(每3周200 mg)。在PEMBRO-RT研究中,在放疗(24 Gy 3次分割照射)结束后1周给予第1剂帕博利珠单抗。在MDACC研究中,在第1次放疗(50 Gy 4次分割照射或45 Gy 15次分割照射)同时给予帕博利珠单抗。仅检测未经照射病灶的应答。本研究的终点为最佳射野外(远隔)应答率(ARR)、最佳射野外疾病控制率(ACR)、12周时ARR、12周时ACR、无进展生存期(PFS)和总生存期(OS)。2个研究的意向治疗(ITT)人群均纳入分析。PEMBRO-RT研究(NCT02492568)和MDACC研究(NCT02444741)均在ClinicalTrials.gov上注册。发现纳入148例患者,76例接受帕博利珠单抗治疗,72例接受帕博利珠单抗+放疗治疗。所有患者随访时间中位数为33个月[四分位距(IQR):32.4~33.6]。148例患者中124例(84%)组织学特征为非鳞癌,111例(75%)患者曾经接受过化疗。组间没有基线特征差异,包括PD-L1表达状态和转移灶体积。最常见的照射部位为肺转移灶(39%,28/72)、胸腔内淋巴结(21%,15/72)和非原发灶(17%,12/72)。帕博利珠单抗组和联合治疗组的最佳ARR分别为19.7%(15/76)和41.7%(30/72),OR=2.96,95%CI:1.42~6.20,P=0.0039;最佳ACR分别为43.4%(33/76)和65.3%(47/72),OR=2.51,95%CI:1.28~4.91,P=0.0071;PFS中位数分别为4.4(IQR:2.9~5.9)和9.0个月(IQR:6.8~11.2),HR=0.67,95%CI:0.45~0.99,P=0.045;OS中位数分别为8.7(IQR:6.4~11.0)和19.2个月(IQR:14.6~23.8),OR=0.67,95%CI:0.54~0.84,P=0.0004。在汇总分析中没有发现新的安全问题。解读帕博利珠单抗免疫疗法+放疗显著提高转移性NSCLC患者的应答和改善治疗结局。这些结果需要在三期临床试验中进行验证。陈大卫(翻译) 于金明(校对) Willemijn S M E Theelen Vivek Verma Brian P Hobbs Heike M U Peulen Joachim G J V Aerts Idris Bahce Anna Larissa N Niemeijer Joe Y Chang Patricia M de Groot Quynh-Nhu Nguyen Nathan I Comeaux George R Simon Ferdinandos Skoulidis Steven H Lin Kewen He Roshal Patel John Heymach Paul Baas James W Welsh 2021中华肿瘤防治杂志2021,28,24:49
3Tectonic evolution of the Dabieshan orogen: In the view from polyphase deformation of the Beihuaiyang metamorphic zone显示文摘The Beihuaiyang metamorphic zone was formed by the Early Mesozoic collision of the North and South China Blocks. It consisted of the Foziling and Luzhenguan complexes and was thought to have never afforded a deep subduction. A similar feature of the deformation in the Dabieshan UHP metamorphic rocks implies that these different tectonic units share the same geodynamic background. The early stage of deformation can be preserved on account of their relatively shallower subduction than that for UHP rocks. 4Up-regulation of PIK3CA promotes metastasis in gastric carcinoma显示文摘AIM:To explore expressions of PIK3CA in the progression of gastric cancer from primary to metastasis and its effects on activation of phosphatidylinositol 3-kinase(PI3K)/Akt pathway.METHODS:mRNA and protein levels of PIK3CA were assessed,respectively,by real-time quantitative polymerase chain reaction and immunohistochemistry in specimens of normal gastric mucosa,primary foci and lymph node and distant metastasis of gastric cancer.Akt and phosphorylated Akt protein were also examined by Western blotting in these tissues,in order to analyze the effect of PIK3CA expression level changes on the activation of PI3K/Akt signaling pathway.RESULTS:PIK3CA mRNA in lymph node metastasis were approximately 5 and 2 folds higher,respectively,than that in the corresponding normal gastric mucosa and primary gastric cancer tissues(P<0.05),while no statistical significance was found compared with distant metastasis.Immunohistochemically,PIK3CA protein expression was discovered in 7(35%)specimens of 20 primary foci vs 10(67%)of 15 of lymph node metastasis or 11(61%)of 18 of distant metastasis(35%vs 67%,P=0.015;35%vs 61%,P=0.044).With the increased level of PIK3CA expression,the total Akt protein expression remained almost unchanged,but p-Akt protein was upregulated markedly.CONCLUSION:Increased expression of PIK3CA is expected to be a promising indicator of metastasis in gastric cancer.Up-regulation of PIK3CA may promote the metastasis of gastric cancer through aberrant activation of PI3K/Akt signaling.Liu JF Zhou XK Chen JH Yi G Chen HG Ba MC Lin SQ Qi YC. 2010World Journal of Gastroenterology2010,16,39:20

7桑叶多糖对早期断奶仔猪生长性能、腹泻、血液指标和肠道菌群的影响显示文摘研究旨在探讨日粮补充桑叶多糖(MLPs)对早期断奶仔猪生长性能、腹泻、血液生化指标和肠道菌群的影响。共选取(28±2)日龄、体重为(9.18±0.46)kg的'杜洛克×长白×约克夏'杂交断奶仔猪150头,随机分成5个处理组,每组6个重复,每个重复5头猪。饮食处理如下:对照组(CT):基础日粮(BD);低剂量桑叶多糖组(LT):0.3 g·kg-1MLPs+BD;中剂量桑叶多糖组(MT):0.6 g·kg-1MLPs+BD;高剂量桑叶多糖组(HT):0.9 g·kg-1MLPs+BD;抗生素处理组(AT):0.15 g·kg-1金霉素+BD。分别在第0、10、21天的早晨,每头猪空腹测量体重并记录采食量,这些数据用来计算平均日增重(ADG)、平均日采食量(ADFI)和料重比(F/G)。每天两次(上午和下午)对每头仔猪的腹泻状况进行检查并记录。在试验结束时,收集血液样本用于生化分析,并收集盲肠、结肠和直肠内容物进行识别和量化相关的肠道菌群。研究结果表明,MLPs处理组间的日增重、平均日采食量和料重比无显著差异(P>0.05),但低剂量组(LT)、中剂量组(MT)、高剂量组(HT)的平均日采食量高于对照组(CT)和抗生素组(AT)。并且,LT、MT和HT组的料重比低于CT和AT组。与CT和AT组相比,LT、MT和HT组仔猪的腹泻发病率也显著降低(P<0.05)。生化分析表明,MLPs各处理组仔猪的血糖(GLU)水平均显著低于CT或AT组(P<0.05)。MT和HT组血液尿素氮(BUN)水平与CT和AT组存在显著差异(P<0.05),HT组T3水平显著高于CT和AT组(P<0.05)。虽然MLPs各处理组的T4水平与AT组并没有显著差异(P>0.05),但MT组的胰岛素样生长因子Ⅰ(IGF-Ⅰ)和生长激素(GH)水平较CT或AT组均显著升高(P<0.05)。LT、MT和HT 3组在抑制肠道大肠杆菌生长和促进肠道乳酸杆菌和双歧杆菌生长的整体效果上均优于CT和AT组。值得注意的是,与CT和AT组相比,HT组(0.9 g·kg-1MLPs)对抑制肠道大肠杆菌以及促进肠道有益菌群具有更显著效果(P<0.05)。结果表明,日粮添加桑叶多糖可以改善早期断奶仔猪肠道菌群的生态环境,降低腹泻率,提高整体生长性能。Zhao X J Lin L Luo Q L Ye M Q Luo G Q Kuang Z S 2015饲料博览2015,0,7:7
8Engineering osteoarthritic cartilage model through differentiating senescent human mesenchymal stem cells for testing disease-modifying drugs显示文摘Significant cellular senescence has been observed in cartilage harvested from patients with osteoarthritis(OA).In this study,we aim to develop a senescence-relevant OA-like cartilage model for developing disease-modifying OA drugs(DMOADs).Spe-cifically,human bone marrow-derived mesenchymal stromal cells(MSCs)were expanded in vitro up to passage 10(P10-MSCs).Following their senescent phenotype formation,P10-MSCs were subjected to pellet culture in chondrogenic medium.Results from qRT-PCR,histology,and immunostaining indicated that cartilage generated from P10-MSCs displayed both senescent and OA-like phenotypes without using other OA-inducing agents,when compared to that from normal passage 4(P4)-MSCs.Interestingly,the same gene expression differences observed between P4-MSCs and P10-MSC-derived cartilage tissues were also observed between the preserved and damaged OA cartilage regions taken from human samples,as demonstrated by RNA sequencing data and other analysis methods.Lastly,the utility of this senescence-initiated OA-like cartilage model in drug development was assessed by testing several potential DMOADs and senolytics.The results suggest that pre-existing cellular senescence can induce the generation of OA-like changes in cartilage.The P4-and P10-MSCs derived cartilage models also represent a novel platform for predicting the efficacy and toxicity of potential DMOADs on both preserved and damaged cartilage in humans.Ning Wang Yuchen He Silvia Liu Meagan J.Makarcyzk Guanghua Lei Alexander Chang Peter G Alexander Tingjun Hao Anne-Marie Padget Nuria de Pedro Tsapekos Menelaos Hang Lin 2022Science China(Life Sciences)2022,65,2:5

11A Study on 3D FE Simulation Method of NC Bending Process of Thin-walled Tube显示文摘The numerical control (NC) precision bending process of thin-walled tube is on e of advanced plastic forming processes with high efficiency, forming precision, strength/weight ratio and low cost, thus it is playing more and more important role in manufacturing parts in aerospace and automobile industries. However, the determination of parameters crucial to make sure tube parts qualified is heavil y experience-based and involves repeated trial-and-errors in practice, which makes the production efficiency reduce drastically and does not fulfill the deve lopment of high technology. With quick development of computer technology and gr adual perfect of plastic forming theory, computer numerical simulation based on finite element method (FEM) has become one of important tools of researching and developing plastic forming technology. Development trend of NC precision bendin g process of tube is simulating its forming process by FEM. Because NC tube bend ing is of 3D nature, it is of great importance to analyze the forming mechanism and find out the influence law of forming parameters on forming process in the N C precision bending process of thin-walled tube quantitatively by 3D FE simulat ion. Based on the rigid-plastic finite element method (FEM) principle, a 3-dimens ional (3D) rigid-plastic FE simulation system named TBS -3D (tube bending simu lation by 3D FEM) for the NC bending process of thin-walled tube has been devel oped, a reasonable FEM model has been established. By use of this FEM simulation system, a NC bending process of thin-walled has been simulated. And deformed m eshes under different bending stages, stress distribution along bending directio n, relationship between maximal wall thickness changing ratio and bending angle have been obtained. And then some forming laws of NC tube bending obtained are a s follows: (1) NC bending process make tube elongate to some extent; (2) Charact eristic of stress distribution is that the outer area is undergoing tensile stre ss, the inner area is undergoing compression stress, and stress neutral layer mo ves close to the inner area, which is in good accordance with the practice; (3) Maximal wall thinning ratio in the outer tensile area changes only a little with increase of bending angle, and maximal wall thickening ratio in the inner compr ession area increases linearly with bending angle. 14Delaying surgery after neoadjuvant chemoradiotherapy improves prognosis of rectal cancer显示文摘AIM To investigate the prognostic effect of a delayed interval between neoadjuvant chemoradiotherapy(CRT) and surgery in locally advanced rectal cancer.METHODS We evaluated 87 patients with locally advanced mid-or distal rectal cancer undergoing total mesorectal excision following an interval period after neoadjuvant CRT at ?i?li Hamidiye Etfal Training and Research Hospital,Istanbul between January 2009 and January 2014.Patients were divided into two groups according to the intervalbefore surgery: < 8 wk(group Ⅰ) and ≥ 8 wk(group Ⅱ).Data related to patients,cancer characteristics and pathological examination were collected and analyzed.RESULTS When the distribution of timing between group Ⅰ(n = 45) and group Ⅱ(n = 42) was viewed,comparison of interval periods(median ± SD) of groups showed a significant difference of as 5 ± 1.28 wk in group Ⅰ and 10.1 ± 2.2 wk in group Ⅱ(P < 0.001).The median follow-up period for all patients was 34.5(9.9-81) mo.group Ⅱ had significantly higher rates of pathological complete response(p CR) than group Ⅰ had(19% vs 8.9%,P = 0.002).Rate of tumor regression grade(TRG) poor response was 44.4% in group Ⅰ and 9.5% in group Ⅱ(P < 0.002).A poor pathological response was associated with worse disease-free survival(P = 0.009).The interval time did not show any association with local recurrence(P = 0.79).CONCLUSION Delaying the neoadjuvant CRT-surgery interval may provide nodal down-staging,improve p CR rate,and decrease the rate of TRG poor response.Mehmet Mihmanli Esin Kabul Gürbulak Ismail Ethem Akgün Mustafa Fevzi Celayir Pinar Yazici Deniz Tuncel Tuba Tülin Bek Ayhan Oz Sinan Omeroglu 2016World Journal of Gastrointestinal Oncology2016,8,9:3

16Tumor necrosis factor-α-induced protein 1 and immunity to hepatitis B virus显示文摘AIM: To compare the gene expression profile in a pair of HBV-infected twins.METHODS: The gene expression profile was compared in a pair of HBV-infected twins.RESULTS: The twins displayed different disease outcomes. One acquired natural immunity against HBV,whereas the other became a chronic HBV carrier. Eightyeight and forty-six genes were found to be up- or downregulated in their PBMCs, respectively. Tumor necrosis factor-alpha-induced protein 1 (TNF-αIP1) that expressed at a higher level in the HBV-immune twins was identified and four pairs of siblings with HBV immunity by RTPCR. However, upon HBV core antigen stimulation,TNF-αIP1 was downregulated in PBMCs from subjects with immunity, whereas it was slightly upregulated in HBV carriers. 19全颗粒饲料对奶牛生产性能的影响显示文摘饲料中的中性洗涤纤维(NDF)含量和饲料颗粒直径是影响瘤胃功能的重要因素。试验评估了以颗粒饲料为主的奶牛瘤胃健康情况、NDF消化率和生产性能。选取8头荷斯坦母牛(336±30 d,体重346±35 kg),随机分配到交叉重复试验组中,奶牛安置于牛栏,自由采食。试验期为3周,前两周是适应期,最后一周为数据收集期。各组饲粮原料相同,但物理形式不同,分为全混合日粮(TMR)和颗粒料(直径=8 mm)。两组间物理有效NDF(pe NDF)不同,TMR组为39.8%,颗粒料组为11.8%。在试验过程中,干物质采食量(DMI)、饮水量、瘤胃反刍时间、瘤胃温度和pH情况均记录。3周后收集粪样,确定潜在的可消化NDF总消化率(pdNDF)。每阶段结束后计算平均日增重和饲料转化率。颗粒料组的DMI、DMI/体重和饮水量更高。而两组间平均日增重和饲料转化率无显著差异。颗粒料组的瘤胃反刍时间为241 min·d-1低于TMR组的507 min·d-1。饲料的不同物理外形对瘤胃温度、瘤胃pH无显著影响。TMR组的pdNDF总消化率为90.25%高于颗粒料组的86.82%。试验结果表明,动物易接受完整的颗粒饲料,DMI较高,且反刍时间减少,但瘤胃pH无显著变化。TMR组的pdNDF消化率显著增加。尽管两组奶牛在DMI和纤维消化方面表现不同,但总体表现相似,两组饲料的保存时间与其物理形式有关。在短期内,提供能满足NDF需求的完全颗粒饲料可改善瘤胃健康状况和动物生产性能,而较长时间的数据则需要进一步试验。Wang J Yang M Cao M Lin Y Che L Q Duraipandiyan V Al-Dhabi N A Fang Z F Xu S Y Feng B Liu G Wu D Bonfante E Palmonari A Mammi L Canestrari G Fustini M Formigoni A 2016饲料博览2016,0,12:2

combinations are proposed where necessary to improve our traditional species concepts and provide updates on their classification.In addition to extensive morphological descriptions and appropriate asexual and sexual connections,DNA sequence data are also analysed from concatenated datasets(rDNA,TEF-a,RBP2 and b-Tubulin)to infer phylogenetic relationships and substantiate systematic position of taxa within appropriate ranks.Wherever new species or combinations are being proposed,we apply an integrative approach(morphological and molecular data as well as ecological features wherever applicable).Notes on 125 fungal taxa are compiled in this paper,including eight new genera,101 new species,two new combinations,one neotype,four reference specimens,new host or distribution records for eight species and one alternative morphs.The new genera introduced in this paper are Alloarthopyrenia,Arundellina,Camarosporioides,Neomassaria,Neomassarina,Neotruncatella,Paracapsulospora and
