| 2 | Evolution of transanal total mesorectal excision for rectal cancer:From top to bottom显示文摘The gold standard for curative treatment of locally advanced rectal cancer involves radical resection with a total mesorectal excision(TME). TME is the most effective treatment strategy to reduce local recurrence and improve survival outcomes regardless of the surgical platform used. However, there are associated morbidities, functional consequences, and quality of life(QoL) issues associated with TME; these risks must be considered during the modern-day multidisciplinary treatment for rectal cancer. This has led to the development of new surgical techniques to improve patient, oncologic, and QoL outcomes. In this work, we review the evolution of TME to the transanal total mesorectal excision(TaTME) through more traditional minimally invasive platforms. The review the development, safety and feasibility, proposed benefits and risks of the procedure, implementation and education models, and future direction for research and implementation of the TaTME in colorectal surgery. | 9 | Pseudocirrhosis in a pancreatic cancer patient with liver metastases: A case report of complete resolution of pseudocirrhosis with an early recognition and management显示文摘We report a case of pseudocirrhosis arising in the setting of regression of liver metastases from pancreatic cancer. A 55-year-old asymptomatic woman presented to our clinic with newly diagnosed metastatic pancreatic cancer with extensive liver metastases. She underwent systemic chemotherapy with gemcitabine and oxaliplatin (GEMOX). After 8 cycles of therapy, she had a remarkable response to the therapy evidenced by decline of carcinoembryonic antigen (CEA) and CA19 by > 50% and nearly complete resolution of hepatic metastases in computed tomography (CT) scan. Shortly after, she developed increasing bilateral ankle edema and ascites, associated with dyspnea, progressive weight gain, and declining performance status. Gemcitabine and oxaliplatin were discontinued as other causes of her symptoms such as congestive heart disease or venous thrombosis were ruled out. CT scan 6 mo after the initiation of GEMOX revealed worsening ascites with a stable pancreatic mass. However, it also revealed a lobular hepatic contour, segmental atrophy, and capsular retraction mimicking the appearance of cirrhosis. She was managed with aggressive diuresis and albumin infusions which eventually resulted in a resolution of the above- mentioned symptoms as well as complete resolution of pseudocirrhotic appearance of the liver and ascites in CT scan. This case demonstrates that pancreatic cancer patients can develop pseudocirrhosis. | 10 | Modeling the influence of ethanol on the adsorption and desorption of selected BTEX compounds on bentonite and kaolin显示文摘The influence of ethanol on the adsorption capacity and desorption kinetics of benzene and toluene on bentonite and kaolin through modeling and experimental study was investigated.The results showed that the adsorption capacity of both soils for the target compounds decreased as ethanol content increased.As ethanol content increased from 0 to 50%,the adsorption capacity for benzene and toluene on bentonite decreased from 3.6 to 0.54 μg n+1 /(L n ·g) (by 85%) and 1.91 to 0.01 μg n+1 /(L n ·g) (by 99.5%),respectively.For benzene and toluene adsorption on kaolin,the adsorption capacity decreased by 86.5% (from 0.26 to 0.04 μg n+1 /(L n ·g) and 98.2% (from 0.13 to 0.002 μg n+1 /(L n ·g)),respectively,as ethanol content increased from 0 to 50%.In addition,the desorption rate of benzene and toluene from bentonite decreased by about one order of magnitude as the ethanol increased from 0 to 25% and 0 to 50%,respectively.It can be inferred that ethanol could affect the effectiveness of natural attenuation processes that rely on adsorption to soils as a containment technique for benzene and toluene by retarding the adsorption to soils and remobilizing compounds that had already been adsorbed to soils. | Temesgen Garoma Lacy Skidmore | 2011 | Journal of Environmental Sciences2011，23，11： | 4 |
| 11 | Gene Expression Profiling of Human Epidermal Keratinocytes in Simulated Microgravity and Recovery Cultures显示文摘Simulated microgravity (SMG) bioreactors and DNA microarray technology are powerful tools to identify 'space genes' that play key roles in cellular response to microgravity. We applied these biotechnology tools to investigate SMG and post-SMG recovery effects on human epidermal keratinocytes by exposing cells to SMG for 3, 4, 9, and 10 d using the high aspect ratio vessel bioreactor followed by recovery culturing for 15, 50, and 60 d in normal gravity. As a result, we identified 162 differentially expressed genes, 32 of which were 'center genes' that were most consistently affected in the time course experiments. Eleven of the center genes were from the integrated stress response pathways and were coordinately down-regulated. Another seven of the center genes, which are all metallothionein MT-I and MT-II isoforms, were coordinately up-regulated. In addition, HLA-G, a key gene in cellular immune response suppression, was found to be significantly up-regulated during the recovery phase. Overall, more than 80% of the differentially expressed genes from the shorter exposures (≤4 d) recovered in 15 d; for longer (≥9 d) exposures, more than 50 d were needed to recover to the impact level of shorter exposures. | 14 | Gastrointestinal Kaposi's sarcoma: Case report and review of the literature显示文摘Kaposi's sarcoma(KS) of the gastrointestinal tract is not an uncommon disease among individuals with acquired immunodeficiency syndrome(AIDS). The majority is asymptomatic, and for this reason, gastrointestinal KS(GI-KS) remains undiagnosed. With continued tumor growth, considerable variation in clinical presentation occurs including abdominal pain, nausea, vomiting, iron deficiency anemia(either chronic or frank gastrointestinal bleeding), and rarely mechanical obstruction alone or combined with bowel perforation. Endoscopy with biopsy allows for histological and immunohistochemical testing to confirm the diagnosis of GI-KS among those with clinical symptoms. In previous studies, dual treatment with highly active antiretroviral therapy and systemic chemotherapy have been associated with improved morbidity and mortality in individuals with visceral KS. Therefore, investigators have suggested performing screening endoscopies in select patients for early detection and treatment to improve outcome. In this review, we describe a 44 years old man with AIDS and cutaneous KS who presented for evaluation of postprandial abdominal pain, vomiting, and weight loss. On upper endoscopy, an extensive, infiltrative,circumferential, reddish mass involving the entire body and antrum of the stomach was seen. Histologic examination later revealed spindle cell proliferation, and confirmatory immunohistochemical testing revealed human herpes virus 8 latent nuclear antigen expression consistent with a diagnosis of gastric KS.
