1Management of pancreatic fluid collections: A comprehensive review of the literature显示文摘Pancreatic fluid collections(PFCs) are a frequent complication of pancreatitis. It is important to classify PFCs to guide management. The revised Atlanta criteria classifies PFCs as acute or chronic, with chronic fluid collections subdivided into pseudocysts and walled-off pancreatic necrosis(WOPN). Establishing adequate nutritional support is an essential step in the management of PFCs. Early attempts at oral feeding can be trialed in patients with mild pancreatitis. Enteral feeding should be implemented in patients with moderate to severe pancreatitis. Jejunal feeding remains the preferred route of enteral nutrition. Symptomatic PFCs require drainage; options include surgical, percutaneous, or endoscopic approaches. With the advent of newer and more advanced endoscopic tools and expertise, and an associated reduction in health care costs, minimally invasive endoscopic drainage has become the preferable approach. An endoscopic ultrasonography-guided approach using a seldinger technique is the preferred endoscopic approach. Both plastic stents and metal stents are efficacious and safe; however, metal stents may offer an advantage, especially in infected pseudocysts and in WOPN. Direct endoscopic necrosectomy is often required in WOPN. Lumen apposing metal stents that allow for direct endoscopic necrosectomy and debridement through the stent lumen are preferred in these patients. Endoscopic retrograde cholangio pancreatography with pancreatic duct(PD) exploration should be performed concurrent to PFC drainage. PD disruption is associated with an increased severity of pancreatitis, an increased risk of recurrent attacks of pancreatitis and long-term complications, and a decreased rate of PFC resolution after drainage. 2Immunogenicity and reactogenicity of a recombinant hepatitis B vaccine in subjects over age of forty years and response of a booster dose among nonresponders显示文摘AIM: The study was initiated to evaluate the reactogenicity and immunogenicity of a recombinant hepatitis B vaccine in age group >40 years and to study the response of a single booster dose in primary non-responders to the hepatitis B vaccination.METHODS: A total of 102 volunteers without markers of hepatitis B infection (negative for HBsAg, anti-HBc antibody,HBeAg and anti-HBs antibody) received 20 pg of recombinant HB vaccine intramuscularly at 0, 1, and 6 months. Anti HBs titers were evaluated by a quantitative Elisa kit at 90 and 210 days. A booster dose of 20 pg HB vaccine was given after 6 months of the 3rd vaccine dose to the 15 nonresponders and anti-HBs titers were measured after 1 month.RESULTS: Seroprotection (anti-HBs GMT3 10 IU/L) was achieved in 85.3 % (87/102) volunteers. The mean GMT titers of the vaccine responders was 136.1 IU/L. Of the seroprotected individuals, there were 32.4 % (33/102) hyporesponders (antiHBs titers <10-99 mlU/ml) and 52.9 % (54/102) were responders (anti-HBs titers >100 IU/L). All the non-responders (15/15) responded to a single dose of the booster dose of recombinant HB vaccine and their mean anti-HBs antibody titers were more than 100.5 mIU/ml after the booster dose.CONCLUSION: Recombinant hepatitis B vaccine offers good seroprotection in the age group >40 years and has a good safety profile. A single booster dose after 6 months in primary non-responders leads to good seroprotective anti-HBs antibody titers. 3Lipid abnormalities in kidney disease and management strategies显示文摘Patients with kidney diseases continue to experience significant cardiovascular disease(CVD) morbidity and mortality. Although there are many important risk factors playing a role in the pathogenesis of CVD in chronic kidney disease(CKD) patients, dyslipidemia(elevated triglycerides, elevated oxidized low-densitylipoprotein and low/dysfunctional low high-density) represents one of the modifiable risk factors. Renal failure patients have unique lipid abnormalities which not only have complex role in pathogenesis of CVD but also cause relative resistance to usual interventions. Most of the randomized trials have been in hemodialysis population and data from CKD non-dialysis, peritoneal dialysis and renal transplant populations is extremely limited. Compared to general population, evidence of mortality benefit of lipid lowering medications in CKD population is scarce. 4Effect of sample size on the fluid flow through a single fractured granitoid显示文摘Most of deep geological engineered structures,such as rock caverns,nuclear waste disposal repositories,metro rail tunnels,multi-layer underground parking,are constructed within hard crystalline rocks because of their high quality and low matrix permeability.In such rocks,fluid flows mainly through fractures.Quantification of fractures along with the behavior of the fluid flow through them,at different scales,becomes quite important.Earlier studies have revealed the influence of sample size on the confining stress-permeability relationship and it has been demonstrated that permeability of the fractured rock mass decreases with an increase in sample size.However,most of the researchers have employed numerical simulations to model fluid flow through the fracture/fracture network,or laboratory investigations on intact rock samples with diameter ranging between 38 mm and 45 cm and the diameter-to-length ratio of 1:2 using different experimental methods.Also,the confining stress,σ_3,has been considered to be less than 30 MPa and the effect of fracture roughness has been ignored.In the present study,an extension of the previous studies on 'laboratory simulation of flow through single fractured granite' was conducted,in which consistent fluid flow experiments were performed on cylindrical samples of granitoids of two different sizes(38 mm and 54 mm in diameters),containing a 'rough walled single fracture'.These experiments were performed under varied confining pressure(σ_3=5-40 MPa),fluid pressure(f_p ≤25 MPa),and fracture roughness.The results indicate that a nonlinear relationship exists between the discharge,Q,and the effective confining pressure,σ_(eff),and Q decreases with an increase in σ_(eff).Also,the effects of sample size and fracture roughness do not persist when σ_(eff)≥ 20 MPa.It is expected that such a study will be quite useful in correlating and extrapolating the laboratory scale investigations to in-situ scale and further improving theoretical/numerical models associated with fluid flow through rock masses.Kunal Kumar Singh Devendra Narain Singh Ranjith Pathegama Gamage 2016Journal of Rock Mechanics and Geotechnical Engineering2016,8,3:5
5Development of Cardanol-Based Polyol via Click Chemistry and Crosslinking with Melamine Formaldehyde Resin for Coating Applications显示文摘The research work presented in this article deals with the synthesis of cardanol-based polyol and its curing with hexabutoxymethyl melamine(HBMM)for application in coatings.Cardanol-based polyol was prepared via thiol-ene click reaction using thioglycerol.Unsaturation present in the long chain of cardanol was successfully utilized to synthesize polyol via thiol-ene coupling.The reaction was carried out between cardanol and thioglycerol in the presence of Irgacure 184(photoinitiator)and 1,8-Diazabicyclo[5.4.0]undec-7-ene(catalyst)under UV light for 12 h at 80°C.After completion of the reaction,one mole of thioglycerol was successfully added across the double bond of a fatty chain of cardanol and confirmed by Fourier transform infrared spectroscopy(FTIR)and proton nuclear magnetic resonance spectroscopy and hydroxyl and iodine values were determined.Furthermore,the polyol thus prepared was cured with commercial HBMM in various proportions,such as 1:0.6,1:0.8 and 1:1,on an equivalent basis.The coatings were then characterized for mechanical,chemical,optical,thermal and anticorrosive properties.It was observed that coatings exhibited excellent performance properties as compared to that of its acrylic counterpart.Kunal Wazarkar Anagha Sabnis 2018Journal of Renewable Materials2018,6,4:4

7Hybrid polymer biomaterials for bone tissue regeneration显示文摘Native tissues possess unparalleled physiochemical and biological functions, which can be attributed to their hybrid polymer composition and intrinsic bioactivity. However, there are also various concerns or limitations over the use of natural materials derived from animals or cadavers, including the potential immunogenicity, pathogen transmission, batch to batch consistence and mismatch in properties for various applications. Therefore, there is an increasing interest in developing degradable hybrid polymer biomaterials with controlled properties for highly efficient biomedical applications. There have been efforts to mimic the extracellular protein structure such as nanofibrous and composite scaffolds, to functionalize scaffold surface for improved cellular interaction, to incorporate controlled biomolecule release capacity to impart biological signaling, and to vary physical properties of scaffolds to regulate cellular behavior. In this review, we highlight the design and synthesis of degradable hybrid polymer biomaterials and focus on recent developments in osteoconductive, elastomeric, photoluminescent and electroactive hybrid polymers. 8Ayurvedic drug induced liver injury显示文摘Drug induced liver injury is responsible for 50% of acute liver failure in developed countries. Ayurvedic and homeopathic medicine have been linked to liver injury. This case describes the first documented case of Punarnava mandur and Kanchnar guggulu causing drug induced liver injury. Drug induced liver injury may be difficult to diagnosis, but use of multi-modalities tools including the ACG algorithms, causative assessment scales, histological findings, and imaging, is recommended. 11Cardioprotective activity of alcoholic extract of Tinospora cordifolia (Willd.) Miers in calcium chloride-induced cardiac arrhythmia in rats显示文摘The present study investigated the antiarrhythmic activity of alcoholic extract of Tinospora cordifolia (T.cordifolia) in CaCl 2 induced arrhythmia.CaCl 2 (25 mg/kg) was administered by intravenous infusion (iv) to produce arrhythmia in rats.The animals were then treated with T.cordifolia extract (150,250,and 450 mg/kg) and verapamil (5 mg/kg,iv).Lead II electrocardiogram was monitored.Plasma calcium,sodium and potassium levels were measured.In CaCl 2 induced arrhythmia,heart rate was decreased by 41.10%,T.cordifolia at 150,300,and 450 mg/kg decreased the heart rate by 26.30%,29.16%,and 38.29%,respectively,and verapamil reduced the heart rate by 9.70% compared to the normal group.The PQRST waves were normalized and atrial and ventricular fibrillation was controlled in rats treated with verapamil and T.cordifolia.CaCl 2 increased calcium and sodium levels and decreased potassium levels in blood.T.cordifolia dose-dependently decreased calcium and sodium levels and increased potassium levels.Hence,T.cordifolia can be used in antiarrhythmic clinical settings and beneficial in atrial and ventricular fibrillation and flutter and may be indicated in ventricular tachyarrhythmia.Ashish Kumar Sharma Kunal Kishore Divya Sharma B.P Srinivasan Shyam Sunder Agarwal Ashok Sharma Santosh Kumar Singh Samir Gaur Vijay Singh Jatav 2011The Journal of Biomedical Research2011,25,4:2
