1Chemokine expression in hepatocellular carcinoma versus colorectal liver metastases显示文摘AIM: To evaluate and compare the expression profiles of CXCL12 (SDF-1), CCL19 (MIP-3β), CCL20 (MIP-3α) and CCL21 (6Ckine, Exodus2) and their receptors on RNA and protein levels in hepatocellular carcinoma (HCC) versus colorectal liver metastases (CRLM) and to elucidate their impact on the carcinogenesis and progression of malignant liver diseases. METHODS: Chemokine expression was analyzed by RT-PCR and ELISA in 11 cases of HCC specimens and in 23 cases of CRLM and corresponding adjacent non- tumorous liver tissues, respectively. Expressions of their receptors CXCR4, CCR6 and CCR7 were analyzed by RT- PCR and Western blot analysis in the same cases of HCC and CRLM. RESULTS: Significant up-regulation for CCL20/CCR6 was detected in both cancer types. Moreover, CCL20 demonstrated significant overexpression in CRLM in relation to the HCC tissues. Being significantly up- regulated only in CRLM, CXCR4 displayed an aberrant expression pattern with respect to the HCC tissues. CONCLUSION: Correlation of CXCR4 expression with CRLM suggests CXCR4 as a potential predictive factor for CRLM. 2藏西南札达盆地磁性地层学特征及其构造意义显示文摘札达盆地是喜马拉雅山北坡一个重要的晚新生代盆地,对盆地地层的年代学研究将为反演喜马拉雅造山带隆升与构造演化提供许多重要的信息.盆地新生代地层厚近750m,共采集古地磁定向样品268块.系统古地磁测量表明岩石经热退磁显示大多数样品在500~690℃之间可以获得稳定的特征剩磁,实测极性柱与标准极性柱对比表明该套地层的古地磁年龄为9.5~2.6Ma.盆地的沉积地层年龄不支持藏南拆离系作为控盆构造的假说,结合盆地内沉积构造特征及盆地北缘活动构造特征,认为盆地形成受喀喇昆仑断裂活动控制.王世锋 张伟林 方小敏 戴霜 Oliver Kempf 2008科学通报2008,53,6:12
3Magnetostratigraphy of the Zanda basin in southwest Tibet Plateau and its tectonic implications显示文摘The Zanda basin is one of the very important basins at the north slope of the Himalaya Range. Thus the study of the basin strata will provide critical information about the tectonic evolution of the Himalayan Orogenic Belt. 268 oriented block samples were collected in the 750-m-thick sections of the Zanda ba- sin. The characteristic remanent magnetization (ChRM) was isolated that decays linearly to the origin between 500oC and 690oC for most studied samples. An age range of 9.5―2.6 Ma was estimated from the correlation between our observed polarity column and the Geomagnetic Polarity Time Scale (GPTS). The age of the Zanda basin does not support the models that the South Tibetan Detach system (STDS) is one of the basin controlling faults. 4Influence of antibiotic-regimens on intensive-care unit-mortality and liver-cirrhosis as risk factor显示文摘AIM: To assess the rate of infection, appropriateness of antimicrobial-therapy and mortality on intensive care unit(ICU). Special focus was drawn on patients with liver cirrhosis.METHODS: The study was approved by the local ethical committee. All patients admitted to the Internal Medicine-ICU between April 1, 2007 and December 31, 2009 were included. Data were extracted retrospectively from all patients using patient charts and electronic documentations on infection, microbiological laboratory reports, diagnosis and therapy. Due to the large hepatology department and liver transplantation center, special interest was on the subgroup of patients with liver cirrhosis. The primary statistical-endpoint was the evaluation of the influence of appropriate versusinappropriate antimicrobial-therapy on in-hospitalmortality.RESULTS: Charts of 1979 patients were available. The overall infection-rate was 53%. Multiresistantbacteria were present in 23% of patients with infection and were associated with increased mortality(p < 0.000001). patients with infection had significantly increased in-hospital-mortality(34% vs 17%, p < 0.000001). Only 9% of patients with infection received inappropriate initial antimicrobial-therapy, no influence on mortality was observed. Independent risk-factors for in-hospital-mortality were the presence of septicshock, prior chemotherapy for malignoma and infection with pseudomonas spp. Infection and mortality-rate among 175 patients with liver-cirrhosis was significantly higher than in patients without liver-cirrhosis. Infection increased mortality 2.24-fold in patients with cirrhosis. patients with liver cirrhosis were at an increased risk to receive inappropriate initial antimicrobial therapy.CONCLUSION: The results of the present study report the successful implementation of early-goal-directed therapy. Liver cirrhosis patients are at increased risk of infection, mortality and to receive inappropriate therapy. 10Severe infection with multidrug-resistant Salmonella choleraesuis in a young patient with primary sclerosing cholangitis显示文摘Massive global spread of multidrug-resistant(MDR) Salmonella spp. expressing extended-spectrum beta-lactamase(ESBL) and additional resistance to fluoroquinolones has often been attributed to high international mobility as well as excessive use of oral antibiotics in livestock farming. However, MDR Salmonella spp. have not been mentioned as a widespread pathogen in clinical settings so far. We demonstrate the case of a 25-year-old male with primary sclerosing cholangitis who tested positive for MDR Salmonella enterica serotype Choleraesuis expressing ESBL and fluoroquinolone resistance. The pathogen was supposedly acquired during a trip to Thailand, causing severe fever, cholangitis andpancreatitis. To our knowledge, this is the first report of Salmonella enterica serotype Choleraesuis in Europe expressing such a multidrug resistance pattern.
