1Functional interaction of endoplasmic reticulum stress and hepatitis B virus in the pathogenesis of liver diseases显示文摘Hepatitis B virus(HBV) is a non-cytopathic virus that causes acute and chronic inflammatory liver diseases,often leading to the pathogenesis of hepatocellular carcinoma(HCC). Although many studies for the roles of HBV on pathogenesis of the liver diseases,such as non-alcoholic fatty liver disease(NAFLD),hepatic inflammation,cirrhosis,and HCC,have been reported,the mechanisms are not fully understood. Endoplasmic reticulum(ER) and mitochondria have the protective mechanisms to restore their damaged function by intrinsic or extrinsic stresses,but their chronic dysfunctions are associated with the pathogenesis of the various diseases. Furthermore,HBV can affect intraor extracellular homeostasis through induction of ER and mitochondrial dysfunctions,leading to liver injury. 2Association of chronic viral hepatitis B with insulin resistance显示文摘AIM:To investigate the relationship between chronic viral hepatitis B(CVHB) and insulin resistance(IR) in Korean adults.METHODS:A total of 7880 adults(3851 men,4029 women) who underwent a comprehensive medical examination were enrolled in this study.Subjects diagnosed with either diabetes mellitus,or any other disorder that could influence their insulin sensitivity,were rejected.Anthropometry,metabolic risk factors,hepatitis B surface antigen,hepatitis B surface antibody,hepatitis B core antibody,fasting plasma glucose and insulin were measured for all subjects.Homeostasis model assessment(HOMA),quantitative insulin check index(QUICKI),and Mf fm index were used for determining insulin sensitivity.Each participant was categorized into a negative,recovery,or CVHB group.To compare variables between groups,a t-test and/or one-way analysis of variance were used.Partial correlation coefficients were computed to present the association between insulin resistance and other variables.Multiple logistic regression analysis was used to assess the independent association between CVHB and IR.RESULTS:The mean age of men and women were 48.9 and 48.6 years,respectively.Subjects in the CVHB group had significantly higher waist circumference [(86.0 ± 7.7 cm vs 87.3 ± 7.8 cm,P = 0.004 in men),(78.3 ± 8.6 cm vs 80.5 ± 8.5 cm,P < 0.001 in women)],cystatin C [(0.96 ± 0.15 mg/dL vs 1.02 ± 0.22 mg/dL,P < 0.001 in men),(0.84 ± 0.15 mg/dL vs 0.90 ± 0.16 mg/dL,P < 0.001 in women)],fasting insulin [(5.47 ± 3.38 U/mL vs 6.12 ± 4.62 U/mL,P < 0.001 in men),(4.57 ± 2.82 U/mL vs 5.06 ± 3.10 U/mL,P < 0.001 in women)] and HOMA index [(1.24 ± 0.86 vs 1.43 ± 1.24,P < 0.001 in men),(1.02 ± 0.76 vs 1.13 ± 0.87,P = 0.033 in women)] compared to control group.The HOMA index revealed a positive correlation with body mass index(BMI)(r = 0.378,P < 0.001),waist circumference(r =0.356,P < 0.001),percent body fat(r = 0.296,P < 0.001),systolic blood pressure(r = 0.202,P < 0.001),total cholesterol(r = 0.134,P < 0.001),triglycerides(r = 0.292,P < 0.001),cystatin C(r = 0.069,P < 0.001) and uric acid(r = 0.142,P < 0.001).The QUICKI index revealed a negative correlation with BMI(r =-0.254,P < 0.001),waist circumference(r = 0-0.243,P < 0.001),percent body fat(r =-0.217,P < 0.001),systolic blood pressure(r =-0.132,P < 0.001),total cholesterol(r =-0.106,P < 0.001),triglycerides(r =-0.205,P < 0.001),cystatin C(r =-0.044,P < 0.001) and uric acid(r =-0.096,P < 0.001).For subjects identified with IR,the odds ratio of an accompanying diagnosis of chronic hepatitis B was 1.534(95% CI:1.158-2.031,HOMA index criteria) or 1.566(95% CI:1.124-2.182,QUICKI criteria) after adjustment for age,gender,BMI,and amount of alcohol consumption.CONCLUSION:Our study demonstrates that CVHB is associated with IR.CVHB may need to be monitored for occurrence of IR and diabetes mellitus.Jeong Gyu Lee Sangyeoup Lee Yun Jin Kim Byung Mann Cho Joo Sung Park Hyung Hoi Kim JaeHun Cheong Dong Wook Jeong Yu Hyun Lee Young Hye Cho Mi Jin Bae Eun Jung Choi 2012World Journal of Gastroenterology2012,18,42:10

12Relationship of meteorological factors and air pollutants with medical care utilization for gastroesophageal reflux disease in urban area显示文摘BACKGROUND Gastroesophageal reflux disease(GERD)is a highly prevalent disease of the upper gastrointestinal tract,and it is associated with environmental and lifestyle habits.Due to an increasing interest in the environment,several groups are studying the effects of meteorological factors and air pollutants(MFAPs)on disease development.AIM To identify MFAPs effect on GERD-related medical utilization.METHODS Data on GERD-related medical utilization from 2002 to 2017 were obtained from the National Health Insurance Service of Korea,while those on MFAPs were obtained from eight metropolitan areas and merged.In total,20071900 instances of GERD-related medical utilizations were identified,and 200000 MFAPs were randomly selected from the eight metropolitan areas.Data were analyzed using a multivariable generalized additive Poisson regression model to control for time trends,seasonality,and day of the week.RESULTS Five MFAPs were selected for the prediction model.GERD-related medical utilization increased with the levels of particulate matter with a diameter≤2.5μm(PM2.5)and carbon monoxide(CO).S-shaped and inverted U-shaped changes were observed in average temperature and air pollutants,respectively.The time lag of each variable was significant around nine days after exposure.CONCLUSION Using five MFAPs,the final model significantly predicted GERD-related medical utilization.In particular,PM2.5 and CO were identified as risk or aggravating factors for GERD.Ho Seok Seo Jinwook Hong Jaehun Jung 2020World Journal of Gastroenterology2020,26,39:1

15Development of exosome membrane materialsfused microbubbles for enhanced stability and efficient drug delivery of ultrasound contrast agent显示文摘Lipid-coated microbubbles are widely used as an ultrasound contrast agent,as well as drug delivery carriers.However,the two main limitations in ultrasound diagnosis and drug delivery using microbubbles are the short half-life in the blood system,and the difficulty of surface modification of microbubbles for active targeting.The exosome,a type of extracellular vesicle,has a preferentially targeting ability for its original cell.In this study,exosome-fused microbubbles(Exo-MBs)were developed by embedding the exosome membrane proteins into microbubbles.As a result,the stability of Exo-MBs is improved over the conventional microbubbles.On the same principle that under the exposure of ultrasound,microbubbles are cavitated and self-assembled into nano-sized particles,and Exo-MBs are self-assembled into exosome membrane proteins-embedded nanoparticles(Exo-NPs).The Exo-NPs showed favorable targeting properties to their original cells.A photosensitizer,chlorin e6,was loaded into Exo-MBs to evaluate therapeutic efficacy as a drug carrier.Much higher therapeutic efficacy of photodynamic therapy was confirmed,followed by cancer immunotherapy from immunogenic cell death.We have therefore developed a novel ultrasound image-guided drug delivery platform that overcomes the shortcomings of the conventional ultrasound contrast agent and is capable of simultaneous photodynamic therapy and cancer immunotherapy.Yongho Jang Jeehun Park Pilsu Kim Eun-Joo Park Hyungjin Sun Yujin Baek Jaehun Jung Tai-kyong Song Junsang Doh Hyuncheol Kim 2023Acta Pharmaceutica Sinica B2023,13,12:1
