| 1 | Acupuncture's Role in Solving the Opioid Epidemic: Evidence, Cost-Effectiveness, and Care Availability for Acupuncture as a Primary, Non-Pharmacologic Method for Pain Relief and Management-White Paper 2017显示文摘The United States(U.S.) is facing a national opioid epidemic, and medical systems are in need of nonpharmacologic strategies that can be employed to decrease the public's opioid dependence. Acupuncture has emerged as a powerful, evidence-based, safe, cost-effective, and available treatment modality suitable to meeting this need. Acupuncture has been shown to be effective for the management of numerous types of pain conditions, and mechanisms of action for acupuncture have been described and are understandable from biomedical, physiologic perspectives. Further, acupuncture's cost-effectiveness can dramatically decrease health care expenditures, both from the standpoint of treating acute pain and through avoiding addiction to opioids that requires costly care, destroys quality of life, and can lead to fatal overdose. Numerous federal regulatory agencies have advised or mandated that healthcare systems and providers offer non-pharmacologic treatment options for pain. Acupuncture stands out as the most evidence-based, immediately available choice to fulfil these calls. Acupuncture can safely, easily, and cost-effectively be incorporated into hospital settings as diverse as the emergency department, labor and delivery suites, andneonatal intensive care units to treat a variety of commonly seen pain conditions. | 4 | Allosteric modulation of cholinergic system:Potential approach to treating cognitive deficits of schizophrenia显示文摘Schizophrenia is a psychiatric disorder affecting approximately 1% of the population worldwide and is characterised by the presence of positive and negative symptoms and cognitive deficits. Whilst current therapeutics ameliorate positive symptoms, they are largely ineffective in improving negative symptoms and cognitive deficits. The cholinergic neurotransmitter system heavily influences cognitive function and there is evidence that implicates disruption of the central cholinergic system in schizophrenia. Historically, targeting the cholinergic system has been impeded by poor selectivity leading to intolerable side effects warranting the need to develop more targeted therapeutic compounds. In this review we will summarise evidence supporting the roles of the cholinergic system, particularly the muscarinic M1 receptor, in the pathophysiology of schizophrenia and discuss the potential of a promising new class of candidate compounds, allosteric ligands, for addressing the difficulties involved in targeting this system. | 5 | Endoscopic ultrasound sedation in the United Kingdom: Is life without propofol tolerable?显示文摘There is compelling evidence to support the quality,cost effectiveness and safety profile of non-anesthesiologist-administered propofol for endoscopic ultrasound (EUS). However in the United Kingdom, it is recommended that the administration and monitoring of propofol sedation for endoscopic procedures should be the responsibility of a dedicated and appropriately trained anaesthetist only. The majority of United Kingdom EUS procedures are performed with opiate and benzodiazepine sedation rather than anaesthetist led propofol lists due to anaesthetist resource availability. We sought to prospectively determine the tolerability and safety of EUS with benzodiazepine and opiate sedation in single United Kingdom centre. Two hundred consecutive patients undergoing either EUS or oesophago-gastroduodenoscopy (OGD) with conscious sedation were prospectively recruited with a 1:1 enrolment ratio. Patients completed questionnaires pre and post procedure detailing anticipated and actual pain experienced on a 1-10 visual analogue scale. Demographics, procedure duration, sedation doses and willingness to repeat the procedure were also recorded. EUS procedures lasted significantly longer than OGDs(15 min vs 6 min, P < 0.0001), however, there was no difference in anticipated pain scores between the groups(EUS 3.37/10 vs OGD 3.47/10, P = 0.46). Pain scores indicated EUS was better tolerated than OGD(1.16/10 vs 1.88/10, P = 0.03) although higher doses of sedation were used for EUS procedures. There were no complications identified in either group. | 6 | A rapid dehydration leaf assay reveals stomatal response differences in grapevine genotypes显示文摘A simple and reliable way of phenotyping plant responses to dehydration was developed.Fully-developed leaves were detached and placed in a closed plastic box containing a salt solution to control the atmospheric water potential in the container.Three hours of dehydration(weight loss of the leaf)was optimal for measuring changes in stomatal response to dehydration.Application of the plant hormone abscisic acid(ABA)prior to leaf detachment decreased the amount of water loss,indicating that the assay was able to detect differences based on a stomatal response to dehydration.Five different Vitis genotypes(V.riparia,V.champinii,V.vinifera cv.Shiraz,V.vinifera cv.Grenache and V.vinifera cv.Cabernet Sauvignon)with known differences in drought tolerance were screened for their dehydration response and the results obtained corresponded to previous reports of stomatal responses in the vineyard.Significant differences in stomatal density along with differences in the amount and rate of water lost indicate differences in dehydration sensitivity among the genotypes screened.Differences in stomatal response to ABA were also detected.Shiraz had the lowest stomatal density and the highest ABA sensitivity among the genotypes screened,yet Shiraz lost the most amount of water,indicating that it was the least sensitive to dehydration.Despite having the highest stomatal density and intermediate stomatal sensitivity to ABA,V.riparia lost the smallest amount of water,indicating that it was the most sensitive to dehydration.The assay presented here represents a simple and reliable phenotyping method for plant responses to leaf dehydration. | Daniel W Hopper Ryan Ghan Grant R Cramer | 2014 | Horticulture Research2014，1，1： | 2 |
