| 1 | Small intestinal bacterial overgrowth in inactive Crohn's disease:Influence of thiopurine and biological treatment显示文摘AIM:To investigate the influence of thiopurines and biological drugs on the presence of small intestinal bacterial overgrowth(SIBO) in patients with inactive Crohn's disease(CD).METHODS:This was a prospective study in patients with CD in remission and without corticosteroid treatment,included consecutively from 2004 to 2010.SIBO was investigated using the hydrogen glucose breath test.RESULTS:One hundred and seven patients with CD in remission were included.Almost 58%of patients used maintenance immunosuppressant therapy and 19.6%used biological therapy.The prevalence of SIBO was16.8%.No association was observed between SIBO and the use of thiopurine Immunosuppressant(12/62patients),administration of biological drugs(2/21 patients),or with double treatment with an anti-tumor necrosis factor drugs plus thiopurine(1/13 patients).Half of the patients had symptoms that were suggestive of SIBO,though meteorism was the only symptom that was significantly associated with the presence of SIBO on univariate analysis(P<0.05).Multivariate analysis revealed that the presence of meteorism and a fistulizing pattern were associated with the presence of SIBO(P<0.05).CONCLUSION:Immunosuppressants and/or biological drugs do not induce SIBO in inactive CD.Fistulizing disease pattern and meteorism are associated with SIBO. | Cristina Sánchez-Montes Vicente Ortiz Guillermo Bastida Ester Rodríguez María Yago Belén Beltrán Mariam Aguas Marisa Iborra Vicente Garrigues Julio Ponce Pilar Nos | 2014 | World Journal of Gastroenterology2014,20,38: | 4 |
| 2 | Adalimumab in prevention of postoperative recurrence of Crohn's disease in high-risk patients显示文摘AIM:To evaluate the effectiveness of adalimumab in preventing recurrence after intestinal resection for Crohn's disease in high-risk patients.METHODS:A multicenter,prospective,observational study was conducted from June 2009 until June 2010.We consecutively included high-risk Crohn's disease patients who had undergone an ileal/ileocolonic resection.High-risk patients were defined as two or more criteria:smokers,penetrating pattern,one or more previous surgical resections or prior extensive resection.Subcutaneous adalimumab was administered 2 wk(± 5 d) after surgery at a dose of 40 mg eow,with an initial induction dose of 160/80 mg at weeks 0 and 2.Demographic data,previous and concomitant treatments(antibiotics,5-aminosalicylates,corticosteroids,immunomodulators or biologic therapies),smoking status at the time of diagnosis and after the index operation and number of previous resections(type and reason for surgery) were all recorded.Biological status was assessed with C-reactive protein,erythrocyte sedimentation rate and fecal calprotectin.One year(± 3 mo) after surgery,an ileocolonoscopy and/or magnetic resonance enterography was performed.Endoscopic recurrence was defined as Rutgeerts score ≥ i2.Morphological recurrence was based on magnetic resonance(MR) score ≥ MR1.RESULTS:Twenty-nine patients(55.2% males,48.3% smokers at diagnosis and 13.8% after the index operation),mean age 42.3 years and mean duration of the disease 13.8 years were included in the study.A mean of 1.76(range:1-4) resections previous to adalimumab administration and in 37.9% was considered extensive resection.51.7% had previously received infliximab.Immunomodulators were given concomitantly to 17.2% of patients.Four of the 29(13.7%) developed clinical recurrence,6/29(20.7%) endoscopic recurrence and 7/19(36.8%) morphological recurrence after 1-year.All patients with clinical recurrence showed endoscopic and morphological recurrence.A high degree of concordance was found between clinical-endoscopic recurrence(k = 0.76,P < 0.001) and clinical-morphological recurrence(k = 0.63,P = 0.003).Correlation between endoscopic and radiological findings was good(comparing the 5-point Rutgeerts score with the 4-point MR score,a score of i4 was classified as MR3,i3 as MR2,and i2-i1 as MR1)(P < 0.001,r s = 0.825).During follow-up,five(17.2%) patients needed adalimumab dose intensification(40 mg/wk);Mean time to intensification after the introduction of adalimumab treatment was 8 mo(range:5 to 11 mo).In three cases(10.3%),a biological change was needed due to a worsening of the disease after the dose intensification to 40 mg/wk.One patient suffered an adverse event.CONCLUSION:Adalimumab seems to be effective and safe in preventing postoperative recurrence in a selected group of patients who had undergone an intestinal resection for their CD. | Mariam Aguas Guillermo Bastida Elena Cerrillo Belén Beltrán Marisa Iborra Cristina Sánchez-Montes Fernando Muoz Jesús Barrio Sabino Riestra Pilar Nos | 2012 | World Journal of Gastroenterology2012,18,32: | 3 |

| 6 | 在经历 transcatheter 的病人的 atrial 纤维性颤动的临床、预示的含意大动脉的阀门培植显示文摘 AIM: To study a cohort of consecutive patients under-going transcatheter aortic valve implantation (TAVI) and compare the outcomes of atrial fibrillation (AF) patients vs patients in sinus rhythm (SR). METHODS: All consecutive patients undergoing TAVI in our hospital were included. The AF group comprised patients in AF at the time of TAVI or with history of AF, and were compared with the SR group. Procedural, echocardiographic and follow-up variables were compared. Likewise, the CHA 2 DS 2-VASC stroke risk score and HAS-BLED bleeding risk score and antithrombotic treatment at discharge in AF patients were compared with that in SR patients. RESULTS: From a total of 34 patients undergoing TAVI, 17 (50%) were allocated to the AF group, of whom 15 (88%) were under chronic oral anticoagulation. Patients in the AF group were similar to those in the SR group except for a trend (P = 0.07) for a higher logistic EuroSCORE (28% vs 19%), and a higher prevalence of hypertension (82% vs 53%) and chronic renal failure (17% vs 0%). Risk of both stroke and bleeding was high in the AF group (mean CHA 2 DS 2-VASC 4.3, mean HAS-BLED 2.9). In the AF group, treatment at discharge included chronic oral anticoagulation in all except one case, and in association with an antiplatelet drug in 57% of patients. During a mean follow-up of 11 mo (maximum 32), there were only two strokes, none of them during the peri-procedural period: one in the AF group at 30 mo and one in the SR group at 3 mo. There were no statistical differences in procedural success, and clinical outcome (survival at 1 year 81% vs 74% in AF and SR groups, respectively, P = NS). CONCLUSION: Patients in AF undergoing TAVI show a trend to a higher surgical risk. | 18 | Fagus sylvatica and Quercus pyrenaica:Two neighbors with few things in common显示文摘Background:The Iberian Peninsula comprises one of the largest boundaries between Mediterranean and Eurosiberian vegetation,known as sub-Mediterranean zone.This ecotone hosts many unique plant species and communities and constitutes the low-latitude(warm)margin of numerous central European species which co-occur with Mediterranean vegetation.Two of the main species found in this region are the Eurosiberian European beech(Fagus sylvatica L.)and the Mediterranean Pyrenean oak(Quercus pyrenaica Willd.).It remains unclear how the different physiological and adaptive strategies of these two species reflect their niche partitioning within a subMediterranean community and to what extent phenotypic variation(intraspecific variability)is driving niche partitioning across Eurosiberian and Mediterranean species.Methods:We quantified functional niche partitioning,based on the n-dimensional hypervolume to nine traits related to resource acquisition strategies(leaf,stem and root)plus relative growth rate as an additional wholeplant trait,and the environmental niche similarity between Pyrenean oak and European beech.Further,we analyzed the degree of phenotypic variation of both target species and its relationship with relative growth rates(RGR)and environmental conditions.Plant recruitment was measured for both target species as a proxy for the average fitness.Results:Species’functional space was highly segregated(13.09%overlap),mainly due to differences in niche breadth(59.7%)rather than niche replacement(25.6%),and beech showed higher trait variability,i.e.,had larger functional space.However,both species shared the environmental space,i.e.,environmental niches were overlapped.Most plant traits were not related to abiotic variables or RGR,neither did RGR to plant traits.Conclusions:Both target species share similar environmental space,however,show notably different functional resource-use strategies,promoting a high complementarity that contributes to maintaining a high functionality in sub-Mediterranean ecosystems.Therefore,we propose that conservation efforts be oriented to preserve both species in these habitats to maximize ecosystem functionality and resilience. | Sergio de Tomás Marín Jesús Rodríguez-Calcerrada Salvador Arenas-Castro Iván Prieto Guillermo González Luis Gil Enrique G.de la Riva | 2023 | Forest Ecosystems2023,10,2: | 0 |

| 20 | Long-term results of the treatment of Hodgkin’s lymphoma in a resource-constrained setting:Real-world data from a single center显示文摘BACKGROUND The outcomes of Hodgkin's lymphoma(HL)in Mexico have not been widely reported.Simplified and affordable treatments have been adopted in middleincome countries.AIM The aim was to evaluate long-used therapies for HL in Mexico in a long-term basis.METHODS In a 34-year time period,88 patients with HL were treated at a single institution in Mexico.Patients were treated with adriamycin bleomycin vinblastine and dacarbazine(ABVD)or mechlorethamine,vincristine,procarbazine,and prednisone(MOPP).Relapsed or refractory patients were given ifosfamide,carboplatin,and etoposide(ICE)followed by autologous or allogeneic stem cell transplants.RESULTS Thirty-seven women and 51 men were included;the median age was 29 years.Patients were followed for a mean of 128 mo.The 310-mo overall survival(OS)was 83%for patients treated with MOPP and 88%for those treated with ABVD.The OS of patients who received autologous stem cell transplantation was 76%(330 mo)vs 93%(402 mo)in those who did not.CONCLUSION HL may be less aggressive in Mexican population than in Caucasians.Combined chemotherapy renders acceptable results,regardless of clinical stage. | Luisa Fernanda Sánchez-Valledor Thomas M Habermann Iván Murrieta-Alvarez Alejandra Carmina Córdova-Ramírez Montserrat Rivera-Álvarez Andrés León-Peña Yahveth Cantero-Fortiz Juan Carlos Olivares-Gazca Guillermo JoséRuiz-Delgado Guillermo JoséRuiz-Argüelles | 2021 | World Journal of Clinical Oncology2021,12,9: | 0 |
