| 1 | The role of landscape pattern analysis in understanding concepts of land cover change显示文摘Landscape ecology and landscape pattern analysis are important components of national-scale programs to identify trends in land cover change because: 1) Statistics on changes in land cover proportions are not spatial. A change matrix derived from GIS provides useful information, but it does not show the spatial form of change in the landscape. Landscape pattern metrics reveal spatial pattern. 2) A growing body of literature has shown that a change in landscape pattern might indicate important changes in ecological functions: forest connectivity and species movements, number and size of farm patches, effects on water quality. Spatial pattern is important in structuring ecological communities and in maintaining existence of competitors. Spatial pattern may be determined by disturbance and may in turn, determine how disturbances propagate through the system. 3) Sometimes landscape pattern may not significantly change, even though land cover proportions do change. Or, vice-versa, sometimes landscape pattern can significantly change, even though land cover proportions don’t significantly change. 4) Landscape pattern is an inherent and important part of describing landscapes: based on the literature, one of the most important descriptive characteristics of a landscape is its texture. | 4 | Conduit necrosis following esophagectomy: An up-to-date literature review显示文摘Esophageal conduit ischaemia and necrosis is an uncommon but devastating complication of esophagectomy and remains one of the most challenging issues in surgical practice. The incidence, time interval to develop symptoms, and clinical presentation are highly variable with no predictable pattern. Evidence comes from case reports and case series rather than randomized controlled trials.We describe the issues surrounding conduit necrosis affecting the stomach,jejunum and colon as an esophageal replacement and the advantages,disadvantages and challenges of each type of reconstruction. Diagnosis is challenging for the most experienced surgeon. Upper gastrointestinal endoscopy and computed tomography thorax with both oral and intravenous contrast is the gold standard. Management, either conservative or interventional is also a difficult decision. Management options include conservative treatment and more aggressive treatments such as stent insertion, surgical debridement and repair of the esophagus using jejunum, colon or a musculocutaneous flap. In spite of recent advances in surgical techniques, there is no reliable strategy to manage esophageal conduit necrosis. | 6 | SLC26A4 mutation testing for hearing loss associated with enlargement of the vestibular aqueduct显示文摘Pendred syndrome(PS) is characterized by autosomal recessive inheritance of goiter associated with a defect of iodide organification, hearing loss, enlargement of the vestibular aqueduct(EVA), and mutations of the SLC26A4 gene. However, not all EVA patients have PSor SLC26A4 mutations. Two mutant alleles of SLC26A4 are detected in 1/4 of North American or European EVA populations, one mutant allele is detected in another 1/4 of patient populations, and no mutations are detected in the other 1/2. The presence of two mutant alleles of SLC26A4 is associated with abnormal iodide organification, increased thyroid gland volume, increased severity of hearing loss, and bilateral EVA. The presence of a single mutant allele of SLC26A4 is associated with normal iodide organification, normal thyroid gland volume, less severe hearing loss and either bilateral or unilateral EVA. When other underlying correlations are accounted for, the presence of a cochlear malformation or the size of EVA does not have an effect on hearing thresholds. This is consistent with observations of an Slc26a4 mutant mouse model of EVA in which hearing loss is independent of endolymphatic hydrops or inner ear malformations. Segregation analyses of EVA in families suggest that the patients carrying one mutant allele of SLC26A4 have a second, undetected mutant allele of SLC26A4, and the probability of a sibling having EVA is consistent with its segregation as an autosomal recessive trait. Patients without any mutations are an etiologically heterogeneous group in which siblings have a lower probability of having EVA.
