| 6 | Randomized trial in malignant biliary obstruction:Plastic vs partially covered metal stents显示文摘AIM:To compare efficacy and complications of par-tially covered self-expandable metal stent(pcSEMS)to plastic stent(PS)in patients treated for malignant,infrahilar biliary obstruction.METHODS:Multicenter prospective randomized clinical trial with treatment allocation to a pcWallstent(SEMS)or a 10 French PS.Palliative patients aged≥18,for infrahilar malignant biliary obstruction and a Karnofsky performance scale index>60%from 6 participating North American university centers.Primary endpoint was time to stent failure,with secondary outcomes of death,adverse events,Karnofsky performance score and short-form-36 scale administered on a three-monthly basis for up to 2 years.Survival analyses were performed for stent failure and death,with Cox proportional hazards regression models to determine significant predictive characteristics.RESULTS:Eighty-five patients were accrued over 37mo,42 were randomized to the SEMS group and 83patients were available for analyses.Time to stent failure was 385.3±52.5 d in the SEMS and 153.3±19.8 d in the PS group,P=0.006.Time to death did not differ between groups(192.3±23.4 d for SEMS vs211.5±28.0 d for PS,P=0.70).The only significant predictor was treatment allocation,relating to the time to stent failure(P=0.01).Amongst other measured outcomes,only cholangitis differed,being more common in the PS group(4.9%vs 24.5%,P=0.029).The small number of patients in follow-up limits longitudinal assessments of performance and quality of life.From an initially planned 120 patients,only 85 patients were recruited.CONCLUSION:Partially covered SEMS result in a longer duration till stent failure without increased complication rates,yet without accompanying measurable benefits in survival,performance,or quality of life. | Peter L Moses Khalid M AlNaamani Alan N Barkun Stuart R Gordon Roger D Mitty M Stanley Branch Thomas E Kowalski Myriam Martel Viviane Adam | 2013 | World Journal of Gastroenterology2013，19，46： | 6 |

| 8 | Rhubarb extract partially improves mucosal integrity in chemotherapy-induced intestinal mucositis显示文摘AIM To investigate the effects of orally gavaged aqueous rhubarb extract(RE) on 5-fluorouracil(5-FU)-induced intestinal mucositis in rats. METHODS Female Dark Agouti rats(n = 8/group) were gavaged daily(1 mL) with water, high-dose RE(HDR; 200 mg/kg) or low-dose RE(LDR; 20mg/kg) for eight days. Intestinal mucositis was induced(day 5) with 5-FU(150 mg/kg) via intraperitoneal injection. Intestinal tissue samples were collected for myeloperoxidase(MPO) activity and histological examination. Xenopus oocytes expressing aquaporin 4 water channels were prepared to examine the effect of aqueous RE on cell volume, indicating a potential mechanism responsible for modulating net fluid absorption and secretion in the gastrointestinal tract. Statistical significance was assumed at P < 0.05 by one-way ANOVA. RESULTS B o d y w e i g h t w a s s i g n i f i c a n t l y r e d u c e d i n r a t s administered 5-FU compared to healthy controls(P < 0.01). Rats administered 5-FU significantly increased intestinal MPO levels(≥ 307%; P < 0.001), compared to healthy controls. However, LDR attenuated this effect in 5-FU treated rats, significantly decreasing ileal MPO activity(by 45%; P < 0.05), as compared to 5-FU controls. 5-FU significantly reduced intestinal mucosal thickness(by ≥ 29% P < 0.001) as compared to healthy controls. LDR significantly increased ileal mucosal thickness in 5-FU treated rats(19%; P < 0.05) relative to 5-FU controls. In xenopus oocytes expressing AQP4 water channels, RE selectively blocked water influx into the cell, induced by a decrease in external osmotic pressure. | 13 | Transient thermal analysis as measurement method for IC package structural integrity显示文摘Practices of IC package reliability testing are reviewed briefly,and the application of transient thermal analysis is examined in great depth.For the design of light sources based on light emitting diode(LED) efficient and accurate reliability testing is required to realize the potential lifetimes of 105 h.Transient thermal analysis is a standard method to determine the transient thermal impedance of semiconductor devices,e.g.power electronics and LEDs.The temperature of the semiconductor junctions is assessed by time-resolved measurement of their forward voltage(Vf).The thermal path in the IC package is resolved by the transient technique in the time domain.This enables analyzing the structural integrity of the semiconductor package.However,to evaluate thermal resistance,one must also measure the dissipated energy of the device(i.e.,the thermal load) and the k-factor.This is time consuming,and measurement errors reduce the accuracy.To overcome these limitations,an innovative approach,the relative thermal resistance method,was developed to reduce the measurement effort,increase accuracy and enable automatic data evaluation.This new way of evaluating data simplifies the thermal transient analysis by eliminating measurement of the k-factor and thermal load,i.e.measurement of the lumen flux for LEDs,by normalizing the transient Vfdata.This is especially advantageous for reliability testing where changes in the thermal path,like cracks and delaminations,can be determined without measuring the k-factor and thermal load.Different failure modes can be separated in the time domain.The sensitivity of the method is demonstrated by its application to highpower white In GaN LEDs.For detailed analysis and identification of the failure mode of the LED packages,the transient signals are simulated by time-resolved finite element(FE) simulations.Using the new approach,the transient thermal analysis is enhanced to a powerful tool for reliability investigation of semiconductor packages in accelerated lifetime tests and for inline inspection.This enables automatic data analysis of the transient thermal data required for processing a large amount of data in production and reliability testing.Based on the method,the integrity of LED packages can be tested by inline,outgoing inspection and the lifetime prediction of the products is improved. | Alexander Han Maximilian Schmid E Liu Gordon Elger | 2015 | Chinese Physics B2015，24，6： | 2 |
