2Damage control orthopaedics: State of the art显示文摘Damage control orthopaedics(DCO) originally consisted of the provisional immobilisation of long bone-mainly femur-fractures in order to achieve the advantages of early treatment and to minimise the risk of complications, such as major pain, fat embolism, clotting, pathological inflammatory response, severe haemorrhage triggering the lethal triad, and the traumatic effects of major surgery on a patient who is already traumatised(the 'second hit' effect). In recent years, new locations have been added to the DCO concept, such as injuries to the pelvis, spine and upper limbs. Nonetheless, this concept has not yet been validated in well-designed prospective studies, and much controversy remains.Indeed, some researchers believe the indiscriminate application of DCO might be harmful and produce substantial and unnecessary expense. In this respect, too,normalised parameters associated with the acid-base system have been proposed,under a concept termed early appropriate care, in the view that this would enable patients to receive major surgical procedures in an approach offering the advantages of early total care together with the apparent safety of DCO. 3Recent advances in bone regeneration using adult stem cells显示文摘Bone is a highly vascularized tissue reliant on the close spatial and temporal association between bloodvessels and bone cells. Therefore, cells that participate in vasculogenesis and osteogenesis play a pivotal role in bone formation during prenatal and postnatal periods. Nevertheless, spontaneous healing of bone fracture is occasionally impaired due to insufficient blood and cellular supply to the site of injury. In these cases, bone regeneration process is interrupted, which might result in delayed union or even nonunion of the fracture. Nonunion fracture is difficult to treat and have a high financial impact. In the last decade, numerous technological advancements in bone tissue engineering and cell-therapy opened new horizon in the field of bone regeneration. This review starts with presentation of the biological processes involved in bone development, bone remodeling, fracture healing process and the microenvironment at bone healing sites. Then, we discuss the rationale for using adult stem cells and listed the characteristics of the available cells for bone regeneration. The mechanism of action and epigenetic regulations for osteogenic differentiation are also described. 5Negative regulation of receptor tyrosine kinases: unexpected links to c-Cbl and receptorubiquitylation显示文摘Intracellular signals mediated by the family of receptor tyrosine kinases play pivotal roles in morphogenesis, cell fate determination and pathogenesis. Precise control of signal amplitude and duration is critical for the fidelity and robustness of these processes. Activation of receptor tyrosine kinases by their cognate growth factors not only leads to propagation of the signal through various biochemical cascades, but also sets in motion multiple attenuation mechanisms that ulti- mately terminate the active state. Early attenuators pre-exist prior to receptor activation and they act to limit signal propagation. Subsequently, late attenuators, such as Lrig and Sprouty, are transcriptionally induced and further act to dampen the signal. Central to the process of signaling attenuation is the role of the E3 ubiquitin ligase c-Cbl. While Cbl- mediated processes of receptor ubiquitylation and endocytosis are relatively well understood, the links of Cbl to other negative regulators are just now beginning to be appreciated. 6In situ chemical fabrication of polyaniline/multi-walled carbon nanotubes composites as supports of Pt for methanol electrooxidation显示文摘In this study,platinum(Pt)-polyaniline(PANI)/MWNTs catalysts were synthesized by two sequential reactions.First,coreshell structural PANI/MWNTs composites were fabricated by in-situ chemical oxidation polymerization,in which MWNTs act as the core and PANI as the shell.Then,Pt particles were deposited on the PANI/MWNTs composites by a chemical reduction method.The morphology and constitution of the products were characterized by FT IR,scanning electron microscopy,transmission electron microscopy and XRD.It was observed that the Pt particles were smaller in size and more uniformly distributed on these composite supports than those on the reference,pure MWNT supports.XRD results showed higher Pt(111) content in the catalyst deposited on PANI/MWNTs supports than that on MWNTs.The cyclic voltammeter(CV) tests demonstrated that the electrode modified by Pt-PANI/MWNTs ternary composite catalyst showed a higher catalytic stability than Pt-MWNT binary catalyst did,due to the synergic interaction between Pt and the composite support.GAL Jean-Yves 2010Science China Chemistry2010,53,9:4

10取代聚苯胺的聚集态结构显示文摘环取代基及掺杂对聚苯胺分子链结构产生影响的同时也对大分子聚集态结构产生影响.通过对聚合物膜的XRD、SEM研究,表明在所合成的环取代聚苯胺衍生物中,聚合物的结晶性依POT(聚邻甲苯胺)、PDMAn(聚-2,5-二甲氧基苯胺)、PmClAn(聚间氯苯胺)顺序增加;与本征态的比较,质子酸掺杂后聚合物的结晶性有所提高.POT基本上表现出了非晶聚合物的性质;PDMAn具有同质多晶的结构特征,但结晶度较低;PmClAn表现出了典型高分子晶体性质,符合单斜晶系的特征(晶胞参数为a=0.679nm,b=2.304nm,c=0.5734nm,α=90°,β=100.9°,γ=90°).采用HyperChem程序包中的MM+分子力学方法对1H-NMR和XRD实验结果进行结构优化,确定了苯式结构/醌式结构为3/2的PmClAn大分子链的构象.戴李宗 许一婷 Jean-Yves GAL 吴辉煌 2002物理化学学报2002,18,3:3

14Efficacy of bevacizumab-containing chemotherapy in metastatic colorectal cancer and CXCL5 expression: Six case reports显示文摘BACKGROUND In metastatic colorectal cancer(mCRC),the anti-vascular endothelial growth factor drug bevacizumab(BVZ)plus chemotherapy significantly improves progression-free survival compared to chemotherapy(CT)alone.This benefit is not,however,observed in all patients.While increased chemokine CXCL5 gene expression promoting angiogenesis has been proposed as a prognostic mCRC biomarker,few studies have examined its relationship with drug efficacy.This study sought to analyze tumor CXCL5 gene expression in six patients with different efficacy of BVZ-containing CT in terms of the tumor response to treatment.CASE SUMMARY We report six cases of stage IV KRAS-mutated mCRC.Patients were given first line treatment with BVZ-containing chemotherapy in University Hospital of Fuenlabrada.The six patients differed in terms of primary tumor location(right/left side),tumor burden(mostly hepatic and peritoneal disease)and clinical disease course.Before treatment onset,total RNA was isolated from paraffinated tumor biopsy specimens and CXCL5 gene expression quantified through conventional RT-qPCR procedures.Our main finding was that CXCL5 expression levels were several times higher in three patients with lower progression free survival(under 6 mo)from the start of treatment.CONCLUSION A higher expression of CXCL5 was observed in the three patients showing worse tumor response to treatment.Apolonia Novillo María Gaibar Alicia Romero-Lorca María Fuencisla Gilsanz Laura Beltrán Miguel Galán Beatriz Antón Diego Malón Amalia Moreno Ana Fernández-Santander 2020World Journal of Gastroenterology2020,26,16:2
15Double balloon enteroscopy examinations in general anesthesia显示文摘AIM:To demonstrate that the double balloon enteroscopy(DBE) can be safely performed in general anesthesia with intubation.METHODS:We performed a retrospective examination between August 2005 and November 2008 amongpatients receiving intubation narcosis due to DBE examination.The patients were grouped based on sex,age and physical status.Anesthesia records includedduration of anesthesia,quantity of medication usedand anesthesia-related complications.We determinedthe frequency of complications in the different groupsand their relation with the quantity of medication usedand the duration of anesthesia.RESULTS:We compiled data for 108 cases of general anesthesia with intubation.We did not observeany permanent anesthesia-related complications;themost frequent side effects of anesthesia were hypo-tension(30.55%),desaturation(21.29%),and apnea(17.59%).These complications were significantly more frequent among patients with multiple additional diseases [hypotension(23.1% vs 76.9%,P = 0.005),desaturation(12.3% vs 69.2%,P < 0.001) and apnea(7.7% vs 53.8%,P = 0.001)],however,their incidence was not proportional to the quantity of medication used or the duration of anesthesia.CONCLUSION:General anesthesia with intubation is definitely a viable option among DBE methods.It is highly recommended in patients with multiple additional diseases.Laszlo Zubek Lena Szabo Peter Laszlo Lakatos Janos Papp Janos Gal Gabor Elo 2010World Journal of Gastroenterology2010,16,27:2
