6Endoscopic retrograde cholangiopancreatography associated pancreatitis:A 15-year review显示文摘The aim of this article is to review the literature regarding post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis.We searched for and evaluated all articles describing the diagnosis,epidemiology,pathophysiology,morbidity,mortality and prevention of post-ERCP pancreatitis(PEP) in adult patients using the PubMed database.Search terms included endoscopic retrograde cholangiopancreatography,pancreatitis,ampulla of vater,endoscopic sphincterotomy,balloon dilatation,cholangiography,adverse events,standards and utilization.We limited our review of articles to those published between January 1,1994 and August 15,2009 regarding human adults and written in the English language.Publicat ions from the reference sections were reviewed and included if they were salient and fell into the time period of interest.Between the dates queried,seventeen large(> 500 patients) prospective and four large retrospective trials were conducted.PEP occurred in 1-15 in the prospective trials and in 1-4 in the retrospective trials.PEP was also reduced with pancreatic duct stent placement and outcomes were improved with endoscopic sphincterotomy compared to balloon sphincter dilation in the setting of choledocholithiasis.Approximately 34 pharmacologic agents have been evaluated for the prevention of PEP over the last f ifteen years in 63 trials.Although 22 of 63 trials published during our period of review suggested a reduction in PEP,no pharmacologic therapy has been widely accepted in clinical use in decreasing the development of PEP.In conclusion,PEP is a well-recognized complication of ERCP.Medical treatment for prevention has been disappointing.Proper patient selection and pancreatic duct stenting have been shown to reduce the complication rate in randomized clinical trials.Kevin E Woods Field F Willingham 2010World Journal of Gastrointestinal Endoscopy2010,2,5:20

9Field test of best management practice pollutant removal efficiencies in Shenzhen,China显示文摘This paper presents a study on the use of best management practices(BMPs)for controlling nonpoint pollution in the Xikeng Reservoir watershed located in Shenzhen,China.A BMP treatment train design,including a pond,a wetland,and a buffer strip placed in series was implemented at the reservoir location.A separate grass swale was also constructed at the site.Low impact development(LID)BMPs,namely a planter box and bioboxes,were used at the parking lot of the reservoir’s Administration Building.Samples were collected during storm events and were analyzed for total suspended solids(TSS),biochemical oxygen demand(BOD5),ammonia nitrogen(NH3-N),and total phosphorus(TP).The removal efficiencies of both BMP systems were evaluated using the Efficiency Ratio(ER)method based on the event mean concentration(EMC)data.In summary,the pond/wetland treatment train removed 70%–90%of TSS,20%–50%of BOD5,and 30%–70%of TP and NH3-N.The swale removed 50%–90%of TSS,30%–55%of BOD5,–10%–35%of NH3-N,and 25%–70%of TP.For the planter box and biobox,the ranges of removal rates were 70%–90%,20%–50%,and 30%–70%for TSS,BOD5,and ammonia and phosphorus,respectively.Ru ZHANG Wenbin ZHOU Richard FIELD Anthony TAFURI Shaw LYU Keli JIN 2009Frontiers of Environmental Science & Engineering2009,3,3:12

11The immune modifying effects of amino acids on gut-associated lymphoid tissue显示文摘The intestine and the gut-associated lymphoid tissue(GALT) are essential components of whole body immune defense,protecting the body from foreign antigens and pathogens,while allowing tolerance to commensal bacteria and dietary antigens.The requirement for protein to support the immune system is well established.Less is known regarding the immune modifying properties of individual amino acids,particularly on the GALT.Both oral and parenteral feeding studies have established convincing evidence that not only the total protein intake,but the availability of specific dietary amino acids(in particular glutamine,glutamate,and arginine,and perhaps methionine,cysteine and threonine) are essential to optimizing the immune functions of the intestine and the proximal resident immune cells.These amino acids each have unique properties that include,maintaining the integrity,growth and function of the intestine,as well as normalizing inflammatory cytokine secretion and improving T-lymphocyte numbers,specific T cell functions,and the secretion of IgA by lamina propria cells.Our understanding of this area has come from studies that have supplemented single amino acids to a mixed protein diet and measuring the effect on specific immune parameters.Future studies should be designed using amino acid mixtures that target a number of specific functions of GALT in order to optimize immune function in domestic animals and humans during critical periods of development and various disease states.Megan R Ruth Catherine J Field 2013Journal of Animal Science and Biotechnology2013,4,3:11
12A new cyamodontoid placodont from Triassic of Guizhou, China显示文摘A new cyamodontoid placodont is named (Pse-phochelys polyosteoderma gen. et. sp. nov.) and described based on a three-dimensionally preserved specimen. The material comes from the Wayao Member of the Falang Formation (Carnian, Late Triassic) in Guizhou Province, southwestern China. The skull of Psephochelys shows a unique combination of characteristics observed in Psepho-derma and Placochelys, and the carapace is diagnostic of a new taxon as well. The ventral dermal armor of Psephochelys differs from all other known cyamodontoids in that it comprises irregular marginal osteoderms in loose contact with each other, and gastralia between them. The cranial suture pattern is in part difficult to analyze due to apparent co-ossification of elements, or poor delimitation of sutures. This affects primarily the demarcation of the nasal from the prefrontal, and the relation between the anterior part of the parietal, posterior ends of the frontals, and postfrontals. 13Intrahepatic natural killer T cell populations are increased in human hepatic steatosis显示文摘AIM: To determine if natural killer T cell (NKT) populations are affected in nonalcoholic fatty liver disease (NAFLD). METHODS: Patients undergoing bariatric surgery underwent liver biopsy and blood sampling during surgery. The biopsy was assessed for steatosis and immunocyte infiltration. Intrahepatic lymphocytes (IHLs) were isolated from the remainder of the liver biopsy, and peripheral blood mononuclear cells (PBMCs) were isolated from the blood. Expression of surface proteins on both IHLs and PBMCs were quantified using flow cytometry. RESULTS: Twenty-seven subjects participated in thisstudy. Subjects with moderate or severe steatosis had a higher percentage of intrahepatic CD3+/CD56+ NKT cells (38.6%) than did patients with mild steatosis (24.1%, P = 0.05) or those without steatosis (21.5%, P = 0.03). Patients with moderate to severe steatosis also had a higher percentage of NKT cells in the blood (12.3%) as compared to patients with mild steatosis (2.5% P = 0.02) and those without steatosis (5.1%, P = 0.05). 14加热环境对人工合成磁赤铁矿热磁行为的影响显示文摘热磁测量,包括高温磁化率和高温磁化强度测量,是根据热磁曲线转折点的温度(居里点、尼尔点或相变点)鉴定样品中磁性矿物种类的有效方法.本文选取两个人工合成磁赤铁矿样品,利用四种热磁测量仪器分析不同的条件下测得的热磁曲线.依据样品与空气接触程度,将测量环境设为开放、封闭、封闭(通入氩气或氮气)三类.结果表明:热磁测量环境的开放程度对居里点和曲线可逆程度产生极大的影响.封闭环境下测得的居里点较开放环境下的低,分别对应磁铁矿和磁赤铁矿;开放系统的热磁曲线不可逆程度高于封闭系统.造成这些差异的原因是氧化还原条件的不同.本文的磁赤铁矿样品在封闭的条件下,加热至250℃左右开始转化为磁铁矿,因此无法通过居里点被正确识别;在开放的氧化环境下,加热的最终产物为赤铁矿,能够测得正确的居里点.本实验结果启发人们:在不同的加热环境下,磁性矿物可能表现出不同的热磁行为,根据单一的热磁曲线,很容易对样品中磁性矿物的种类造成误判.全面对比不同条件下的测量结果,才能够得出更为准确的结果.刘秀铭 吕镔 李平原 Field Russell 毛学刚 郭晖 马明明 赵国永 2013地球物理学报2013,56,5:10

17Metastatic colorectal cancer-past,progress and future显示文摘The clinical management of metastatic (stage Ⅳ) colorectal cancer (CRC) is a common challenge faced by surgeons and physicians. The last decade has seen exciting developments in the management of CRC, with significant improvements in prognosis for patients diagnosed with stage Ⅳ disease. Treatment options have expanded from 5-fluorouracil alone to a range of pharmaceutical and interventional therapies, improving survival, and providing a cure in selected cases. Enhanced understanding of the biologic pathways most important in colorectal carcinogenesis has led to a new generation of drugs showing promise in advanced disease. It is hoped that in the near future the treatment paradigm of metastatic CRC will be analogous to that of a chronic illness, rather than a rapidly terminal condition. 19Hijacking antibody-induced CTLA-4 lysosomal degradation for safer and more effective cancer immunotherapy显示文摘It remains unclear why the clinically used anti-CTLA-4 antibodies, popularly called checkpoint inhibitors, have severe immunotherapy-related adverse effects (irAEs) and yet suboptimal cancer immunotherapeutic effects (CITE). Here we report that while irAE-prone Ipilimumab and TremeIgG1 rapidly direct cell surface CTLA-4 for lysosomal degradation, the non-irAE-prone antibodies we generated, HL12 or HL32, dissociate from CTLA-4 after endocytosis and allow CTLA-4 recycling to cell surface by the LRBA-dependent mechanism. Disrupting CTLA-4 recycling results in robust CTLA-4 downregulation by all anti-CTLA-4 antibodies and confers toxicity to a non-irAE-prone anti-CTLA-4 mAb. Conversely, increasing the pH sensitivity of TremeIgG1 by introducing designed tyrosine-to-histidine mutations prevents antibody-triggered lysosomal CTLA-4 downregulation and dramatically attenuates irAE. Surprisingly, by avoiding CTLA-4 downregulation and due to their increased bioavailability, pH-sensitive anti-CTLA-4 antibodies are more effective in intratumor regulatory T-cell depletion and rejection of large established tumors. 20Mycophenolate mofetil for maintenance of remission in steroid-dependent autoimmune pancreatitis显示文摘Systemic corticosteroids represent the standard treatment for autoimmune pancreatitis with IgG4-associated cholangitis.For steroid-dependent disease,azathioprine has been used for maintenance of remission.Mycophenolate mofetil has been used for transplant immunosuppression and more recently for autoimmune hepatitis;however,there are no case reports to date on the use of mycophenolate mofetil in adult patients with autoimmune pancreatitis.A patient with IgG4-mediated autoimmune pancreatitis and IgG4-associated cholangitis refractory to steroids and intolerant of azathioprine was treated with mycophenolate mofetil,which inhibits de novo guanosine synthesis and blockade of both B and T lymphocyte production.Introduction of mycophenolate mofetil and uptitration to 1000 mg by mouth twice daily over a treatment period of 4 mo was associated with improvement in the patient's energy level and blood glucose control and was not associated with any adverse events.The patient was managed without a biliary stent.However,there was a return of symptoms,jaundice,increase in transaminases,and hyperbilirubinemia when the prednisone dose reached 11 mg per day.In the first report of mycophenolate mofetil use in an adult patient with IgG4-associated autoimmune pancreatitis and IgG4-associated cholangitis,the introduction of mycophenolate mofetil was safe and well-tolerated without adverse events,but it did not enable discontinuation of the steroids.Mycophenolate mofetil and other immunomodulatory therapies should continue to be studied for maintenance of remission in the large subset of patients with refractory or recurrent autoimmune pancreatitis.Jamie B Sodikoff Steven A Keilin Sheila J Bharmal Melinda M Lewis Gottumukkala S Raju Field F Willingham 2012World Journal of Gastroenterology2012,18,18:6
