2Modulation of immunity and inflammatory gene expression in the gut, in inflammatory diseases of the gut and in the liver by probiotics显示文摘The potential for the positive manipulation of the gut microbiome through the introduction of beneficial microbes, as also known as probiotics, is currently an active area of investigation. The FAO/WHO define probiotics as live microorganisms that confer a health benefit to the host when administered in adequate amounts. However, dead bacteria and bacterial molecular components may also exhibit probiotic properties. The results of clinical studies have demonstrated the clinical potential of probiotics in many pathologies, such as allergic diseases, diarrhea, inflammatory bowel disease and viral infection. Several mechanisms have been proposed to explain the beneficial effects of probiotics, most of which involve gene expression regulation in specific tissues, particularly the intestine and liver. Therefore, the modulation of gene expression mediated by probiotics is an important issue that warrants further investigation. In the present paper, we performed a systematic review of the probiotic-mediated modulation of gene expression that is associated with the immune system and inflammation. Between January 1990 to February 2014, Pub Med was searched for articles that were published in English using the Me SH terms 'probiotics ' and ' gene expression ' combined with 'intestines', 'liver', 'enterocytes', 'antigen-presenting cells', 'dendritic cells', 'immune system', and 'inflammation'. Two hundred and five original articles matching these criteria were initially selected, although only those articles that included specific gene expression results(77) were later considered for this review and separated into three major topics: the regulation of immunity and inflammatory gene expression in the gut, in inflammatory diseases of the gut and in the liver. Particular strains of Bifidobacteria, Lactobacilli, Escherichia coli, Propionibacterium, Bacillus and Saccharomyces influence the gene expression of mucins, Toll-like receptors, caspases, nuclear factor-κB, and interleukins and lead mainly to an anti-inflammatory response in cultured enterocytes. In addition, the interaction of commensal bacteria and probiotics with the surface of antigenpresenting cells in vitro results in the downregulation of pro-inflammatory genes that are linked to inflammatory signaling pathways, whereas other anti-inflammatory genes are upregulated. The effects of probiotics have been extensively investigated in animal models ranging from fish to mice, rats and piglets. These bacteria induce a tolerogenic and hyporesponsive immune response in which many genes that are related to the immune system, in particular those genes expressing anti-inflammatory cytokines, are upregulated. By contrast, information related to gene expression in human intestinal cells mediated by the action of probiotics is scarce. There is a need for further clinical studies that evaluate the mechanism of action of probiotics both in healthy humans and in patients with chronic diseases. 7对乙酰氨基酚引起的急性肝衰竭:一项美国多中心前瞻性研究结果显示文摘Severe acetaminophen hepatotoxicity frequently leads to acute liver failure (ALF). We determined the incidence, risk factors, and outcomes of acetaminophen- induced ALF at 22 tertiary care centers in the United States. Detailed prospective data were gathered on 662 consecutive patients over a 6- year period fulfilling standard criteria for ALF (coagulopathy and encephalopathy), from which 275 (42% )were determined to result from acetaminophen liver injury. The annual percentage of acetaminophen-related ALF rose during the study from 28% in 1998 to 51% in 2003. Median dose ingested was 24 g (equivalent to 48 extra-strength tablets). Unintentional overdoses accounted for 131 (48% ) cases, intentional (suicide attempts) 122 (44% ), and 22 (8% ) were of unknown intent. In the unintentional group, 38% took two or more acetaminophen preparations simultaneously, and 63% used narcotic-containing compounds. Eighty-one percent of unintentional patients reported taking acetaminophen and/or other analgesics for acute or chronic pain syndromes. Overall, 178 subjects (65% ) survived, 74 (27% ) died without transplantation, and 23 subjects (8% ) underwent liver transplantation; 71% were alive at 3 weeks. Transplant free survival rate and rate of liver transplantation were similar between intentional and unintentional groups. In conclusion, acetaminophen hepatotoxicity far exceeds other causes of acute liver failure in the United States. Susceptible patients have concomitant depression, chronic pain, alcohol or narcotic use, and/or take several preparations simultaneously. 8Low efficacy of levofloxacin-doxycycline-based third-line triple therapy for Helicobacter pylori eradication in Italy显示文摘AIM:To evaluate a levofloxacin-doxycycline-based triple therapy with or without a susceptibility culture test in non-responders to Helicobacter pylori(H.pylori) eradication.METHODS:A total of 142(99 women,43 men; mean 53.0 ± 12.7 years) non-responders to more than two H.pylori eradication therapies underwent susceptibility culture tests or were treated with a seven-day triple therapy consisting of esomeprazole,20 mg b.i.d.,levofloxacin,500 mg b.i.d.,and doxycycline,100 mg b.i.d.,randomly associated with(n = 71) or without(n = 71) Lactobacillus casei DG.H.pylori status was checked in all patients at enrollment and at least 8 wk after the end of therapy.Compliance and tolerability of regimens were also assessed.RESULTS:H.pylori eradication was achieved in < 50% of patients [per prototol(PP) = 49%; intention to treat(ITT) = 46%].Eradication rate was higher in patients administered probiotics than in those without(PP = 55% vs 43%; ITT = 54% vs 40%).Estimated primary resistance to levofloxacin was 18% and multiple resistance was 31%.Therapy was well tolerated,and side effects were generally mild,with only one patient experiencing severe effects.CONCLUSION:Third-line levofloxacin-doxycycline triple therapy had a low H.pylori eradication efficacy,though the success and tolerability of this treatment may be enhanced with probiotics.Omero Alessandro Paoluzi Giovanna Del Vecchio Blanco Emanuela Visconti Manuela Coppola Carla Fontana Marco Favaro Francesco Pallone 2015World Journal of Gastroenterology2015,21,21:5
9Gut microbiota and Ma-Pi 2 macrobiotic diet in the treatment of type 2 diabetes显示文摘In the past 10 years the prevalence of type 2 diabetes mel itus(T2DM) has increased hugely worldwide,driven by a rise in the numbers of overweight and obese individuals.A number of diets have been shown to be effective for the management of T2DM:the Mediterranean diet,the vegetarian diet and the low-calorie diet.Results of studies clearly indicate,however,that the efficacy of these diets is not solely related to the biochemical structure of the individual nutrients they contain.This review discusses this point with reference to the potential role of the intestinal microbiota in diabetes.The macrobiotic Ma-Pi 2 diet is rich in carbohydrates,whole grains and vegetables,with no animal fat or protein or added sugar.In shortand medium-term trials conducted in patients with T2 DM,the Ma-Pi 2 diet has been found to significantly improve indicators of metabolic control,including fasting blood glucose,glycosylated hemoglobin,the serum lipid profile,body mass index,body weight and blood pressure.The diet may also alter the gut microbiota composition,which could additionally affect glycemic control.As a result,the Ma-Pi 2 diet could be considered a valid additional shortto medium-term treatment for T2 DM.Francesco Fallucca Lucia Fontana Sara Fallucca Mario Pianesi 2015World Journal of Diabetes2015,6,3:3

11Genetics of barley tiller and leaf development显示文摘In cereals, tillering and leaf development are key factors in the concept of crop ideotype, introduced in the 1960 s to enhance crop yield, via manipulation of plant architecture. In the present review, we discuss advances in genetic analysis of barley shoot architecture,focusing on tillering, leaf size and angle. We also discuss novel phenotyping techniques, such as 2 D and 3 D imaging, that have been introduced in the era of phenomics, facilitating reliable trait measurement. We discuss the identification of genes and pathways that are involved in barley tillering and leaf development,highlighting key hormones involved in the control of plant architecture in barley and rice.
