1Serum bicarbonate may independently predict acute kidney injury in critically ill patients:An observational study显示文摘AIM: To explore whether serum bicarbonate at admission to intensive care unit(ICU) predicted development of acute kidney injury(AKI).METHODS:We studied all patients admitted to our ICU over a 2 year period(February 2010 to 2012).The ICU has a case mix of medical and surgical patients excluding cardiac surgical,trauma and neurosurgical patients.We analysed 2035 consecutive patients admitted to ICU during the study period.Data were collected by two investigators independently and in duplicate using a standardised spread sheet to ensure accuracy.Ambiguous data were checked for accuracy where indicated.AKI was defined using the Kidney Disease Improving Global Outcomes criteria.Patients were divided into two groups;patients who developed AKI or those who did not,in order to compare the baseline characteristics,and laboratory and physiologic data of the two cohorts.Regression analysis was used to identify if serum bicarbonate on admission predicted the development of AKI.RESULTS:Of 2036 patients 152(7.5%)were excluded due to missing data.AKI developed in 43.1%of the patients.The AKI group,compared to the nonAKI group,was sicker based on their lower systolic,diastolic and mean arterial pressures and a higher acutephysiology and chronic health evaluation(APACHE)Ⅲand SAPSⅡscores.Moreover,patients who developed AKI had more co-morbidities and a higher proportion of patients who developed AKI required mechanical ventilation.The multi-regression analysis of independent variables showed that serum bicarbonate on admission(OR=0.821;95%CI:0.796-0.846;P<0.0001),APACHEⅢ(OR=1.011;95%CI:1.007-1.015;P<0.0001),age(OR=1.016;95%CI:1.008-1.024;P<0.0001)and presence of sepsis at ICU admission(OR=2.819;95%CI:2.122-23.744;P=0.004)were each significant independent predictors of AKI.The area under the ROC curve was 0.8(95%CI:0.78-0.83),thereby demonstrating that the predictive model has relatively good discriminating power for predicting AKI.CONCLUSION:Serum bicarbonate on admission may independently be used to make a diagnosis of AKI.Anuksha Gujadhur Ravindranath Tiruvoipati Elizabeth Cole Saada Malouf Erum Sahid Ansari Kim Wong 2015World Journal of Critical Care Medicine2015,4,1:3
2Geochemistry and Depositional Setting of Fort Munro Formation,Middle and Lower Indus Basins,Pakistan显示文摘Fort Munro Formation represents the products of the Upper Cretaceous (Maastrichtian) in the middle and lower Indus basins. The formation is exposed in the Rakhi Nala (Sulaiman Range), Bara Nala (Lakhi Range) and Naka Pabni (Southern Pab Range) areas. Major and trace elemental geochemistry and petrographic studies of the formation have been carried out to understand the facies trends in the middle and lower Indus basins. A high amount of acid-insoluble fraction, Ca/Mg and Mg vs. Ca/Sr ratio reveal that the formation was deposited in a shallow marine regressive environment. High amounts of clastic reflect abundant influx of terrigenous materials from the east (Indian craton) and west (Bibai volcanic). High Sr content indicates that aragonite was the precursor mineral, which was transformed into stable low-Mg calcite during diagenesis. 9Biogeochemical evaluation of Nannorrhops ritchiana: A Mg-flora from Khuzdar, Balochistan, Pakistan显示文摘Nannorrhops ritchiana (Mazari Palm) is a distinctive flora growing in the Saharo-Sindian region. It is well distributed on the ultramafic soil, derived from the Bela Ophiolite in the Khuzdar District, Balochistan, Pakistan. Quantitative estimation of Ca, Mg, Fe and Ni in soil and plant ash has been carried out. The constituents of plant ash have been discussed in relation to soil chemistry, pH, climate, mobility, average abundance in plant ash and exclusion mechanism of the flora. Relationship among Ca, Mg, Fe and Ni has been established using scattergrams to evaluate the biogeochemistry of the plant. High contents of Mg and high coefficient of biological absorption allow it to be classed as Mg-flora. Both Ca and Fe appeared to be antagonistic to Mg. The metal assemblage of N. ritchiana nicely reflected the nature of the bed rock as being serpentinized ultramafic, and its corresponding soils. Good exclusion mechanism of N. ritchiana did not allow it to absorb high Ni from the soil. Relatively high concentrations of Ni in N. ritchiana from the Baran Lak area can be used to localize Ni-mineralization in the study area.; Statistical analyses, such as minimum, maximum, mean, mode, median, standard deviations, and coefficient of correlation, were also made to improve raw geochemical data and interpretations.Sadaf Naseem Shahid Naseem Erum Bashir Khaula Shirin Shamim A. 10First principles investigation of protactinium-based oxide-perovskites for flexible opto electronic devices显示文摘The structural,elastic,mechanical,electronic,and optical properties of KPaO_3 and RbPaO_3 compounds are investigated from first-principles calculations by using the WIEN2 k code in the frame of local density approximation(LDA) and generalized gradient approximation(GGA).The calculated ground state quantities,such as lattice constant(α_0),ground state energy(E),bulk modulus(S),and their pressure derivative(B_p) are in reasonable agreement with the present analytical and previous theoretical results and available experimental data.Based on several elastic and mechanical parameters,the structural stability,hardness,stiffness and the brittle and ductile behaviors are discussed,which reveal that protactiniumbased oxide series of perovskites is mechanically stable and possesses weak resistance to shear deformation compared with resistance to unidirectional compression while flexible and covalent behaviors are dominated in them.The analysis of band profile through Trans-Blaha modified Becke-Johnson(TB-mBJ) potential highlights the underestimation of bandgap with traditional density functional theory(DFT) approximation.Specific contribution of electronic states is investigated by means of total and partial density of states and it can be evaluated that both compounds are(Γ-Γ) direct bandgap semiconductors.All fundamental optical properties are analyzed while attention is paid to absorption and reflection spectra to explore extensive absorptions and reflections of these compounds in high frequency regions.The present method represents an influential approach to calculating the whole set of elastic,mechanical,and opto-electronic parameters,which would conduce to the understanding of various physical phenomena and empower the device engineers to implement these materials in flexible opto-electronic applications.Nazia Erum Muhammad Azhar Iqbal 2017Chinese Physics B2017,26,4:1
