| 5 | Obstructive jaundice due to hepatobiliary cystadenoma or cystadenocarcinoma显示文摘Hepatobiliary cystadenomas (HBC) and cystadenocarci- nomas are rare cystic lesions. Most patients with these lesions are asymptomatic, but presentation with ob- structive jaundice may occur. The first patient presented with intermittent colicky pain and recurrent obstructive jaundice. Imaging studies revealed a polypoid lesion in the left hepatic duct. The second patient had recurrent jaundice and cholangitis. Endoscopic retrograde cholan- giopancreatography (ERCP) showed a cystic lesion at the confluence of the hepatic duct. In the third patient with intermittent jaundice and cholangitis, cholangioscopy re- vealed a papillomatous structure protruding into the left bile duct system. In the fourth patient with obstructive jaundice, CT-scan showed slight dilatation of the intrahe- patic bile ducts and dilatation of the common bile duct of 3 cm. ERCP showed filling of a cystic lesion. All patients underwent partial liver resection, revealing HBC in the specimen. In the fifth patient presenting with obstructive jaundice, ultrasound examination showed a hyperecho- genic cystic lesion centrally in the liver. The resection specimen revealed a hepatobiliary cystadenocarcinoma. HBC and cystadenocarcinoma may give rise to obstruc- tive jaundice. Evaluation with cross-sectional imaging techniques is useful. | 8 | Fiber effects in nutrition and gut health in pigs显示文摘Dietary fiber is associated with impaired nutrient utilization and reduced net energy values.However,fiber has to be included in the diet to maintain normal physiological functions in the digestive tract.Moreover,the negative impact of dietary fiber will be determined by the fiber properties and may differ considerably between fiber sources.Various techniques can be applied to enhance nutritional value and utilization of available feed resources.In addition,the extent of fiber utilization is affected by the age of the pig and the pig breed.The use of potential prebiotic effects of dietary fiber is an attractive way to stimulate gut health and thereby minimize the use of anti-microbial growth promoters.Inclusion of soluble non-starch polysaccharides(NSP) in the diet can stimulate the growth of commensal gut microbes.Inclusion of NSP from chicory results in changes in gut micro-environment and gut morphology of pigs,while growth performance remains unaffected and digestibility was only marginally reduced.The fermentation products and pH in digesta responded to diet type and were correlated with shifts in the microbiota.Interestingly,fiber intake will have an impact on the expression of intestinal epithelial heat-shock proteins in the pig.Heat-shock proteins have an important physiological role in the gut and carry out crucial housekeeping functions in order to maintain the mucosal barrier integrity.Thus,there are increasing evidence showing that fiber can have prebiotic effects in pigs due to interactions with the gut micro-environment and the gut associated immune system. | Jan Erik Lindberg | 2014 | Journal of Animal Science and Biotechnology2014,5,3: | 18 |

| 10 | 肿瘤恶性转化与C/EBPβ和STAT3基因相关(英文)显示文摘The inference of transcriptional networks that regulate transitions into physiological or pathological cellular states remains a central challenge in systems biology. A mesenchymal phenotype is the hallmark of tumour aggressiveness in human malignant glioma,but the regulatory programs responsible for implementing the associated molecular signature are largely unknown. Here we show that reverse-engineering and an unbiased interrogation of a glioma-specific regulatory network reveal the transcriptional module that activates expression of mesenchymal genes in malignant glioma. Two transcription factors (C/EBPβ and STAT3) emerge as synergistic initiators and master regulators of mesenchymal transformation. Ectopic co-expression of C/EBPβ and STAT3 reprograms neural stem cells along the aberrant mesenchymal lineage,whereas elimination of the two factors in glioma cells leads to collapse of the mesenchymal signature and reduces tumour aggressiveness. In human glioma,expression of C/EBPβ and STAT3 correlates with mesenchymal differentiation and predicts poor clinical outcome. | 11 | Results of percutaneous sclerotherapy and surgical treatment in patients with symptomatic simple liver cysts and polycystic liver disease显示文摘AIM: To evaluate the results of the treatment of simple liver cysts (solitary and multiple) and polycystic liver disease (PLD) using percutaneous sclerotherapy and/or surgical procedures in a single tertiary referral centre. METHODS: Retrospective analysis of 54 patients referred for evaluation and possible treatment of simple liver cysts (solitary and multiple) and PLD, from January 1997 to July 2006. RESULTS: Simple liver cysts were treated in 41 pts (76/) with a mean size of 12.6 cm. The most common reason for referral was abdominal pain or discomfort (85/). Percutaneous sclerotherapy was performed as initial treatment in 30 pts, showing cyst recurrence in 6 pts (20/). Surgical treatment was initially performed in 11 pts with cyst recurrence in 3 pts (27/). PLD was treated in 13 pts (24/) with a mean size of the dominant cyst of 13 cm. Percutaneous sclerotherapy for PLD was performed in 9 pts with recurrence in 7 pts (77.8/). Surgical treatment for PLD was undertaken in 4 pts (30.8/) with recurrence in all. Eventually, 2 pts with PLD in the presence of polycystic kidney disease underwent liver-and kidney transplantation because of deterioration of liver and kidney function. CONCLUSION: The majority of patients with simple liver cysts and PLD are referred for progressive abdominal pain. As initial treatment, percutaneous sclerotherapy is appropriate. Surgical deroofing is indicated in caseof cyst recurrence after percutaneous sclerotherapy. However, the results of percutaneous sclerotherapy and surgical treatment for PLD are disappointing. | 13 | Failed medial patellofemoral ligament reconstruction:Causes and surgical strategies显示文摘Patel ar instability is a common clinical problem encountered by orthopedic surgeons specializing in the knee. For patients with chronic lateral patellar instability, the standard surgical approach is to stabilize the patella through a medial patellofemoral ligament(MPFL) reconstruction. Foreseeably, an increasing number of revision surgeries of the reconstructed MPFL will be seen in upcoming years. In this paper, the causes of failed MPFL reconstruction are analyzed:(1) incorrect surgical indication or inappropriate surgical technique/patient selection;(2) a technical error; and(3) an incorrect assessment of the concomitant risk factors for instability. An understanding of the anatomy and biomechanics of the MPFL and cautiousness with the imaging techniques while favoring clinical over radiological findings and the use of common sense to determine the adequate surgical technique for each particular case, are critical to minimizing MPFL surgery failure. | 19 | Induction of therapeutic hypothermia via the esophagus:a proof of concept study显示文摘BACKGROUND:Induction of hypothermia(a 4℃decrease from baseline)improves outcomes in adult cardiac arrest and neonatal hypoxic ischemic encephalopathy,and may benefit other conditions as well.Methods used to implement or prevent hypothermia typically require skin contact with blankets or pads or intravascular access with catheter devices.The study was to evaluate the potential to induce mild therapeutic hypothermia via an esophageal route in a porcine model.METHODS:Single-animal proof-of-concept study of a prototype esophageal device in a 70 kg Yorkshire swine.We measured the rate of temperature change after placement of a prototype device to induce hypothermia via the esophagus,and compared this rate to known temperature changes that occur under similar laboratory conditions without a hypothermic device.RESULTS:Swine temperature decreased from a starting temperature of 37.8℃to 33.8℃(achieving the goal of a 4℃decrease)in 175 minutes,resulting in a cooling rate of 1.37℃/h.Histopathology of the esophagus showed normal tissue without evidence of injury.CONCLUSION:A prototype of an esophageal cooling device induced hypothermia effectively in a large single-swine model. | Erik B.Kulstad D.Mark Courtney Donald Waller | 2012 | World Journal of Emergency Medicine2012,3,2: | 11 |
