| 1 | Metabolic-associated Fatty Liver Disease(MAFLD):A Multi-systemic Disease Beyond the Liver显示文摘Nonalcoholic fatty liver disease(NAFLD)is a multisystemic clinical condition that presents with a wide spectrum of extrahepatic manifestations,such as obesity,type 2 diabetes mellitus,metabolic syndrome,cardiovascular diseases,chronic kidney disease,extrahepatic malignancies,cogni-tive disorders,and polycystic ovarian syndrome.Among NAFLD patients,the most common mortality etiology is cardiovascular disorders,followed by extrahepatic malignancies,diabetes mellitus,and liver-related complications.Furthermore,the severity of extrahepatic diseases is parallel to the severity of NAFLD.In clinical practice,awareness of the associations of concomitant diseases is of major importance for initiating prompt and timely screening and multidisciplinary management of the disease spectrum.In 2020,a consensus from 22 countries redefined the disease as metabolic(dysfunction)-associated fatty liver disease(MAFLD),which resulted in the redefinition of the corre-sponding population.Although the patients diagnosed with MAFLD and NAFLD mostly overlap,the MAFLD and NAFLD populations are not identical.In this review,we compared the associations of key extrahepatic diseases between NAFLD and MAFLD. | Eda Kaya Yusuf Yilmaz | 2022 | Journal of Clinical and Translational Hepatology2022,10,2: | 8 |
| 2 | Direct-fed microbes: A tool for improving the utilization of low quality roughages in ruminants显示文摘For many years, ruminant nutritionists and microbiologists have been interested in manipulating the microbial ecosystem of the rumen to improve production efficiency of different ruminant species. Removal and restriction of antibiotics subtherapeutic uses from ruminant diets has amplified interest in improving nutrient utilization and animal performance and search for more safe alternatives. Some bacterial and fungal microorganisms as a direct-fed microbial(DFM) can be the most suitable solutions. Microorganisms that are commonly used in DFM for ruminants may be classified mainly as lactic acid producing bacteria(LAB), lactic acid utilizing bacteria(LUB), or other microorganism's species like Lactobacillus, Bifidobacterium, Enterococcus, Streptococcus, Bacillus, Propionibacterium, Megasphaera elsdenii and Prevotellabryantii, in addition to some fungal species of yeast such as Saccharomyces and Aspergillus. A definitive mode of action for bacterial or fungal DFM has not been established; although a variety of mechanisms have been suggested. Bacterial DFM potentially moderate rumen conditions, and improve weight gain and feed efficiency. Fungal DFM may reduce harmful oxygen from the rumen, prevent excess lactate production, increase feed digestibility, and alter rumen fermentation patterns. DFM may also compete with and inhibit the growth of pathogens, immune system modulation, and modulate microbial balance in the gastrointestinal tract. Improved dry matter intake, milk yield, fat corrected milk yield and milk fat content were obtained with DFM administration. However, the response to DFM is not constant; depending on dosages, feeding times and frequencies, and strains of DFM. | 4 | Understanding the need for pre-injection from permeability measurements: What is the connection?显示文摘Pre-grouting ahead of tunnels has three main functions:to control water inflow into the tunnel,to limit groundwater drawdown above the tunnel,and to make tunnelling progress more predictable since rock mass quality is effectively improved.It helps to avoid settlement damage caused by consolidation of clay deposits beneath built-up areas,since towns tend to be built where terrain is more flat,due to the clay deposits.There are so many instances of settlement damage that the profession needs to take note of the need for high-pressure pre-grouting,to use micro-cements and micro-silica additives.The use of highpressure injection may cause joint jacking,but this is local in extent when the rapid pressure decay away from an injection hole is understood.This effect is variable and depends on the geometrical parameters of the joints.This pressure-decay advantage must not be violated by maintaining high pressure when grout flow from the injection hole has ceased.The latter can cause damage to the grouting already achieved.Simplified methods of estimating mean hydraulic apertures(e)from Lugeon testing are described,and from more sophisticated three-dimensional(3D)permeability measurement.The estimation of the larger mean physical joint apertures(E)is based on the joint roughness coefficient(JRC).Comparison is then made with the empirical aperture-particle size criterion E>4d95,where d95 represents almost the largest cement particle size.Depending on joint set orientations and on the available micro-cements,the decision must be made of which range of pre-injection pressure should be aimed for,using successive reductions of the water-cement ratio w/c.More simple estimation of permeability,also with depth dependence,can be made with the empirical link between a modified rock mass quality Q and permeability,which is termed QH2O.The value of this parameter can be based on core-logging or intunnel face logging.The 3D before-and-after-grouting permeability measurements have been used to justify the quantification of rock mass quality Q-parameter improvement,and the consequent increases in expected P-wave velocity and deformation modulus,for application in dam foundation treatment and its monitoring. | Nick Barton Eda Quadros | 2019 | Journal of Rock Mechanics and Geotechnical Engineering2019,11,3: | 5 |
| 5 | Voltage gated calcium channel antibody-related neurological diseases显示文摘Voltage gated calcium channel(VGCC) antibodies are generally associated with Lambert-Eaton myasthenic syndrome. However the presence of this antibody has been associated with paraneoplastic as well as nonparaneoplastic cerebellar degeneration. Most patients with VGCC-antibody-positivity have small cell lung cancer(SCLC). Lambert-Eaton myasthenic syndrome(LEMS)is an autoimmune disease of the presynaptic part of the neuromuscular junction. Its classical clinical triadis proximal muscle weakness, areflexia and autonomic dysfunction. Fifty to sixty percent of LEMS patients have a neoplasia, usually SCLC. The co-occurrence of SCLC and LEMS causes more severe and progressive disease and shorter survival than non-paraneoplastic LEMS. Treatment includes 3,4 diaminopyridine for symptomatic purposes and immunotherapy with prednisolone, azathioprine or intravenous immunoglobulin in patients unresponsive to 3,4 diaminopyridine. Paraneoplastic cerebellar degeneration(PCD) is a syndrome characterized with severe, subacute pancerebellar dysfunction. Serum is positive for VGCC antibody in 41%-44% of patients, usually with the co-occurrence of SCLC. Clinical and electrophysiological features of LEMS are also present in 20%-40% of these patients. Unfortunately, PCD symptoms do not improve with immunotherapy. The role of VGCC antibody in the immunopathogenesis of LEMS is well known whereas its role in PCD is still unclear.
