| 2 | 限制性与自由输血策略对心血管疾病患者非心脏手术效果的影响:系统回顾与Meta分析显示文摘目的比较采取限制性与自由输血策略的心血管疾病患者的非心脏手术结局。数据来源搜集涉及医院红细胞输注阈值的随机控制试验。检索CENTRAL、Medline、EMBase、CINAHL、PubMed、LILACS、NHSBT输血证据库、Clinical Trials.gov、WHO国际临床试验注册平台、国际标准随机对照临床试验编号登记、欧盟临床试验登记。检索时间为2015-11-02之前。试验选择选取发表与未发表的比较限制性与自由输血阈值的随机控制试验,试验均纳入心血管疾病患者。结果检索到41项试验,其中7项试验纳入心血管疾病患者。另选取4项纳入心血管疾病患者的试验。最终共搜集11项纳入心血管疾病患者(n=3 033)的试验进行Meta分析(限制性输血1 514例,自由输血1 519例)。输血阈值与30 d死亡率之间联系的汇集风险比例为1.15〔95%CI(0.88,1.50),P=0.50〕,异质性较小(I^2=14%)。限制性输血患者的急性冠脉综合征风险与自由输血患者相比更高〔9项试验;风险比例1.78,95%CI(1.18,2.70),P=0.01,I^2=0%〕。结论对急性冠脉综合征或慢性心血管疾病患者采取80 g/L以下的限制性输血阈值可能并不安全,对病死率与其他结局的影响并不确定。这些数据可暂时支持对急性或慢性心血管疾病患者使用更加自由的输血阈值(>80 g/L),但有待更具有说服力的高质量的心血管疾病患者随机试验进行验证。 | Docherty AB O' Donnell R Brunskill S 本刊编辑部 | 2016 | 中国全科医学2016，19，15： | 12 |

| 5 | Determinants of the impact of blood pressure variability on neurological outcome after acute ischaemic stroke显示文摘Introduction: Increased blood pressure variability (BPV) is detrimental after acute ischaemic stroke, but the interaction between BPV and neuroimaging factors that directly influence stroke outcome has not been explored. Methods: We retrospectively reviewed inpatients from 2007 to 2014 with acute anterior circulation ischaemic stroke, CT perfusion and angiography at hospital admission, and a modified Rankin Scale (MRS) 30- 365 days after stroke onset. BPV indices included SD, coefficient of variation and successive variation of the systolic blood pressure between 0 and 120 hours after admission. Ordinal logistic regression models were fitted to MRS with predictor variables of BPV indices. Models were further stratified by CT perfusion volumetric measurements, proximal vessel occlusion and collateral score. Results: 110 patients met the inclusion criteria. The likelihood of a 1-point rise in the MRS increased with every 10 mm Hg increase in BPV (OR for the 3 BPV indices ranged from 2.27 to 5.54), which was more pronounced in patients with larger ischaemic core volumes (OR 8.37 to 18.0) and larger hypoperfused volumes (OR 6.02 to 15.4). This association also held true for patients with larger mismatch volume, proximal vessel occlusion and good collateral vessels. Conclusions: These results indicate that increased BPV is associated with worse neurological outcome after stroke, particularly in patients with a large lesion core volume, concurrent viable ischaemic penumbra, proximal vessel occlusion and good collaterals. | 10 | The contribution of botanic gardens to ex situ conservation through seed banking显示文摘Target 8 of the Global Strategy for Plant Conservation calls for 'at least 75 per cent of threatened plant species in ex situ collections, preferably in the country of origin, and at least 20 per cent available for recovery and restoration programmes by 2020'.Botanic gardens make a significant contribution to ex situ conservation of wild species with more than a third of plant species represented in botanic gardens collections. These collections are a combination of living collection and seed banked material. Seed banking can provide an efficient form of conservation for wild plant genetic diversity.Information from Botanic Gardens Conservation International's(BGCI) databases(GardenSearch,PlantSearch, ThreatSearch and GlobalTreeSearch) has been analysed as well as survey data to report on global, regional and national seed banking trends.Information from BGCI's databases indicates that there are at least 350 seed banking botanic gardens in 74 countries. In total 56,987 taxa have been banked including more than 9000 taxa that are threatened with extinction. 6881 tree species are stored in ex situ seed bank collections. More than half(3562) of these tree species are single country endemics and represent species from more than 166 countries.This study suggests that institutions are increasingly conserving plant species via seed banking.However the majority of species in collections that have a conservation assessment are not threatened with extinction. This disjunction between species that are threatened and those conserved in seed banks needs to be addressed. Data from BGCI's databases can be used to enable prioritisation of threatened plant species for collection and conservation in seed banks. | 11 | Decreased expression of hyperpolarisation-activated cyclic nucleotide-gated channel 3 in Hirschsprung's disease显示文摘AIM: To determine if hyperpolarisation-activated nucleotide-gated(HCN) channels exist in human colon, and to investigate the expression of HCN channels in Hirschsprung's disease.METHODS:We investigated HCN1,HCN2,HCN3 and HCN4 protein expression in pull-through specimens from patients with Hirschsprung’s disease(HSCR,n=10)using the proximal-most ganglionic segment and distalmost aganglionic segment,as well as in healthy control specimens obtained at the time of sigmoid colostomy closure in children who had undergone anorectoplasty for imperforate anus(n=10).Fluorescent immunohistochemistry was performed to assess protein distribution,which was then visualized using confocal microscopy.RESULTS:No HCN1 channel expression was observed in any of the tissues studied.Both HCN2 and HCN4proteins were found to be equally expressed in the aganglionic and ganglionic bowel in HSCR and controls.HCN3 channel expression was found to be markedly decreased in the aganglionic colon vs ganglionic colon and controls.HCN2-4 channels were seen to be expressed within neurons of the myenteric and submucosal plexus of the ganglionic bowel and normal controls,and also co-localised to interstitial cells of Cajal in all tissues studied.CONCLUSION:We demonstrate HCN channel expression in human colon for the first time.Reduced HCN3expression in aganglionic bowel suggests its potential role in HSCR pathophysiology. | Anne Marie O’Donnell David Coyle Prem Puri | 2015 | World Journal of Gastroenterology2015，21，18： | 4 |

| 15 | The Importance of Heparan Sulfate in Herpesvirus Infection显示文摘Herpes simplex virus type-1 (HSV-1) is one of many pathogens that use the cell surface glycosaminoglycan heparan sulfate as a receptor. Heparan sulfate is highly expressed on the surface and extracellular matrix of virtually all cell types making it an ideal receptor. Heparan sulfate interacts with HSV-1 envelope glycoproteins gB and gC during the initial attachment step during HSV-1 entry. In addition,a modified form of heparan sulfate,known as 3-O-sulfated heparan sulfate,interacts with HSV-1 gD to induce fusion between the viral envelope and host cell membrane. The 3-O-sulfation of heparan sulfate is a rare modification which occurs during the biosynthesis of heparan sulfate that is carried out by a family of enzymes known as 3-O-sulfotransferases. Due to its involvement in multiple steps of the infection process,heparan sulfate has been a prime target for the development of agents to inhibit HSV entry. Understanding how heparan sulfate functions during HSV-1 infection may not only be critical for inhibiting infection by this virus,but it may also be crucial in the fight against many other pathogens as well. | Christopher D.
