| 1 | Acinar cell injury induced by inadequate unfolded protein response in acute pancreatitis显示文摘Acute pancreatitis (AP) is an inflammatory disorder of pancreatic tissue initiated in injured acinar cells. Severe AP remains a significant challenge due to the lack of effective treatment. The widely-accepted autodigestion theory of AP is now facing challenges, since inhibiting protease activation has negligible effectiveness for AP treatment despite numerous efforts. Furthermore, accumulating evidence supports a new concept that malfunction of a self-protective mechanism, the unfolded protein response(UPR), is the driving force behind the pathogenesis of AP. The UPR is induced by endoplasmic reticulum(ER) stress, a disturbance frequently found in acinar cells, to prevent the aggravation of ER stress that can otherwise lead to cell injury. In addition, the UPR's signaling pathways control NFκB activation and autophagy flux, and these dysregulations cause acinar cell inflammatory injury in AP, but with poorly understood mechanisms. We therefore summarize the protective role of the UPR in AP, propose mechanistic models of how inadequate UPR could promote NFκB's pro-inflammatory activity and impair autophagy's protective function in acinar cells, and discuss its relevance to current AP treatment. | 3 | Managements of osteoporotic vertebral compression fractures:A narrative review显示文摘Osteoporotic vertebral compression fractures(OVCFs)are the most common fragility fracture and significantly influence the quality of life in the elderly.Currently,the literature lacks a comprehensive narrative review of the management of OVCFs.The purpose of this study is to review background information,diagnosis,and surgical and non-surgical management of the OVCFs.A comprehensive search of PubMed and Google Scholar for articles in the English language between 1980 and 2021 was performed.Combinations of the following terms were used:compression fractures,vertebral compression fractures,osteoporosis,osteoporotic compression fractures,vertebroplasty,kyphoplasty,bisphosphonates,calcitonin,and osteoporosis treatments.Additional articles were also included by examining the reference list of articles found in the search.OVCFs,especially those that occur over long periods,can be asymptomatic.Symptoms of acute OVCFs include pain localized to the mid-line spine,a loss in height,and decreased mobility.The primary treatment regimens are pain control,medication management,vertebral augmentation,and anterior or posterior decompression and reconstructions.Pain control can be achieved with acetaminophen or nonsteroidal anti-inflammatory drugs for mild pain or opioids and/or calcitonin for moderate to severe pain.Bisphosphonates and denosumab are the first-line treatments for osteoporosis.Vertebroplasty and kyphoplasty are reserved for patients who have not found symptomatic relief through conservative methods and are effective in achieving pain relief.Vertebroplasty is less technical and cheaper than kyphoplasty but could have more complications.Calcium and vitamin D supplementation can have a protective and therapeutic effect.Management of OVCFs must be combined with multiple approaches.Appropriate exercises and activity modification are important in fracture prevention.Medication with different mechanisms of action is a critical long-term causal treatment strategy.The minimally invasive surgical interventions such as vertebroplasty and kyphoplasty are reserved for patients not responsive to conservative therapy and are recognized as efficient stopgap treatment methods.Posterior decompression and fixation or Anterior decompression and reconstruction may be required if neurological deficits are present.The detailed pathogenesis and related targeted treatment options still need to be developed for better clinical outcomes. | Devon Patel Jiayong Liu Nabil A Ebraheim | 2022 | World Journal of Orthopedics2022，13，6： | 5 |

| 6 | Burial and exhumation history of the Xigaze forearc basin, Yarlung suture zone. Tibet显示文摘The Cretaceous-Eocene Xigaze forearc basin is a crucial data archive for understanding the tectonic history of the Asian continental margin prior to and following collision with India during the early Cenozoic Era. This study reports apatite and zircon(U-Th)/He thermochronologic data from fourteen samples from Albian-Ypresian Xigaze forearc strata to determine the degree and timing of heating(burial) and subsequent cooling(exhumation) of two localities along the Yarlung suture zone(YSZ) near the towns of Saga and Lazi. Thirty-seven individual zircon He ages range from 31.5 ± 0.8 Ma to6.06 ± 0.18 Ma,with the majority of grains yielding ages between 30 Ma and 10 Ma. Twenty apatite He ages range from 12.7 ± 0.5 Ma to 3.9 ± 0.3 Ma,with the majority of grains yielding ages between 9 Ma and 4 Ma. These ages suggest that the Xigaze forearc basin was heated to 140-200 ℃ prior to cooling in Oligocene-Miocene time. Thermal modeling supports this interpretation and shows that the samples were buried to maximum temperatures of ~140-200 0 C by 35-21 Ma, immediately followed by the onset of exhumation. The zircon He and apatite He dataset and thermal modeling results indicate rapid exhumation from ~21 Ma to 15 Ma, and at ~4 Ma. The 21-15 Ma thermochronometric signal appears to be regionally extensive, affecting all the lithotectonic units of the YSZ, and coincides with movement along the north-vergent Great Counter Thrust system. Thrusting, coupled with enhanced erosion possibly related to the paleo-Yarlung River, likely drove Early Miocene cooling of the Xigaze forearc basin.In contrast, the younger phase of rapid exhumation at ~4 Ma was likely driven by enhanced rock uplift in the footwall of north-striking rifts that cross-cut the YSZ. | Devon A.Orme | 2019 | Geoscience Frontiers2019，10，3： | 2 |
