| 1 | Chronic pancreatitis, pancreatic adenocarcinoma and the black box in-between显示文摘Pancreatic cancer is a challenging disease for patients, doctors and researchers who for decades have searched for a cure for this deadly malignancy. Although existing mouse models of pancreatic cancer have shed light on the mecha- nistic basis of the neoplastic conversion of the pancreas, their impact in terms of offering new diagnostics and therapeu- tic modalities remains limited. Chronic pancreatitis is an inflammatory disease of the pancreas that is associated with a gradual damage of the organ and an increased risk of developing neoplastic lesions. In this review, we propose that detailed studies of chronic inflammatory processes in the pancreas will provide insights into the evolution of pancreatic cancer. | 2 | Systematic review of macular ganglion cell complex analysis using spectral domain optical coherence tomography for glaucoma assessment显示文摘AIM: To review the use of spectral domain optical coherence tomography(SD-OCT) for macular retinal ganglion cells(RGC) and ganglion cell complex(GCC) measurement in glaucoma assessment, specifically for early detection and detection of disease progression. METHODS: A systematic review was performed by searching Pub Med, Medline, and Web of Science for articles published in English through July 2014 describing the various macular SD-OCT scanning strategies developed for glaucoma assessment. The review focused on papers evaluating the use of macular RGC/GCC SDOCT to detect early glaucoma and its progression. The search included keywords corresponding to the index test(macular ganglion cell/RGC/GCC/Spectral domain OCT), the target condition(glaucoma), and diagnostic performance. The RGC/GCC SD-OCT scanning strategies used to assess glaucoma of most commonly used SD-OCT instruments were described and compared. These included the Cirrus high definition-OCT(Carl Zeiss Meditec, Inc., Dublin, CA, United States), RTVue(Optovue, Inc., Fremont, CA, United States), Spectralis(Heidelberg Engineering, Heidelberg, Germany) and the 3D OCT 2000(Topcon Corporation, Tokyo, Japan). Studies focusing on the ability of RGC/GCC SD-OCT to detect early glaucomatous damage and on the correlation between glaucomatous progression and RGC/GCC measurement by SD-OCT were reviewed.RESULTS: According to the literature, macular RGC/GCC SD-OCT has high diagnostic power of preperimetric glaucoma, reliable discrimination ability to differentiate between healthy eyes and glaucomatous eyes, with good correlation with visual filed damage. The current data suggests that it may serve as a sensitive detection tool for glaucomatous structural progression even with mild functional progression as the rate of change of RGC/GCC thickness was found to be significantly higher in progressing than in stable eyes. Glaucoma assessment with RGC/GCC SD-OCT was comparable with and sometimes better than circumpapillary retinal nerve fiber layer thickness measurement.CONCLUSION: An increasing body of evidence supports using macular RGC/GCC thickness as an indicator for early glaucoma. | 9 | Predictors for advanced fibrosis in morbidly obese nonalcoholic fatty liver patients显示文摘AIM To investigate predictors for fibrosis specifically in a high risk population of morbidly obese patients, including detailed evaluation of lifestyle. METHODS We conducted a cross-sectional study among morbidly obese patients attending the bariatric clinic at the TelAviv Medical Center between the years 2013-2014 with body mass index(BMI) above 40 or above 35 with co-morbidity. Patients with serum hepatitis B surface antigen or anti-hepatitis C virus antibodies, genetic liver diseases, autoimmune disease or high alcohol intake(≥ 30 g/d in men or ≥ 20 g/d in women) were excluded from the study. Liver fibrosis was estimated by transient elastography(Fibro Scan?), using the ‘‘XL'' probe. We collected data on age and gender, education, smoking status and amount, medical history, nutrition and lifestyle habits. All these data were collected using structured and validated questionnaires. Fasting blood test were available for a subsample. RESULTS Fibroscan was performed on a total of 91 patients, of which 77 had a valid examination according to the accepted criteria. Of those, 21% had significant fibrosis(F2) and 39% had advanced or severe fibrosis(F3 or F4). In multivariate analysis, male gender and BMI had a positive association with advanced fibrosis; the OR for fibrosis F ≥ 2 was 7.93(95%CI: 2.36-26.64, P = 0.001) for male gender and 1.33(1.11-1.60 kg/m2, P = 0.002) for BMI. The OR for fibrosis F ≥ 3 was 2.92(1.08-7.91, P = 0.035) for male gender and 1.17(1.03-1.33, P = 0.018) for BMI. Subjects were categorized to subgroups based on the combination of male gender and BMI of 40 and above. A significant dose response association with stiffness level was noted across these categories, with the highest stiffness among men with a higher BMI(P = 0.001).
