1Management of necrotizing pancreatitis显示文摘Infection complicating pancreatic necrosis leads to persisting sepsis, multiple organ dysfunction syndrome and accounts for about half the deaths that occur following acute pancreatitis. Severe cases due to gallstones require urgent endoscopic sphincterotomy. Patients with pancreatic necrosis should be followed with serial contrast enhanced computed tomography (CE-CT) and if infection is suspected fine needle aspiration of the necrotic area for bacteriology (FNAB) should be undertaken. Treatment of sterile necrosis should initially be non-operative. In the presence of infection necrosectomy is indicated. Although traditionally this has been by open surgery, minimally invasive procedures are a promising new alternative. There are many unresolved issues in the management of pancreatic necrosis. 2Review of evolution of tunnel position in anterior cruciate ligament reconstruction显示文摘Anterior cruciate ligament(ACL) rupture is one of the commonest knee sport injuries. The annual incidence of the ACL injury is between 100000-200000 in the United States. Worldwide around 400000 ACL reconstructions are performed in a year. The goal of ACL reconstruction is to restore the normal knee anatomy and kinesiology. The tibial and femoral tunnel placements are of primordial importance in achieving this outcome. Otherfactors that influence successful reconstruction are types of grafts, surgical techniques and rehabilitation programmes. A comprehensive understanding of ACL anatomy has led to the development of newer techniques supplemented by more robust biological and mechanical concepts. In this review we are mainly focussing on the evolution of tunnel placement in ACL reconstruction, focusing on three main categories, i.e., anatomical, biological and clinical outcomes. The importance of tunnel placement in the success of ACL reconstruction is well researched. Definite clinical and functional data is lacking to establish the superiority of the single or double bundle reconstruction technique. 14Socio-economic status and lifestyle factors are associated with achalasia risk: A population-based case-control study显示文摘AIM: To evaluate the association between various lifestyle factors and achalasia risk.METHODS: A population-based case-control study was conducted in Northern Ireland, including n = 151 achalasia cases and n = 117 age- and sexmatched controls. Lifestyle factors were assessed via a face-to-face structured interview. The association between achalasia and lifestyle factors was assessed by unconditional logistic regression, to produce odds ratios(OR) and 95% confidence interval(CI).RESULTS: Individuals who had low-class occupations were at the highest risk of achalasia(OR = 1.88, 95%CI: 1.02-3.45), inferring that high-class occupation holders have a reduced risk of achalasia. A history of foreign travel, a lifestyle factor linked to upper socioeconomic class, was also associated with a reduced risk of achalasia(OR = 0.59, 95%CI: 0.35-0.99). Smoking and alcohol consumption carried significantly reduced risks of achalasia, even after adjustment for socio-economic status. The presence of pets in the house was associated with a two-fold increased risk of achalasia(OR = 2.00, 95%CI: 1.17-3.42). Nochildhood household factors were associated with achalasia risk.CONCLUSION: Achalasia is a disease of inequality, and individuals from low socio-economic backgrounds are at highest risk. This does not appear to be due to corresponding alcohol and smoking behaviours. 18Hypoxia and its impact on the tumour microenvironment of gastroesophageal cancers显示文摘The malfeasant role of the hypoxic tumour microenvironment(TME)in cancer progression was recognized decades ago but the exact mechanisms that augment the hallmarks of cancer and promote treatment resistance continue to be elucidated.Gastroesophageal cancers(GOCs)represent a major burden of worldwide disease,responsible for the deaths of over 1 million people annually.Disentangling the impact of hypoxia in GOCs enables a better overall understanding of the disease pathogenesis while shining a light on novel therapeutic strategies and facilitating precision treatment approaches with the ultimate goal of improving outcomes for patients with these diseases.This review discusses the underlying principles and processes of the hypoxic response and the effect of hypoxia in promoting the hallmarks of cancer in the context of GOCs.We focus on its bidirectional influence on inflammation and how it drives angiogenesis,innate and adaptive immune evasion,metastasis,and the reprogramming of cellular bioenergetics.The contribution of the hypoxic GOC TME to treatment resistance is examined and a brief overview of the pharmacodynamics of hypoxiatargeted therapeutics is given.The principal methods that are used in measuring hypoxia and how they may enhance prognostication or provide rationale for individually tailored management in the case of tumours with significant hypoxic regions are also discussed.Ross King Conall Hayes Claire L Donohoe Margaret R Dunne Maria Davern Noel E Donlon 2021World Journal of Gastrointestinal Oncology2021,13,5:0
19Linking the effect of localised pitting corrosion with mechanical integrity of a rare earth magnesium alloy for implant use显示文摘This study presents a computational framework that investigates the effect of localised surface-based corrosion on the mechanical performance of a magnesium-based alloy.A finite element-based phenomenological corrosion model was used to generate a wide range of corrosion profiles,with subsequent uniaxial tensile test simulations to predict the mechanical response to failure.The python-based detection framework PitScan provides detailed quantification of the spatial phenomenological features of corrosion,including a full geometric tracking of corroding surface.Through this approach,this study is the first to quantitatively demonstrate that a surface-based non-uniform corrosion model can capture both the geometrical and mechanical features of a magne-sium alloy undergoing corrosion by comparing to experimental data.Using this verified corrosion modelling approach,a wide range of corrosion scenarios was evaluated and enabled quantitative relationships to be established between the mechanical integrity and key phenomenological corrosion features.In particular,we demonstrated that the minimal cross-sectional area parameter was the strongest predictor of the remaining mechanical strength(R2=0.98),with this relationship being independent of the severity or spatial features of localised surface corrosion.Interestingly,our analysis demonstrated that parameters described in ASTM G46-94 showed weaker correlations to the mechanical integrity of corroding specimens,compared to parameters determined by Pitscan.This study establishes new mechanistic insight into the performance of the magnesium-based materials undergoing corrosion.Kerstin van Gaalen Conall Quinn Felix Benn Peter E.McHugh Alexander Kopp Ted J.Vaughan 2023Bioactive Materials2023,,3:0
20Predicting localised corrosion and mechanical performance of a PEO surface modified rare earth magnesium alloy for implant use through in-silico modelling显示文摘In this study,the influence of a plasma electrolytic oxidation(PEO)surface treatment on a medical-grade WE43-based magnesium alloy is examined through an experimental and computational framework that considers the effects of localised corrosion features and mechanical properties throughout the corrosion process.First,a comprehensive in-vitro immersion study was performed on WE43-based tensile specimens with and without PEO surface modification,which included fully automated spatial reconstruction of the phenomenological features of corrosion through micro-CT scanning,followed by uniaxial tensile testing.Then the experimental data of both unmodified and PEO-modified groups were used to calibrate parameters of a finite element-based surface corrosion model.In-vitro,it was found that the WE43-PEO modified group had a significantly lower corrosion rate and maintained significantly higher mechanical properties than the unmodified.While corrosion rates were~50%lower in the WE43-PEO modified specimens,the local geometric features of corroding surfaces remained similar to the unmodified WE43 group,however evolving after almost the double amount of time.We were also able to quantitatively demonstrate that the PEO surface treatment on magnesium continued to protect samples from corrosion throughout the entire period tested,and not just in the early stages of corrosion.Using the results from the testing framework,the model parameters of the surface-based corrosion model were identified for both groups.This enabled,for the first time,in-silico prediction of the physical features of corrosion and the mechanical performance of both unmodified and PEO modified magnesium specimens.This simulation framework can enable future in-silico design and optimisation of bioabsorbable magnesium devices for load-bearing medical applications.Kerstin van Gaalen Conall Quinn Marek Weiler Felix Gremse Felix Benn Peter E.McHugh Ted J.Vaughan Alexander Kopp 2023Bioactive Materials2023,,8:0
