| 1 | Clinical characteristics and outcome of a cohort of 101 patients with hepatocellular carcinoma显示文摘AIM To conduct a cohort study of 101 patients with hepatocellular carcinoma (HCC) presenting to a tertiary care medical referral center in Germany between 1997 and 1999.METHODS AND RESULTS Data were retrospectively analyzed by chart review. In 95cases (72 males and 23 females) sufficient data were available for analysis. Twenty-five (29%)of 8.5 patients were HBsAg or anti-HBc positive,21/85 (25%) were anti-HCV positive, and 6/85(7%) were positive for both HBV and HCV-markers. Age was significantly lower in HBV-positive patients than in the other two groups.Thirty-one (34%) of 90 patients had histories of alcohol abuse, In 79/ 94 (84%) patients,cirrhosis was diagnosed. Of these cirrhotic patients, 29/79 (37%) belonged to Child-Pugh's group (CHILD) A, 32/79 (40%) to CHILD B, and 18/79 (23%) to CHILD C. AFP was elevated in 61/91 (67%) patients. A single tumor nodule was found in 38/94 (40%), more than one nodule in 31/94 (34%), and 25/94 (26%) had a diffusely infiltrating tumor, i.e. the tumor margins could not be seen on imaging procedures. Portal vein thrombosis was present in 19/ 94 (20%).Imaging data consistent with lymph node metastases were found in 10/92 (11%), while distant metastases were found in 8/93 (9%).According to Okuda 28/94 (30%) were grouped to stage Ⅰ, 53/94 (56%) were grouped to stage Ⅱ, and 13/94 (14%) were grouped to stage Ⅲ.Survival data were available for 83 patients. The Kaplan-Meier estimate for median survival was 8.4 months. Factors influencing survival were the Okuda score, the presence of portal vein thrombosis, and the presence of ascites. The presence of non-complicated liver cirrhosis by itself, distant metastases, or infection with hepatitis viruses did not influence survival. AFP positivity by itself did not influence survival,though patients with an AFP value greater than 100μg/L did experience shortened survival.Treatment besides tamoxifen or supportive care was associated with prolonged survival. The influence of therapy on survival was most pronounced in Okuda stage Ⅱ patients. There was longer survival in those Okuda stage Ⅱpatients who were treated with percutaneous ethanol injection.CONCLUSION Even in a Iow incidence area such as Germany, the majority of HCC is caused by viral hepatitis and therefore potentially preventable. Reflecting the high proportion of advanced stage tumors in our patients, the median survival was poor. | 2 | 'Micro-deletions' of the human Y chromosome and their relationship with male infertility显示文摘The Y chromosome evolves from an autochromosome and accumulates male-related genes including sex-determining region of Y-chromosome (SRY) and several spermatogenesis-related genes. The human Y chromosome (60 Mb long) is largely composed of repeti-tive sequences that give it a heterochromatic appearance, and it consists of pseudoautosomal, euchromatic, and heterochromatic regions. Located on the two extremities of the Y chromosome, pseudoautosomal regions 1 and 2 (PAR1 and PAR2, 2.6 Mb and 320 bp long, re-spectively) are homologs with the termini of the X chromosome. The euchromatic region and some of the repeat-rich heterochromatic parts of the Y chromosome are called 'male-specific Y' (MSY), which occupy more than 95% of the whole Y chromosome. After evolu-tion, the Y chromosome becomes the smallest in size with the least number of genes but with the most number of copies of genes that are mostly spermatogenesis-related. The Y chromosome is characterized by highly repetitive sequences (including direct repeats, inverted repeats, and palindromes) and high polymorphism. Several gene rearrangements on the Y chromosome occur during evolution owing to its specific gene structure. The consequences of such rearrangements are not only loss but also gain of specific genes. One hundred and fifty three haplotypes have been discovered in the human Y chromosome. The structure of the Y chromosome in the GenBank belongs to haplotype R1. There are 220 genes (104 coding genes, 111 pseudogenes, and 5 other uncategorized genes) according to the most recent count. The 104 coding genes encode a total of about 48 proteins/protein families (including putative proteins/protein families). Among them, 16 gene products have been discovered in the azoospermia factor region (AZF) and are related to spermatogenesis. It has been dis-covered that one subset of gene rearrangements on the Y chromosome, 'micro-deletions', is a major cause of male infertility in some populations. However, controversies exist about different Y chromosome haplotypes. Six AZFs of the Y chromosome have been discov-ered including AZFa, AZFb, AZFc, and their combinations AZFbc, AZFabc, and partial AZFc called AZFc/gr/gr. Different deletions in AZF lead to different content spermatogenesis loss from teratozoospermia to infertility in different populations depending on their Y hap-lotypes. This article describes the structure of the human Y chromosome and investigates the causes of micro-deletions and their relation-ship with male infertility from the view of chromosome evolution. After analysis of the relationship between AZFc and male infertility, we concluded that spermatogenesis is controlled by a network of genes, which may locate on the Y chromosome, the autochromosomes, or even on the X chromosome. | 4 | Clinically detected gastroenteropancreatic neuroendocrine tumors are on the rise: Epidemiological changes in Germany显示文摘AIM:To study the epidemiologic changes of gastroenteropancreatic neuroendocrine tumors(GEP-NET)in Germany,we analyzed two time periods 1976-1988 and1998-2006.METHODS:We evaluated epidemiological data of GEP-NET from the former East German National Cancer Registry(DDR Krebsregister,1976-1988)and its successor,the Joint Cancer Registry(GKR,1998-2006),which was founded after German reunification.Due to a particularly substantial database the epidemiological data from the federal states of Mecklenburg-Western Pomerania,Saxony,Brandenburg and Thuringia,covering a population of more than 10.8 million people,were analyzed.Survival probabilities were calculated using life table analysis.In addition,GEP-NET patients were evaluated for one or more second(non-GEP-NET)primary malignancies.RESULTS:A total of 2821 GEP neuroendocrine neoplasms were identified in the two registries.The overall incidence increased significantly between 1976 and2006 from 0.31(per 100.000 inhabitants per year)to2.27 for men and from 0.57 to 2.38 for women.In the later period studied(2004-2006),the small intestine was the most common site.Neuroendocrine(NE)neoplasms of the small intestine showed the largest absolute increase in incidence,while rectal NE neoplasms exhibited the greatest relative increase.Only the incidence of appendiceal NET in women showed little change between 1976 and 2006.Overall survival of patients varied for sex,tumor site and the two periods studied but improved significantly over time.Interestingly,about 20%of the GEP-NET patients developed one or more second malignancies.Their most common location was the gastrointestinal tract.GEP-NET patients without second malignancies fared better than those with one or more of them.CONCLUSION:The number of detected GEP-NET increased about 5-fold in Germany between 1976 and2006.At the same time,their anatomic distribution changed,and the survival of GEP-NET patients improved significantly.Second malignancies are common and influence the overall survival of GEP-NET patients.Thus,GEP-NET warrant our attention as well as intensive research on their tumorigenesis. | Hans Scherübl Brigitte Streller Roland Stabenow Hermann Herbst Michael Hopfner Christoph Schwertner Joachim Steinberg Jan Eick Wanda Ring Krishna Tiwari Soren M Zappe | 2013 | World Journal of Gastroenterology2013,19,47: | 16 |

| 6 | The enhanced X-ray Timing and Polarimetry mission—eXTP显示文摘In this paper we present the enhanced X-ray Timing and Polarimetry mission—eXTP. eXTP is a space science mission designed to study fundamental physics under extreme conditions of density, gravity and magnetism. The mission aims at determining the equation of state of matter at supra-nuclear density, measuring effects of QED, and understanding the dynamics of matter in strong-field gravity. In addition to investigating fundamental physics, eXTP will be a very powerful observatory for astrophysics that will provide observations of unprecedented quality on a variety of galactic and extragalactic objects. | 9 | Effects of grazing intensity on seed production of Caragana stenophylla along a climatic aridity gradient in the Inner Mongolia Steppe,China显示文摘As the increases of climatic aridity and grazing intensity,shrubs play an increasingly important role in grassland ecosystem in arid and semi-arid regions,and its abundance also generally increases.However,the effects of climatic aridity and grazing intensity on sexual reproduction of shrubs in grassland remain largely unclear.In order to understand the effects of grazing intensity and climatic drought stress,and their interaction on seed production of shrub species,we examined the seed number,seed weight and seed yield of Caragana stenophylla under three grazing intensities(fenced,mild grazing and severe grazing) across a climatic aridity gradient(semi-arid,arid,very arid and intensively arid zones) in the Inner Mongolia Steppe,northern China during 2012–2013.The seed number,seed weight and seed yield gradually increased from the semi-arid to the very arid zones,but decreased from the very arid to the intensively arid zones in fenced plots.The seed number and seed yield decreased from the semi-arid to the intensively arid zones in mild and severe grazing treatment plots,therefore,grazing enhanced the suppression effect of climatic aridity on seed production of C.stenophylla.The seed number and seed yield gradually decreased as grazing intensity increased.The seed weight was highest in severe grazing plots,followed by the mild grazing plots and then the fenced plots.Precipitation varied interannually during the study period.We observed that the seed number,seed weight and seed yield were lower in the low precipitation year(2013) than in the high precipitation year(2012).As climatic drought stress increased,the negative effects of grazing on seed production of C.stenophylla also gradually increased.Our results indicated that climatic drought stress may contribute to the encroachment of C.stenophylla shrub in arid zones by promoting its seed production.However,grazing had negative effects on sexual reproduction of C.stenophylla,and the combined effects of drought stress and grazing seriously suppressed sexual reproduction of C.stenophylla in the intensively arid zone. | XIE Lina CHEN Weizhong Christopher A GABLER HAN Lei GUO Hongyu CHEN Qing MA Chengcang GU Song | 2016 | Journal of Arid Land2016,8,6: | 5 |

| 12 | Tyrosine aminotransferase: biochemical and structural properties and molecular dynamics simulations显示文摘Tyrosine aminotransferase(TAT)catalyzes the transamination of tyrosine and other aromatic amino acids.The enzyme is thought to play a role in tyrosinemia type II,hepatitis and hepatic carcinoma recovery.The objective of this study is to investigate its biochemical and structural characteristics and substrate specificity in order to provide insight regarding its involvement in these diseases.Mouse TAT(mTAT)was cloned from a mouse cDNA library,and its recombinant protein was produced using Escherichia coli cells and purified using various chromatographic techniques.The recombinant mTAT is able to catalyze the transamination of tyrosine usingα-ketoglutaric acid as an amino group acceptor at neutral pH.The enzyme also can use glutamate and phenylalanine as amino group donors and p-hydroxyphenylpyruvate,phenylpyruvate and alpha-ketocaproic acid as amino group acceptors.Through macromolecular crystallography we have determined the mTAT crystal structure at 2.9Åresolution.The crystal structure revealed the interaction between the pyridoxal-5′-phosphate cofactor and the enzyme,as well as the formation of a disulphide bond.The detection of disulphide bond provides some rational explanation regarding previously observed TAT inactivation under oxidative conditions and reactivation of the inactive TAT in the presence of a reducing agent.Molecular dynamics simulations using the crystal structures of Trypanosoma cruzi TAT and human TAT provided further insight regarding the substrate-enzyme interactions and substrate specificity.The biochemical and structural properties of TAT and the binding of its cofactor and the substrate may help in elucidation of the mechanism of TAT inhibition and activation. | Prajwalini Mehere Qian Han Justin A.Lemkul Christopher J.Vavricka Howard Robinson David R.Bevan Jianyong Li | 2010 | Protein & Cell2010,1,11: | 4 |

In contrast to other pancreatic tumors,the solid-pseudopapillary tumor has a favorable prognosis.The 60-year-old female patient we report on here was treated by left pancreatic resection combined with splenectomy for a non-metastasizing tumor of the pancreas. A solid-pseudopapillary tumor was found on histology. The patient had no signs of metastases at present.Since a microscopically invasive tumor growth is assumed,oncologically curative resection should be preferred vs the less radical enucleation. | 16 | Significance of scintigraphy for the localisation of obscure gastrointestinal bleedings显示文摘AIM: To determine the role of scintigraphy in patients with gastrointestinal (GI) bleeding of unknown localisation. METHODS: We performed retrospective analyses on 92 patients receiving scintigraphies from 1993 to 2000 in the University of Regensburg hospital, which were done for localisation of GI bleeding as a diagnostic step after an unsuccessful endoscopy. In addition to the scintigraphies, further diagnostic steps such as endoscopy, angiography or operations were performed. In some of the scintigraphies with negative results, a provocation test for bleeding with heparinisation was carried out. RESULTS: 73% of all scintigraphies showed a positive result. In 4.5% of the positive results, the source was located in the stomach, in 37% the source was the small bowel, in 25% the source was the right colon, in 4.5% the source was the left colon, and in 20% no clear localisation was possible. Only 4% of all scintigraphies were false positive. A reliablepositive scintigraphy was independent of the age of the examined patient. A provocation test for bleeding with heparin resulted in an additional 46% of positive scintigraphies with a reliable localisation in primary negative scintigraphies. CONCLUSION: Our results show that scintigraphy and scintigraphy with heparin provocation tests are reliable procedures. They enable a reliable localisation in about half of the obscure GI-bleeding cases. Scintigraphy is superior to angiography in locating a bleeding. | 17 | Chinese Herbal Medicine for Systemic Lupus Erythematosus:A Systematic Review and Meta-Analysis of Randomized,Placebo-Controlled Trials显示文摘Objective:To provide evidence on the efficacy and safety of Chinese herbal medicine(CHM)as interventions for systemic lupus erythematosus(SLE).Methods:Seven electronic databases,including the Cochrane Library,Chinese National Knowledge Infrastructure(CNKI),Chinese Scientific Journal Database(VIP),Chinese Biomedical Literature Service System(SinoMed),Wanfang,Embase,and PubMed,were comprehensively searched,from their inception to August 16,2020,for all randomized controlled trials(RCTs)that focused on CHM used alone or in combination with conventional medicine for SLE.Outcomes were SLE activity index(SLEDAI),traditional Chinese medicine symptom/syndrome score(TCMSS),dosage of glucocorticoids,main serological testing,and incidence of adverse events.Data were extracted and pooled using Review Manager 5.3 software.Results:A total of 13 RCTs enrolling 856 participants met our inclusion criteria.Meta-analyses showed that,compared to placebo,CHM had statistically significant effect on reducing SLEDAI score(MD=-1.74,95%Cl:-2.29 to-1.18),diminishing TCMSS(SMD=-0.89,95%Cl:-1.16 to-0.62),decreasing dosage of glucocorticoids(MD=-2.41 mg/d,95%Cl:-3.34 to-1.48),lowering erythrocyte sedimentation rate(MD=-4.78 mm/h,95%Cl:-8.86 to-0.71),and increasing serum complement C4 level(MD=0.03 mg/dL,95%Cl:0.00 to 0.06).No significant difference was found between CHM and placebo on adverse events.Conclusions:CHM provided significant beneficial effect on controlling disease activity and reducing dose of glucocorticoids used among SLE patients.Future advanced designed RCTs for CHM treating moderate to severe SLE with multicenter and longer follow-up are urgently needed. | WANG Ying HAN Mei Christopher EPedigo XIE Zhi-min WANG Wei-jie LIU Jian-ping | 2021 | Chinese Journal of Integrative Medicine2021,27,10: | 3 |
