| 1 | Plasma and aqueous humor levels of adiponutrin and pannexin 1 in patients with and without diabetic retinopathy显示文摘AIM: To evaluate plasma and aqueous levels of adiponutrin and pannexin 1 in patients with and without diabetic retinopathy.METHODS: The study included three age and gendermatched groups of 20 cataract patients with no diabetes or additional disease(Group C), 20 cataract patients with diabetes and no retinopathy(Group DM+C), and 20 cataract patients with diabetic retinopathy(Group DR+C).All the patients were examined with respect to body mass index(BMI), fasting plasma glucose, hemoglobin A1c(HbA1c), and lipid profile.Phacoemulsification and intraocular lens(Phaco+IOL) implantation were performed to all patients in all the groups, and aqueous samples were taken during the operation. The plasma and aqueous adiponutrin and pannexin 1 levels were analyzed using enzyme-linked immunosorbent assays.RESULTS: A statistically significant difference was determined between the groups with respect to BMI, fasting plasma glucose, and HbA1c levels(P<0.05 for all parameters tested). The plasma adiponutrin levels of Group DR+C were statistically significantly lower than those of Group C and Group DM+C(P<0.001, P=0.004).No statistically significant difference was determined in the aqueous adiponutrin levels in three groups. The plasma pannexin 1 levels of Groups DM+C and DR+C were statistically significantly lower than those of Group C(both P=0.001).The aqueous pannexin 1 levels of Group DR+C were statistically significantly higher than those of Group C and Group DM+C(P=0.001, P<0.001).CONCLUSION: Adiponutrin and pannexin 1, which play an important role in the pathophysiology of diabetes and obesity, and have a regulatory role in hyperglycemia and insulin resistance. The measurement of adiponutrin and pannexin 1 levels may support clinicians in determining the risk of DR development. | Fatih Cem Gül Sabiha Güngör Kobat FatihÇelik Süleyman Aydin Ramazan Fazil Akkoç | 2022 | International Journal of Ophthalmology(English edition)2022,15,3: | 3 |
| 2 | Mesenchymal stem cell therapy in patients with small bowel transplantation:Single center experience显示文摘AIM:To study the effects of mesenchymal stem cell(MSC) therapy on the prevention of acute rejection and graft vs host disease following small bowel transplantation.METHODS: In our transplantation center, 6 isolated intestinal transplants have been performed with MSC therapy since 2009. The primary reasons for transplants were short gut syndrome caused by surgical intestine resection for superior mesenteric artery thrombosis(n = 4), Crohn's disease(n = 1) and intestinal aganglionosis(n = 1). Two of the patients were children. At the time of reperfusion, the first dose of MSCs cultured from the patient's bone marrow was passedinto the transplanted intestinal artery at a dose of 1000000 cells/kg. The second and third doses of MSCs were given directly into the mesenteric artery through the arterial anastomosis using an angiography catheter on day 15 and 30 post-transplant.RESULTS: The median follow-up for these patients was 10.6 mo(min: 2 mo-max: 30 mo). Three of the patients developed severe acute rejection. One of these patients did not respond to bolus steroid therapy. Although the other two patients did respond to antirejection treatment, they developed severe fungal and bacterial infections. All of these patients died in the 2nd and 3rd months post-transplant due to sepsis. The remaining patients who did not have acute rejection had good quality of life with no complications observed during the follow-up period. In addition, their intestinal grafts were functioning properly in the 13th, 25th and 30th month post-transplant. The patients who survived did not encounter any problems related to MSC transplantation.CONCLUSION: Although this is a small case series and not a randomized study, it is our opinion that small bowel transplantation is an effective treatment for intestinal failure, and MSC therapy may help to prevent acute rejection and graft vs host disease following intestinal transplantation. | Sait Murat Do?an Sel?uk K?l?n? | 18 | Evaluation of proximal tubule functions in children with COVID‑19:a prospective analytical study显示文摘Background There are limited numbers of studies focusing on renal efects of coronavirus disease 2019(COVID-19)infection and proximal tubular dysfunction in children with COVID-19 infections. The purpose of this study was to evaluate the functions of the proximal tubule in hospitalized children with confrmed acute COVID-19.Methods The children who were hospitalized for confrmed COVID-19 were included in this prospective descriptive analysis. The presence of at least two of the following four abnormalities was used to diagnose proximal tubule injury:abnormal tubular reabsorption of phosphate, normoglycemic glycosuria, hyperuricosuria, and proteinuria. Results A total of 115 patients were included in the study.About a third of the individuals had elevated blood creatinine levels or proteinuria. In addition, abnormal renal tubular phosphate loss measured by renal tubular phosphate loss was found in 10(8.7%)patients,as was hyperuricosuria in 28.6%.As a result, total proximal tubular dysfunction was found in 24(20.9%)patients.Conclusions One in every fve children with acute COVID-19 infections had proximal tubular dysfunction, according to our data.Although, the rate of proximal tubular dysfunction was lower than in adults,it should be noted. The recovery of proximal tubular function in children with COVID-19 should be followed. | Fatma Devrim Elif Böncüoğlu Elif Kıymet ŞahikaŞahinkaya Ela Cem Mine Düzgöl Aybüke Akaslan Kara KamileÖtiken Arıkan AslıKantar Ebru Yılmaz Nida Dinçel Nuri Bayram İlker Devrim | 2022 | World Journal of Pediatrics2022,18,9: | 0 |
| 19 | Multi-aspect analysis of ureteral access sheath usage in retrograde intrarenal surgery: A RIRSearch group study显示文摘Objective:To evaluate the effect of ureteral access sheath(UAS)use and calibration change on stone-free rate and complications of retrograde intrarenal surgery(RIRS).Methods:Data from 568 patients undergoing RIRS for kidney or upper ureteral stones were retrospectively included.Firstly, patients were compared after 1:1 propensity score matching, according to UAS usage during RIRS(UAS used[+]87 and UAS non-used[−]87 patients). Then all UAS+patients(n=481)were subdivided according to UAS calibration:9.5-11.5 Fr,10-12 Fr,11-13 Fr,and 13-15 Fr.Primary outcomes of the study were the success and complications of RIRS.Results:Stone-free rate of UAS+patients(86.2%)was significantly higher than UAS−patients(70.1%)after propensity score matching(p=0.01).Stone-free rate increased with higher caliber UAS(9.5-11.5 Fr:66.7%;10-12 Fr:87.0%;11-13 Fr:90.6%;13-15 Fr:100%;p<0.001).Postoperative complications of UAS+patients(11.5%)were significantly lower than UAS−patients(27.6%)(p=0.01).Complications(8.7%)with 9.5-11.5 Fr UAS was lower than thicker UAS(17.2%)but was not statistically significant(p=0.09).UAS usage was an independent factor predicting stone-free status or peri-and post-operative complications(odds ratio[OR]3.654,95%confidence interval[CI]1.314-10.162;OR 4.443,95%CI 1.350-14.552;OR 4.107,95%CI 1.366-12.344,respectively).Conclusion:Use of UAS in RIRS may increase stone-free rates, which also increase with higher caliber UAS.UAS usage may reduce complications;however, complications seemingly increase with higher UAS calibration. | OktayÖzman HacıM.Akgül Cem Başataç ÖnderÇınar Eyüp B.Sancak Cenk M.Yazıcı BülentÖnal Haluk Akpınar on behalf of the RIRSearch Study Group | 2024 | Asian Journal of Urology2024,11,1: | 0 |
