| 2 | Geotechnical investigations and remediation design for failure of tunnel portal section: a case study in northern Turkey显示文摘Mass movements are very common problems in the eastern Black Sea region of Turkey due to its climate conditions, geological, and geomorphological characteristics. High slope angle, weathering, dense rainfalls, and anthropogenic impacts are generally reported as the most important triggering factors in the region. Following the portal slope excavations in the entrance section of Cankurtaran tunnel, located in the region, where the highly weathered andesitic tuff crops out, a circular toe failure occurred. The main target of the present study is to investigate the causes and occurrence mechanism of this failure and to determine the feasible remedial measures against it using finite element method(FEM) in four stages. These stages are slope stability analyses for pre-and postexcavation cases, and remediation design assessments for slope and tunnel. The results of the FEM-SSR analyses indicated that the insufficient initial support design and weathering of the andesitic tuffs are the main factors that caused the portal failure. After installing a rock retaining wall with jet grout columns and reinforced slope benching applications, the factor of safety increased from 0.83 to 2.80. In addition toslope stability evaluation, the Rock Mass Rating(RMR), Rock Mass Quality(Q) and New Austrian Tunneling Method(NATM) systems were also utilized as empirical methods to characterize the tunnel ground and to determine the tunnel support design. The performance of the suggested empirical support design, induced stress distributions and deformations were analyzed by means of numerical modelling. Finally, it was concluded that the recommended stabilization technique was essential for the dynamic long-term stability and prevents the effects of failure. | 6 | Coronary artery disease in congenital single coronary artery in adults:A Dutch case series显示文摘AIM:To assess the current diagnostic and therapeutic management and the clinical implications of congenital single coronary artery(SCA) in adults.METHODS:We identified 15 patients with a SCA detected from four Dutch angiography centers in the period between 2010 and 2013.Symptomatic patients who underwent routine diagnostic coronary angiography(CAG) for suspected coronary artery disease and who incidentally were found to have isolated SCA were analyzed.RESULTS:Fifteen(7 females) with a mean age of 58.5 ± 13.78 years(range 43-86) had a SCA.ConventionalCAG demonstrated congenital isolated SCA originating as a single ostium from the right sinus of Valsalva in 6 patients and originating from the left in 9 patients.Minimal to moderate coronary atherosclerotic changes were found in 4,and severe stenotic lesions in another 4 patients.Seven patients were free of coronary atherosclerosis.Runs of non-sustained ventricular tachycardia were documented in 2 patients,one of whom demonstrated transmural ischemic changes on presentation.Myocardial perfusion scintigraphic evidence of transmural myocardial ischemia was found in 1 patient due to kinking and squeezing of the SCA with an interarterial course between the aorta and pulmonary artery.Multi-slice computed tomography(MSCT) was helpful to delineate the course of the anomalous artery relative to the aorta and pulmonary artery.Percutaneous coronary intervention was successfully performed in 3 patients.Eight patients were managed medically.Arterial bypass graft was performed in 4 patients with the squeezed SCA.CONCLUSION:SCA may be associated with transient transmural myocardial ischemia and aborted sudden death in the absence of coronary atherosclerosis.The availability and sophistication of MSCT facilitates the delineation of the course of a SCA.We present a Dutch case series and review of the literature. | Salah AM Said Willem G de Voogt Suat Bulut Jacques Han Peter Polak Rogier LG Nijhuis Jeroen W op den Akker Andries Slootweg | 2014 | World Journal of Cardiology2014，6，4： | 3 |
| 7 | What is the relationship between frailty and orthostatic hypotension in older adults?显示文摘Background Frailty and orthostatic hypotension (OH),which is common in older adults,is associated with morbidity and mortality.The relationship between them remains unclear.The aim of the study is to determine whether there is a relationship between frailty and OH.Methods A total of 496 patients who were admitted to the geriatric clinic and underwent comprehensive geriatric assessment were retrospectively reviewed.In a cross-sectional and observational study,OH was measured by the Head-up Tilt Table test at 1,3,and 5 min (respectively,OH1,OH3,and OH5) and the frailty was measured by the Fried’s frailty scale.Results The mean age of all patients was 75.4 ± 7.38.The prevalence of females was 69.8%.When the frail people were compared with the pre-frail and the robust ones,the frailty was associated with OH1.There was no relationship between the groups in terms of OH1 when the pre-frail group was compared with the robust group.OH3 were higher in the frail group than in the pre-frail group (P < 0.05) and the OH5 were higher in the frail group than in the pre-frail and robust group (P < 0.05),but OH3 and OH5 were not associated with frailty status when they were adjusted for age (P > 0.05).Slowness and weakness were associated with OH1 (P < 0.05),whereas the other components of the Fried’s test were not.Conclusions Frailty may be a risk factor for OH1.The 1^st min measurements of OH should be routinely evaluated in frail older adults to prevent OH-related poor outcomes. | Suleyman Emre Kocyigit Pinar Soysal Esra Ates Bulut Ali Ekrem Aydin Ozge Dokuzlar Ahmet Turan Isik | 2019 | Journal of Geriatric Cardiology2019，16，3： | 3 |

| 9 | Effect of sodium tungstate on visual evoked potentials in diabetic rats显示文摘AIM: To evaluate the effect of sodium tungstate on visual evoked potentials(VEPs) in diabetic rats.· METHODS: Wistar rats were randomly divided into three groups as normal control, diabetic control and diabetic rats treated with sodium tungstate. Diabetes was induced by single intraperitoneal injection of streptozotocin(50 mg/kg). Sodium tungstate [40 mg/(kg·d)]was administered for 12 wk and then VEPs were recorded.Additionally, thiobarbituric acid reactive substance(TBARS) levels were measured in brain tissues.·RESULTS: The latencies of P1, N1, P2, N2 and P3 waves were significantly prolonged in diabetic rats compared with control group. Diabetes mellitus caused an increase in the lipid peroxidation process that was accompanied by changes in VEPs. However, prolonged latencies of VEPs for all components returned to control levels in sodium tungstate-treated group.
