| 1 | Full-face excavation of large tunnels in difficult conditions显示文摘Following a few preliminary remarks on the tunneling methods at the beginning of the 20th century,the successful applications of the full-face method also in difficult conditions are underlined.The attention is posed on the use of a systematic reinforcement of the face and of the ground,by means of fiber-glass elements.A selection of tunnels where this method was used successfully is reported with the purpose of illustrating the wide spectrum of ground conditions where it has been applied.Then,following a description of the main concepts behind the method,the attention moves from the so-called 'heavy method',where deformations are restrained,to the 'light method',where deformations are allowed with the intention to decrease the stresses acting on the primary and final linings.The progress in the application of the 'light method' is underlined,up to the development of a novel technique,which relies on the use of a yielding support composed of top head steel sets with sliding joints and special deformable elements inserted in the primary lining.The well-known case study of the Saint Martin La Porte access adit,along the Lyon-Turin Base Tunnel,is described.In this tunnel,a yield-control support system combined with full-face excavation has been adopted successfully in order to cope with the large deformations experienced during face advance through the Carboniferous formation.The monitoring results obtained during excavation are illustrated,together with the modeling studies performed when paying attention to the rock mass time-dependent behavior. | Giovanni Barla | 2016 | Journal of Rock Mechanics and Geotechnical Engineering2016,8,3: | 13 |

| 3 | Congenital H-type anovestibuler fistula显示文摘The congenital H-type fistula between the anorectum and genital tract besides a normal anus is a rare entity in the spectrum of anorectal anomalies. We described a girl with an anovestibuler H-type fistula and left vulvar abscess. A 40-day-old girl presented symptoms after her parents noted the presence of stool at the vestibulum. On the physical examination, anus was in normal location and size, and had normal sphincter tone. A vestibuler opening was seen in the midline just below of the hymen. A fistulous communication was found between the vestibuler opening and the anus, just above the dentate line. There was a vulvar abscess which had a left lateral vulvar drainage opening 15 mm left lateral to the perineum. After the management of local inflammation and abscess, the patient was operated for primary repair of the fistula. A protective colostomy wasn′t performed prior the operation. A profuse diarrhea started after 5 hours of postoperation. After the diarrhea, a recurrent fistula was occurred on the second postoperative day. A divided sigmoid colostomy was performed. 2 months later, and anterior sagital anorectoplasty was reconstructed and colostomy was closed 1 month later. Various surgical techniques with or without protective colostomy have been described for double termination repair. | 4 | Colorectal cancer of the young displays distinct features of aggressive tumor biology:A single-center cohort study显示文摘BACKGROUND In recent years,a decrease in incidence and mortality of colorectal cancer(CRC)has been observed in developed nations,presumably through public disease awareness and increased screening efforts.However,a rising incidence of CRC in young patients below the age of 50 years has been reported in several studies.AIM To study tumor biology in CRC patients below 50 years of age.METHODS All patients with CRC were prospectively enrolled in our single-center oncologic database from January 2013 to December 2018 and were grouped and analyzed according to age(≥50 and<50 years).Clinical as well as histopathological features were analyzed and compared.The study was approved by the local Ethics Committee.Fisher’s exact test or t-test was used to test for differences between the groups,as appropriate.All statistical analysis was performed with IBM SPSS software Version 25(SPSS Inc,Armonk,NY,United States)and with RStudio using R Version 3.4.1(RStudio,Boston,MA,United States).RESULTS Seventeen percent of the 411 patients were younger than 50 years.Young patients were more often diagnosed with locally advanced T4-tumors and lymph node metastases(36.6%and 62%vs 17.7%and 42%;P<0.01).In addition,a higher frequency of poorly differentiated(G3)tumors(40%vs 22.4%P<0.05)was observed.More than every second patient below 40 years of age(51.8%)had distant metastases at diagnosis with a significant higher rate ring of signet cell differentiation compared to patients≥50 years(14.8%,P<0.05).Mutational status(KRAS,NRAS,BRAF,MSI)as well as selected behavioral risk factors showed no significant differences.CONCLUSION Distinct histopathologic features of increased biologic aggressiveness are found in patients with CRC of young-onset.Those patients present more frequently with more advanced tumor stages compared to older patients.Features of aggressive tumor biology underscore the need for earlier uptake of routine screening measures. | Matteo Mueller Marcel Andre Schneider Barla Deplazes Daniela Cabalzar-Wondberg Andreas Rickenbacher Matthias Turina | 2021 | World Journal of Gastrointestinal Surgery2021,13,2: | 3 |
| 5 | Numerical modeling of deep-seated landslides interacting with man-made structures显示文摘This paper describes the interaction between deep-seated landslides and man-made structures such as dams, penstocks, viaducts, and tunnels. Selected case studies are reported first with the intent to gain insights into the complexities associated with the interaction of these structures with deep-seated landslides(generally referred to as deep-seated gravity slope deformations, DSGSDs). The main features, which characterize these landslides, are mentioned together with the interaction problems encountered in each case. Given the main objective of this paper, the numerical modeling methods adopted are outlined as means for increase in the understanding of the interaction problems being investigated. With the above in mind, the attention moves to an important and unique case history dealing with the interaction of a large-size twin-tunnel excavated with an earth pressure balance(EPB)tunnel boring machine(TBM) and a deep-seated landslide, which was reactivated due to the stress changes induced by tunnel excavation in landslide shear zone. The geological and geotechnical conditions are described together with the available monitoring data on the landslide movements, based on the advanced and conventional monitoring tools used. Numerical modeling is illustrated as an aid to back-analyze the monitored surface and subsurface deformations and to assist in finding the appropriate engineering solution for putting the tunnel into service and as a follow-up means for future understanding and control of the interaction problems. | 6 | Energy tunnels:concept and design aspects显示文摘Geotechnical structures are increasingly employed as energy geostructures in Europe and worldwide.Besides being constructed for their primary structural role,they are equipped to exchange heat with the ground and supply thermal energy for heating and cooling of buildings and de-icing of infrastructures.This technology can play a fundamental role in the current challenge of addressing the increasing need for clean and renewable sources of energy.This study investigates the possibility of thermal activation of tunnel linings.Particularly,attention will be paid on a new energy segment,which can be used together with tunnel boring machine tunneling to create so-called energy tunnels.Thermal and mechanical designs need to be developed by making effective use of computational methods to quantify the exploitable heat and assess the possible consequences on the surrounding ground and the structure itself.Guidance on how to proceed in this direction will be provided in this study,showing how thermo-hydro and thermo-mechanical numerical analyses can be used to achieve a proper and effective design of energy tunnels.Two examples of possible applications will also be presented. | Marco Barla Alice Di Donna | 2018 | Underground Space2018,3,4: | 3 |
