2H pylori:Treatment for the patient only or the whole family?显示文摘AIM:To compare the effects of treatment of H pylori-infected individuals with the effects of treatment of individuals as well as all H pylori-infected family members. METHODS:H pylori-positive patients with similar demographic specifications were prospectively randomized with respect to treatment, with a triple regimen of either patients and all H pylori-positive family members living together (groupⅠ) or patients only (group Ⅱ). Nine months after treatment, all patients were assessed for H pylori positivity. RESULTS:There were 70 H pylori-positive patients in each group;patients in groupsⅠand Ⅱ lived with 175 and 190 H pylori-positive relatives, respectively. Age, sex and H pylori positivity rate were similar in both groups of relatives. Nine months after 14 d standard triple therapy, H pylori positivity was 7.1% in groupⅠpatients and 38.6% in group Ⅱ patients [P < 0.01, OR = 8.61 95% confidence interval (CI):2.91-22.84]. CONCLUSION:The present results indicate bad environmental hygienic conditions and close intra-familial relationships are important in H pylori contamination. 3Soil biodiversity and crop diversification are vital components of healthy soils and agricultural sustainability显示文摘The fast-growing world population exerts great pressure on the land to produce enough nutritious food. It is projectedthat global population will be 50% greater than at present by 2050 and the demand for global grain will havedoubled[1]. The pressure further intensifies with the stresses resulting from climate change, associated extremeweather[2], and expansion of urbanization. Humanity has already transgressed three of the nine interlinked planetaryboundaries, and agriculture is the major driving force behind this development^1. The agricultural system must betransformed to simultaneously provide global food security and environmental integrity[4]. To address thesechallenges, sustainability in agriculture must be enhanced13,51. This is particularly true for rapidly developingcountries such as China. While intensive, industrial agriculture achieved enormous successes, such as feeding 20%of the global population by producing 25% of the world’s grain with less than 10% of world arable land, theseachievements came at the expense of low resource use efficiency and environmental problems such as airpollution161, water pollution171, and soil acidification[8]. 5Correlation of endoscopic disease severity with pediatric ulcerative colitis activity index score in children and young adults with ulcerative colitis显示文摘AIM To investigate of pediatric ulcerative colitis activity index(PUCAI) in ulcerative colitis correlate with mucosal inflammation and endoscopic assessment of disease activity(Mayo endoscopic score).METHODS We reviewed charts from ulcerative colitis patients who had undergone both colonoscopy over 3 years. Clinical assessment of disease severity within 35 d(either before or after) the colonoscopy were included. Patients were excluded if they had significant therapeutic interventions(such as the start of corticosteroids or immunosuppressive agents) between the colonoscopy and the clinical assessment. Mayoendoscopic score of the rectum and sigmoid were done by two gastroenterologists. Inter-observer variability in Mayo score was assessed.RESULTS We identified 99 patients(53% female, 74% pancolitis) that met inclusion criteria. The indications for colonoscopy included ongoing disease activity(62%), consideration of medication change(10%), assessment of medication efficacy(14%), and cancer screening(14%). Based on PUCAI scores, 33% of patients were in remission, 39% had mild disease, 23% had moderate disease, and 4% had severe disease. There was 'moderate-substantial' agreement between the two reviewers in assessing rectal Mayo scores(kappa = 0.54, 95%CI: 0.41-0.68). CONCLUSION Endoscopic disease severity(Mayo score) assessed by reviewing photographs of pediatric colonoscopy has moderate inter-rater reliability, and agreement was less robust in assessing patients with mild disease activity. Endoscopic disease severity generally correlates with clinical disease severity as measured by PUCAI score. However, children with inflamed colons can have significant variation in their reported clinical symptoms. 10Procedural complexity independent of P2Y12 reaction unit(PRU)values is associated with acute in situ thrombosis in Pipeline flow diversion of cerebral aneurysms显示文摘background Acute in situ thrombosis is an ischaemic phenomenon during Pipeline embolisation device(PED)procedures with potentially high morbidity and mortality.There is controversy regarding the role of platelet function testing with P2Y12 assay as a predictor of intraprocedural thromboembolic events.There is limited knowledge on whether procedural complexity influences these events.Methods Data were collected retrospectively on 742 consecutive PED cases at a single institution.Patients with intraprocedural acute thrombosis were compared with patients without these events.results A cohort of 37 PED cases with acute in situ thrombosis(mean age 53.8 years,mean aneurysm size 8.4 mm)was matched with a cohort of 705 PED cases without intraprocedural thromboembolic events(mean age 56.4 years,mean aneurysm size 6.9 mm).All patients with in situ thrombosis received intra-arterial and/or intravenous abciximab.The two groups were evenly matched in patient demographics,previous treatment/subarachnoid hemorrhage(SAH)and aneurysm location.There was no statistical difference in postprocedural P2Y12 reaction unit(PRU)values between the two groups,with a mean of 156 in the in situ thrombosis group vs 148 in the control group(p=0.5894).Presence of cervical carotid tortuosity,high cavernous internal carotid artery grade,need for multiple PED and vasospasm were not significantly different between the two groups.The in situ thrombosis group had statistically significant longer fluoroscopy time(60.4 vs 38.4 min,p<0.0001),higher radiation exposure(3476 vs 2160 mGy,p<0.0001),higher rates of adjunctive coiling(24.3% vs 8.37%,p=0.0010)and higher utilisation of balloon angioplasty(37.8% vs 12.2%,p<0.0001).Clinically,the in situ thrombosis cohort had higher incidence of major and minor stroke,intracerebral haemorrhage and length of stay.Conclusions Predictors of procedural complexity(higher radiation exposure,longer fluoroscopy time,adjunctive coiling and need for balloon angioplasty)are associated with acute thrombotic events during PED placement,independent of PRU values.Bowen Jiang Matthew T Bender Erick M Westbroek Jessica K Campos Li-Mei Lin Risheng Xu Rafael J Tamargo Judy Huang Geoffrey P Colby Alexander L Coon 2018Stroke & Vascular Neurology2018,3,3:2
11高羊茅干草部分替代玉米和紫花苜蓿青贮饲料对奶牛消化、泌乳性能的影响显示文摘试验旨在研究高羊茅干草替代玉米或紫花苜蓿青贮对奶牛中性洗涤纤维(NDF)消化率、泌乳性能的影响。选取初产哺乳荷斯坦奶牛(产奶75±35 d)24头,经产荷斯坦奶牛(产奶68±19 d)40头,随机分成4组,共设置32个进料闸门,记录采食量。每组8个进料阀门,以1头牛为1个试验单位,每头牛试验过程中可接触8个进料闸门。试验用高羊茅干草(TF)取代玉米青贮饲料(CS)或紫花苜蓿青贮饲料(AS),试验设计为:33%AS和67%CS(空白组)、60%TF和40%AS(试验1组)、60%TF和40%CS(试验2组)、33%TF和67%CS(试验3组)。试验进行7周,前2周进行预饲。试验结果表明,各组间牛奶产量无显著差异,平均值为40.4 kg·d^(-1)。各组间牛奶中脂肪含量和浓度、蛋白质含量和浓度均无显著差异,平均值为1.58 kg·d^(-1)和3.94%、1.28 kg·d^(-1)和3.15%。与试验1组(22.1 kg·d^(-1))、试验3组(22.7 kg·d^(-1))相比,空白组的干物质摄入量最高,为24.5kg·d^(-1)。各组间总的干物质消化率无显著差异,平均值为66.2%。与试验2组(44.4%)、试验3组(45.3%)相比,空白组的NDF消化率较低,为37.8%,与试验1组(42.4%)相近。各组间奶牛体内NDF消化率(42.3%)与预估的NDF消化率(42.6%)相近。试验结果表明,饲喂高羊茅干草可提高饲料中NDF消化率,部分替代玉米和紫花苜蓿时对产奶量无显著影响,在市场价格较低时,可作为经济实用的奶牛饲料。Bender R W Lopes F Cook D E Combs D K 2016饲料博览2016,0,8:2
