2SHRIMP single zircon U-Pb dating of the Kongling high-grade metamorphic terrain: Evidence for >3.2 Ga old continental crust in the Yangtze craton显示文摘Single zircons from two trondhjemitic gneisses and two clastic metasedimentary rocks without Eu anomaly of the Kongling high-grade metamorphic terrain are dated by the in situ SHRIMP U-Pb method. The results show that the trondhjemitic magma emplaced at 2947-2903 Ma. Concordant age of as old as 3.3 Ga is present in the detrital zircons from the clastic metasedimentary rocks. 10Novel concepts in radiation-induced cardiovascular disease显示文摘Radiation-induced cardiovascular disease(RICVD) is the most common nonmalignant cause of morbidity and mortality among cancer survivors who have undergone mediastinal radiation therapy(RT).Cardiovascular complications include effusive or constrictive pericarditis,cardiomyopathy,valvular heart disease,and coronary/vascular disease.These are pathophysiologically distinct disease entities whose prevalence varies depending on the timing and extent of radiation exposure to the heart and great vessels.Although refinements in RT dosimetry and shielding will inevitably limit future cases of RICVD,the increasing number of long-term cancer survivors,including those treated with older higher-dose RT regimens,will ensure a steady flow of afflicted patients for the foreseeable future.Thus,there is a pressing need for enhanced understanding of the disease mechanisms,and improved detection methods and treatment strategies.Newly characterized mechanisms responsible for the establishment of chronic fibrosis,such as oxidative stress,inflammation and epigenetic modifications,are discussed and linked to potential treatments currently under study.Novel imaging modalities may serve as powerful screening tools in RICVD,and recent research and expert opinion advocating their use is introduced.Data arguing for the aggressive use of percutaneous interventions,such as transcutaneous valve replacement and drug-eluting stents,are examined and considered in the context of prior therapeutic approaches.RICVD and its treatment options are the subject of a rich and dynamic body of research,and patients who are at risk or suffering from this disease will benefit from the care of physicians with specialty expertise in the emerging field of cardiooncology.Jason R Cuomo Gyanendra K Sharma Preston D Conger Neal L Weintraub 2016World Journal of Cardiology2016,8,9:17
11Review of the current targeted therapies for non-small-cell lung cancer显示文摘The last decade has witnessed the development of oncogene-directed targeted therapies that have significantly changed the treatment of non-small-cell lung cancer(NSCLC). In this paper we review the data demonstrating efficacy of gefitinib, erlotinib, and afatinib, which target the epidermal growth factor receptor(EGFR), and crizotinib which targets anaplastic lymphoma kinase(ALK). We discuss the challenge of acquired resistance to these small-molecular tyrosine kinase inhibitors and review promising agents which may overcome resistance, including the EGFR T790 Mtargeted agents CO-1686 and AZD9291, and the ALKtargeted agents ceritinib(LDK378), AP26113, alectinib(CH/RO5424802), and others. Emerging therapies directed against other driver oncogenes in NSCLC including ROS1, HER2, and BRAF are covered as well. 12Effect of longer battery life on small bowel capsule endoscopy显示文摘AIM:To determine if longer battery life improves capsule endoscopy(CE) completion rates.METHODS:A retrospective study was performed at a tertiary,university-affiliated hospital in Vancouver,Canada.Patients who underwent CE with either PillCamTM SB2 or SB2 U between 01/2010 and 12/2013 were considered for inclusion.SB2 and SB2 U share identical physical dimensions but differ in their battery lives(8 h vs 12 h).Exclusion criteria included history of gastric or small bowel surgery,endoscopic placement of CE,interrupted view of major landmarks due to technical difficulty or significant amount of debris,and repeat CE using same system.Basic demographics,comorbidities,medications,baseline bowel habits,and previous surgeries were reviewed.Timing of major landmarks in CE were recorded,and used to calculate gastric transit time,small bowel transit time,and total recording time.A complete CE study was defined as visualization of cecum.Transit times and completion rates were compared.RESULTS:Four hundred and eight patients,including 208(51.0%) males,were included for analysis.The mean age was 55.5 ± 19.3 years.The most common indication for CE was gastrointestinal bleeding(n =254,62.3%),followed by inflammatory bowel disease(n =86,21.1%).There was no difference in gastric transit times(group difference 0.90,95%CI:0.72-1.13,P =0.352) and small bowel transit times(group difference 1.07,95%CI:0.95-1.19,P =0.261) between SB2 U and SB2,but total recording time was about 14% longer in the SB2 U group(95%CI:10%-18%,P < 0.001) and there was a corresponding trend toward higher completion rate(88.2% vs 93.2%,OR =1.78,95%CI 0.88-3.63,P =0.111).There was no statistically significant difference in the rates of positive findings(OR =0.98,95%CI:0.64-1.51,P =0.918).CONCLUSION:Extending the operating time of CE may be a simple method to improve completion rate although it does not affect the rate of positive findings.George Ou Neal Shahidi Cherry Galorport Oliver Takach Terry Lee Robert Enns 2015World Journal of Gastroenterology2015,21,9:13
13Magnetic anchor guidance for endoscopic submucosal dissection and other endoscopic procedures显示文摘Endoscopic submucosal dissection(ESD) is a wellestablished, minimally invasive treatment for superficial neoplasms of the gastrointestinal tract. The universal adoption of ESD has been limited by its slow learning curve, long procedure times, and high risk of complications. One technical challenge is the lack of a second hand that can provide traction, as in conventional surgery. Reliable tissue retraction that exposes the submucosal plane of dissection would allow for safer and more efficient dissection. Magnetic anchor guided endoscopic submucosal dissection(MAGESD) has potential benefits compared to other current traction methods. MAG-ESD offers dynamic tissue retraction independent of the endoscope mimicking a surgeon's 'second hand'. Two types of magnets can be used: electromagnets and permanent magnets. In this article we review the MAG-ESD technology, published work and studies of magnets in ESD. We also review the use of magnetic anchor guidance systems in natural orifice transluminal endoscopic surgery and the idea of magnetic non-contact retraction using surface ferromagentization. 14Knockout mouse models of insulin signaling: Relevance past and future显示文摘Insulin resistance is a hallmark of type 2 diabetes. In an effort to understand and treat this condition, re searchers have used genetic manipulation of mice to uncover insulin signaling pathways and determine the effects of their perturbation. After decades of research much has been learned, but the pathophysiology o insulin resistance in human diabetes remains contro versial, and treating insulin resistance remains a chal lenge. This review will discuss limitations of mouse models lacking select insulin signaling molecule genes In the most influential mouse models, glucose metabo lism differs from that of humans at the cellular, organ and whole-organism levels, and these differences limi the relevance and benefit of the mouse models both in terms of mechanistic investigations and therapeutic development. These differences are due partly to im mutable differences in mouse and human biology, and partly to the failure of genetic modifications to produce an accurate model of human diabetes. Several fac tors often limit the mechanistic insights gained from experimental mice to the particular species and strain including: developmental effects, unexpected meta bolic adjustments, genetic background effects, and technical issues. 15One hundred and seventy-eight consecutive pancreatoduodenectomies without mortality:role of the multidisciplinary approach显示文摘BACKGROUND:Pancreatoduodenectomy offers the only chance of cure for patients with periampullary cancers.This,however,is a major undertaking in most patients and is associated with a significant morbidity and mortality.A multidisciplinary approach to the workup and follow-up of patients undergoing pancreatoduodenectomy was initiated at our institution to improve the diagnosis,resection rate,mortality and morbidity.We undertook the study to assess the effect of this approach on diagnosis,resection rates and short-term outcomes such as morbidity and mortality.METHODS:A prospective database of patients presenting with periampullary cancers to a single surgeon between April 2004 and April 2010 was reviewed.All cases were discussed at a multidisciplinary meeting comprising surgeons,gastroenterologists,radiologists,oncologists,radiation oncologists,pathologists and nursing staff.A standardized investigation and management algorithm was followed.Complications were graded according to the Clavien-Dindo classification.RESULTS:A total of 295 patients with a periampullary lesion were discussed and 178 underwent pancreatoduodenectomy (resection rate 60%).Sixty-one patients (34%) required either a vascular or an additional organ resection.Eighty-nine patients experienced complications,of which the commonest was blood transfusion (12%).Thirty-four patients (19%) had major complications,i.e.grade 3 or above.There was no in-hospital,30-day or 60-day mortality.CONCLUSIONS:Pancreatoduodenectomy can safely be performed in high-volume centers with very low mortality.The surgeon's role should be careful patient selection,intensive preoperative investigations,use of a team approach,and an unbiased discussion at a multidisciplinary meeting to optimize the outcome in these patients.Jaswinder S Samra Raul Alvarado Bachmann Julian Choi Anthony Gill Michael Neale Vikram Puttaswamy Cameron Bell Ian Norton Sarah Cho Steven Blome Ritchie Maher Sivakumar Gananadha Thomas J Hugh 2011Hepatobiliary & Pancreatic Diseases International2011,10,4:8

19100-Gb/s undersea transmission with high spectral efficiency using pre-filtered QPSK modulation format显示文摘We provide a review of our recent 100-Gb/s, high spectral efficiency (SE) experiment targeting transoceanic and regional undersea transmission distances. We demonstrated that simple pre-filtering at the transmitter together with a maximum a posteriori probability (MAP) detection algorithm can significantly improve SE. We transmitted 96×100-Gb/s pre-filtered polarization division multiplexed return-to-zero quadrature phase shift keyed (PDM-RZ-QPSK) channels with 300% SE over 10608 km using 52-km spans of 150-μm 2 fiber and simple single-stage erbium-doped fiber amplifiers (EDFAs).
