1亚太地区胃食管反流病的处理共识:更新版显示文摘背景与目的:自从2004年亚太地区胃食管反流病(GERD)共识发表以来,更多关于GERD流行病学和处理的文献资料相继出现。有必要对这些资料进行循证综述,对共识作出更新。方法:由多学科专家组应用德尔菲(Delphi)法制定共识条文,提呈相关资料,并对证据质量、推荐力度和共识水平进行分级。结果:亚洲GERD发生率日益增加。其危险因素包括老年、男性、种族、家族史、社会经济地位高、体重指数增加和吸烟。对于有典型症状而无报警症状的患者,对质子泵抑制剂(PPI)试验有症状应答具有诊断意义。如PPI试验失败,停止治疗后pH监测结果阴性可排除GERD。窄带成像、胶囊内镜检查和无线pH监测的作用尚未明确。亚洲诊断策略的制定须考虑到并存的胃癌和消化性溃疡。减轻体质量和抬高床头可改善反流症状。PPIs是最有效的内科治疗手段。对于非糜烂性反流病(NERD)患者,按需治疗较为适宜。有慢性咳嗽、喉炎和典型GERD症状的患者在排除非GERD病因后,应予PPI每天两次治疗。如有经验丰富的外科医师,GERD患者可行胃底折叠术。除临床试验外,GERD不应采用内镜治疗。结论:新的诊断方法和内镜治疗的作用有待进一步研究阐明。亚洲GERD诊断策略的制定须考虑到并存的胃癌和消化性溃疡。PPIs仍为治疗的基石。Kwong Ming Fock Nicholas J Talley Ronnie Fass Khean Lee Goh Peter Katelaris Richard Hunt Michio Hongo Tiing Leong Ang Gerald Holtmann Sanjay Nandurkar San Ren Lin Benjamin CY Wong Francis KL Chan Abdul Aziz Rani Young-Tae Bak Jose Sollano Khek Yu Ho Sathoporn Manatsathit 钱本余 2008胃肠病学2008,13,7:21
2Association between Ambient Air Pollution and Outpatient Visits for Acute Bronchitis in a Chinese City显示文摘Objective To investigate the short-term association between outdoor air pollution and outpatient visits for acute bronchitis,which is a rare subject of research in the mainland of China.Methods A time-series analysis was conducted to examine the association of outdoor air pollutants with hospital outpatient visits in Shanghai by using two-year daily data(2010-2011).Results Outdoor air pollution was found to be associated with an increased risk of outpatient visits for acute bronchitis in Shanghai.The effect estimates of air pollutants varied with the lag structures of the concentrations of the pollutants.For lag06,a 10 μg/m3 increase in the concentrations of PM10,SO2,and NO2 corresponded to 0.94%(95% CI:0.83%,1.05%),11.12%(95% CI:10.76%,11.48%),and 4.84%(95% CI:4.49%,5.18%) increases in hospital visits for acute bronchitis,respectively.These associations appeared to be stronger in females(P<0.05).Between-age differences were significant for SO2(P<0.05),and between-season differences were also significant for SO2(P<0.05).Conclusion Our analyses have provided the first evidence that the current air pollution level in China has an effect on acute bronchitis and that the rationale for further limiting air pollution levels in Shanghai should be strengthened.GUO Li Juan ZHAO Ang CHEN Ren Jie KAN Hai Dong KUANG Xing Ya 2014Biomedical and Environmental Sciences2014,27,11:13
3Output Waveform Analysis of an Electro-hydraulic Vibrator Controlled by the Multiple Valves显示文摘The existing research of the electro-hydraulic vibrator mainly focuses on system stability,working frequency width and output waveform distortion.However,this high frequency performance of the electro-hydraulic vibrator is difficult to be improved greatly due to fast insufficiently frequency response of the servo valve itself and limited compensation capability of the control structure in the vibrator system.In this paper,to realize high frequency vibration,an improved two-dimensional valve(here within defined as a2D valve)as a main control component is adopted to the parallel connection with a servo valve to control the electro-hydraulic vibrator.Because the output waveforms of this electro-hydraulic vibrator are incapable to be verified through timely feedback as in the conventional electro-hydraulic servo system,the analysis to the output waveform becomes crucial to the design and control of the electro-hydraulic vibrator.The mathematical models of hydraulic actuation mechanism and the orifice area of the parallel valves connection are established first.And then the vibration process is divided into two sections in terms of the direction of the flow,the analytical expression of the excited waveform is solved.Based on the analytical results,the vibration boundary positions and the relationships exist between working states and the control parameters are derived.Finally an experimental system was built to validate the theoretical analysis.It is verified that this electro-hydraulic vibration system could achieve high working frequency,up to 2 000 Hz.The excited waveform is similar to the sinusoidal waveform.And the ascent and decent slopes of the waveforms are somewhat asymmetrical.This asymmetry is not only caused by the change of the direction of the elastic force but also dependent on the bias position of the vibration.Consequently the distortion of effective working waveform is less than 10%.This electro-hydraulic vibrator controlled by the multiple valves could not only greatly enhance the working frequency but also precisely control the vibration characteristic variables such as waveform shape.REN Yan RUAN Jian JIA Wen'ang 2014Chinese Journal of Mechanical Engineering2014,27,1:10

5Inhibiting autophagy enhances sulforaphane-induced apoptosis via targeting NRF2 in esophageal squamous cell carcinoma显示文摘Sulforaphane(SFN), a natural anti-tumor compound from cruciferous vegetables, has been reported to induce protective autophagy to cancer cells, which might impair the anti-tumor efficiency of SFN. However, the accurate function and mechanism of SFN inducing autophagy in cancers are still obscure, especially in esophageal squamous cell carcinoma(ESCC), one of malignancies with high incidence in North China. Here, we mainly explored the potential function of autophagy upon SFN treatment in ESCC and molecular mechanism. We demonstrated that SFN could inhibit cell proliferation and induce apoptosis by activating caspase pathway. Moreover, we found activation of NRF2 pathway by SFN was responsible for the induction of autophagy and also a disadvantage element to the anti-tumor effects of SFN on ESCC, indicating that SFN might induce protective autophagy in ESCC. We, therefore,investigated effects of autophagy inhibition on sensitivity of ESCC cells to SFN and found that chloroquine(CQ) could neutralize the activation of SFN on NRF2 and enhance the activation of SFN on caspase pathway, thus improved the anti-tumor efficiency of SFN on ESCC in vitro and in vivo. 7Separate Control of High Frequency Electro-hydraulic Vibration Exciter显示文摘The working frequency of the conventional electro-hydraulic vibration exciters,which consist of a servo valve and a hydraulic cylinder,is generally restricted within a narrow range due to limited frequency response capability of the servo valve itself.To counteract such restriction,a novel scheme for an electro-hydraulic vibrator,controlled by a two-dimensional valve(2D valve) and a bias valve in parallel,is therefore proposed.The frequency,amplitude and offset are independently controlled by rotary speed,axial sliding of the spool of the 2D valve and axial sliding of the spool of the bias valve.The principle of separate control was presented and the regulation approach of frequency,amplitude and offset was discussed.A mathematical model of the hydraulic power mechanism for the proposed vibration exciter was established to investigate the relationship between the amplitude and the axial sliding of the 2D valve' spool,as well as that between the offset and the axial sliding of the bias valve's spool at various frequencies.An experimental system was built to validate the theoretical analysis.It is verified that the 2D exciter is capable of working smoothly in a frequency range of 5- 200 Hz.And its frequency,amplitude and offset can be controlled respectively by either closed loop or open loop method.There is a linear relationship between the output amplitude and the spool axial opening of the 2D valve until a point when the flow rate becomes saturate and the amplitude remains constant.The offset displacement of the cylinder's piston is linearly proportional to the axial displacement of the spool of the bias valve,when the valve opening is less than 25%.Thereafter,the slop of the offset curve decreases and tends to saturate.The proposed electro-hydraulic vibration controlled by the 2D valve not only facilitates the realization of high-frequency electro-hydraulic vibration,the high-accuracy of vibration can also be achieved by means of independent controls to the frequency,amplitude and offset.JIA Wen'ang RUAN Jian REN Yan 2011Chinese Journal of Mechanical Engineering2011,24,2:7
8Optimal immunosuppressor induces stable gut microbiota after liver transplantation显示文摘AIM To study the influence of different doses of tacrolimus(FK506)on gut microbiota after liver transplantation(LT)in rats.METHODS Specific pathogen-free Brown Norway(BN)rats and Lewis rats were separated into five groups:(1)Tolerance group(BN-BN LT,n=8);(2)rejection group(Lewis-BN LT,n=8);(3)high dosage FK506(FK506-H)group(Lewis-BN LT,n=8);(4)middle dosage FK506(FK506-M)group(Lewis-BN LT,n=8);and(5)low dosage FK506(FK506-L)group(LewisBN LT,n=8).FK506 was administered to recipients at a dose of 1.0 mg/kg,0.5 mg/kg,and 0.1 mg/kg body weight for 29 d after LT to the FK506-H,FK506-M,and FK506-L groups,respectively.On the 30^(th) day after LT,all rats were sampled and euthanized.Blood samples were harvested for liver function and plasma endotoxin testing.Hepatic graft and ileocecal tissues were collected for histopathology observation.Ileocecal contents were used for DNA extraction,Real-time quantitative polymerase chain reaction(RT-PCR)and digital processing of denaturing gradient gel electrophoresis(DGGE)profiles and analysis.RESULTS Compared to the FK506-H and FK506-L groups,FK506-M was optimal for maintaining immunosuppression and inducing normal graft function;the FK506-M maintained gut barrier integrity and low plasma endotoxin levels;furthermore,DGGE results showed that FK506-M induced stable gut microbiota.Diversity analysis indicated that FK506-M increased species richness and rare species abundance,and cluster analysis confirmed the stable gut microbiota induced by FK506-M.Phylogenetic tree analysis identified crucial bacteria associated with FK506-M;seven of the nine bacteria that were decreased corresponded to Bacteroidetes,while increased bacteria were of the Bifidobacterium species.FK506-M increased Faecalibacterium prausnitzii and Bifidobacterium spp.and decreased Bacteroides-Prevotella and Enterobacteriaceae,as assessed by RT-PCR,which confirmed the crucial bacterial alterations identified through DGGE.CONCLUSION Compared to the low or high dosage of FK506,an optimal dosage of FK506 induced immunosuppression,normal graft function and stable gut microbiota following LT in rats.The stable gut microbiota presented increased probiotics and decreased potential pathogenic endotoxin-producing bacteria.These findings provide a novel strategy based on gut microbiota for immunosuppressive dosage assessment for recipients following LT.Jian-Wen Jiang Zhi-Gang Ren Hai-Feng Lu Hua Zhang Ang Li Guang-Ying Cui Jun-Jun Jia Hai-Yang Xie Xin-Hua Chen Yong He Li Jiang Lan-Juan Li 2018World Journal of Gastroenterology2018,24,34:5
9Mandible exosomal ssc-mir-133b regulates tooth development in miniature swine via endogenous apoptosis显示文摘Signal transduction between different organs is crucial in the normal development of the human body. As an important medium for signal communication, exosomes can transfer important information, such as microRNAs(miRNAs), from donors to receptors.MiRNAs are known to fine-tune a variety of biological processes, including maxillofacial development; however, the underlying mechanism remains largely unknown. In the present study, transient apoptosis was found to be due to the expression of a miniature swine maxillofacial-specific miRNA, ssc-mir-133b. Upregulation of ssc-mir-133b resulted in robust apoptosis in primary dental mesenchymal cells in the maxillofacial region. Cell leukemia myeloid 1(Mcl-1) was verified as the functional target, which triggered further downstream activation of endogenous mitochondria-related apoptotic processes during tooth development.More importantly, mandible exosomes were responsible for the initial apoptosis signal. An animal study demonstrated that ectopic expression of ssc-mir-133 b resulted in failed tooth formation after 12 weeks of subcutaneous transplantation in nude mice. 10Radiofrequency ablation vs surgical resection in elderly patients with hepatocellular carcinoma in Milan criteria显示文摘BACKGROUND Surgical resection and radiofrequency ablation(RFA)represent two possible strategy in treatment of hepatocellular carcinoma(HCC)in Milan criteria.AIM To evaluate short-and long-term outcome in elderly patients(>70 years)with HCC in Milan criteria,which underwent liver resection(LR)or RFA.METHODS The study included 594 patients with HCC in Milan criteria(429 in LR group and 165 in RFA group)managed in 10 European centers.Statistical analysis was performed using the Kaplan-Meier method before and after propensity score matching(PSM)and Cox regression.RESULTS After PSM,we compared 136 patients in the LR group with 136 patients in the RFA group.Overall survival at 1,3,and 5 years was 91%,80%,and 76%in the LR group and 97%,67%,and 41%in the RFA group respectively(P=0.001).Diseasefree survival at 1,3,and 5 years was 84%,60%and 44%for the LR group,and 63%,36%,and 25%for the RFA group(P=0.001).Postoperative Clavien-Dindo IIIIV complications were lower in the RFA group(1%vs 11%,P=0.001)in association with a shorter length of stay(2 d vs 7 d,P=0.001).In multivariate analysis,Model for End-stage Liver Disease(MELD)score(>10)[odds ratio(OR)=1.89],increased value of international normalized ratio(>1.3)(OR=1.60),treatment with radiofrequency(OR=1.46),and multiple nodules(OR=1.19)were independent predictors of a poor overall survival while a high MELD score(>10)(OR=1.51)and radiofrequency(OR=1.37)were independent factors associated with a higher recurrence rate.CONCLUSION Despite a longer length of stay and a higher rate of severe postoperative complications,surgery provided better results in long-term oncological outcomes as compared to ablation in elderly patients(>70 years)with HCC in Milan criteria.Maria Conticchio Riccardo Inchingolo Antonella Delvecchio Letizia Laera Francesca Ratti Maximiliano Gelli Ferdinando Anelli Alexis Laurent Giulio Vitali Paolo Magistri Giacomo Assirati Emanuele Felli Taiga Wakabayashi Patrick Pessaux Tullio Piardi Fabrizio di Benedetto Nicola de'Angelis Javier Briceño AntonioRampoldi RenèAdam Daniel Cherqui Luca Antonio Aldrighetti Riccardo Memeo 2021World Journal of Gastroenterology2021,27,18:4
11Human microbiome is a diagnostic biomarker in hepatocellular carcinoma显示文摘Background:Hepatocellular carcinoma(HCC)is the third leading cause of cancer mortality worldwide.Increasing evidence indicates a close relationship between HCC and the human microbiota.Herein,we reviewed the important potential of the human microbiota as a diagnostic biomarker of HCC.Data sources:Several innovative studies have investigated the characteristics of the gut and oral micro-biomes in patients with HCC and proposed that the human microbiome has the potential to be a diag-nostic biomarker of HCC.Literature from February 1999 to February 2019 was searched in the PubMed database using the keywords'microbiota'or'microbiome'or'microbe'and'liver cancer'or'hepato-cellular carcinoma',and the results of clinical and experimental studies were analyzed.Results:Specific changes occur in the human microbiome of patients with HCC.Moreover,the gut mi-crobiome and oral microbiome can be used as non-invasive diagnostic biomarkers for HCC.Furthermore,they also have certain diagnostic potential for precancerous diseases of HCC.The diagnostic potential of the blood microbiota and ascites microbiota in HCC will be gradually discovered in the future.Conclusions:The human microbiome is valuable to the diagnosis of HCC and provides a novel strategy for targeted therapy of HCC.The human microbiome may be widely used in the diagnosis,treatment and prognosis for multiple system diseases or cancers in the future.Ben-Chen Rao Jia-Min Lou Wei-Jie Wang Ang Li Guang-Ying Cui Zu-Jiang Yu Zhi-Gang Ren 2020Hepatobiliary & Pancreatic Diseases International2020,19,2:4
12Thermoresponsive Lignin-Reinforced Poly(Ionic Liquid)Hydrogel Wireless Strain Sensor显示文摘To meet critical requirements on flexible electronic devices,multifunctionalized flexible sensors with excellent electromechanical performance and temperature perception are required.Herein,lignin-reinforced thermoresponsive poly(ionic liquid)hydrogel is prepared through an ultrasound-assisted synthesized method.Benefitting from the electrostatic interaction between lignin and ionic liquid,the hydrogel displays high stretchability(over 1425%),excellent toughness(over 132 kPa),and impressive stress loading-unloading cyclic stability.The hydrogel strain sensor presents excellent electromechanical performance with a high gauge factor(1.37)and rapid response rate(198 ms),which lays the foundation for human body movement detection and smart input.Moreover,owing to the thermal-sensitive feature of poly(ionic liquid),the as-prepared hydrogel displays remarkable thermal response sensitivity(0.217℃^(-1))in body temperature range and low limit of detection,which can be applied as a body shell temperature indicator.Particularly,the hydrogel can detect dual stimuli of strain and temperature and identify each signal individually,showing the specific application in human-machine interaction and artificial intelligence.By integrating the hydrogel strain sensor into a wireless sensation system,remote motion capture and gesture identification is realized in real-time.Xinyu Qu Ye Zhao Zi’ang Chen Siying Wang Yanfang Ren Qian Wang Jinjun Shao Wenjun Wang Xiaochen Dong 2021Research2021,,1:3
13Gut microbiota alteration and modulation in hepatitis B virus-related fibrosis and complications:Molecular mechanisms and therapeutic inventions显示文摘Hepatitis B virus(HBV)has posed a threat to public health,mainly resulting in liver damage.With long-term accumulation of extracellular matrix,patients with chronic hepatitis B are at high risk of developing into liver fibrosis and cirrhosis and even life-threatening hepatic carcinoma.The occurrence of complications such as spontaneous bacterial peritonitis and hepatic encephalopathy greatly increases disability and mortality.With deeper understanding of the bidirectional interaction between the liver and the gut(gut-liver axis),there is a growing consensus that the human health closely relates to the gut microbiota.Supported by animal and human studies,the gut microbiota alters as the HBV-related liver fibrosis initials and progresses,characterized as the decrease of the ratio between“good”and“potentially pathogenic”microbes.When the primary disease is controlled via antiviral treatment,the gut microbiota dysfunction tends to be improved.Conversely,the recovery of gut microbiota can promote the regression of liver fibrosis.Therapeutic strategies targeted on gut microbiota(rifaximin,probiotics,engineered probiotics and fecal microbiota transplantation)have been applied to animal models and patients,obtaining satisfactory results.Yao-Guang Li Zu-Jiang Yu Ang Li Zhi-Gang Ren 2022World Journal of Gastroenterology2022,28,28:3

15Characterization and biodegradation kinetics of a new cold-adapted carbamazepine-degrading bacterium, Pseudomonas sp. CBZ-4显示文摘Carbamazepine is frequently detected in waters and hardly eliminated during conventional wastewater treatment processes due to its complicated chemical structure and resistance to biodegradation. A carbamazepine-degrading bacterium named CBZ-4 was isolated at a low temperature(10°C) from activated sludge in a municipal wastewater treatment plant. Strain CBZ-4, which can use carbamazepine as its sole source of carbon and energy, was identified as Pseudomonas sp. by the 16S rRNA gene sequence. The composition and percentage of fatty acids, which can reveal the cold-adaptation mechanism of strain CBZ-4, were determined. Strain CBZ-4 can efectively degrade carbamazepine at optimal conditions: pH 7.0, 10°C, 150 r/min rotation speed, and 13% inoculation volume. The average removal rate of carbamazepine was 46.6% after 144 hr of incubation. The biodegradation kinetics of carbamazepine by CBZ-4 was fitted via the Monod model. 17Role of interstitial flow in tumor migration through 3D ECM显示文摘Cancer invasion in tissue is simultaneously regulated by chemical and mechanical cues.Evidences suggest that interstitial flow plays a critical role in tumor metastasis.On one hand,the distribution of chemokines around cell is influenced by flow.On the other hand,interstitial flow may reconfigure the alignment of fiber matrix,which greatly changes the contact force between cell and extracellular matrix.In this study,we have upgraded a model by which we can quantitatively investigate the influence of flow on tumor cell migration.A hydrodynamic analysis of shear stress on a slender body is introduced to simulate the fiber realignment.Factors such as subtle flow and cell-matrix interaction which dominate tumor migration are integrated in this novel model.Simulation results show interstitial flow facilitates tumor cell migration in the flow direction.Moreover,the flow-related chemical and mechanical cues have a synergistic effect on the migration.This model provides better understanding on cancer metastasis and helps design vitro experiment precisely.Ang Li Ren Sun 2020Acta Mechanica Sinica2020,36,3:2
18Modulation of the cutoff wavelength in the spectra for solar selective absorbing coating based on high-entropy films显示文摘This paper demonstrates an intrinsic modulation of the cutoff wavelength in the spectra for solar selective absorbing coating based on high-entropy films.The(NiCuCrFeSi)N((NCCFS)N)films were deposited by a magnetron sputtering system.Rutherford backscattering spectroscopy analysis confirms the uniform composition and good homogeneity of these high-entropy films.The real and imaginary parts of the permittivity for the(NCCFS)N material are calculated on the basis of the reflectance spectral fitting results.A redshift cutoff wavelength of the reflectance spectrum with increasing nitrogen gas flow rate exists because of the different levels of dispersion when changing nitrogen content.To realize significant solar absorption,the film surface was reconstituted to match its impedance with air by designing a pyramid nanostructure metasurface.Compared with the absorptance of the as-deposited films,the designed metasurface obtains a significant improvement in solar absorption with the absorptance increasing from 0.74 to 0.99.The metasurfaces also show low mid-infrared emissions with thermal emittance that can be as low as 0.06.These results demonstrate a new idea in the design of solar selective absorbing surface with controllable absorptance and low infrared emission for high-efficiency photo-thermal conversion.Ping Song Cong Wang Jie Ren Ying Sun Yong Zhang Angélique Bousquet Thierry Sauvage Eric Tomasella 2020International Journal of Minerals,Metallurgy and Materials2020,27,10:2

20Rational integration of defense and repair synergy on PEEK osteoimplants via biomimetic peptide clicking strategy显示文摘Polyetheretherketone(PEEK)has been widely used as orthopedic and dental materials due to excellent mechanical and physicochemical tolerance.However,its biological inertness,poor osteoinduction,and weak antibacterial activity make the clinical applications in a dilemma.Inspired by the mussel adhesion mechanism,here we reported a biomimetic surface strategy for rational integration and optimization of anti-infectivity and osteo-inductivity onto PEEK surfaces using a mussel foot proteins(Mfps)-mimic peptide with clickable azido terminal.The peptide enables mussel-like adhesion on PEEK biomaterial surfaces,leaving azido groups for the further steps of biofunctionalizations.In this study,antimicrobial peptide(AMP)and osteogenic growth peptide(OGP)were bioorthogonally clicked on the azido-modified PEEK biomaterials to obtain a dual-effect of host defense and tissue repair.Since bioorthogonal clicking allows precise collocation between AMP and OGP through changing their feeding molar ratios,an optimal PEEK surface was finally obtained in this research,which could long-term inhibit bacterial growth,stabilize bone homeostasis and facilitate interfacial bone regeneration.In a word,this upgraded mussel surface strategy proposed in this study is promising for the surface bioengineering of inert medical implants,in particular,achieving rational integration of multiple biofunctions to match clinical requirements.Meng Li Jiaxiang Bai Huaqiang Tao Li Hao Weiling Yin Xiaoxue Ren Ang Gao Ning Li Miao Wang Shiyuan Fang Yaozeng Xu Liang Chen Huilin Yang Huaiyu Wang Guoqing Pan Dechun Geng 2022Bioactive Materials2022,7,2:2
