| 1 | Roux-en-Y versus BillrothⅠreconstruction after distal gastrectomy for gastric cancer:A meta-analysis显示文摘AIM:To conduct a meta-analysis to compare Rouxen-Y(R-Y) gastrojejunostomy with gastroduodenal Billroth Ⅰ(B-Ⅰ) anastomosis after distal gastrectomy(DG) for gastric cancer.METHODS:A literature search was performed to identify studies comparing R-Y with B-Ⅰ after DG for gastric cancer from January 1990 to November 2012 in Medline,Embase,Science Citation Index Expanded and the Cochrane Central Register of Controlled Trials in The Cochrane Library.Pooled odds ratios(OR) or weighted mean differences(WMD) with 95%CI were calculated using either fixed or random effects model.Operative outcomes such as operation time,intraoperative blood loss and postoperative outcomes such as anastomotic leakage and stricture,bile reflux,remnant gastritis,reflux esophagitis,dumping symptoms,delayed gastric emptying and hospital stay were the main outcomes assessed.Meta-analyses were performed using RevMan 5.0 software(Cochrane library).RESULTS:Four randomized controlled trials(RCTs) and 9 non-randomized observational clinical studies(OCS) involving 478 and 1402 patients respectively were included.Meta-analysis of RCTs revealed that R-Y reconstruction was associated with a reduced bile reflux(OR 0.04,95%CI:0.01,0.14;P < 0.00 001) and remnant gastritis(OR 0.43,95%CI:0.28,0.66;P = 0.0001),however needing a longer operation time(WMD 40.02,95%CI:13.93,66.11;P = 0.003).Metaanalysis of OCS also revealed R-Y reconstruction had a lower incidence of bile reflux(OR 0.21,95%CI:0.08,0.54;P = 0.001),remnant gastritis(OR 0.18,95%CI:0.11,0.29;P < 0.00 001) and reflux esophagitis(OR 0.48,95%CI:0.26,0.89;P = 0.02).However,this reconstruction method was found to be associated with a longer operation time(WMD 31.30,95%CI:12.99,49.60;P = 0.0008).CONCLUSION:This systematic review point towards some clinical advantages that are rendered by R-Y compared to B-Ⅰ reconstruction post DG.However there is a need for further adequately powered,welldesigned RCTs comparing the same. | Jun-Jie Xiong Kiran Altaf Muhammad A Javed Quentin M Nunes Wei Huang Gang Mai Chun-Lu Tan Rajarshi Mukherjee Robert Sutton Wei-Ming Hu Xu-Bao Liu | 2013 | World Journal of Gastroenterology2013,19,7: | 33 |

| 3 | Meta-analysis of laparoscopic vs open liver resection for hepatocellular carcinoma显示文摘AIM:To conduct a meta-analysis to determine the safety and efficacy of laparoscopic liver resection(LLR) and open liver resection(OLR) for hepatocellular carcinoma(HCC).METHODS:PubMed(Medline),EMBASE and Science Citation Index Expanded and Cochrane Central Register of Controlled Trials in the Cochrane Library were searched systematically to identify relevant comparative studies reporting outcomes for both LLR and OLR for HCC between January 1992 and February 2012.Two authors independently assessed the trials for inclusion and extracted the data.Meta-analysis was performed using Review Manager Version 5.0 software(The Cochrane Collaboration,Oxford,United Kingdom).Pooled odds ratios(OR) or weighted mean differences(WMD) with 95%CI were calculated using either fixed effects(Mantel-Haenszel method) or random effects models(DerSimonian and Laird method).Evaluated endpoints were operative outcomes(operation time,intraoperative blood loss,blood transfusion requirement),postoperative outcomes(liver failure,cirrhotic decompensation/ascites,bile leakage,postoperative bleeding,pulmonary complications,intraabdominal abscess,mortality,hospital stay and oncologic outcomes(positive resection margins and tumor recurrence).RESULTS:Fifteen eligible non-randomized studies were identified,out of which,9 high-quality studies involving 550 patients were included,with 234 patients in the LLR group and 316 patients in the OLR group.LLR was associated with significantly lower intraoperative blood loss,based on six studies with 333 patients [WMD:-129.48 mL;95%CI:-224.76-(-34.21) mL;P = 0.008].Seven studies involving 416 patients were included to assess blood transfusion requirement between the two groups.The LLR group had lower blood transfusion requirement(OR:0.49;95%CI:0.26-0.91;P = 0.02).While analyzing hospital stay,six studies with 333 patients were included.Patients in the LLR group were found to have shorter hospital stay [WMD:-3.19 d;95%CI:-4.09-(-2.28) d;P < 0.00001] than their OLR counterpart.Seven studies including 416 patients were pooled together to estimate the odds of developing postoperative ascites in the patient groups.The LLR group appeared to have a lower incidence of postoperative ascites(OR:0.32;95%CI:0.16-0.61;P = 0.0006) as compared with OLR patients.Similarly,fewer patients had liver failure in the LLR group than in the OLR group(OR:0.15;95%CI:0.02-0.95;P =0.04).However,no significant differences were found between the two approaches with regards to operation time [WMD:4.69 min;95%CI:-22.62-32 min;P = 0.74],bile leakage(OR:0.55;95%CI:0.10-3.12;P = 0.50),postoperative bleeding(OR:0.54;95%CI:0.20-1.45;P = 0.22),pulmonary complications(OR:0.43;95%CI:0.18-1.04;P = 0.06),intra-abdominal abscesses(OR:0.21;95%CI:0.01-4.53;P = 0.32),mortality(OR:0.46;95%CI:0.14-1.51;P = 0.20),presence of positive resection margins(OR:0.59;95%CI:0.21-1.62;P = 0.31) and tumor recurrence(OR:0.95;95%CI:0.62-1.46;P = 0.81).CONCLUSION:LLR appears to be a safe and feasible option for resection of HCC in selected patients based on current evidence.However,further appropriately designed randomized controlled trials should be undertaken to ascertain these findings. | Jun-Jie Xiong Kiran Altaf Muhammad A Javed Wei Huang Rajarshi Mukherjee Gang Mai Robert Sutton Xu-Bao Liu Wei-Ming Hu | 2012 | World Journal of Gastroenterology2012,18,45: | 25 |
| 4 | 不同品种芥菜对镉胁迫的敏感性分布及抗氧化特征显示文摘为探讨不同品种芥菜(Brassica juncea L.)对土壤中镉(Cd)胁迫的敏感性分布规律,筛选出耐性强、吸收高的品种,并了解其抗氧化特征和Cd累积特征,为我国Cd污染土壤植物修复提供理论依据。以国内市场上常见的21个芥菜品种为材料,通过盆栽试验,研究不同品种芥菜对Cd胁迫的剂量-效应关系、敏感性分布差异,在此基础上对两种代表性芥菜的抗氧化酶活性及Cd含量进行测定。结果表明:21个芥菜品种对Cd的敏感性分布频次具有明显差异,低浓度Cd(≤1.23 mg·kg^(-1))对17种芥菜生长产生一定的刺激效应,其生物量增长了2.0%~30.5%,而过量的Cd(≥6.90 mg·kg^(-1))则会产生明显的胁迫作用。特选九头鸟大叶芥菜(JTN)对Cd最为敏感,而包心芥菜(BJ)则表现出最高耐性。通过对JTN和BJ体内丙二醛(MDA)和抗氧化酶活性(SOD、POD、CAT)分析,BJ体内的MDA含量明显低于JTN,而抗氧化酶活性高于JTN。耐性品种BJ根部及地上部分的富集系数均大于1,地上部Cd积累量最高可达156.03μg·株-1,且地上部对Cd污染土壤的净化率最大可达11.0%。研究表明,耐性品种BJ具有修复Cd污染土壤的潜力。 | 郭瞻宇 张子杨 蒋亚辉 Altaf Hussain Lahori 张增强 李荣华 | 2018 | 农业环境科学学报2018,37,12: | 16 |
| 5 | Validation of the moderate severity category of acute pancreatitis defined by determinant-based classification显示文摘BACKGROUND: Recent international multidisciplinary consultation proposed the use of local(sterile or infected pancreatic necrosis) and/or systemic determinants(organ failure) in the stratification of acute pancreatitis. The present study was to validate the moderate severity category by international multidisciplinary consultation definitions.METHODS: Ninety-two consecutive patients with severe acute pancreatitis(according to the 1992 Atlanta classification) were classified into(i) moderate acute pancreatitis group with the presence of sterile(peri-) pancreatic necrosis and/or transient organ failure; and(ii) severe/critical acute pancreatitis group with the presence of sterile or infected pancreatic necrosis and/or persistent organ failure. Demographic and clinical outcomes were compared between the two groups.RESULTS: Compared with the severe/critical group(n=59), the moderate group(n=33) had lower clinical and computerized tomographic scores(both P<0.05). They also had a lower incidence of pancreatic necrosis(45.5% vs 71.2%, P=0.015),infection(9.1% vs 37.3%, P=0.004), ICU admission(0% vs27.1%, P=0.001), and shorter hospital stay(15±5 vs 27±12days; P<0.001). A subgroup analysis showed that the moderate group also had significantly lower ICU admission rates, shorter hospital stay and lower rate of infection compared with the severe group(n=51). | 11 | Update summarising the conclusions of the international consultation on male lower urinary tract symptoms显示文摘The International Consultation on Urological Disease have recently published comprehensive conclusions, based on evidence reviewed by eight committees, on aspects of male lower urinary tract symptoms(LUTS). In this review, we summarise the conclusions from fourof the committees, namely, the evidence regarding the epidemiology of male LUTS, patient assessment, nocturia and medical management. It is indisputable that with an expanding and ageing global population the prevalence of male LUTS is likely to increase. Therefore symptom prevention and preservation of quality of life(Qo L) feature highly in the guidelines. There are now a number of different medical options, proven to lead to significant improvements in symptom scores, flow rate and Qo L available to men with LUTS. Metaanalyses have shown the benefits for alpha blockers, antimuscarinics, 5-α reductase and phosphodiesterase-5 inhibitors. High level evidence also exists for combinations of all of the above with alpha blockers and so men with concomitant storage symptoms, prostate volume > 30 mL, PSA > 1.4 or erectile dysfunction may be considered for combination treatment of an alpha blocker with an antimuscarinic, 5-α reductase inhibitor or phosphodiesterase-5 inhibitor respectively. | 12 | Prediction of the severity of acute pancreatitis on admission by urinary trypsinogen activation peptide: A meta-analysis显示文摘AIM: To undertake a meta-analysis on the value of urinary trypsinogen activation peptide (uTAP) in predicting severity of acute pancreatitis on admission. METHODS: Major databases including Medline, Embase, Science Citation Index Expanded and the Cochrane Central Register of Controlled Trials in the Cochrane Library were searched to identify all relevant studies from January 1990 to January 2013. Pooled sensitivity, specificity and the diagnostic odds ratios (DORs) with 95%CI were calculated for each study and were compared to other systems/biomarkers if mentioned within the same study. Summary receiver-operating curves were conducted and the area under the curve (AUC) was evaluated. RESULTS: In total, six studies of uTAP with a cut-off value of 35 nmol/L were included in this meta-analysis. Overall, the pooled sensitivity and specificity of uTAP for predicting severity of acute pancreatitis, at time of admission, was 71% and 75%, respectively (AUC = 0.83, DOR = 8.67, 95%CI: 3.70-20.33). When uTAP was compared with plasma C-reactive protein, the pooled sensitivity, specificity, AUC and DOR were 0.64 vs 0.67, 0.77 vs 0.75, 0.82 vs 0.79 and 6.27 vs 6.32, respectively. Similarly, the pooled sensitivity, specificity, AUC and DOR of uTAPvs Acute Physiology and Chronic Health Evaluation Ⅱ within the first 48 h of admission were found to be 0.64 vs 0.69, 0.77 vs 0.61, 0.82 vs 0.73 and 6.27vs 4.61, respectively. Recently, there have been important advances in the molecular, pathological and biological understandingof pancreatic cancer. Even after the emergence of recent new targeted agents and the use of multiple therapeutic combinations, no treatment option is viable in patients with advanced cancer. Developing novel strategies to target progression of PC is of intense interest. A small population of pancreatic cancer stem cells(CSCs) has been found to be resistant to chemotherapy and radiation therapy. CSCs are believed to be responsible for tumor initiation, progression and metastasis. The CSC research has recently achieved much progress in a variety of solid tumors, including pancreatic cancer to some extent. This leads to focus on understanding the role of pancreatic CSCs. The focus on CSCs may offer new targets for prevention and treatment of this deadly cancer. We review the most salient developments in important areas of pancreatic CSCs. | 14 | Concomitant-chemoradiotherapy-associated oral lesions in patients with oral squamous-cell carcinoma显示文摘Objective:Oral squamous-cell carcinoma(OSCC)accounts for >90% of oral cancers affecting adults mostly between the fourth to seventh decades of life.The most common OSCC treatment is concomitant chemoradiotherapy(CCRT)having both locoregional and distant control,but CCRT has acute and chronic toxic effects on adjacent normal tissue.This study aimed to determine the side effects of CCRT on the oral mucosa and to characterize the clinicopathology of oral lesions in patients with OSCC.Methods:This descriptive,cross-sectional study was certified by the Ethical Review Committee(UHS/Education/126-12/2728)of the University of Health Sciences,Lahore,Pakistan.OSSC patients(n=81)with various histological subtypes,grades,and stages were recruited,and findings on their oral examination were recorded.These patients received 70,90,and 119 Gy of radiotherapy dosages in combination with the chemotherapy drugs cisplatin and 5-fluorouracil.Data were analyzed using SPSS 20.0.Results:The most common presentation of OSCC was a nonhealing ulcer(63%) involving tongue(55.6%).Clinical findings included mucositis(92.6%)and xerostomia of mild,moderate,and severe degrees in 11.1%,46.9%,and 35.8% cases,respectively.Ulcers(87.7%),palpable lymph nodes(64.2%),limited mouth opening(64.2%)and fistula(40.7%) were also observed.In females,the association of radiotherapy dosage with limited mouth opening,xerostomia,and histological grading was statistically significant(P<0.05).The association of chemotherapy drugs with xerostomia(P=0.003)was also statistically significant.Conclusions:CCRT induced mucositis,xerostomia,and trismus in patients with OSCC. | Sadia Minhas Muhammad Kashif Wasif Altaf Nadeem Afzal Abdul Hanan Nagi | 2017 | Cancer Biology & Medicine2017,14,2: | 6 |

| 17 | Residual effects of tobacco biochar along with different fixing agents on stabilization of trace elements in multi-metal contaminated soils显示文摘The residual effect of tobacco biochar(TB≥500°C)mono and co-application with Cahydroxide(CH),Ca-bentonite(CB)and natural zeolite(NZ)on the bio-availability of trace elements TE(s)in alkaline soils has not been deeply studied yet.A pot study that had earlier been investigated TB mono and blended with CH,CB and NZ on the immobilization of Pb,Cu Cd,and Zn by Chinese cabbage.Maize crop in the rotation was selected as test plant to assess the residual impact of amendments on stabilization of Pb,Cu Cd,and Zn in mine polluted(M-P),smelter heavily and low polluted(S-HP and S-LP,respectively)soils.The obtained results showed that stabilization of Pb,Cd,Cu and Zn reached 63.84% with TB+CB,61.19% with TB+CH,83.31% with TB+CH and 35.27% with TB+CH for M-P soil,36.46% with TB+NZ,38.46% with TB+NZ,19.40% with TB+CH and 62.43% with TB+CH for S-LP soil,52.94%TB+NZ,57.65% with TB+NZ,52.94% with TB+NZ,and 28.44% with TB+CH for S-LP soil.Conversely,TB+CH and TB alone had mobilized Pb and Zn up to19.29% and 34.96% in M-P soil.The mobility of Zn reached 8.38% with TB+CB and 66.03% with TB for S-HP and S-LP soils.The uptake and accumulation of Pb,Cd,Cu and Zn in shoot and root were reduced in three polluted soils.Overall,the combination of TB along with CH,CB and NZ has been proven to be effective in Pb,Cd,Cu and Zn polluted mine/smelter soils restoration. | Altaf Hussain Lahori Monika Mierzwa-Hersztek Muhammad Rashid Shahmir Ali Kalhoro Mehrunisa Memon Zobia Naheed Muneer Ahmed Zengqiang Zhang | 2020 | Journal of Environmental Sciences2020,32,1: | 3 |
| 18 | High-dose-rate intraluminal brachytherapy during preoperative chemoradiation for locally advanced rectal cancers显示文摘AIM:To determine the feasibility and safety of high dose rate intraluminal brachytherapy(HDR-ILBT) boost during preoperative chemoradiation for rectal cancer.METHODS:Between 2008 and 2009,thirty-six patients with locally advanced rectal cancer(≥ T3 or N+),were treated initially with concurrent capecitabine(825 mg/m2 oral twice daily) and pelvic external beam radiotherapy(EBRT)(45 Gy in 25 fractions),then were randomized to group A;HDR-ILBT group(n = 17) to receive 5.5-7 Gy×2 to gross tumor volume(GTV) and group B;EBRT group(n = 19) to receive 5.4 Gy×3 fractions to GTV with EBRT.All patients underwent total mesorectal excision.RESULTS:Grade 3 acute toxicities were registered in 12 patients(70.6%) in group A and in 8(42.1%) in group B.Complete pathologic response of T stage(ypT0) in group A was registered in 10 patients(58.8%) and in group B,3 patients(15.8%) had ypT0(P < 0.0001).Sphincter preservation was reported in 6/9 patients(66.7%) in group A and in 5/10 patients(50%) in group B(P < 0.01).Overall radiological response was 68.15% and 66.04% in Group A and B,respectively.During a median follow up of 18 mo,late grade 1 and 2 sequelae were registered in 3 patients(17.6%) and 4 patients(21.1%) in the groups A and B,respectively.CONCLUSION:HDR-ILBT was found to be effective dose escalation technique in preoperative chemoradiation for rectal cancers,with higher response rates,downstaging and with manageable acute toxicities. | Mutahir Ali Tunio Mansoor Rafi Altaf Hashmi Rehan Mohsin Abdul Qayyum Mujahid Hasan Amjad Sattar Muhammad Mubarak | 2010 | World Journal of Gastroenterology2010,16,35: | 3 |

| 20 | Degradation of malathion by Pseudomonas during activated sludge treatment system using principal component analysis (PCA)显示文摘Popular descriptive multivariate statistical method currently employed is the principal component analyses (PCA) method. PCA is used to develop linear combinations that successively maximize the total variance of a sample where th ere is no known group structure. This study aimed at demonstrating the performan ce evaluation of pilot activated sludge treatment system by inoculating a strain of Pseudomonas capable of degrading malathion which was isolated by enrichment technique. An intensive analytical program was followed for evaluating the effic iency of biosimulator by maintaining the dissolved oxygen (DO) concentration at 4.0 mg/L. Analyses by high performance liquid chromatographic technique revealed that 90% of malathion removal was achieved within 29 h of treatment whereas COD got reduced considerably during the treatment process and mean removal efficien cy was found to be 78%. The mean pH values increased gradually during the treatm ent process ranging from 7.36—8.54. Similarly the mean ammonia-nitrogen (NH3-N) values were found to be fluctuating between 19.425—28.488 mg/L, mean nitrite-n itrogen (NO3-N) ranging between 1.301—2.940 mg/L and mean nitrate-nitrogen (NO3 -N) ranging between 0.0071—0.0711 mg/L. The study revealed that inoculation of bacterial culture under laboratory conditions could be used in bioremediation of environmental pollution caused by xenobiotics. The PCA analyses showed that pH, COD, organic load and total malathion concentration were highly correlated and emerged as the variables controlling the first component, whereas dissolved oxyg en, NO3-N and NH3-N governed the second component.
