| 2 | Diagnostic accuracy of endoscopic ultrasound in pancreatic neuroendocrine tumors: A systematic review and meta analysis显示文摘AIM: To detect pancreatic neuroendocrine tumors (PNETs) has been varied. This study is undertaken to evaluate the accuracy of endoscopic ultrasound (EUS) in detecting PNETs.METHODS: Only EUS studies confirmed by surgery or appropriate follow-up were selected. Articles were searched in Medline, Ovid journals, Medline nonindexed citations, and Cochrane Central Register of Controlled Trials and Database of Systematic Reviews. Pooling was conducted by both fixed and random effects model). RESULTS: Initial search identified 2610 reference articles, of these 140 relevant articles were selected and reviewed. Data was extracted from 13 studies (n = 456) which met the inclusion criteria. Pooled sensitivity of EUS in detecting a PNETs was 87.2% (95%CI: 82.2-91.2). EUS had a pooled specificity of 98.0% (95%CI: 94.3-99.6). The positive likelihood ratio of EUS was 11.1 (95%CI: 5.34-22.8) and negative likelihood ratio was 0.17 (95%CI: 0.13-0.24). The diagnostic odds ratio, the odds of having anatomic PNETs in positive as compared to negative EUS studies was 94.7 (95%CI: 37.9-236.1). Begg-Mazumdar bias indicator for publication bias gave a Kendall's tau value of 0.31 (P = 0.16), indication no publication bias. The P for χ2 heterogeneity for all the pooled accuracy estimates was > 0.10. CONCLUSION: EUS has excellent sensitivity and specificity to detect PNETs. | 7 | Synthesis, characterization, cytotoxicity, DNA cleavage and antimicrobial activity of homodinuclear lanthanide complexes of phenylthioacetic acid显示文摘Lanthanide complexes of Eu(III),Gd(III),Nd(III),Sm(III),and Tb(III)with phenylthioacetic acid were synthesized and characterized by elemental analysis,mass,infrared radiation(IR),electronic spectra,molar conductance,thermogravimetric analysis(TGA),and powder X-ray diffraction(XRD).The results showed that the lanthanide complexes were homodinuclear in nature.The two lanthanide ions were bridged by eight oxygen atoms from four carboxylate groups.Thermal decomposition profiles were consistent with the proposed formulations.Powder XRD studies showed that all the complexes were amorphous in nature.Antimicrobial studies indicated that these complexes exhibited more activity than the ligand itself.The DNA cleavage activity of the ligand and its complexes were assayed on CT DNA using gel electrophoresis in the presence of H2O2.The result showed that the Eu(III)and Nd(III)complexes completely cleaved the DNA.The anticancer activities of the complexes were also studied towards human cervical cancer cell line(HeLa)and colon cancer cells(HCT116)and it was found that the Eu(III)and Nd(III)complexes were more active than the corresponding Gd(III),Sm(III),Tb(III)complexes and the free ligand on both the cancer cells. | T. Considering the connections between host and environment in TB显示文摘Accurate prediction of which patient will progress from a sub-clinical Mycobacterium tuberculosis infection to active tuberculosis represents an elusive,yet critical,clinical research objective.From the individual perspective,progression can be considered to be the product of a series of unfortunate events or even a run of bad luck.Here,we identify the subtle physiological relationships that can influence the odds of progression to active TB and how this progression may reflect directed dysbiosis in a number of interrelated systems.Most infected individuals who progress to disease have apparently good immune responses,but these responses are,at times,compromised by either local or systemic environmental factors.Obvious disease promoting processes,such as tissue-damaging granulomata,usually manifest in the lung,but illness is systemic.This apparent dichotomy between local and systemic reflects a clear need to define the factors that promote progression to active disease within the context of the body as a physiological whole.We discuss aspects of the host environment that can impact expression of immunity,including the microbiome,glucocorticoid-mediated regulation,catecholamines and interaction between the gut,liver and lung.We suggest the importance of integrating precision medicine into our analyses of experimental outcomes such that apparently conflicting results are not contentious,but rather reflect the impact of these subtle relationships with our environment and microbiota. | John E Pearl Mrinal Das Andrea M Cooper | 2018 | Cellular & Molecular Immunology2018，15，3： | 3 |

| 15 | A three-dimensional k-ε-k_p model in curvilinear coordinates for sediment movement and bed evolution显示文摘To aim at the substitution of the magnitude and direction of water flow movement near bed for those of bed load transport in solid-liquid two-phase one-fluid model, and to simulate the effect of secondary flow on transverse bed load transport in channel bends and the effect of bed slope on bed load trans- port in a better way, a three-dimensional k-ε-kp solid-liquid two-phase two-fluid model in curvilinear coordinates is solved numerically with a finite-volume method on an adaptive grid for studying wa- ter-sediment movements and bed evolution in a 120° channel bend. Numerical results show that the trajectories of solid-phase deviate from those of liquid-phase in the channel bend, and the deviation increases with the increase of the particle diameters. The calculated bed deformation by the k-ε-kp model is in better agreement with measured bed deformation than those by one-fluid model.
