| 1 | Gallstone recurrence after successful percutaneous cholecystolithotomy:a 10-year follow-up of 439 cases显示文摘BACKGROUND: Stone recurrence is a major problem in the medication of gallstones with gallbladder preservation. The aim of this study was to determine the long-term recurrence rate of gallstones and the clinical outcome after successful percutaneous cholecystolithotomy (PCCL) treatment, and to investigate the possible risk factors for gallstone recurrence. METHODS: After successful PCCL for gallstones, 439 patients were followed up during a 10-year period. The long-term gallstone recurrence rate and clinical outcome were evaluated. Risk factors associated with stone recurrence were identified. RESULTS: Gallstone recurrence was detected in 182 of 439 PCCL patients, giving an overall recurrence rate of 41.46%. The cumulative gallstone recurrence rate for each of the 10 post-operative years was 9.57%, 18.91%, 27.33%, 34.14%, 37.59%, 39.86%, 41.90%, 42.73%, 42.85%, and 43.21%, respectively. Among these recurrent patients, 94 were asymptomatic, 80 suffered from nonspecific upper gastrointestinal symptoms and 8 suffered from abdominal pain or biliary colic. Thirty-eight of the 182 patients were retreated with cholecystectomy. The risk factors for stone recurrence included a family history of gallstones, preference for fatty food, accompanying liver disease, multiple stones and poor gallbladder function pre-PCCL. CONCLUSIONS: In this study, the overall recurrence rate of gallstone was 41.46% during a 10-year period. The highest frequency of gallstone recurrence was during the 5th to 6th postoperative years and then continued to slowly increase. | 3 | Intraoperative radiofrequency ablation combined with ^(125)iodine seed implantation for unresectable pancreatic cancer显示文摘AIM: To evaluate the feasibility, efficacy and safety of intraoperative radiofrequency ablation (RFA) combined with 125 iodine seed implantation for unresectable pancreatic cancer. METHODS: Thirty-two patients (21 males and 11 females) at the age of 68 years (range 48-90 years) with unresectable locally advanced pancreatic cancer admitted to our hospital from January 2006 to May 2008 were enrolled in this study. The tumor, 4-12 cm in diameter, located in pancreatic head of 23 patients and in pancreatic body and tail of 9 patients, was found to be unresectable during operation. Diagnosis of pancreatic cancer was made through intraoperative biopsy. Patients were treated with FRA combined with 125 iodine seed implantation. In brief, a RFA needle was placed, which was confirmed by intraoperative ultrasound to decrease the potential injury of surrounding vital structures, a 125 iodine seed was implanted near the blood vessels and around the tumor border followed by bypass palliative procedure (cholangio-jejunostomy and/or gastrojejunostomy) in 29 patients.RESULTS: The serum CA 19-9 level was decreased from 512 ± 86 U/mL before operation to 176 ± 64 U/mL, 108 ± 42 U/mL and 114 ± 48 U/mL, respectively, 1, 3 and 6 mo after operation (P < 0.05). The pain score on day 7 after operation, 1 and 3 mo after combined therapy was decreased from 5.86 ± 1.92 before operation to 2.65 ± 1.04, 1.65 ± 0.88 and 2.03 ± 1.16, respectively, after operation (P < 0.05). The rate of complete response (CR), partial response (PR), stable disease (SD), progressive disease (PD) in 32 patients was 21.8% (7/32), 56.3% (18/32), 15.6% (5/32) and 6.3% (2/32), respectively, 6 mo after operation, with a median overall survival time of 17. 5 mo. The median survival time of patients at stage Ⅲ was longer than that of those at stage Ⅳ (19 mo vs 10 mo, P = 0.0026). The median survival time of patients who received and did not receive chemotherapy after operation was 20 mo and 16 mo, respectively (P = 0.0176). Of the 32 patients, 3 (10.6%) experienced postoperative complications including transient biliary leaks in 2 patients and acute pancreatitis in 1 patient. All the patients recovered well after conservative support treatment. | 4 | Effect of Rare Earth on Microstructures and Properties of High Speed Steel With High Carbon Content显示文摘The effects of rare earth (RE) on the microstructures and properties of high carbon high speed steel (HCHSS) were investigated. The results show that when suitable RE is added to the HCHSS, the effect of RE on the austenite and eutectic carbides is obvious. The austenite grain and coarse eutectic structure are refined, and flake carbides in the eutectic structures become short and fine. After heat treatment, most of the eutectic carbides are spheroidized and distributed in a uniform manner. The hardness and red hardness of modified HCHSS are slightly increased; impact toughness is greatly increased by 37.81% and reaches 10.17 J/cm^2. | 5 | Submucosal tunneling endoscopic resection: An effective and safe therapy for upper gastrointestinal submucosal tumors originating from the muscularis propria layer显示文摘AIM To evaluate the effectiveness and safety of submucosal tunneling endoscopic resection(STER) and compare its outcomes in esophageal and cardial submucosal tumors(SMTs) of the muscularis propria(MP) layer.METHODS From May 2012 to November 2017, 173 consecutive patients with upper gastrointestinal(GI) SMTs of the MP layer underwent STER. Overall, 165 patients were included, and 8 were excluded. The baseline characteristics of the patients and SMTs were recorded. The en bloc resection rate, complete resection rate,residual rate, and recurrence rate were calculated to evaluate the effectiveness of STER, and the complication rate was recorded to evaluate its safety. Effectiveness and safety outcomes were compared between esophageal and cardial SMTs.RESULTS One hundred and twelve men and 53 women with a mean age of 46.9 ± 10.8 years were included. The mean tumor size was 22.6 ± 13.6 mm. Eleven SMTs were located in the upper esophagus(6.7%), 49 in the middle esophagus(29.7%), 46 in the lower esophagus(27.9%), and 59 in the cardia(35.7%). Irregular lesions accounted for 48.5% of all lesions. STER achieved an en bloc resection rate of78.7%(128/165) for GI SMTs with an overall complication rate of 21.2%(35/165).All complications resolved without intervention or were treated conservatively without the need for surgery. The en bloc resection rates of esophageal and cardial SMTs were 81.1%(86/106) and 72.1%(42/59), respectively(P = 0.142), and the complication rates were 19.8%(21/106) and 23.7%(14/59), respectively,(P =0.555). The most common complications for esophageal SMTs were gas-related complications and fever, while mucosal injury was the most common for cardial SMTs.CONCLUSION STER is an effective and safe therapy for GI SMTs of the MP layer. | 9 | Big-data analysis: A clinical pathway on endoscopic retrograde cholangiopancreatography for common bile duct stones显示文摘BACKGROUND A clinical pathway(CP)is a standardized approach for disease management.However,big data-based evidence is rarely involved in CP for related common bile duct(CBD)stones,let alone outcome comparisons before and after CP implementation.AIM To investigate the value of CP implementation in patients with CBD stones undergoing endoscopic retrograde cholangiopancreatography(ERCP).METHODS This retrospective study was conducted at Nanjing Drum Tower Hospital in patients with CBD stones undergoing ERCP from January 2007 to December 2017.The data and outcomes were compared by using univariate and multivariable regression/linear models between the patients who received conventional care(non-pathway group,n=467)and CP care(pathway group,n=2196).RESULTS At baseline,the main differences observed between the two groups were the percentage of patients with multiple stones(P<0.001)and incidence of cholangitis complication(P<0.05).The percentage of antibiotic use and complications in the CP group were significantly less than those in the nonpathway group[adjusted odds ratio(OR)=0.72,95%confidence interval(CI):0.55-0.93,P=0.012,adjusted OR=0.44,95%CI:0.33-0.59,P<0.001,respectively].Patients spent lower costs on hospitalization,operation,nursing,medication,and medical consumable materials(P<0.001 for all),and even experienced shorter length of hospital stay(LOHS)(P<0.001)after the CP implementation.No significant differences in clinical outcomes,readmission rate,or secondary surgery rate were presented between the patients in the non-pathway and CP groups.CONCLUSION Implementing a CP for patients with CBD stones is a safe mode to reduce the LOHS,hospital costs,antibiotic use,and complication rate. | Wei Zhang Bing-Yi Wang Xiao-Yan Du Wei-Wei Fang Han Wu Lei Wang Yu-Zheng Zhuge Xiao-Ping Zou | 2019 | World Journal of Gastroenterology2019,25,8: | 14 |

| 11 | Insight-HXMT observations of the first binary neutron star merger GW170817显示文摘Finding the electromagnetic(EM) counterpart of binary compact star merger, especially the binary neutron star(BNS) merger,is critically important for gravitational wave(GW) astronomy, cosmology and fundamental physics. On Aug. 17, 2017,Advanced LIGO and Fermi/GBM independently triggered the first BNS merger, GW170817, and its high energy EM counterpart,GRB 170817 A, respectively, resulting in a global observation campaign covering gamma-ray, X-ray, UV, optical, IR, radio as well as neutrinos. The High Energy X-ray telescope(HE) onboard Insight-HXMT(Hard X-ray Modulation Telescope) is the unique high-energy gamma-ray telescope that monitored the entire GW localization area and especially the optical counterpart(SSS17 a/AT2017 gfo) with very large collection area(~1000 cm^2) and microsecond time resolution in 0.2-5 MeV. In addition,Insight-HXMT quickly implemented a Target of Opportunity(ToO) observation to scan the GW localization area for potential X-ray emission from the GW source. Although Insight-HXMT did not detect any significant high energy(0.2-5 MeV) radiation from GW170817, its observation helped to confirm the unexpected weak and soft nature of GRB 170817 A. Meanwhile,Insight-HXMT/HE provides one of the most stringent constraints(~10^(-7) to 10^(-6) erg/cm^2/s) for both GRB170817 A and any other possible precursor or extended emissions in 0.2-5 MeV, which help us to better understand the properties of EM radiation from this BNS merger. | 15 | Towards visible-wavelength passively mode-locked lasers in all-fibre format显示文摘Mode-locked fibre lasers(MLFLs)are fundamental building blocks of many photonic systems used in industrial,scientific and biomedical applications.To date,1–2μm MLFLs have been well developed;however,passively modelocked fibre lasers in the visible region(380–760 nm)have never been reported.Here,we address this challenge by demonstrating an all-fibre visible-wavelength passively mode-locked picosecond laser at 635 nm.The 635 nm mode-locked laser with an all-fibre figure-eight cavity uses a Pr/Yb codoped ZBLAN fibre as the visible gain medium and a nonlinear amplifying loop mirror as the mode-locking element.First,we theoretically predict and analyse the formation and evolution of 635 nm mode-locked pulses in the dissipative soliton resonance(DSR)regime by solving the Ginzburg-Landau equation.Then,we experimentally demonstrate the stable generation of 635 nm DSR mode-locked pulses with a pulse duration as short as~96 ps,a radio-frequency signal-to-noise ratio of 67 dB and a narrow spectral bandwidth of <0.1 nm.The experimental results are in excellent agreement with our numerical simulations.In addition,we also observe 635 nm noise-like pulse operation with a wide(>1 nm)and modulated optical spectrum.This work represents an important step towards miniaturized ultrafast fibre lasers in the visible spectral region. | Jinhai Zou Chuchu Dong Hongjian Wang Tuanjie Du Zhengqian Luo | 2020 | Light(Science & Applications)2020,9,1: | 9 |
| 16 | ε-Poly(L-lysine)-based Hydrogels with Fast-acting and Prolonged Antibacterial Activities显示文摘Bacterial infections and the associated morbidity and mortality due to bacterial pathogens in wounds and medical implants have been increasing as most of current coatings cannot fulfill all the requirements including excellent intrinsically antibacterial activity,low cytotoxicity, and favorable physical properties. Herein, we present a kind of antibacterial hydrogel based on ε-poly(L-lysine)(EPL)grafted carboxymethyl chitosan(CMC-g-EPL) as the inherently antibacterial matrix and the surplus EPL as highly efficient antimicrobial agent. Such hydrogels possess tunable swelling abilities with water absorption percentages of 800%–2000% and modulus varying from10 kPa to 100 kPa, and exhibit two-stage excellent antibacterial behavior. | 20 | A Study of the Rhenium-Osmium Geochronometry of Molybdenites显示文摘This study deals with the first application of the isotope dilution - inductively coupled plasma mass spectrometry in the rhenium-osmium geochronometry in China. The chemical separation procedure included decomposition of samples by means of alkali fusion, extraction of rhenium by acetone and extraction of osmium by distillation. The recovery of both elements in the entire process was more than 90% . The blanks of analyses were 0.07 ng for rhenium and 0.01 ng for 187Os. The mineralization ages of molybdenites from four molybdenum deposits in China were determined by using this method, with a precision (2σ)of about ±3% . These results agree satisfactorily with their geological backgrounds. The Re-Os method can directly determine ages of metal deposits and reflect the true ages more reliably than other dating methods that only determine the ages of country rocks from which mineralization ages are inferred.
