| 1 | Measures of patient radiation exposure during endoscopic retrograde cholangiography:Beyond fluoroscopy time显示文摘AIM:to determine whether fluoroscope time is a good predictor of patient radiation exposure during endoscopic retrograde cholangiopancreatography.METHODS:this is a prospective observational study of consecutive patients undergoing endoscopic retrograde cholangiopancreatography in a tertiary care setting.Data related to radiation exposure were collected.The following measures were obtained:Fluoroscopy time(FT),dose area product(DAP)and dose at reference point(DOSERP).Coefficients of determination were calculated to analyze the correlation between FT,DAP and DOSRP.Agreement between FT and DAP/DOSRP was assessed using Bland Altman plots.RESULTS:Four hundred sixty-three data sets were obtained.Fluoroscopy time average was 7.3 min.Fluoroscopy related radiation accounted for 86%of the total DAP while acquisition films related radiation accounted for 14%of the DAP.For any given FT there are wide ranges of DAP and DOSERP and the variability in both increases as fluoroscopy time increases.The coefficient of determination(R2)on the non transformed data for DAP and DOSERP versus FT were respectively 0.416 and0.554.While fluoroscopy use was the largest contributor to patient radiation exposure during endoscopic retrograde cholangiography(ERCP),there is a wide variability in DAP and DOSERP that is not accounted for by FT.DAP and DOSERP increase in variability as FT increases.This translates into poor accuracy of FT in predicting DAP and DOSERP at higher radiation doses.CONCLUSION:DAP and DOSERP in addition to FT should be adopted as new ERCP quality measures to estimate patient radiation exposure. | Toufic Kachaamy Edwyn Harrison Rahul Pannala William Pavlicek Michael D Crowell Douglas O Faigel | 2015 | World Journal of Gastroenterology2015,21,6: | 2 |
| 2 | Mathematical analysis of corneal remodelling after intracorneal ring surgery in keratoconus显示文摘AIM:To represent mathematically the intersection between the ectatic corneal geometry and the plane of intracorneal ring implants(ICRS)in order to determine the corneal response to ICRS surgery in keratoconus(KC).Thereafter,to present the concept and early results of a newly derived topography-guided nomogram for ICRS surgery for the treatment of keratectasia. METHODS:The corneal rings plane intersection was modelled to a conic section. Ring effect was the result of:the ring size,position(steep vs flat),location(distance from the geometric centre of the cornea),and the discrepancy between the ring's curvature and the tunnel's curvature.Femtosecond laser was used to create the tunnels and the incision sites were chosen according to the nomogram in order to place the thickest ring in the steepest portion of the cornea regardless of the astigmatism axis of refraction.RESULTS:The conic section had a more prolate shape in the steep area of the cornea than in the flat area,depending on the corneal sagittal curvature. Equal ring size had more flattening effect in the steep area than in the flat area. Thick segment should be implanted under the steep portion of the cornea regardless of the cylinder axis of refraction. Single segment in the steep area was sufficient in early and moderate cases of KC. The new nomogram provided more topographic regularity with significant reduction of astigmatism and better improvement in uncorrected visual acuity(UCVA)and best-corrected visual acuity(BCVA)than the conventional nomogram. CONCLUSION:The newly derived nomogram can produce better results than the conventional nomogram. | 16 | Transanal minimally invasive surgery to rescue anastomosis following leak after low anterior resection: A case report显示文摘Restorative low anterior resection(LAR)for rectal cancer carries a significant risk of anastomotic leak:One of the most feared complications in colorectal surgery.Operative management may include takedown of the anastomosis and end colostomy which,in some cases,is permanent.Other contemporary operative measures include over the scope clips and Endosponge.Recently,there have been case reports and a Society of American Endoscopic and Gastrointestinal Surgeons video on the novel use of transanal minimally invasive surgery(TAMIS)in the management of anastomotic leak.We present a 59-year-old female who underwent LAR after declining radiotherapy for a bulky 9 cm rectal tumour 9-10 cm from the anal verge.Following clinical deterioration,computed tomography demonstrated an anastomotic leak communicating with a 5-cm pelvic collection containing gas.At laparoscopy,pus and faeculent material were washed from the pelvic cavity and drains were placed.Intra-operative endoscopy demonstrated a 7-8 mm dehiscence at the anastomosis.The defect(approximately 7 cm from the anal verge)was successfully closed using TAMIS and a running V-lock suture.The patient recovered well and was discharged home on post-operative day 20.In this case,a low colorectal anastomotic leak was successfully rescued with TAMIS.This novel technique may be useful in the armamentarium of colorectal surgeons experienced in TAMIS. | James Wei Tatt Toh Henry Wang Geoffrey Collins Chelsea Beinke Elissa Zhang Alistair Escott Toufic El-Khoury Nimalan Pathma-Nathan | 2021 | Laparoscopic, Endoscopic and Robotic Surgery2021,4,4: | 0 |
