| 7 | Declining responsiveness of childhood Plasmodium falciparum infections to artemisinin-based combination treatments ten years following deployment as first-line antimalarials in Nigeria显示文摘Background:The development and spread of artemisinin-resistant Plasmodium falciparum malaria in Greater Mekong Subregion has created impetus for continuing global monitoring of efficacy of artemisinin-based combination therapies(ACTs). This post analyses is aimed to evaluate changes in early treatment response markers 10 years after the adoption of ACTs as first-line treatments of uncomplicated falciparum malaria in Nigeria.Methods: At 14 sentinel sites in six geographical areas of Nigeria, we evaluated treatment responses in 1341 children under 5 years and in additional 360 children under 16 years with uncomplicated malaria enrolled in randomized trials of artemether-lumefantrine versus artesunate-amodiaquine at 5-year interval in 2009-2010 and 2014-2015 and at 2-year interval in 2009-2010 and 2012-2015, respectively after deployment in 2005.Results: Asexual parasite positivity 1 day after treatment initiation (APPD1) rose from 54 to 62% and 2 days after treatment initiation from 5 to 26% in 2009-2010 to 2014-2015(P=0.002 and P<0.0001, respectively).Parasite clearance time increased significantly from 1.6 days (95% confidence interval [CI]: 1.55-1.64) to 1.9 days (95% C,1.9-2.0) and geometric mean parasite reduction ratio 2 days after treatment initiation decreased significantly from 11 000 to 4700 within the same time period (P< 00001 for each). Enrolment parasitaemia > 75 000 μl^-1, haematocrit > 27% 1 day post-treatment initiatiortreatment with artemether-lumefantrine and enrolment in 2014-2015 independently predicted APPD1. In paralle , Kaplan-Meier estimated risk of recurrent infections by day 28 rose from 8 to 14% (P=0005) and from 9 to 15%(P=0.02) with artemether-lumefantrine and artesunate-amodiaquine, respectively. | 8 | Bioactive and nutritional characterization of modeled and optimized consumer-ready flakes from pseudocereal(Amaranthus viridis), high-protein soymeal and modified corn starch显示文摘Flake is consumed in many parts of the world.Flakes are majorly prepared from cereals.However,most flakes are deficient in protein and some other healthful substances.High-protein soymeal is rich in protein,mineral,amino acids,antioxidants,and other healthful substances.Formulating flakes with high-protein soymeal would improve the health status of consumers.This work investigated consumer-ready flake from amaranth,high-protein soymeal,and modified corn starch produced under the optimized condition and characterized with the aim to develop models that would give a healthful consumer-ready flake.Amaranthus viridis,corn,and soybean grains were sorted,wet-cleaned,and dried.Soybean grains were processed into high-protein soymeal,starch was extracted from corn grains while A.viridis grains were processed into flour.Formulated flour mixtures were developed into flakes using three-level factorial categoric factor design of response surface methodology.The flakes were analyzed using standard procedures.Optimal flour mixtures of high-protein soymeal(34.78 g/100 g),amaranth(56.52 g/100 g),and modified corn starch(8.70 g/100 g)were established.Results showed the optimized flakes contained per 100 g:29.05 g protein,6.00 g fat,4.10 g fibre,3.84 g ash,8.96 g moisture,249.74 mg calcium,272.35 mg magnesium,12.08 mg iron,618.42 mg phosphorus,6.41 mg niacin,4.85 mg pyridoxine,0.21 g tannin,1.85 mg phytate,2.96 mg alkaloids,908.24GAE total phenolics and 12.75mgRE flavonoids with good quality characteristics in amino acids.The study illustrated the feasibility of formulating quality consumer-ready flakes from amaranth,high-protein soymeal,and modified corn starch.The production process is scalable and could be employed for both domestic and industrial purposes. | Temitope Omolayo Fasuan Kenneth Chigozie Asadu Cynthia Chioma Anyiam Linda Obianuju Ojokoh Titilope Modupe Olagunju Judith Uchenna Chima Kingsley Osita Okpara | 2021 | Food Production, Processing and Nutrition2021,3,1: | 0 |
| 9 | Clinical outcomes in patients with native valve infective endocarditis and diabetes mellitus显示文摘BACKGROUND There is a lack of data on the clinical outcomes in patients with native valve infective endocarditis(NVIE)and diabetes mellitus(DM).AIM To investigate(1)trends in the prevalence of DM among patients with NVIE;and(2)the impact of DM on NVIE outcomes.METHODS We identified 76385 with NVIE from the 2004 to 2014 National Inpatient Sample,of which 22284(28%)had DM.We assessed trends in DM from 2004 to 2014 using the Cochrane Armitage test.We compared baseline comorbidities,microorganisms,and in-patients procedures between those with vs without DM.Propensity match analysis and multivariate logistic regression were used to investigate study outcomes in in-hospital mortality,stroke,acute heart failure,cardiogenic shock,septic shock,and atrioventricular block.RESULTS Crude rates of DM increased from in 22%in 2004 to 30%in 2014.There were significant differences in demographics,comorbidities and NVIE risk factors between the two groups.Staphylococcus aureus was the most common organism identified with higher rates in patients with DM(33.1%vs 35.6%;P<0.0001).After propensity matching,in-hospital mortality(11.1%vs 11.9%;P<0.0001),stroke(2.3%vs 3.0%;P<0.0001),acute heart failure(4.6%vs 6.5%;P=0.001),cardiogenic shock(1.5%vs 1.9%;P<0.0001),septic shock(7.2%vs 9.6%;P<0.0001),and atrioventricular block(1.5%vs 2.4%;P<0.0001),were significantly higher in patients with DM.Independent predictors of mortality in NVIE patients with DM include hemodialysis,congestive heart failure,atrial fibrillation,staphylococcus aureus,and older age.CONCLUSION There is an increasing prevalence of DM in NVIE and it is associated with poorer outcomes.Further studies are crucial to identify the clinical,and sociodemographic contributors to this trend and develop strategies to mitigate its attendant risk. | Temidayo Abe Harry Onoriode Eyituoyo Gabrielle De Allie Titilope Olanipekun Valery Sammah Effoe Kikelomo Olaosebikan Paul Mather | 2021 | World Journal of Cardiology2021,13,1: | 0 |
| 10 | Opening of liver mitochondrial permeability transition pore in streptozotocin-induced diabetic rats and its inhibition by methanol fraction of Ficus mucoso(Welw) root bark显示文摘Objective: Several pathologies arise from the inappropriate opening of the mitochondrial permeability transition(mPT) pore. In this regard, inhibition of mPT pore represents a cytoprotective approach to preserve mitochondrial function for treatment of diseases characterized by excessive tissue wastage such as diabetes mellitus. The aim of this study, therefore, was to study the effects of fractions of Ficus mucoso, a medicinal plant used in the traditional treatment of diabetes, on mPT pore in normal and streptozotocin(STZ)-induced diabetic rat liver.Methods: Different solvent fractions of the crude methanol extract of F. mucoso were obtained by vacuum liquid chromatography and were tested on the mPT pore. Of all the fractions tested, methanol fraction of F. mucoso(MFFM) was the most potent and was used for in vivo studies. Diabetes mellitus was induced by a single intraperitoneal injection of 60 mg/kg STZ, while treatment lasted for 14 d. In vivo, 30 male Wistar rats were divided into five groups: A, normo-glycemic control(distilled water);B, STZ(65 mg/kg;diabetic control);C, STZ + MFFM(25 mg/kg);D, STZ + MFFM(50 mg/kg);E, STZ + glibenclamide(5 mg/kg). The mPT, mitochondrial ATPase activity, lipid peroxidation and cytochrome c release were assessed spectrophotometrically while blood glucose levels were monitored using glucometer.Results: In vitro, the solvent fractions of F. mucoso, at all concentrations tested, had no effect on the mPT pore, in the absence of calcium, with no significant release of cytochrome c. Interestingly, calciumdependent pore opening was inhibited by all solvent fractions of F. mucoso, with the MFFM having the highest inhibitory effect of 83% at 3 mg/mL. Induction of opening of the mPT pore, significant(P < 0.001) enhancement of mitochondrial ATPase activity and elevated malondialdehyde(MDA) levels in STZ-induced diabetes were significantly(P < 0.001) reversed by MFFM and were comparable with the effects of glibenclamide, a standard antidiabetic drug. | 11 | Bioactive profile of borno brown Vigna unguiculata grains as influenced by pre-harvest synthetic chemicals显示文摘This study investigated the impact of pre-harvest synthetic chemical treatments on the post-harvest bioactive contents,pasting,colour,and thermal properties of borno brown V.unguiculata(L.)Walp grains.Seeds of borno brown V.unguiculata(L.)Walp was planted on an already ploughed,harrowed and ridged plot of land with a portion cultivated as organic and another non-organic.After 90 days,mature grains were harvested and analyzed using standard procedures.Results showed that a total of 44.33 g bioactive compounds/100g grain was present in the organic borno brown V.unguiculata grains whereas,20.56 g bioactive compounds/100g grain was present in the non-organic(control).The identified bioactive compounds in the organic grains included[heneicosane(4.50g/100g grain)];[12-Methyl-E,E-2,13-octadecadien-1-ol(4.09/100g grain)];[3-(2-Hydroxy-6-methylphenyl)-4(3H)-quinazolinone(7.40 g/100g grain)];[Methaqualone(9.08 g/100g grain)].The non-organic contained[heneicosane(4.07g/100g grain)];[3-(2-Hydroxy-6-methylphenyl)-4(3H)-quinazolinone(5.09 g/100g grain)];[Methaqualone(10.61 g/100g grain)].Flavonoids,alkaloids,total phenolic,2,2-diphenyl-1-picryl-hydrazl(DPPH)and ferric ion reducing antioxidant power(FRAP)contents of the organic grains were 19 mgRE/100g,3.40 mg/100g,1550 mgGAE/100g,2840μmolTEAC/100g,and 20.08 mmol Fe^(2+)/100g,respectively.The non-organic grains had flavonoids(11.31 mgRE/100g),alkaloids(1.86 mg/100g),total phenolic(1110 mgGAE/100g),DPPH(1779μmolTEAC/100g)and FRAP(12.24 mmol Fe^(2+)/100g).The peak viscosity of 111.52 RVU,final viscosity(141.25 RVU)and pasting temperature(82.01℃)were obtained for the organic grains while non-organic had peak viscosity of 93.10RVU,final viscosity(95.20 RVU)and pasting temperature(80.10℃).The colour parameters L^(*),a^(*),b^(*)and ΔE were 73.10,0.68,14.47 and 12.98 for the organic and 84.25,0.30,21.10,and 0.00,respectively for non-organic grains.The thermal property showed enthalpy(ΔH)and peak temperature(T_(p))of 3.12 J/g and 78.25℃ for organic,and 2.14 J/g and 76.28℃ for non-organic grains,respectively.Phenolic compounds profile showed that organic grains were richer in epicatechin,gallic acid,ferulic acid,chlorogenic acid,4-hydroxy benzoic acid,and catechin.Organic grains showed significant(p<0.05)superiority in bioactive compounds,phytochemicals,and antioxidants over non-organic grains.Good pasting properties of the organic grains showed that it could function as a food thickener.Organic borno brown V.unguiculata grains are rich in health-promoting compounds and could,therefore,be used as an ingredient in the development of functional foods. | Temitope Omolayo Fasuan Nneka Nkechi Uchegbu Titilope Modupe Olagunju Oyenike Olubunmi Falola | 2022 | Food Bioscience2022,45,1: | 0 |

| 13 | Nutritional assessment,glycemic indices and anti-diabetic potentials of dough meal generated from optimized blends of matured plantain,soya cake and wheat bran flours显示文摘Management of blood glucose levels associated with type 2 diabetes using nutrients derived from food without having a harmful effect on one’s health is gradually gaining research interest.Plantain dough meal blends were developed using optimal mixture design of response surface methodology(RSM).The optimum blends based on the protein and crude fibre contents were as follows:PSW1:mature plantain 70.0%+soya cake 25.0%+wheat bran 5.0%;PSW2:mature plantain 68.0%+soya cake 23.0%+wheat bran 9.00%;PSW3:mature plantain 69.0%+soya cake 24.0%+wheat bran 7.0%,while 100% mature plantain flour(PLNT)and cerolina were used as controls.The nutritional value,amino acid profile,pasting properties,α-amylase,α-glucosidase inhibition potential,in-vitro glycemic index and consumer acceptability were determined using standard methods.Results showed that the inclusion of soya cake and wheat bran significantly increased(P<0.05)protein 14.08 g/100 g and fibre 3.02 g/100 g as observed.Significantly higher(P<0.05)branched chain and aromatic amino acids were observed in the composite dough meal compared to PLNT.The peak viscosity(Pv),trough viscosity(Tv)and final viscosity(Fv)are lower in dough meal blends with increase in incorporation of soya cake and wheat bran.PSW3 showed the highest essential amino acid index,predicted biological value,α-glucosidase,α-amylase inhibition and lowest estimated glycaemic index at(P≤0.05).Hence,the study established that PSW3 showed the best potentials to modulate blood sugar level. | Adedamola Iyioluwa Akinyede Ebenezer Oladipupo Ayibiowu Titilope Fakologbon Olugbenga Olufemi Awolu Tayo Nathaniel Fagbemi | 2023 | Journal of Future Foods2023,3,4: | 0 |
| 14 | Intensive care unit hospitalizations and outcomes in patients with severe COVID-19 during summer and fall surges in Georgia显示文摘BACKGROUND There is limited data on the difference in the clinical characteristics and outcomes of patients with severe coronavirus disease 2019(COVID-19)infection in the summer compared to the fall surge.AIM To compare the sociodemographic,clinical characteristics,and outcomes among mechanically ventilated patients with severe COVID-19 infection admitted to the intensive care unit(ICU)during the summer and fall surges in the year 2020.METHODS We included patients admitted to the ICU and treated with invasive mechanical ventilation for COVID-19 associated respiratory failure between April 1 and December 31,2020.Patients were categorized into summer surge for ICU admissions between June 15,2020,and August 15,2020,and fall surge between October 15,2020,and December 31,2020.We compared patients'characteristics and outcomes using descriptive and inferential statistics.RESULTS A total of 220 patients were admitted to the Grady Memorial Hospital ICU and mechanically ventilated for COVID-19 associated hypoxemic respiratory failure during the period considered(125 during the summer surge and 95 during the fall surge).More women were admitted in the fall compared to summer(41.1%vs 36.8%,difference,4.3%;95%CI:1.2,7.5).Patients admitted in the fall had fewer comorbidities(chronic obstructive pulmonary disease,stroke,diabetes mellitus,obstructive sleep apnea and body mass index≥35 kg/m2).Overall,patients in the fall had a lower ICU mortality rate(27.4%vs 38.4%,difference,-11.0;95%CI:-6.4,-18.2),shorter length of stay on the mechanical ventilator(7 d vs 11 d,difference,4 d;95% CI:2.1,6.6)and shorter ICU length of stay(9 d vs 14 d,difference,5 d;95% CI:2.7,9.4).CONCLUSION Patients admitted with severe COVID-19 infection requiring mechanical ventilation had better outcomes in the fall than summer.This difference observed is likely attributable to a better understanding of the condition and advances in treatment strategies. | Titilope Olanipekun Temidayo Ayotomiwa Abe Valery Sammah Effoe Joffi E Musonge-Effoe Agusiegbe Chuks Esther Kwara Alexandra Caldwell Samed Obeng Nicolas Bakinde Gloria Westney Richard Snyder | 2021 | World Journal of Critical Care Medicine2021,10,6: | 0 |
