2Efficacy of an orally active small-molecule inhibitor of RANKL in bone metastasis显示文摘Bone is one of the preferred sites for the metastasis of malignant tumours,such as breast cancer,lung cancer and malignant melanoma.Tumour cells colonizing bone have the capacity to induce the expression of receptor activator of nuclear factor-κB ligand(RANKL),which promotes osteoclast differentiation and activation.Tumour-induced osteoclastic bone resorption leads to a vicious cycle between tumours and bone cells that fuels osteolytic tumour growth,causing bone pain and hypercalcaemia.Furthermore,RANKL contributes to bone metastasis by acting as a chemoattractant to bone for tumour cells that express its receptor,RANK.Thus inhibition of the RANKL–RANK pathway is a promising treatment for bone metastasis,and a human monoclonal anti-RANKL antibody,denosumab,has been used in the clinic.However,orally available drugs targeting RANKL must be developed to increase the therapeutic benefits to patients.Here we report the efficacy of the small-molecule RANKL inhibitor AS2676293 in treating bone metastasis using mouse models.Oral administration of AS2676293 markedly inhibited bone metastasis of human breast cancer cells MDA-MB-231-5a-D-Luc2 as well as tumour-induced osteolysis.AS2676293 suppressed RANKLmediated tumour migration in the transwell assay and inhibited bone metastasis of the murine cell line B16F10,which is known not to trigger osteoclast activation.Based on the results from this study,RANKL inhibition with a small-molecule compound constitutes a promising therapeutic strategy for treating bone metastasis by inhibiting both osteoclastic bone resorption and tumour migration to bone.Yuta Nakai Kazuo Okamoto Asuka Terashima Shogo Ehata Jun Nishida Takeshi Imamura Takashi Ono Hiroshi Takayanagi 2019Bone Research2019,7,1:8
3Familial pancreatic cancer: Concept, management and issues显示文摘Familial pancreatic cancer (FPC) is broadly defined as two first-degree-relatives with pancreatic cancer (PC) and accounts for 4%-10% of PC. Several genetic syndromes, including Peutz-Jeghers syndrome, hereditary pancreatitis, hereditary breast-ovarian cancer syndrome(HBOC), Lynch syndrome, and familial adenomatous polyposis (FAP), also have increased risks of PC, but the narrowest definition of FPC excludes these known syndromes. When compared with other familial tumors, proven genetic alterations are limited to a small proportion (<20%) and the familial aggregation is usually modest. However, an ethnic deviation (Ashkenazi Jewish>Caucasian) and a younger onset are common also in FPC. In European countries, 'anticipation' is reported in FPC families, as with other hereditary syndromes; a trend toward younger age and worse prognosis is recognized in the late years. The resected pancreases of FPC kindred often show multiple pancreatic intraepithelial neoplasia (Pan IN) foci, with various K-ras mutations, similar to colorectal polyposis seen in the FAP patients. As with HBOC patients, a patient who is a BRCA mutation carrier with unresectable pancreatic cancer (accounting for 0%-19% of FPC patients) demonstrated better outcome following platinum and Poly (ADP-ribose) polymerase inhibitor treatment. Western countries have established FPC registries since the 1990 s and several surveillance projects for highrisk individuals are now ongoing to detect early PCs. Improvement in lifestyle habits, including non-smoking, is recommended for individuals at risk. 4Hierarchical structures hydrogel evaporator and superhydrophilic water collect device for efficient solar steam evaporation显示文摘Efficient light absorption and trapping are of vital importance for the solar water evaporation by hydrogel-based photothermal conversion materials.Conventional strategies are focused on the development of the composition and structure of the hydrogers internal network.In our point of view,the importance of the surface structure of hydrogel has usually been underestimated or ignored.Here inspired by the excellent absorbance and water transportation ability of biological surface structure,the hierarchical structured hydrogel evaporators(HSEs)increased the light absorption,trapping,water transportation and water-air interface,which is the beneficial photothermal conversion and water evaporation.The HSEs showed a rapid evaporation rate of 1.77 kg·m^(-2)·h^(-1)at about 92%energy efficiency under one sun(1 kW·m^(-2)).Furthermore,the superhydrophilic window device was used in this work to collect the condensed water,which avoids the light-blocking caused by the water mist formed by the small droplets and the problem of the droplets stick on the device dropping back to the bulk water.Integrated with the excellent photothermal conversion hydrogel and superhydrophilic window equipment,this work provides efficient evaporation and desalination of hydrogel-based solar evaporators in practical large-scale applications.Wenwei Lei Sovann Khan Lie Chen Norihiro Suzuki Chiaki Terashima Kesong Liu Akira Fujishima Mingjie Liu 2021Nano Research2021,14,4:4

6Factors affecting the quality of life of patients after gastrectomy as assessed using the newly developed PGSAS-45 scale: A nationwide multi-institutional study显示文摘AIM To identify certain clinical factors other than the type of gastrectomy which affect the postoperative quality of life(QOL) of patients after gastrectomy.METHODS The postgastrectomy syndrome assessment scale(PGSAS)-45 was designed to assess the severity of symptoms, the living status and the QOL of gastrectomized patients. It consists of 45 items, of which 22 are original items while 23 were retrieved from the SF-8 and Gastrointestinal Symptoms Rating Scale questionnaires with permission. A nationwide surveillance study to validate PGSAS was conducted and 2368 gastric cancer patients who underwent various types of gastrectomy at 52 medical institutions were enrolled. Of these, 1777 patients who underwent total gastrectomy(TG) reconstructed with Roux-Y(n = 393), distal gastrectomy(DG) reconstructed with Billroth-I(n = 909), or DG reconstructed with Roux-Y(n = 475) were evaluated in the current study. The influence of the type of gastrectomy and other clinical factors such as age, sex, duration after surgery, the symptom severity, the degree of weight loss, dietary intake, and the ability for working on the postoperative QOL(i.e., dissatisfaction for daily life subscale, physical component summary and mental component summary of the SF-8) were examined by multiple regression analysis(MRA). In addition, importance of various symptoms such as esophageal reflux, abdominal pain, meal-related distress, indigestion, diarrhea, constipation and dumping on the postoperative living status and QOL were also appraised by MRA.RESULTS The postoperative QOL were significantly deteriorated in patients who underwent TG compared to those after DG. However, the extent of gastrectomy was not an influential factor on patients' QOL when adjusted by the MRA. Among various clinical factors, the symptom severity, ability for working, and necessity for additional meals were the most influential factorsto the postoperative QOL. As for the individual symptoms, meal-related distress, dumping, abdominal pain, and esophageal reflux significantly affected the postoperative QOL in that order, while the influence of indigestion, diarrhea and constipation was insignificant. 8Quality of life after total vs distal gastrectomy with Rouxen-Y reconstruction: Use of the Postgastrectomy Syndrome Assessment Scale-45显示文摘AIM To investigate the detrimental impact of loss of reservoir capacity by comparing total gastrectomy(TGRY) and distal gastrectomy with the same Rouxen-Y(DGRY) reconstruction. The study was conducted using an integrated questionnaire, the Postgastrectomy Syndrome Assessment Scale(PGSAS)-45, recently developed by the Japan Postgastrectomy Syndrome Working Party.METHODS The PGSAS-45 comprises 8 items from the Short Form-8, 15 from the Gastrointestinal Symptom Rating Scale, and 22 newly selected items. Uni-and multivariate analysis was performed on 868 questionnaires completed by patients who underwent either TGRY(n = 393) or DGRY(n = 475) for stage I gastric cancer(52 institutions). Multivariate analysis weighed of six explanatory variables, including the type of gastrectomy(TGRY/DGRY), interval after surgery, age, gender, surgical approach(laparoscopic/open), and whether the celiac branch of the vagus nerve was preserved/divided on the quality of life(QOL).RESULTS The patients who underwent TGRY experienced the poorer QOL compared to DGRY in the 15 of 19 main outcome measures of PGSAS-45. Moreover, multiple regression analysis indicated that the type of gastrectomy, TGRY, most strongly and broadly impaired the postoperative QOL among six explanatory variables. 10Pathological response measured using virtual microscopic slides for gastric cancer patients who underwent neoadjuvant chemotherapy显示文摘BACKGROUND Although pathological response is a common endpoint used to assess the efficacy of neoadjuvant chemotherapy (NAC) for gastric cancer, the problem of a low rate of concordance from evaluations among pathologists remains unresolved. Moreover, there is no globally accepted consensus regarding the optimal evaluation. A previous study based on a clinical trial suggested that pathological response measured using digitally captured virtual microscopic slides predicted patients’ survival well. However, the pathological concordance rate of this approach and its usefulness in clinical practice were unknown. AIM To investigate the prognostic utility of pathological response measured using digital microscopic slides in clinical practice. METHODS We retrospectively evaluated pathological specimens of gastric cancer patients who underwent NAC followed by surgery and achieved R0 resection between March 2009 and May 2015. Residual tumor area and primary tumor beds were measured in one captured image slide, which contained the largest diameter of the resected specimens. We classified patients with < 10% residual tumor relative to the primary tumorous area as responders, and the rest as non-responders;we then compared overall survival (OS) and relapse-free survival (RFS) between these two groups. Next, we compared the prognostic utility of this method using conventional Japanese criteria. RESULTS Fifty-four patients were evaluated. The concordance rate between two evaluators was 96.2%. Median RFS of 25 responders and 29 non-responders was not reached (NR) vs 18.2 mo [hazard ratio (HR)= 0.35, P = 0.023], and median OS was NR vs 40.7 mo (HR = 0.3, P = 0.016), respectively. This prognostic value was statistically significant even after adjustment for age, eastern cooperative oncology group performance status, macroscopic type, reason for NAC, and T- and Nclassification (HR = 0.23, P = 0.018). This result was also observed even in subgroup analyses for different macroscopic types (Borrmann type 4/non-type 4) and histological types (differentiated/undifferentiated). Moreover, the adjusted HR for OS between responders and non-responders was lower in this method than that in the conventional histological evaluation of Japanese Classification of Gastric Carcinoma criteria (0.23 vs 0.39, respectively). 11VISUALIZATION OF TURBULENT REACTIVE JET BY USING DIRECT NUMERICAL SIMULATION显示文摘Direct numerical simulation(DNS)of turbulent planar jet with a second-order chemical reaction(A+B→R)is performed to investigate the processes of mixing and chemical reactions in spatially developing turbulent free shear flows.Reactant A is premixed into the jet flow,and reactant B is premixed into the ambient flow.DNS is performed at three different Damk¨ohler numbers(Da=0.1,1,and 10).Damk¨ohler number is a ratio of a time scale of a flow to that of chemical reactions,and in this study,the large Da means a fast chemical reaction,and the small Da means a slow chemical reaction.The visualization of velocity field shows that the jet flow is developed by entraining the ambient fluid.The visualization of concentration of reactant A shows that concentration of reactant A for Da=1 and 10 becomes very small in the downstream region because the chemical reaction consumes the reactants and reactant A is diffused with the jet development.By comparison of the profiles of chemical reaction rate and concentration of product R,it is found that product R for Da=10 is produced by the chemical reaction at the interface between the jet and the ambient fluids and is diffused into the jet flow,whereas product R for Da=0.1 is produced in the jet flow after reactants A and B are well mixed.TOMOAKI WATANABE YASUHIKO SAKAI KOUJI NAGATA OSAMU TERASHIMA HIROKI SUZUKI TOSHIYUKI HAYASE YASUMASA ITO 2013International Journal of Modeling, Simulation, and Scientific Computing2013,4,S01:2
12Cellular irinotecan resistance in colorectal cancer and overcoming irinotecan refractoriness through various combination trials including DNA methyltransferase inhibitors: a review显示文摘Treatment with pharmacological drugs for colorectal cancer(CRC)remains unsatisfactory.A major cause of failure in pharmacotherapy is the resistance of colon cancer cells to the drugs,creating an urgent issue.In this review,we summarize previous studies on the resistance of CRC cells to irinotecan and discuss possible reasons for refractoriness.Our review presents the following five major causes of irinotecan resistance in human CRC:(1)cellular irinotecan resistance is induced mainly through the increased expression of the drug efflux transporter,ABCG2;(2)cellular irinotecan resistance is also induced in association with a nuclear receptor,pregnane/steroid X receptor(PXR/SXR),which is enriched in the CYP3A4 gene enhancer region in CRC cells by exposing the cells to SN-38;(3)irinotecan-resistant cells possess either reduced DNA topoisomerase I(Top1)expression at both the mRNA and protein levels or Top1 missense mutations;(4)alterations in the tumor microenvironment lead to drug resistance through intercellular vesicle-mediated transmission of miRNAs;and(5)CRC stem cells are the most difficult targets to successfully treat CRC.In the clinical setting,CRC gradually develops resistance to initially effective irinotecan-based therapy.To solve this problem,several clinical trials,such as irinotecan plus cetuximab vs.cetuximab monotherapy,have been conducted.Another clinical trial on irinotecan plus guadecitabine,a DNA-methyltransferase inhibitor,has also been conducted.Shogo Ozawa Toshitaka Miura Jun Terashima Wataru Habano 2021Cancer Drug Resistance2021,4,4:2
