1Interaction between microbiota and immunity in health and disease显示文摘The interplay between the commensal microbiota and the mammalian immune system development and function includes multifold interactions in homeostasis and disease.The microbiome plays critical roles in the training and development of major components of the host's innate and adaptive immune system,while the immune system orchestrates the maintenance of key features of host-microbe symbiosis.In a genetically susceptible host,imbalances in microbiota-immunity interactions under defined environmental contexts are believed to contribute to the pathogenesis of a multitude of immune-mediated disorders.Here,we review features of microbiome-immunity crosstalk and their roles in health and disease,while providing examples of molecular mechanisms orchestrating these interactions in the intestine and extra-intestinal organs.We highlight aspects of the current knowledge,challenges and limitations in achieving causal understanding of host immune-microbiome interactions,as well as their impact on immune-mediated diseases,and discuss how these insights may translate towards future development of microbiometargeted therapeutic interventions.Danping Zheng Timur Liwinski Eran Elinav 2020Cell Research2020,30,6:58

3支架自显影定位法体外开窗技术在主动脉弓部腔内修复术中的应用:附113例国际多中心病例回顾性分析显示文摘背景与目的:胸主动脉腔内修复术(TEVAR)已成为治疗降主动脉疾病的一线治疗方式。在治疗累及主动脉弓部疾病时,体外开窗(PMF)是目前常用于重建弓部分支动脉的技术之一。它是指在选择合适的主动脉覆膜支架后,根据弓上分支动脉的位置,在覆膜支架相应位置上除去覆膜,建立人造的“窗孔”,在将有窗孔的大动脉支架准确对位释放后再释放分支支架。本文介绍一种利用主动脉支架本身所携带的不透X线标志快速、准确地将窗孔与分支动脉开口对准的开窗方法—支架自显影定位PMF技术(S-F技术),并通过总结利用该方法进行TEVAR的一组患者临床资料和手术以及随访数据,探讨利用S-F技术在主动脉弓部疾病腔内治疗中的技术特点和其应用价值。方法:回顾性分析2015年12月—2020年6月利用S-F技术行TEVAR治疗的中国湖南中南大学湘雅二医院血管外科和其他7个国家的国家级血管外科中心共113例主动脉弓部疾病患者的基本数据、术中技术数据和术后随访数据。结果:患者均根据S-F技术的操作标准流程进行的TEVAR,即时手术成功率为100%,共植入胸主动脉覆膜支架118枚,其中植入桥接支架35例。手术时间(62.9±17.4)min,X线操作时间(23.1±9.8)min,造影剂用量(81.2±22.5)mL。术中开窗对位准确率为98.2%。补救性植入烟囱支架1例(0.9%),矫正后对位回准1例(0.9%)。开窗同时保留弓上三分支动脉2例(1.8%),开窗同时保留左颈总动脉(LCCA)和左锁骨下动脉(LSA)6例(5.3%),保留LSA 110例(97.3%)。术后1个月内无死亡病例,无严重并发症发生。术后1周复查发现Ⅰ型内漏7例(6.2%)。随访时间中位时间24.5个月,死亡2例(1.8%);术后分支动脉闭塞3例(2.7%),分别发生在术后3个月(2例)和术后6个月(1例)。结论:S-F技术为治疗累及主动脉弓部疾病重建弓上分支动脉提供了一种可供选择的方法,是全腔内重建主动脉弓部方法技术上的一项进步。舒畅 李鑫 李全明 何昊 黎明 王暾 杨晨紫 王沫 周海洋 JoséFernando Teixeira Suko Adiarto Tao Fan Bogdan Cherpark Imaev Timur Makhkamov Najmiddin Pavel Charnahlaz Giorgi Kheladze 2020中国普通外科杂志2020,29,12:10

5One-step electrochemical reduction of stibnite concentrate in molten borax显示文摘In this study,antimony production from a stibnite concentrate(Sb2S3)was performed in one step using a molten salt electrolysis method and borax as an electrolyte.Electrochemical reduction of the stibnite concentrate was performed at 800℃under galvanostatic conditions and explained in detail by the reactions and intermediate compounds formed in the borax.The effects of current density(100 800 mA cm^-2)and electrolysis time(10 40 min)on cathodic current efficiency and antimony yields were systematically investigated.During the highest current efficiency,which was obtained at 600 mA cm^-2,direct metal production was possible with 62%cathodic current efficiency and approximately 6 kWh/kg energy consumption.At the end of the 40-min electrolysis duration at 600 mA cm^-2 current density,antimony reduction reached 30.7 g and 99%of the antimony fed to the cell was obtained as metal.Levent Kartal Mehmet Baris Daryal Güldem Kartal Sireli Servet Timur 2019International Journal of Minerals,Metallurgy and Materials2019,26,10:6

7Role of the microbiome in non-gastrointestinal cancers显示文摘'The forgotten organ',the human microbiome,comprises a community of microorganisms that colonizes various sites of the human body.Through coevolution of bacteria,archaea and fungi with the human host over thousands of years,a complex host-microbiome relationship emerged in which many functions,including metabolism and immune responses,became codependent.This coupling becomes evident when disruption in the microbiome composition,termed dysbiosis,is mirrored by the development of pathologies in the host.Among the most serious consequences of dysbiosis,is the development of cancer.As many as 20% of total cancers worldwide are caused by a microbial agent.To date,a vast majority of microbiomecancer studies focus solely on the microbiome of the large intestine and the development of gastrointestinal cancers.Here,we will review the available evidence implicating microbiome involvement in the development and progression of non-gastrointestinal cancers,while distinguishing between viral and bacterial drivers of cancer,as well as 'local' and 'systemic','cancer-stimulating' and 'cancer-suppressing' effects of the microbiome.Developing a system-wide approach to cancer-microbiome studies will be crucial in understanding how microbiome influences carcinogenesis,and may enable to employ microbiome-targeting approaches as part of cancer treatment.Meirav Pevsner-Fischer Timur Tuganbaev Mariska Meijer Sheng-Hong Zhang Zhi-Rong Zeng Min-Hu Chen Eran Elinav 2016World Journal of Clinical Oncology2016,7,2:5
8Direct electrochemical reduction of copper sulfide in molten borax显示文摘In this study,for the first time,direct copper production from copper sulfide was carried out via direct electrochemical reduction method using inexpensive and stable molten borax electrolyte.The effects of current density(100–800 mA/cm^2)and electrolysis time(15–90 min)on both the cathodic current efficiency and copper yield were systematically investigated in consideration of possible electrochemical/chemical reactions at 1200℃.The copper production yield reached 98.09%after 90 min of electrolysis at a current density of 600 mA/cm^2.Direct metal production was shown to be possible with 6 kWh/kg energy consumption at a 600 mA/cm2 current density,at which the highest current efficiency(41%)was obtained.The suggested method can also be applied to metal/alloy production from single-and mixed-metal sulfides coming from primary production and precipitated sulfides,which are produced in the mining and metallurgical industries during treatment of process solutions or wastewaters.Levent Kartal Servet Timur 2019International Journal of Minerals,Metallurgy and Materials2019,26,8:4

10Intestinal injury can be reduced by intra-arterial postischemic perfusion with hypertonic saline显示文摘AIM:To investigate the effect of local intestinal perfusion with hypertonic saline(HTS) on intestinal ischemia-reperfusion injury(IRI) in bothex vivo andin vivo rat models.METHODS:All experiments were performed on male Wistar rats anesthetized with pentobarbital sodium given intraperitoneally at a dose of 60 mg/kg.Ex vivo vascularly perfused rat intestine was subjected to 60-min ischemia and either 30-min reperfusion with isotonic buffer(controls),or 5 min with HTS of 365 or 415 mOsm/L osmolarity(HTS 365mOsm or HTS 415mOsm,respectively) followed by 25-min reperfusion with isotonic buffer.The vascular intestinal perfusate flow(IPF) rate was determined by collection of the effluent from the portal vein in a calibrated tube.Spontaneous intestinal contraction rate was monitored throughout.Irreversible intestinal injury or area of necrosis(AN) was evaluated histochemically using 2.3.5-triphenyltetrazolium chloride staining.In vivo,30-min ischemia was followed by either 30-min blood perfusion or 5-min reperfusion with HTS 365mOsm through the superior mesenteric artery(SMA) followed by 25-min blood perfusion.Arterial blood pressure(BP) was measured in the common carotid artery using a miniature pressure transducer.Histological injury was evaluated in both preparations using the Chui score.RESULTS:Ex vivo,intestinal IRI resulted in a reduction in the IPF rate during reperfusion(P < 0.05 vs sham).The postischemic recovery of the IPF rate did not differ between the controls and the HTS 365mOsm group.In the HTS 415mOsm group,postischemic IPF rates were lower than in the controls and the HTS 365mOsm group(P < 0.05).The intestinal contraction rate was similar at baseline in all groups.An increase in this parameter was observed during the first 10 min of reperfusion in the control group as compared to the sham-treated group,but no such increase was seen in the HTS 365mOsm group.In controls,AN averaged 14.8% ± 5.07% of the total tissue volume.Administration of HTS 365mOsm for 5 min after 60-min ischemia resulted in decrease in AN(5.1% ± 1.20% vs controls,P < 0.01).However,perfusion of the intestine with the HTS of greater osmolarity(HTS 415mOsm) failed to protect the intestine from irreversible injury.The Chiu score was lower in the HTS 365mOsm group in comparison with controls(2.4 ± 0.54 vs 3.2 ± 0.44,P = 0.042),while intestinal perfusion with HTS 415mOsm failed to improve the Chiu score.Intestinal reperfusion with HTS 365mOsm in the in vivo series secured rapid recovery of BP after its transient fall,whereas in the controls no recovery was seen.The Chiu score was lower in the HTS 365mOsm group vs controls(3.1 ± 0.26 and 3.8 ± 0.22,P = 0.0079 respectively,),although the magnitude of the effect was lower than in the ex vivo series.CONCLUSION:Brief intestinal postischemic perfusion with HTS 365mOsm through the SMA followed by blood flow restoration is a protective procedure that could be used for the prevention of intestinal IRI.Oleg Kornyushin Michael Galagudza Anna Kotslova Gelfia Nutfullina Nina Shved Alexey Nevorotin Valeriy Sedov Timur Vlasov 2013World Journal of Gastroenterology2013,19,2:4
11Gastrointestinal microbiome and Helicobacter pylori:Eradicate,leave it as it is,or take a personalized benefit-risk approach?显示文摘Helicobacter pylori(H.pylori)is generally regarded as a human pathogen and a class 1 carcinogen,etiologically related to gastric and duodenal ulcers,gastric cancer,and mucosa-associated lymphoid tissue lymphoma.However,H.pylori can also be regarded as a commensal symbiont.Unlike other pathogenic/opportunistic bacteria,H.pylori colonization in infancy is facilitated by T helper type 2 immunity and leads to the development of immune tolerance.Fucosylated gastric mucin glycans,which are an important part of the innate and adaptive immune system,mediate the adhesion of H.pylori to the surface of the gastric epithelium,contributing to successful colonization.H.pylori may have beneficial effects on the host by regulating gastrointestinal(GI)microbiota and protecting against some allergic and autoimmune disorders and inflammatory bowel disease.The potential protective role against inflammatory bowel disease may be related to both modulation of the gut microbiota and the immunomodulatory properties of H.pylori.The inverse association between H.pylori and some potentially proinflammatory and/or procarcinogenic bacteria may suggest it regulates the GI microbiota.Eradication of H.pylori can cause various adverse effects and alter the GI microbiota,leading to short-term or long-term dysbiosis.Overall,studies have shown that gastric Actinobacteria decrease after H.pylori eradication,Proteobacteria increase during short-term follow-up and then return to baseline levels,and Enterobacteriaceae and Enterococcus increase in the short-term and interim follow-up.Various gastric mucosal bacteria(Actinomyces,Granulicatella,Parvimonas,Peptostreptococcus,Prevotella,Rothia,Streptococcus,Rhodococcus,and Lactobacillus)may contribute to precancerous gastric lesions and cancer itself after H.pylori eradication.H.pylori eradication can also lead to dysbiosis of the gut microbiota,with increased Proteobacteria and decreased Bacteroidetes and Actinobacteria.The increase in gut Proteobacteria may contribute to adverse effects during and after eradication.The decrease in Actinobacteria,which are pivotal in the maintenance of gut homeostasis,can persist for>6 mo after H.pylori eradication.Furthermore,H.pylori eradication can alter the metabolism of gastric and intestinal bacteria.Given the available data,eradication cannot be an unconditional recommendation in every case of H.pylori infection,and the decision to eradicate H.pylori should be based on an assessment of the benefit-risk ratio for the individual patient.Thus,the current guidelines based on the unconditional'test-and-treat'strategy should be revised.The most cautious and careful approach should be taken in elderly patients with multiple eradication failures since repeated eradication can cause antibiotic-associated diarrhea,including severe Clostridioides difficile-associated diarrhea and colitis and antibiotic-associated hemorrhagic colitis due to Klebsiella oxytoca.Furthermore,since eradication therapy with antibiotics and proton pump inhibitors can lead to serious adverse effects and/or dysbiosis of the GI microbiota,supplementation of probiotics,prebiotics,and microbial metabolites(e.g.,butyrate+inulin)should be considered to decrease the negative effects of eradication.Stanislav Sitkin Leonid Lazebnik Elena Avalueva Svetlana Kononova Timur Vakhitov 2022World Journal of Gastroenterology2022,28,7:3

13Intraperitoneal dedifferentiated liposarcoma:A case report显示文摘Dedifferentiated liposarcoma is a variant of liposarcoma with a more aggressive course. Mutations of the p53 gene have been found in different types of soft tissue sarcoma. It is generally accepted that p53 mutations in human malignant tumors are often related to a poor prognosis. In our case, analysis of p53 gene mutation in tumor samples was performed. p53 gene mutation was observed in dedifferentiated tumor tissue samples but not in well-differentiated tumor tissue samples. It has been reported that p53 gene mutation occurs most commonly in the retroperitoneum and rarely in other anatomic locations. combinations are proposed where necessary to improve our traditional species concepts and provide updates on their classification.In addition to extensive morphological descriptions and appropriate asexual and sexual connections,DNA sequence data are also analysed from concatenated datasets(rDNA,TEF-a,RBP2 and b-Tubulin)to infer phylogenetic relationships and substantiate systematic position of taxa within appropriate ranks.Wherever new species or combinations are being proposed,we apply an integrative approach(morphological and molecular data as well as ecological features wherever applicable).Notes on 125 fungal taxa are compiled in this paper,including eight new genera,101 new species,two new combinations,one neotype,four reference specimens,new host or distribution records 18Biepicondylar fracture dislocation of the elbow joint concomitant with ulnar nerve injury显示文摘In this article, we present a case of humeral biepicondylar fracture dislocation concomitant with ulnar nerve injury in a seventeen year-old male patient. Physical examination of our patient in the emergency room revealed a painful, edematous and deformed-looking left elbow joint. Hypoesthesia of the little finger was also diagnosed on the left hand. Radiological assessment ended up with a posterior fracture dislocation of the elbow joint accompanied by intra-articular loose bodies. Open reduction-Internal fixation of the fracture dislocation and ulnar nerve exploration were performed under general anesthesia at the same session as surgical treatment of our patient. Physical therapy and rehabilitation protocol was implemented at the end of two weeks post-operatively. Union of the fracture lines, as well as the olecranon osteotomy site, was achieved at the end of four months post-operatively. Ulnar nerve function was fully restored without any sensory or motor loss.
