1贵州省2003~2004年麻疹疫苗强化免疫效果评价显示文摘目的总结贵州省实施麻疹疫苗(MV)强化免疫的效果。方法综合分析强化免疫期间的现场调查、项目年度评估报告和强化免疫后1年的麻疹监测系统和法定传染病报告系统的疫情资料。结果本次强化免疫共计接种目标儿童8611528人,报告接种率和调查接种率均〉95%;共报告预防接种异常反应354例,发生率4.11/10万;接种后经过1个最长潜伏期,麻疹发病数明显减少,每周报告疑似麻疹2~34例,且近1/3为风疹;目前78.3%的麻疹病例发生在8月龄~14岁,病例71.1%无免疫史或免疫史不详。结论高质量MV强化免疫能够迅速降低麻疹发病,实施该项工作的关键环节包括广泛的社会动员、细致的培训和清晰的物流是前提保障;目标人群的确定必须结合当地流行病学监测的资料;尽可能在较大范围内开展,以形成有效的人群免疫屏障阻断麻疹病毒传播;接种时间选择在麻疹低发季节,且尽可能在15d内完成;保证〉95%接种率是活动的前提目标;必须针对可能发生的预防接种后异常反应做好充分准备;如果该地区常规免疫服务质量在1~2年内不能明显加强,需要同时考虑4~5年后的后续免疫。左树岩 梁晓峰 殷大鹏 尤雪丹 郭飚 童亦兵 朱青 张大勇 杜雯 Lisa lee Yoshihiro Takashima 于竞进 2006中国计划免疫2006,12,1:67

3Elevation of the glycated albumin to glycated hemoglobin ratio during the progression of hepatitis C virus related liver fibrosis显示文摘AIM: To analyze the relationship between the glycated albumin (GA) to glycated hemoglobin (HbA1c) ratio and the histological grading of liver fibrosis.METHODS: The study retrospectively included consecutive hepatitis C virus positive chronic liver disease patients (n = 142) who had undergone percutaneous liver biopsy between January 2008 and March 2010 at our institution. The ratios of GA/HbA1c were calculated in all patients to investigate the relationship with the degree of the liver fibrosis. The values of the aspartate aminotransferase-to-platelet ratio index (APRI), an excellent marker for the evaluation of liver fibrosis, were also calculated. In addition, we combined the ratio of GA/HbA1c and the APRI in order to improve our ability to detect the presence of significant liver fibrosis. RESULTS: Sixty-one (43%) patients had either no fibrosis or minimal fibrosis (METAVIR score: F0-F1), while 25 (17%) had intermediate fibrosis (F2). Fifty-six (39%) patients had severe fibrosis (F3-F4) and 27 of them had cirrhosis (F4). The mean values of the GA/HbA1c increased with the progression of the fibrosis (F0-1: 2.83 ± 0.24, F2: 2.85 ± 0.24, F3: 2.92 ± 0.35, F4: 3.14 ± 0.54). There was a significant dif- ference between the F0-F1 vs F4, F2 vs F4, and F3 vs F4 groups (P < 0.01, P < 0.01, P < 0.01 and P < 0.05, respectively). The GA/HbA1c ratio was significantly higher in the patients with cirrhosis (F4) than in those without cirrhosis (F0-F3) (3.14 ± 0.54 vs 2.85 ± 0.28, P < 0.0001). The GA/HbA1c ratio was also significantly higher in the patients with severe fibrosis (F3-F4) than in those without severe liver fibrosis (F0-F2) (3.03 ± 0.41 vs 2.84 ± 0.24, P < 0.001). Furthermore, the GA/ HbA1c ratio was also significantly higher in the patients with significant fibrosis (F2-F4) than in those without significant liver fibrosis (F0-F1) (2.98 ± 0.41 vs 2.83 ± 0.24, P < 0.001). The diagnostic performance of the increased GA/HbA1c ratio (> 3.0) was as follows: its sensitivity and specificity for the detection of liver cirrhosis (F4) were 59.3% and 70.4%, respectively and its sensitivity and specificity for the detection of severe liver fibrosis (F3-F4) were 50.0% and 74.4%,respectively. With regard to the detection of significant fibrosis (F2-F4), its sensitivity was 44.4% and its specificity was 77.0%. Although even the excellent marker APRI shows low sensitivity (25.9%) for distinguishing patients with or without significant fibrosis, the combination of the APRI and GA/HbA1c ratio increased the sensitivity up to 42.0%, with only a modest decrease in the specificity (from 90.2% to 83.6%). 5Near-infrared photoimmunotherapy of pancreatic cancer using an indocyanine green-labeled anti-tissue factor antibody显示文摘AIM To investigate near-infrared photoimmunotherapeutic effect mediated by an anti-tissue factor(TF) antibody conjugated to indocyanine green(ICG) in a pancreatic cancer model.METHODS Near-infrared photoimmunotherapy(NIR-PIT) is a highly selective tumor treatment that utilizes an antibody-photosensitizer conjugate administration, followed by NIR light exposure. Anti-TF antibody 1849-ICG conjugate was synthesized by labeling of rat IgG2 b anti-TF monoclonal antibody 1849(anti-TF 1849) to a NIR photosensitizer,ICG. The expression levels of TF in two human pancreatic cancer cell lines were examined by western blotting. Specific binding of the 1849-ICG to TF-expressing BxPC-3 cells was examined by fluorescence microscopy. NIR-PITinduced cell death was determined by cell viability imaging assay. In vivo longitudinal fluorescence imaging was used to explore the accumulation of 1849-ICG conjugate in xenograft tumors. To examine the effect of NIRPIT, tumor-bearing mice were separated into 5 groups:(1) 100 μg of 1849-ICG i.v. administration followed by NIR light exposure(50 J/cm2) on two consecutive days(Days 1 and 2);(2) NIR light exposure(50 J/cm2) only on two consecutive days(Days 1 and 2);(3) 100 μg of 1849-ICG i.v. administration;(4) 100 μg of unlabeled antiTF 1849 i.v. administration; and(5) the untreated control. Semiweekly tumor volume measurements, accompanied with histological and immunohistochemical(IHC) analyses of tumors, were performed 3 d after the 2nd irradiation with NIR light to monitor the effect of treatments. RESULTS High TF expression in BxPC-3 cells was observed via western blot analysis, concordant with the observed preferential binding with intracellular localization of 1849-ICG via fluorescence microscopy. NIR-PIT-induced cell death was observed by performing cell viability imaging assay. In contrast to the other test groups, tumor growth was significantly inhibited by NIR-PIT with a statistically significant difference in relative tumor volumes for 27 d after the treatment start date [2.83 ± 0.38(NIR-PIT) vs 5.42 ± 1.61(Untreated), vs 4.90 ± 0.87(NIR), vs 4.28 ±1.87(1849-ICG), vs 4.35 ± 1.42(anti-TF 1849), at Day 27, P < 0.05]. 6黄南藏族自治州2004年新生儿乙型肝炎疫苗首针及时接种率及影响因素调查显示文摘目的了解影响黄南藏族自治州新生儿乙型肝炎(乙肝)疫苗首针(HepB1)及时接种的因素,探索提高农牧区新生儿HepB1及时接种率的方法。方法通过查阅文献、现场访问、入户调查、问卷调查等方式收集有关资料及信息。结果2004年黄南藏族自治州新生儿HepB1报告及时接种率为42.61%,其中在医院出生新生儿HepB1及时接种率为91.77%,在家出生新生儿HepB1及时接种率为19.58%;全州住院分娩率为31.90%;入户接种占51.06%;HepB1未及时接种原因主要是家长不知道,占39.66%。结论2004年全州新生儿HepB1及时接种率较低。今后要加大宣传和开展健康教育活动,掌握在家分娩孕妇的分娩信息和合理安排在家出生新生儿及时接种,是提高新生儿HepB1及时接种率的有效手段之一。李承宁 万玛加 李秀芹 张永基 张昕伟 巴文生 李青旗 乔华 胡苑笙 马耀军 崔富强 梁晓峰 Yoshihiro Takashima 倪大新 2007中国计划免疫2007,13,1:6
7黄南藏族自治州提高新生儿乙型肝炎疫苗首针及时接种率策略研究显示文摘目的调查影响边远贫困农牧区新生儿乙型肝炎(乙肝)疫苗首针(HepB1)及时接种的主要因素,探索提高新生儿HepB1及时接种率的有效手段和模式。方法在黄南藏族自治州开展基线调查,制定针对性策略并实施。包括开展多途径、多渠道的健康教育及宣传活动;加强对医院相关人员的培训,确保在医院出生新生儿得到及时接种;加强疾病预防控制机构与妇幼保健机构、村医与接生员之间的沟通与合作,做好孕妇产前登记和随访工作,确保在家出生新生儿得到及时接种;加强督导和技术支持,定期监测、分析相关指标,不断完善相关策略。结果项目实施前、后全州HepB1及时接种率由2004年的42.61%上升到2005年的75.02%。结论通过项目的实施,显著提高了农牧区新生儿HepB1及时接种率。张永基 胡苑笙 张昕伟 石西安 万玛加 李青旗 李承宁 李秀芹 崔富强 梁晓峰 倪大新 Yoshihiro Takashima 2007中国计划免疫2007,13,1:6

9Ulcerative colitis patients in clinical remission demonstrate correlations between fecal immunochemical test results, mucosal healing, and risk of relapse显示文摘AIM: To assess the risk of relapse in ulcerative colitis(UC) patients in clinical remission using mucosal status and fecal immunochemical test(FIT) results. METHODS: The clinical outcomes of 194 UC patients in clinical remission who underwent colonoscopy were based on evaluations of Mayo endoscopic subscores(MESs) and FIT results.RESULTS: Patients with an MES of 0(n = 94, 48%) showed a ten-fold lower risk of relapse than those with an MES of 1-3(n = 100, 52%)(HR = 0.10, 95%CI: 0.05-0.19). A negative FIT result(fecal hemoglobin concentrations ≤ 100 ng/m L) was predictive of patients with an MES of 0, with a sensitivity of 0.94 and a specific of 0.76. Moreover, patients with a negative FIT score had a six-fold lower risk of clinical relapse than those with a positive score(HR = 0.17, 95%CI: 0.10-0.28). Inclusion of the distinguishing parameter, sustaining clinical remission > 12 mo, resulted in an even stronger correlation between negative FIT results and an MES of 0 with respect to the risk of clinical relapse(HR = 0.11, 95%CI: 0.04-0.23).CONCLUSION: Negative FIT results one year or more after remission induction correlate with complete mucosal healing(MES 0) and better prognosis. 12定西市2005年出生儿童乙型肝炎疫苗首针及时接种率及影响因素调查显示文摘[目的]寻找影响定西市新生儿乙型肝炎(乙肝)疫苗首针(HepB1)及时接种的因素,探索提高贫困山区新生儿HepB1及时接种率的方法。[方法]通过查阅资料、入户调查、问卷调查等方式收集相关资料。[结果]共调查2005年出生新生儿840名,住院分娩率48.93%,HepB1及时接种率61.31%,其中住院分娩新生儿HepB1及时接种率88.56%,在家分娩新生儿HepB1及时接种率35.20%。未住院分娩的原因主要是住院费用太高、离医院太远和认为住院分娩没有必要(占87.18%)。村级接种工作仍以入户接种(占82.16%)为主,从所在村到乡镇卫生院领取疫苗和入户接种主要通过步行(分别占43.11%和68.58%)完成。[结论]定西市2005年新生儿HepB1及时接种率处于较低水平。主要影响因素是经济文化落后、新生儿住院分娩率较低、免疫规划宣传不够、对辖区内的孕产妇和新生儿摸底不清等。今后要通过加大宣传教育力度、开展孕产妇和新生儿入户调查摸底、加强与妇幼机构和妇检机构的合作等措施,进一步提高新生儿HepB1及时接种率。漆可发 李慧 高丽 张晓曙 崔富强 Yoshihiro Takashima 倪大新 彭博识 2010卫生软科学2010,24,2:2
