| 1 | 中国心力衰竭流行病学调查及其患病率显示文摘目的 了解我国成年人慢性心力衰竭 (心衰 )的患病率和分布特征。方法 中国心血管健康多中心合作研究应用四阶段整群随机抽样方法 ,在全国 1 0个省市 (南方和北方各 5个省市 )抽取具有代表性的样本 ,年龄在 35~ 74岁之间 ,城市和农村各半 ,男、女人数均衡。统计不同年龄组、不同性别和不同地区人群的心衰患病率。结果 共抽样调查 35~ 74岁城乡居民 1 5 51 8人 ,心衰患病率为0 9% ;其中男性为 0 7% ,女性为 1 0 % ,女性患病率高于男性 (P <0 0 5)。 35~ 44岁、45~ 54岁、55~64岁、65~ 74岁年龄组的心衰患病率分别为 0 4%、1 0 %、1 3 %和 1 3 % ;随着年龄增高 ,心衰的患病率显著上升 (P <0 0 1 )。我国北方地区心衰患病率为 1 4% ,南方地区心衰患病率为 0 5 % ,北方明显高于南方 (P <0 0 1 ) ;城市人群心衰患病率为 1 1 % ,农村人群心衰患病率为 0 8% ,城市人群心衰患病率高于农村。 | 顾东风 黄广勇 吴锡桂 段秀芳 何江 Paul K Whelton Stephen Mac Mahon | 2003 | 中华心血管病杂志2003，31，1： | 1271 |

| 3 | Current strategies for reproductive management of gilts and sows in North America显示文摘Many advances in genetic selection, nutrition, housing and disease control have been incorporated into modern pork production since the 1950 s resulting in highly prolific females and practices and technologies, which significantly increased efficiency of reproduction in the breeding herd. The objective of this manuscript is to review the literature and current industry practices employed for reproductive management. In particular the authors focus on assisted reproduction technologies and their application for enhanced productivity. Modern maternal line genotypes have lower appetites and exceptional lean growth potential compared to females of 20 yr ago. Thus,nutrient requirements and management techniques and technologies, which affect gilt development and sow longevity, require continuous updating. Failure to detect estrus accurately has the greatest impact on farrowing rate and litter size. Yet, even accurate estrus detection will not compensate for the variability in the interval between onset of estrus and actual time of ovulation. However, administration of Gn RH analogs in weaned sows and in gilts after withdrawal of altrenogest do overcome this variability and thereby synchronize ovulation, which makes fixed-time AI practical. Seasonal infertility, mediated by temperature and photoperiod, is a persistent problem. Training workers in the art of stockmanship is of increasing importance as consumers become more interested in humane animal care.Altrenogest, is used to synchronize the estrous cycle of gilts, to prolong gestation for 2–3 d to synchronize farrowing and to postpone post-weaning estrus. P.G. 600? is used for induction of estrus in pre-pubertal gilts and as a treatment to overcome seasonal anestrous. Sperm cell numbers/dose of semen is significantly less for post cervical AI than for cervical AI. Real-time ultrasonography is used to determine pregnancy during wk 3–5. PGF2αeffectively induces farrowing when administered within two d of normal gestation length. Ovulation synchronization, single fixed-time AI and induced parturition may lead to farrowing synchronization, which facilitates supervision and reduces stillbirths and piglet mortality. Attendance and assistance at farrowing is important especially to ensure adequate colostrum consumption by piglets immediately after birth. | 5 | 卒中相关性肺炎的诊断卒中肺炎共识小组的推荐意见显示文摘背景和目的下呼吸道感染常并发于卒中并对转归造成不利影响。对于并发于卒中的下呼吸道感染性疾病,目前尚无公认的专业术语或诊断金标准,这导致临床诊疗和研究受到影响。本共识旨在提出急性卒中并发下呼吸道感染的标准化术语和可操作性诊断标准。方法对多个文献数据库进行系统检索,经过证据回顾和2轮讨论,最后于2014年9月在英国曼彻斯特召开共识会议,经75%以上共识小组成员达成一致意见后形成共识。结果达成的共识如下:(1)推荐使用卒中相关性肺炎(stroke-associated pneumonia, SAP )作为卒中发病后7 d内发生的下呼吸道感染性疾病的专业术语。(2)提议对 SAP 使用经过改良的美国疾病控制预防中心(Centers for Disease Control and Prevention, CDC)诊断标准:①很可能的 SAP:符合 CDC 诊断标准,但复查或动态监测胸部 X 片仍缺乏典型改变;②确诊的 SAP:符合 CDC 诊断标准,且胸部 X 片有典型改变。(3)关于白细胞计数或 C反应蛋白在 SAP 诊断中的作用证据有限。(4)其他生物学标记物(如降钙素原)的证据不足。结论提议根据 CDC 诊断标准对 SAP 的术语和诊断制定可操作性的共识标准,需要在卒中患者中进行前瞻性研究来确定其信度、效度以及对医生行为(包括抗生素选择)和临床转归的影响。 | Craig J. | 6 | Assessment of noise and ground vibration induced during blasting operations in an open pit mine——A case study on Ewekoro limestone quarry,Nigeria显示文摘Our study was carried out to assess the level of noise generated and ground vibrations induced during blasting operations at the Ewekoro limestone quarry in Nigeria.To achieve this objective,vibro monitor equipment was used to take readings related to noise generated and ground vibrations during all blasting operations that took place in the quarry for a period of one month.As well,a digital camera was used to take photographs of residential structures within villages near the quarry.The results obtained indicate that the ground vibration readings fall between 0.5 mm/s and 2.1 mm/s and the noise generated during the blasting operations between 82 dB and 89 dB.These readings when compared with the limits set by FEPA(Federal Environmental Protection Agency) of 5.0 mm/s and 150 dB) all fall within the permissible limits.However the photographs of most structures near the quarry reveal cracks and dilapidated building walls.Recommendations are made on how to sustain and improve current blasting techniques. | Afeni Thomas B Osasan Stephen K | 2009 | Mining Science and Technology2009，19，4： | 9 |

| 8 | Current concepts in the immunohistochemical evaluation of liver tumors显示文摘Immunohistochemistry often plays an important role in the evaluation of liver tumors. Recent advances have established a classification system for hepatocellular adenomas(HCAs) based on morphology,molecular alterations,and immunohistochemistry. Specifically,loss of liver fatty acid binding protein is seen in HNF1α-inactivated HCA,staining with serum amyloid A isseen in inflammatory HCA,and diffuse staining with glutamine synthetase(GS) is seen in β-catenin activated HCA. A panel of immunohistochemical stains including glypican-3(GPC-3),heat shock protein 70,and GS are useful in distinguishing HCC from non-malignant dysplastic nodules. Immunohistochemistry is also useful to determine whether a liver tumor is of primary hepatocellular or metastatic origin. Recently described markers useful for this purpose include arginase-1,GPC-3,and bile salt export pump. These newer markers may offer superior utility when compared to traditional markers of hepatocellular differentiation such as alpha-fetoprotein,hepatocyte paraffin-1,polyclonal carcinoembryonic antigen,and CD10. | 10 | Parvovirus B19 induced hepatic failure in an adult requiring liver transplantation显示文摘Parvovirus B19 induced acute hepatitis and hepatic failure have been previously reported, mainly in children. Very few cases of parvovirus induced hepatic failure have been reported in adults and fewer still have required liver transplantation. We report the case of a 55-year-old immunocompetent woman who developed fulminant hepatic failure after acute infection with Parvovirus B19 who subsequently underwent orthotopic liver transplantation. This is believed to be the first reported case in the literature in which an adult patient with fulminant hepatic failure associated with acute parvovirus B19 infection and without hematologic abnormalities has been identified prior to undergoing liver transplantation. | 11 | Th1 cytokines promote T-cell binding to antigen-presenting cells via enhanced hyaluronan production and accumulation at the immune synapse显示文摘Hyaluronan(HA)production by dendritic cells(DCs)is known to promote antigen presentation and to augment T-cell activation and proliferation.We hypothesized that pericellular HA can function as intercellular‘glue’directly mediating T cell–DC binding.Using primary human cells,we observed HA-dependent binding between T cells and DCs,which was abrogated upon pre-treatment of the DCs with 4-methylumbelliferone(4-MU),an agent which blocks HA synthesis.Furthermore,T cells regulate HA production by DCs via T cell-derived cytokines in a T helper(Th)subset-specific manner,as demonstrated by the observation that cell-culture supernatants from Th1 but not Th2 clones promote HA production.Similar effects were seen upon the addition of exogenous Th1 cytokines,IL-2,interferon c(IFN-c)and tumor necrosis factor a(TNF-a).The critical factors which determined the extent of DC–T cell binding in this system were the nature of the pre-treatment the DCs received and their capacity to synthesize HA,as T-cell clones which were pre-treated with monensin,added to block cytokine secretion,bound equivalently irrespective of their Th subset.These data support the existence of a feedforward loop wherein T-cell cytokines influence DC production of HA,which in turn affects the extent of DC–T cell binding.We also document the presence of focal deposits of HA at the immune synapse between T-cells and APC and on dendritic processes thought to be important in antigen presentation.These data point to a pivotal role for HA in DC–T cell interactions at the IS. | Paul L Bollyky Stephen P Evanko Rebecca P Wu Susan Potter-Perigo S Alice Long Brian Kinsella Helena Reijonen Kelly Guebtner Brandon Teng Christina K Chan Kathy R Braun John A Gebe Gerald T Nepom Thomas N Wight | 2010 | Cellular & Molecular Immunology2010，7，3： | 3 |
| 12 | Advanced non-alcoholic steatohepatitis cirrhosis: A high-risk population for pre-liver transplant portal vein thrombosis显示文摘AIM To examine if liver transplant recipients with high-risk non-alcoholic steatohepatitis(NASH) are at increased risk for pre-transplant portal venous thrombosis.METHODS Data on all liver transplants in the United States from February 2002 through September 2014 were analyzed. Recipients were sorted into three distinct groups: High-risk(age > 60, body mass index > 30 kg/m2, hypertension and diabetes), low-risk and non-NASH cirrhosis. Multivariable logistic regression models were constructed.RESULTS Thirty-five thousand and seventy-two candidates underwent liver transplantation and of those organ recipients, 465 were transplanted for high-risk and 2775 for lowrisk NASH. Two thousand six hundred and twentysix(7.5%) recipients had pre-transplant portal vein thrombosis; 66(14.2%) of the high-risk NASH group had portal vein thrombosis vs 328(11.8%) of the lowrisk NASH group. In general, all NASH recipients were less likely to be male or African American and more likely to be obese. In adjusted multivariable regression analyses, high-risk recipients had the greatest risk ofpre-transplant portal vein thrombosis with OR = 2.11(95%CI: 1.60-2.76, P < 0.001) when referenced to the non-NASH group.CONCLUSION Liver transplant candidates with high-risk NASH are at the greatest risk for portal vein thrombosis development prior to transplantation. These candidates may benefit from interventions to decrease their likelihood of clot formation and resultant downstream hepatic decompensating events.
